Emergency repair followed by complete-coronal restoration of a fractured mandibular incisor.

Severely fractured anterior teeth often require immediate repair, while long-term restoration is delayed until a healing period is completed. This paper details a technique of immediate emergency repair of a fractured mandibular incisor followed by placement of a bonded composite resin complete-coronal restoration months later. Careful diagnosis, gentle handling of the freshly traumatized tooth and surrounding tissues, and ideal selection and use of the respective bonding materials are necessary to ensure eventual replication of form, function, and tooth shade when the long-term restoration is placed.

Materials and techniques in fixed prosthodontics.

If we have learned anything from our experiences with all-ceramic systems such as Cerestore, Cerapearl, and even Dicor, it is that it takes years to uncover the strengths, weaknesses, and limitations of new materials and technologies. Manufacturers' initial claims for fit, strength, esthetics, biocompatibility, wear characteristics, and clinical performance should be viewed as preliminary until supported by independent laboratory and clinical studies. Laboratory testing alone may not provide sufficient insight into the relative strengths and weaknesses of products or assure comparable performance in the commercial dental laboratory or the ultimate testing arena, the oral environment. Moreover, the diversity of the new materials and techniques discussed demonstrates the breadth and depth of the technologic changes in dental biomaterials. The rate at which these dental products enter the marketplace and their sheer number alone are indeed staggering. Furthermore, these advances are not emerging from any single nation but from manufacturers all around the world (Japan, Germany, as well as the United States), indicating our international reliance on one another. Clearly, the decade of the 1990s is emerging as an exciting period in the development of biomaterials in comprehensive fixed prosthodontics and dentistry in general.

[Observations on prosthetic restoration in children].

In young children there are two frequent factors, that cause edentation: the syndrome of the sugar "baby's bottle" or the "honey dummy" and tooth-loss by trauma. Multiple extractions can be programmed in children for reason of general condition, in the major concern to remove present or potential infectious foci. Still less frequently, sometimes lately diagnosed anomalies in number or structure oblige to replacement of missing or deficient tooth. The complexity of these restorations can be related to technical problems, less frequent, in contradiction to the current opinion, to psychological problems. They have to be integrated in structures in growth, the key character of each treatment in children or adolescents. Consequently it is necessary to take account of all this factors, before to attempt this charge, and to foresee adaptations, regular control, succession of the case and interception.

[Multi-caries in the young child].

This article describes the clinical and therapeutic aspects of multi-caries in young children. These rampant cervical caries usually begin with the eruption of the first teeth. Two typical cases are presented in which the prolonged use of nursing bottle at bedtime and the use of pacifiers dipped into honey are responsible for the development of multi-caries. Although such patient can be treated successfully, efforts should be made to promote preventive measures and parents education.

Teenage luxation injury: report of case.

The endodontic, restorative, and orthodontic treatment sequence of the accidental injury of three maxillary incisors has been presented. The treatment objective was to achieve an esthetically acceptable result for a young adult, until a definitive fixed prosthetic restoration can be planned. The ankylosed maxillary right permanent central incisor (11) is being maintained for reasons of arch-length space and alveolar bone height.

Evaluation of surface finish and polish of eight provisional restorative materials using acrylic bur and abrasive disk with and without pumice.

This study evaluated the effects of two finishing techniques and pumice polishing on the surface roughness of eight different provisional materials. Provisional materials included polymethylmethacrylate-based Alike, Snap, Trim and Jetand composite-based provisional materials Temphase, Protemp 3 Garant, Luxatemp and Integrity. Baseline surface roughness was measured by a profilometer, then the provisional materials were finished using extra fine acrylic burs or medium abrasive disks. The surface roughness of each sample was measured following finishing using a profilometer as previously stated. Each surface was then polished with pumice and the surface roughness was measured again. The data were analyzed using repeated measures of ANOVA and Bonferroni pairwise comparisons (alpha = 0.05). The results indicated that with composite provisional materials, the unfinished surfaces are smoother than with bur or abrasive-disk finished surfaces. Pumice application did not smooth the surface finish for all materials. The different types of provisional materials required different finishing techniques to produce the smoothest finishes.

[Single-tooth replacement in the aesthetic zone with immediate provisionalization].

OBJECTIVE: To investigate the clinical results of immediate loading of a single anterior implant. METHODS: 8 Frialit-2 implants were placed into anterior sites. Provisional prosthesis were placed at the time of fixture placement. RESULTS: No fixture has been lost since the immediate restoration was delivered, the minimum being 7 months and the maximum 14 months. CONCLUSION: Clinical studies on the placement of immediate restoration in anterior implant have revealed predictable results.

Prototype provisional restorations for implant-supported, full-mouth, metal-ceramic reconstructions: the hybrid technique.

The restoration of fully edentulous patients using fixed, metal-ceramic prostheses is one of the most challenging tasks in aesthetic dentistry. Provisional restorations provide the treatment team with a valuable means of addressing criteria that are required throughout the surgical, prosthetic, and technical stages. They also enable the clinician to place and evaluate a prototype of the definitive restoration in the patient's intraoral environment. This presentation describes a technique for the fabrication of provisional restorations for patients receiving fixed implant prostheses.

Tooth buildup prior to endodontic treatment.

Inadequate isolation will greatly compromise routine endodontic therapy. Loss of coronal tooth structure to caries or trauma often makes adequate rubber dam isolation difficult to achieve. This article presents some techniques for placing a buildup prior to endodontic therapy. The few minutes spent placing a buildup prior to the initiation of root canal therapy are an excellent investment in time. The time spent will be repaid by the ease of placement of the rubber dam, the quality of isolation achieved, the reduced chance of losing the provisional restoration, and the increased quality of the completed endodontic treatment.

Fabrication of a fixed provisional restoration utilizing a light-curing acrylic resin.

The properly fabricated provisional restoration must take into account the following interrelated factors: pulpal protection, periodontal health, occlusion, esthetics, and phonetics. The techniques advocated for construction of a provisional restoration include direct, indirect, and a combination of direct and indirect techniques. Two simple techniques that use light-curing acrylic resin for the fabrication of fixed provisional restorations are presented, and advantages and disadvantages of these techniques are discussed.

Immediate implant placement and provisionalization using implants with an internal connection.

Single-tooth implant restoration has evolved from traditional two-stage surgical procedures to techniques that allow implant placement and provisionalization immediately following tooth extraction. Restorative success using an immediately provisionalized implant system is based upon adherence to several clinical and biological parameters. This article will discuss the clinical and restorative considerations that must be addressed when immediately loading single-tooth implants. Four cases are presented demonstrating the clinical versatility of an immediate implant system for predictable and aesthetic restoration of the posterior region.

Contemporary implant concepts in aesthetic dentistry--Part 2: Immediate single-tooth implants.

Immediate implant placement and provisionalization in the aesthetic region has demonstrated predictable long-term results. Use of the immediate loading protocol significantly reduces the need for a second surgical intervention following implant placement. Based on the patient's preoperative condition, this technique can also be incorporated in select cases without the need for complicated bone grafting and augmentation procedures in compromised ridges. This article discusses the placement of single-tooth implants in the anterior region and presents the advantages and limitations of the immediate placement protocol.

A comparison of two methods for removing zinc oxide-eugenol provisional cement.

Two methods of removing zinc oxide-eugenol (ZOE) luting cement from amalgam were compared by determining their effect on the tensile bond strength of amalgam cylinders luted together using a 4-META resin cement. The samples were examined using scanning electron microscopy and energy dispersive x-ray analysis. Results showed that mechanical cleansing with pumice leaves ZOE residue mixed with pumice, which inhibits bonding, while the bond strength of samples cleansed with a dental degreasing agent was similar to that of an uncontaminated control group.
