Cleidocranial dysplasia--a late diagnosis.

UNLABELLED: Cleidocranial dysplasia is a rare congenital disorder of bone characterized by abnormalities of clavicle, skull and dentition. There are features which are pathognomic for this condition and diagnosis can usually be made early in life. This report presents a case that was diagnosed very late in life, with a resultant compromised dental state, and discusses current treatment modalities and the differential diagnosis with another condition with which this was mistaken. CLINICAL RELEVANCE: Delayed eruption of permanent teeth must be diagnosed and investigated early so that it can be managed appropriately.

IOTN (DHC): is it supported by evidence?

UNLABELLED: The strength of the evidence linking some common malocclusion traits (which are used as the basis for IOTN classification) with dental disease is reviewed. It is concluded that IOTN is not a reliable predictor of the potential impact of malocclusion on future dental health, and that alternative indices, which place greater emphasis on the aesthetic impairment and take account of patient perceptions, are more appropriate for assessing an individual's orthodontic treatment need. CLINICAL RELEVANCE: IOTN is likely to assume greater significance with the advent of the new GDS/PDS regulations pertaining to orthodontics. However, all practitioners, including those who commission specialist services, should be aware of its limitations, especially in relation to the Dental Health Component.

Study of the maturation of the organic (type I collagen) and mineral (nonstoichiometric apatite) constituents of a calcified tissue (dentin) as a function of location: a Fourier transform infrared microspectroscopic investigation.

Fourier transform infrared microspectroscopy (FTIRM) was used to investigate the organic and mineral phases of a calcified tissue (dentin) as a function of its location from predentin toward enamel. Thin dentin slices (decalcified or not) were fixed in formaldehyde and embedded in glycolmethylmethacrylate (GMA). Fixation did not denature collagen, and GMA did not interact with organic or mineral constituents of dentin. The v1v3 PO4 domain was studied in particular in order to estimate mineral maturity and amide I, II, A, and B to obtain data on protein conformation. The results showed that dentin apatite became increasingly mature (stoichiometric) from the mineralization front toward the enamel, especially through loss of HPO4(2-) groups and vacancies. Moreover, collagen fibrils became less and less hydrated, suggesting that intrafibrillar mineralization partially dehydrated the collagen. Combined study of the organic and mineral fractions of calcified tissues may help clarify their relationships in physiological and pathological tissues.

The case of the disappearing third molar.

A mandibular third molar moved into the lower border during a period of several years. When the patient experienced discomfort, an incisional biopsy and marsupialization were performed. Eight months later, adequate bone had regenerated and the molar and remaining cyst structure were removed. Full-mouth and panoramic radiographs would have allowed the dentist to locate the missing molar before surgery was necessary.

Cleidocranial dysostosis: a multidisciplinary approach to treatment.

To ensure eruption of the dentition in patients with cleidocranial dysostosis, close collatoration between the oral surgeon and the orthodontist is essential to determine the sequence of extraction, the teeth to be ligated, the preferred technique of ligation, and the approach for surgical exposure.

Premolar enucleation and autogenous transplantation.

Tooth transplantation by autograft and orthodontic treatment can be used to correct occlusal disharmony and avoid fixed partial dentures. Premolars are used in this case to accomplish almost normal dentition.

Panoramic radiographic survey of dentists participating in ADA health screening programs: 1976, 1977, and 1978.

Results of the panoramic examination of dentists participating in the health evaluation program of the ADA annual sessions held at Las Vegas (1976), Miami Beach (1977), and Anaheim (1978) were reported and compared as accurately as possible with the results of the panoramic examinations of dentists taking part in the health evaluation programs of the ADA annual sessions in 1964, 1966, and 1968. The panoramic radiograph continues to serve as a diagnostic aid in dental health evaluation programs. Only general impressions may be drawn from surveys of this nature for various reasons: the inability to follow up each case with a clinical examination; the lack of correlation of findings between examiners; and the use of different criteria each year. Generally, dentists examined in the health evaluation programs in 1976, 1977, and 1978 were in good dental health.

Supernumerary mandibular premolars: report of cases.

Six cases, totaling seven supernumerary mandibular premolars, were seen in a sample population of 4,000 patient records. Four were from a suburban private practice sample of 2,000, the others from a similar group randomly drawn from an urban dental school. Five of seven appeared on the left side, six of seven were impacted or erupted ectopically. Only one patient was female. Four were of Spanish or Hispanic descent, one was white and one was black. The only other supernumerary teeth seen in the population sampled were two paramolars and one mesiodens. The recommended course of treatment is extraction of the supernumerary teeth, unless they may be useful for orthodontic purposes or if the surgery would be unjustifiably hazardous to adjacent structures. Complications could result in resorption of other teeth, malocclusion, and possible cystic degeneration and ameloblastoma formation.

Identifying a deciduous dentigerous cyst.

While the dentigerous cyst is not uncommon, its development as a result of an impacted deciduous tooth is rare. According to the authors' search, this case is only the fifth of its type reported in the dental literature. The authors review the presentation and treatment of a deciduous dentigerous cyst.

Treatment options for impacted teeth.

BACKGROUND: A compilation of treatment options for impacted teeth is presented to assist dentists in discussing the sequelae of impacted teeth as well as the complications of treatment with their patients. OVERVIEW: A differential diagnosis for an impacted tooth is not possible without clinical assessment. However, an ankylosed tooth or a tooth with failure of its eruption mechanism may be mistaken for an impacted tooth. This review of national and international dental texts, journals and publications concerning impacted teeth spans more than 50 years. PRACTICE IMPLICATIONS: Treatment options for the management of impacted teeth are separated into four categories: observation, intervention, relocation and extraction. The indications, contraindications and complications of each option are presented. This information will help the clinician identify developmental conditions associated with an increased probability of tooth impaction and assess available treatment options.

Non-syndromal multiple buried supernumerary teeth: report of two cases from the English-speaking Caribbean and a review of the literature.

Multiple supernumerary teeth affecting all four quadrants of the jaw are a rare dental anomaly which has become a chance finding on routine dental panoramic tomography (DPT). In this paper, two cases from the English-speaking Caribbean are reported. The role of radiography in the diagnosis and management of this rare developmental dental anomaly is emphasized. The paper stresses the importance of ruling out associated syndromes such as Gardner's Syndrome, cleidocranial dysostosis and cleft lip and palates, as multiple supernumerary teeth are usually related to such conditions. There is a review of the literature as it relates to supernumerary teeth.
