Viable intrauterine pregnancy with acute salpingitis progressing to septic abortion. A case report.

BACKGROUND: A viable intrauterine pregnancy with salpingitis has been reported rarely. CASE: A 27-year-old woman at 10 weeks' gestation developed abdominal pain, fever, leukocytosis, peritoneal signs, closed cervix and a viable pregnancy. Progression from acute salpingitis to septic abortion was documented. CONCLUSION: Acute salpingitis in the presence of a viable pregnancy warrants aggressive intervention.

[Relation between abnormal pregnancies and Toxoplasma gondii infection].

3,012 pregnant women were divided into three groups according to the status of Toxoplasma gondii infection, and the pregnancy outcomes were followed up. Results showed that the total incidence rates of spontaneous abortion, premature birth, still birth and congenital defects were 0.70, 3.39, 0.83 and 0.90% respectively. The incidence rates of the four groups of abnormal pregnancies having serologically diagnosed recent or active infection were significantly higher than that having no evidence of infection. The relative risk estimates (ORmh) of the 4 groups were 7.01, 3.44, 8.76 and 9.34% respectively. The incidence rates of the groups diagnosed as having past infection were not significantly different from that having no evidence of infection. Analysis of the time lapse from positive serodiagnosis to the appearance of abnormal pregnancies showed that the development of congenital defects was statistically related with Toxoplasma gondii infection in the first trimester of gestation.

Study on toxoplasmosis in Quasseem, Saudi Arabia.

This study presents evidence of Toxoplasma infection in Quasseem, Saudi Arabia and directs the attention that Toxoplasmosis is an important cause of repeated abortion there. Pregnant women should avoid handling or caring for pet cats. Also, raw meat and raw milk from infected animals should be avoided.

[Apropos of 2 cases of female genital tuberculosis discovered at operation].

The authors report two cases of female, genital tuberculosis with predominantly abdominal symptomatology, which required a laparotomy. In one case, the affliction was discovered during the gynecological period, whereas the second was found during the gravidic period and led to an abortion. The authors cover the classical clinical, biological and radiological aspects of this affliction, which is still current in their region. They stress the need to intensify anti-tubercular prophylaxis as the only means of improving obstetrical prognostic for the genital localization.

[Prevention of congenital toxoplasmosis].

Toxoplasma gondii can be transmitted from mother to fetus during primary maternal infection acquired after or, possibly, slightly before conception. The incidence of congenital infection is highest in the third trimester, while severity is greatest when maternal infection is acquired during the first trimester. About 50 per cent of mothers who acquire the infection during gestation, if not treated, will give birth to infected infants. Incidence of congenital toxoplasmosis varies from 0.5 to 6.5 cases per 1000 live births. Serologic screening before or very early in pregnancy is required to identify seronegative women who are at risk to acquire the infection during pregnancy. Prevention of congenital toxoplasmosis is obtained by educating pregnant women at risk about how to prevent the infection and by diagnosing acute infection of mother. Every mother who demonstrates seroconversion for toxoplasmosis during pregnancy has to be treated as soon as possible. Therapy is based on spiramycin that achieves high concentrations in the placenta; if the fetus is infected pyrimethamine plus sulphonamides are administered since fourth month. Chemotherapy of the infected pregnant mother reduces the incidence of congenital toxoplasmosis and the severity of the disease in the newborn. Intrauterine infection can be detected by fetal blood sampling, by amniocentesis and ultrasound examination; prenatal diagnosis is mandatory if an abortion is being considered.

[HIV screening of pregnant women and women seeking abortion. A preventive measure against HIV?].

In our opinion, screening of women seeking abortion and pregnant women does not benefit the work of HIV-prevention. Of the 115,600 tested from September 1987 to December 1988 in Norway, four new and HIV-positive and 92 false positive women were diagnosed. From the time screening started in September 1987 to the end of February 1989, the program has cost NOK 6-7 million. The prevalence of HIV among pregnant and abortion-seeking women is too low, and the problems associated with false positive results are too great for the project to be recommended. Pregnant women and women seeking abortion, and their partners, should be given the possibility of taking the HIV-test in the same manner as other low prevalence groups, where information and counselling are integrated and confidentiality is secured.

Sarcocystis induced abortion and fetal death.

There are many unanswered questions and observations that appear contradictory within the subject of how sarcocystosis affects fetal health. Some fetuses are infected and have lesions, others are unaffected. Some fetal placentas are infected and have lesions, others are unaffected. Some maternal placentomes are infected and have lesions, others are unaffected. Most pregnant animals have overt clinical illness at the time the fetus is affected, others do not. Several possible mechanisms have been hypothesized by which sarcocystosis may directly or indirectly affect fetal health.

Pregnancy outcomes after first-trimester vaginitis drug therapy.

Prescription frequencies in the first trimester were compared for miconazole, clotrimazole, nystatin, candicidin, aminacrine compounds, and metronidazole before deliveries involving congenital anomalies versus those not linked to congenital anomalies. Prescriptions before spontaneous abortions were compared with those in the first trimester of deliveries and with those before legal abortions. No statistically significant association was observed with any of these agents for the overall frequency of birth defects or for specific birth defects analyzed (cardiovascular defects, oral clefts, and spina bifida). Two hundred fifty miconazole exposures among 4264 spontaneous abortions, compared with 2236 in the first trimester of 55,736 deliveries, provided an estimated relative risk of 1.4 (95% confidence limits 1.2-1.5). One hundred twelve treatments with clotrimazole among the spontaneous abortions, compared with 1086 among the deliveries, provided a relative risk of 1.4 (95% confidence limits 1.1-1.6). In contrast, large numbers of exposures to nystatin and aminacrine compounds did not show this association, suggesting that spontaneous abortions are caused by the imidazole agents miconazole and clotrimazole rather than the condition being treated. Because many associations were examined without previous hypotheses, and because the data were inadequate to show an elevated risk for clotrimazole when comparing spontaneous with legal abortion exposures, these findings are considered to be a signal for further studies rather than definitive in themselves.

Excretion of Leptospira interrogans serovar hardjo following calving or abortion.

Leptospira interrogans serovar hardjo was demonstrated in the vaginal discharges of 12 experimentally infected heifers for up to eight days after abortion or calving. Organisms were recovered from the oviducts of heifers examined at slaughter eight to 22 days after calving or 32 to 91 days after infection. They were also recovered from the uteri of four heifers eight to 22 days after calving.
