Head-to-head comparison of three-dimensional navigator-gated magnetic resonance imaging and 16-slice computed tomography to detect coronary artery stenosis in patients.

OBJECTIVES: The purpose of this research was to compare the diagnostic accuracy of three-dimensional navigator-gated magnetic resonance (MR) imaging and 16-slice multidetector row computed tomography (MDCT) versus quantitative coronary angiography (QCA) for the detection of coronary artery stenosis in patients. BACKGROUND: Both MR and MDCT are novel non-invasive tests, which have been proposed for noninvasive detection of coronary artery disease. Yet their diagnostic accuracy has not been directly compared in the same population. METHODS: Fifty-two patients underwent coronary MR and 16-slice MDCT before invasive coronary angiography. Diameter stenosis (DS) severity in vessels >1.5-mm reference diameter were graded visually and measured quantitatively on both MR and MDCT images. Diagnostic accuracy of both methods was compared using QCA as the reference test. RESULTS: According to QCA, 81 of 452 (18%) coronary segments with >1.5 mm diameter had >50% DS. By visual analysis, MR and MDCT had similar sensitivity (75% vs. 82%, p = NS), specificity (77% vs. 79%, p = NS), and diagnostic accuracy (77%, vs. 80%, p = NS) for detection of >50 % DS. Quantitative measures of DS by MR (r = 0.60, p < 0.001) and MDCT (r = 0.75, both p < 0.001) correlated well with QCA. Receiver-operating characteristic analysis demonstrated that quantification of DS severity improved the diagnostic accuracy of MDCT (area under curve [AUC] 0.81 vs. 0.92, p < 0.001) but not that of MR (AUC 0.78 vs. 0.83, p = NS). CONCLUSIONS: Visual assessment of coronary diameter stenosis severity by MR or MDCT allows identification of significant coronary artery disease with a similar high diagnostic accuracy. Quantitative analysis significantly further improves the diagnostic accuracy of MDCT but not that of MR.

DATA-MEAns: an open source tool for the classification and management of neural ensemble recordings.

The number of laboratories using techniques that allow to acquire simultaneous recordings of as many units as possible is considerably increasing. However, the development of tools used to analyse this multi-neuronal activity is generally lagging behind the development of the tools used to acquire these data. Moreover, the data exchange between research groups using different multielectrode acquisition systems is hindered by commercial constraints such as exclusive file structures, high priced licenses and hard policies on intellectual rights. This paper presents a free open-source software for the classification and management of neural ensemble data. The main goal is to provide a graphical user interface that links the experimental data to a basic set of routines for analysis, visualization and classification in a consistent framework. To facilitate the adaptation and extension as well as the addition of new routines, tools and algorithms for data analysis, the source code and documentation are freely available.

Picture the difference: electrophysiological investigations of picture processing in the two cerebral hemispheres.

The nature of semantic memory and the role of the two cerebral hemispheres in meaning processing were examined using event-related brain potentials (ERPs) elicited by pictures in sentences. Participants read sentence pairs ending with the lateralized presentation of three target types: (1) expected pictures, (2) unexpected pictures from the expected semantic category, and (3) unexpected pictures from an unexpected category. ERPs to contextually unexpected pictures were more negative 350-500ms (larger N400s) than those to expected pictures in both visual fields. However, while N400s to the two types of unexpected items did not differ with left visual field presentations, they were smaller to the unexpected items from the expected category with right visual field presentations. This pattern, previously observed to words [Brain Language 62 (1998) 149], suggests general differences in how the two hemispheres use context on-line. Other aspects of the N400 response-and effects on earlier ERP components-reveal differences between pictures and words, suggesting that semantic memory access is not modality-independent. The P2 component varied with ending type for right but not left visual field presentations, suggesting that the left hemisphere may use contextual information to prepare for the visual analysis of upcoming stimuli. Furthermore, there was clear evidence for an earlier negativity ("N300"), which varied with ending type but, unlike the N400, was unaffected by visual field of presentation. Overall, the results support our hypothesis that the left hemisphere actively uses top-down information to preactivate perceptual and semantic features of upcoming stimuli, while the right hemisphere adopts a "wait and see" integrative approach.

Visual acuity, environmental complexity, and social organization in African cichlid fishes.

The authors studied the effects of habitat complexity and social organization on visual acuity in closely related cichlid fishes (the Ectodini clade). The authors quantified habitat complexity among sand, intermediate, and rock habitats using chromatic difference measures (intensity analysis). Visual acuity was measured behaviorally, using optomotor/optokinetic responses to rotating square-wave stimuli. The rock-dwelling Asprotilapia leptura exhibited the best visual ability, compared with the intermediate and sand-dwelling species, Xenotilapia spilotera and Xenotilapia flavipinnis. The authors then compared effects of social organization. The lek-forming, polygamous Enantiopus melanogenys showed better visual acuity than that of the pair-bonding, monogamous X. flavipinnis. The authors' results are the first to demonstrate that species-specific differences in visual acuity are associated with differences in both the physical and social environment.

Techniques of remote sensing applied to the environmental analysis of part of an aquifer located in the São José dos Campos Region sp, Brazil.

The anthropogenic activity on the surface can modify and introduce new mechanisms of recharging the groundwater system, modifying the tax, the frequency and the quality of recharge of underground waters. The understanding of these mechanisms and the correct evaluation of such modifications are fundamental in determining the vulnerability of groundwater contamination. The groundwater flow of the South Paraíba Compartment, in the region of São José dos Campos, São Paulo, is directly related to structural features of the Taubaté Basin and, therefore, the analysis of its behaviour enhances the understanding of tectonic structure. The methodology adopted for this work consists in pre-processing and processing of the satellite images, visual interpretation of HSI products, field work and data integration. The derivation of the main structural features was based on visual analysis of the texture elements of drainage, and the relief in sedimentary and crystalline rocks. Statistical analysis of the feature densities and the metric-geometric relations between the analysed elements have been conducted. The crystalline rocks, on which the sediments were laying, conditions and controls the structural arrangement of sedimentary formations. The formation of the South Paraíba Grabén is associated with Cenozoic distensive movement which reactivated old features of crust weakness and generated previous cycles with normal characteristics. The environmental analysis is based on the integration of the existing methodology to characterise vulnerability of an universal pollutant and density fracture zone. The digital integration was processed using GIS (Geographic Information System) to delineate five defined vulnerability classes. The hydrogeological settings were analysed in each thematic map and, using fuzzy logic, an index for each different vulnerability class was compiled. Evidence maps could be combined in a series of steps using map algebra.

Diagnostic efficacy of optimised evaluation of planar MIBI myocardium perfusion scintigraphy: a probabilistic approach.

BACKGROUND: The Bayesian (probabilistic) approach to the results of a diagnostic test appears to be more informative than an interpretation of results in binary terms (having disease or not). The aim of our study was the analysis of the effect of an optimised evaluation of myocardium perfusion scintigrams on the probability of CAD in individual patients. METHODS: 197 patients (132 males and 65 females) suspected of CAD, with no history of myocardial infarction were examined. Scintigraphic images were evaluated applying two methods of analysis: visual (semiquantitative) and quantitative, and the combination of both. The sensitivity and specificity of both methods (and their combination) in the detection of CAD were determined and optimal methods of scintigram evaluation, separately for males and females, were selected. All patients were subjected to coronary angiography. The pre-test probability of CAD was assessed according to Diamond and the post-test probability was evaluated in accordance with Bayes's theorem. Patients were divided, according to a pre-test probability of CAD, into 3 groups: with low, medium and high probability of the disease. The same subdivision was made in relation to post-test probability of CAD. The numbers of patients in respective subgroups, before and after the test, were compared. Moreover, in order to test the reliability of post-test probability, its values were compared with real percentages of CAD occurrence among the patients under study, as demonstrated by the angiography. RESULTS: The combination of visual and quantitative methods was accepted as the optimal method of male scintigram evaluation (with sensitivity and specificity equalling 95% and 82%, respectively) and a sole quantitative analysis as the optimal method of female scintigram evaluation (sensitivity and specificity amounted to 81% and 84%, respectively). In the subgroup of males the percentage of individuals with medium pre-test CAD probability equalled 52 and after the scintigraphic study it decreased to 21 (p < 0.0001). In the subgroup of females it changed from 60 to 43 (p = 0.05). The verification of the values of post-test probability revealed its high concordance with the real frequencies of CAD, with correlation coefficient being 0.98 and the regression line differing only slightly from the line of identity. CONCLUSIONS: The results confirm the high reliability of the values of post-scintigraphic probability of CAD obtained in this way, and support the Bayesian, probabilistic interpretation of the study outcome and its application in the diagnostic process.

Computer-assisted detection of nocturnal leg motor activity in patients with restless legs syndrome and periodic leg movements during sleep.

STUDY OBJECTIVES: To assess the performance of a new method for automatic detection of periodic leg movements during sleep. METHODS: Leg movements during sleep were visually detected in the tibialis anterior muscles recordings of 15 patients with restless legs syndrome and 15 normal controls. Leg movements were detected automatically by means of a new computer method with which electromyogram signals are first digitally band-pass filtered and then rectified; subsequently, the detection of leg movements is performed by using 2 thresholds: one for the starting point and another to detect the end point of each leg movement. Sensitivity and false-positive rate were obtained; the American Sleep Disorders Association parameters were also computed, and the results analyzed by means of the Kendall W coefficient, the linear correlation coefficient and the Bland-Altman plots. SETTING: N/A. PARTICIPANTS: Fifteen patients with restless legs syndrome and periodic leg movements and 15 controls. MEASUREMENTS AND RESULTS: High values of the Kendall W coefficient of concordance between automatic and visual analysis were found with values close to 1 and the linear correlation coefficient for leg movements index and total leg movements index was > 0.950 (p < .000001). The Bland-Altman plots provided the limits of agreement between visual and computer detection, which were -9.01 and +9.89 for the periodic leg movement index. None of the normal controls was found to have periodic leg movement indexes >5 after automatic analysis. CONCLUSIONS: Our method can be applied to the clinical evaluation of periodic leg movements during sleep, with some caution in patients with a low periodic leg movement indexes. Large-scale research application is possible and can be considered as reliable.

Automatic chromosome analysis. II. Karyotyping of banded human chromosomes using band transition sequences.

Human chromosomes, represented by band transition sequences, chromosome area, centromeric index by area and centromeric index by density, were karyotyped by computer. A reference set of chromosomes provided frequencies of occurrence of each density class and difference class of the band transition sequence as well as of each of the three global features. The karyotyping program was designed to handle all metaphases, even those from which severely bent and overlapped chromosomes were excluded. In one experiment, 21 metaphases were karyotyped on the basis of a reference set and the results were compared with earlier results of visual analysis of band transition profiles developed from band transition sequences: 0.8% errors were made in the visual experiment and 1.4% errors were made in the computer based experiment. In a second experiment, 179 metaphases were divided into reference and test sets and karyotyped by computer with an error rate of 3.4%. By further analysis it was found that metaphases with many misclassified chromosomes could often be automatically distinguished from metaphases with few errors. Thus by automatic rejection of 7% of the metaphases the error rate could be reduced to 2.6%. The computer program for chromosome karyotyping will now be implemented in a semi-automatic system for practical clinical chromosome analysis.

Orientation discrimination in the cat: its cortical locus II. Extrastriate cortical areas.

Luminance-defined edges or bars are among the basic units of visual analysis: a "primitive" component of perception. We have utilized this stimulus in a psychophysical study of bar orientation discrimination in the cat before and after selective lesions in visual cortical areas. The cortices have been divided on the basis of their connectivity into three tiers. Tier I refers to areas 17 and 18, tier II includes areas that receive directly from tier I, and tier III includes those areas that receive directly from tier II. Previous studies (Vandenbussche et al. [1991] J. Comp. Neurol. 305:632-658) have shown that the discrimination of bar orientation depends heavily upon the integrity of areas 17 and 18 (tier I). The present study indicates that several extrastriate areas in tiers II and III contribute to this discrimination task. Our data suggest that the anterior medial lateral suprasylvian, the posterior lateral lateral suprasylvian (tier II), and the anterior lateral lateral suprasylvian (tier III) areas are most likely to contribute to bar orientation discrimination.

Ultrasound of the infant hip. Part 2. Validity of the method.

The reproducibility of classifications and measurements used in ultrasound of the infant hip has been evaluated in a prospective, double-blind study. Normal values and range distribution are defined for the main types as well as the potential for discriminating these types by means of ROC-analysis. The clinical validity of the method in defining normal and pathologic findings is discussed in comparison with X-ray diagnosis and follow-up sonographic investigations. Visual analysis focussing on a few diagnostic criteria, mainly the inclination of the acetabular roof and the position of the femoral head, provides sufficiently high accuracy (94% agreement rate) and the best diagnostic results. A correct examination which includes tests of the mobility of the hip joint and limitation of the section level to the maximal depth of the acetabular fossa, allows nearly perfect diagnosing of dysplasia and dislocation.

Subendocardial versus transmural ischaemia in myocardial perfusion SPECT--a Monte Carlo study.

UNLABELLED: Myocardial perfusion imaging with single-photon emission computed tomography (SPECT) is useful for the evaluation of patients with known or suspected coronary artery disease. Parameters of interest are the reduction in the blood perfusion (severity) and the lesion volume (extent). The aim of this study was to evaluate these parameters, as calculated by automatic quantification software, for different cases of subendocardial and transmural myocardial lesions. METHODS: A computer phantom was used to simulate 32 male patients with different defect locations and activity uptakes, which were based on clinical patient studies. The Monte Carlo program SIMIND was used to simulate realistic SPECT projections which were reconstructed to give short-axis images, analysed by the AutoQUANT program using the same procedure as for a real patient. RESULTS: The results showed a disparity between the quantification of transmural and subendocardial lesions with the same lesion activity uptake reduction and this could be confirmed by visual interpretation. Neither the parameters given by the quantification program nor visual interpretation could distinguish between the transmural lesions and the subendocardial lesions with activity uptake reduction twice as high as in the corresponding transmural lesions. CONCLUSION: Transmural lesions and the corresponding subendocardial lesions with the same activity uptake reduction could be separated by the quantification software for SPECT imaging and visual analysis. The subendocardial lesions with activity uptake reduction twice as high as in the corresponding transmural lesions could not be differentiated neither by the quantification software nor by visual interpretation. Thus these lesions will get the same scoring when analysed by the AutoQUANT program.

Modelling the propulsion characteristics of a standard wheelchair.

Recent wheelchair modelling work has focused on the efficiency of the human-racing wheelchair interaction. This paper builds on this work, investigating the development of a model appropriate for those using standard wheelchairs. A wheelchair racing model was initially used as the starting point for the generation of a number of model variations. Force predictions from these variations were compared to load cell data taken from an instrumented wheelchair during propulsion. Additional models where then developed, based on the characteristics of the models that performed best, and used to predict the forces in a second group of subjects. The analysis procedure was originally based on the calculation of a model index as a mathematical estimation of the theoretical closeness each model prediction had with the observed force. Visual comparisons of the force versus time were then incorporated into the procedure for evaluating the physical appearances of the profiles. The combination of the statistical and visual analysis led to selection of the final models for estimating the starting, constant, and stopping phases of wheelchair propulsion. The resulting models provide insight into the effects of a variety of factors on efficiency during propulsion in a standard chair.

Differential influence of the visual framework on end point accuracy and trajectory specification of arm movements.

In this study the influence of visual scene on both arm end point accuracy and spatial path kinematics was evaluated. Eight subjects, immersed in a virtual environment, were required to point to one of ten targets located at two distances and in five directions. Targets were presented in frameworks of different complexity. The simplest framework was constituted by a uniform background, the most complex framework was constituted by a perspective-arranged grid. In the other two conditions it consisted of lines having a direction parallel to either the subject's sagittal or frontal body axis. Movements were executed without vision of both target and framework. The results showed that pointing movements were hypometric in all conditions. No difference in end point localization was observed between movements executed after presentation of the simplest and the most complex scenes. However, hypometria significantly increased when the scene was formed by lines parallel to the subject's sagittal axis. Visual information on the scene was also used to specify hand path parameters. Trajectory curvature increased with decreasing complexity of the framework. Correspondingly, the pointing kinematics varied. Taken together, these results suggest that visual analysis of cues surrounding the target can influence both target localisation and hand path planning. However, scene complexity is directly related only to determining trajectory curvature. We conclude that planning an arm movement consists of at least two processes: target localisation and hand path specification. Environmental visual cues forming the scene are taken into account differently during the two processes.

Comparative value of visual and quantitative analysis of thallium-201 imaging after exercise in patients with an abnormal baseline repolarization on the electrocardiogram at rest.

In this study the role of visual assessment and quantitative analysis of thallium-201 myocardial stress perfusion scintigraphy (MPS) was evaluated in patients with a normal electrocardiogram (ECG) (group I) and in patients with an abnormal baseline repolarization on the ECG (group II). One-hundred-and-thirty-five patients were studied. Group I consisted of 93 patients, 65 of whom had coronary artery disease (CAD), i.e. a luminal stenosis of greater than or equal to 50%; group II consisted of 42 patients, 32 of whom had CAD. In group II the exercise ECG was considered uninterpretable and thus only visual and quantitative analysis of MPS was investigated. Sensitivity and specificity of exercise electrocardiography, visual assessment and quantitative analysis of MPS were not significantly different in group I: sensitivities of the three methods were 66, 62 and 55% respectively, and specificities 86, 79 and 96% respectively. In group II there were no significant differences in the sensitivities and specificities of visual assessment or quantitative analysis of MPS: sensitivities of the two methods being both 88% and specificities both 80%. Visual assessment of redistribution (P less than 0.05) and quantitative analysis of washout (P less than 0.005) was significantly superior in group II than in group I; probably because of a higher incidence of three-vessel disease in the second group.(ABSTRACT TRUNCATED AT 250 WORDS)

[Long-term electrocardiography: value and reliability of various systems].

25 years after the introduction of ambulatory electrocardiography four concepts are available: the conventional Holter-ECG, computer-based systems, entire ECG recording in miniaturized form and the event recorder. In contrast to visual analysis with the aid of an arrhythmia computer a more precise and quantitative evaluation of long-term ECG is possible. The reliability of a computer-based system is indicated by its sensitivity, positive predictive accuracy and reproducibility of automatic analysis. Only few of the computerized systems have been repeatedly tested on a company independent basis using these criteria in a representative number of patients, with frequent arrhythmias, and with a separate documentation of false negative, false positive and true positive events. Data available so far show that a reasonable reliability is possible, but 100% remains the exception to the rule. Thus, there is no complete automatic analysis of cardiac arrhythmias, and corrections by the physician are mandatory. This is further emphasized by the fact that only a part of all possible arrhythmias may be identified by computer whereas the other part has to be evaluated by visual control. In this context entire ECG recording is helpful, as an isolated method, however, it cannot be recommended, since the miniaturized ECG prevents analysis of subtle details and provides no quantitative data. Event recorders rely exclusively on the accuracy of the portable arrhythmia detector. Due to the concept of discontinuous ECG recording evaluation of this accuracy is pronounced problematic. Only by changing to the continuous recording mode does validation of the event recorder become possible. The first data demonstrate that the reliability of real-time analysis by the portable arrhythmia detector is not superior to highspeed analysis by the arrhythmia computer. Thus, quality control is necessary when using the event recorder as it is with continuous long-term ECG. However, there are significant limitations: with discontinuous ECG recording false negative events are not documented, separation of false positive from correct positive findings is restricted to the limited recording capacity (12-120 min real time) and the short ECG samples (6-12s) do not allow verification of sustained arrhythmias. Furthermore, questions regarding arrhythmias, ST-segment changes and pacemaker dysfunctions unidentifiable by computer remain unanswered with the use of the event recorder. For these reasons, continuous ambulatory electrocardiography providing complete data for every kind of analysis is still the standard method.(ABSTRACT TRUNCATED AT 400 WORDS)

[11C]-Methionine PET: dysembryoplastic neuroepithelial tumours compared with other epileptogenic brain neoplasms.

BACKGROUND AND OBJECTIVES: Brain tumours responsible for longstanding partial epilepsy are characterised by a high prevalence of dysembryoplastic neuroepithelial tumour (DNT), whose natural evolution is much more benign than that of gliomas. The preoperative diagnosis of DNT, which is not yet feasible on the basis of available clinical and imaging data, would help optimise the therapeutic strategy for this type of tumour. This study tested whether [(11)C]-methionine positron emission tomography (MET-PET) could help to distinguish DNTs from other epileptogenic brain tumours. METHODS: Prospective study of 27 patients with partial epilepsy of at least six months duration related to a non-rapidly progressing brain tumour on magnetic resonance imaging (MRI). A structured visual analysis, which distinguished between normal, moderately abnormal, or markedly abnormal tumour methionine uptake, as well as various regions of interest and semiquantitative measurements were conducted. RESULTS: Pathological results showed 11 DNTs (41%), 5 gangliogliomas (18%), and 11 gliomas (41%). MET-PET visual findings significantly differed between the various tumour types (p<0.0002), regardless of gadolinium enhancement on MRI, and were confirmed by semiquantitative analysis (p<0.001 for all calculated ratios). All gliomas and gangliogliomas were associated with moderately or markedly increased tumour methionine uptake, whereas 7/11 DNTs had a normal methionine uptake, including all six located in the mesiotemporal structures. No DNT presented with a marked MET-PET abnormality. CONCLUSION: Normal MET-PET findings in patient with an epileptogenic and non-rapidly progressing brain tumour are suggestive of DNT, whereas a markedly increased tumour methionine uptake makes this diagnosis unlikely.

Macroscopic brain dynamics during verbal and pictorial processing of affective stimuli.

Emotions can be viewed as action dispositions, preparing an individual to act efficiently and successfully in situations of behavioral relevance. To initiate optimized behavior, it is essential to accurately process the perceptual elements indicative of emotional relevance. The present chapter discusses effects of affective content on neural and behavioral parameters of perception, across different information channels. Electrocortical data are presented from studies examining affective perception with pictures and words in different task contexts. As a main result, these data suggest that sensory facilitation has an important role in affective processing. Affective pictures appear to facilitate perception as a function of emotional arousal at multiple levels of visual analysis. If the discrimination between affectively arousing vs. nonarousing content relies on fine-grained differences, amplification of the cortical representation may occur as early as 60-90 ms after stimulus onset. Affectively arousing information as conveyed via visual verbal channels was not subject to such very early enhancement. However, electrocortical indices of lexical access and/or activation of semantic networks showed that affectively arousing content may enhance the formation of semantic representations during word encoding. It can be concluded that affective arousal is associated with activation of widespread networks, which act to optimize sensory processing. On the basis of prioritized sensory analysis for affectively relevant stimuli, subsequent steps such as working memory, motor preparation, and action may be adjusted to meet the adaptive requirements of the situation perceived.

Computer-assisted interpretation of electro- and vectorcardiograms. Chapter IV. Achievements in the field of computer-assisted interpretation of electrocardiograms.

The most important advantage of computer-assisted ECG/VCG interpretation in our center is the enhancement of the diagnostic accuracy of ECG interpretation, as compared to that reported in the literature for the visual analysis of ECGs and VCGs. Favourable side effects are also obtained from the optimization of various functions involved n ECG data processing, like data handling, editing, reporting, storage and retrieval. Improvement of the arrhythmia program and creation of a new pediatric VCG program are currently in progress in our laboratory. Computerized electrocardiography has made us aware of the limitations of conventional, visual reading of electrocardiograms. Thanks to the current efforts towards standardization of the quantitative analysis of ECGs and VCGs and towards optimization of diagnostic criteria, computerized electrocardiography will undoubtedly lead to a generalized improved accuracy of ECG interpretation.
