Limb salvage procedure for neoplasms about the knee by spherocentric total knee arthroplasty and autogenous autoclaved bone grafting.

Spherocentric total joint arthroplasty was performed for treatment of neoplasm in 15 patients. The average follow-up period was 30 months. In nine of the patients concomitant autoclaved autologous bone implants were also prepared from the resected specimens. The results were satisfactory in ten patients and unsatisfactory in five. One patient had an infection that required amputation. One patient with osteosarcoma had a recurrence at 20 months and was treated by amputation. Poor results were caused by stem failure in two patients and by component loosening in one patient. Total joint arthroplasty combined with autoclaved bone implantation is a useful limb salvage procedure for resected neoplasms about the knee.

[Computed tomography of chondrogenic tumors].

The computed tomography (CT) morphology of chondrogenic tumors and the utility of CT in their diagnostic work-up is presented on the basis of 19 cases. CT provided unique informations regarding definition of tumor extent and tumor relationship to adjacent structures particularly in the axial skeleton. CT has diminished the indications for angiography in chondrogenic tumors.

[Alveolar soft-part sarcomas in the skeleton (author's transl)].

The rare alveolar soft-part sarcomas, tumours with well defined histological pattern but uncertain histogenesis, are up to now described only in soft tissues, especially in those of the extremities of adolescents and young adults. We can report two cases of alveolar soft-part sarcomas in the sceleton, both in the right femur of 22 years old women. The delimitation of these tumours against others with similar histological pattern may be very difficult. The occurrence of these tumours in the sceleton does not require to substitute the term alveolar soft-part sarcoma by an other one.

[Morphological characteristics of mesenchymal chondrosarcoma].

Complex morphological examination of 11 cases of mesenchymal chondrosarcoma is performed. This tumour has sufficiently characteristic clinical and morphological features distinguishing it from a usual chondrosarcoma. Clinically, mesenchymal chondrosarcoma is characterised by a shorter anamnesis of the disease, a rapid course and shorter life-span. It occurs more frequently during the 2nd and 3rd decades of life. Histologically, apart from the structures characteristic of typical chondrosarcoma, poorly differentiated areas resembling hemangiopericytoma occur and sometimes predominate. Electron microscopic examination of the differentiated parts of mesenchymal chondrosarcoma revealed the presence of cells with chondroid direction of the structurally-functional differentiation while in the areas with pericytal structures there were found structurally and functionally undifferentiated cells.

Giant-cell tumor of bone in skeletally immature patients.

Giant-cell tumor of bone is predominantly an affliction of adults and is rarely found in skeletally immature patients. We are reporting six new cases of histologically confirmed giant-cell tumor of bone in patients with radiographically open epiphyseal plates. These patients account for 1.8 per cent of the 326 giant-cell tumor of bone observed at the Bone Tumor Center of the Istituto Ortopedico Rizzoli in Bologna, Italy.

[Serological analysis of melanoma patients receiving autologous melanoma cell vaccines].

By autologous serological typing we have defined 3 classes of cell surface antigens of cultured malignant melanoma, Class 1--unique (presents only on autologous melanoma cells); Class 2-a--shared tumor-restricted, Class 2-b--differentiation antigen of malignant melanoma; Class 3--widely distributed and not restricted to tumor cells. With this background, we studied whether humoral immune reactions to surface antigens of melanoma cells can be developed by immunization with autologous cultured melanoma cells. Vaccines were prepared by irradiating cultured melanoma cells with 10,000 rads from a Cobalt-60 source. An appropriate number of viable units of BCG were added. This mixture was injected intradermally at 3 week intervals. As the serological procedures, protein A assay (PA), immune adherence assay (IA), and anti-C3 mixed hemadsorption assay (C3-MHA) were performed. Absorption tests were also performed to define the specificity of cell surface antigens by using various kinds of cells. Ten melanoma patients who were vaccinated 5 or more times were analyzed by serological tests. Six patients showed an increased titer of antibodies to autologous melanoma cell antigens after vaccination. In two of these patients (AT, BD), the antibodies had specificity for shared melanoma antigens. In four patients (AV, BG, BT, CH), the antibodies were directed solely against antigens related to the fetal calf serum used in growing cells for tissue culture. Other four patients (AQ, BC, BW, CL) did not show any reactivity or showed low reactivity with Class 3.

[Morphological characteristics of osteogenic sarcoma after chemo-and radiotherapy].

Adriamycin was administered intraarterially and the tumor was irradiated at a total focal dose of 3600 rad in the preoperational period in 25 children suffering from osteogenic sarcoma. Histological examinations of the tumors revealed marked necrobiotic, necrotic and sclerotic changes. Within 2 months after the beginning of the therapy the number of tumor cells decreased, they were localized predominantly in the peripheral parts of the extraosseal component. Later in the same areas new generations of tumor cells were found. In 3 cases tumor cells were completely absent. The results suggest the efficacy of adriamycin in combination with radiation therapy in treatment of osteogenic sarcoma.

Autogenous cortical bone grafts in the reconstruction of segmental skeletal defects.

The results of using segmental cortical autogenous bone grafts to reconstruct defects created by resection of tumors were analyzed in forty patients. Thirty-three patients had dual grafts while seven had a single fibular graft. Dual grafts were used for major bones (humerus, femur, and tibia without fibula) while single grafts were used for the radius and for the tibia when the ipsilateral fibula was intact. Thirty patients had good or excellent results; seven, fair; and three, poor results. In twenty-five patients primary union was achieved within tweleve months and in two, in twenty months, while twelve patients required a second, supplementary cancellous graft at the site of non-union to obtain stability. One patient required removal of an infected graft and had a poor result. Stress fractures of the grafts occurred in eighteen of the forty patients after union had occurred. The stress fractures healed in fifteen of these patients: in six with no treatment (the fracture was identified retrospectively), in seven with external immobilization, and in two after bone-grafting of the ununited fracture. There were three persistent non-unions of stress fractures despite bone-grafting, internal fixation, and electrical stimulation, and these account for two of the three poor results. The length of the defect did not affect the incidence of non-union but it did affect the number of fatigue fractures. The shorter grafts (7.5 to twelve centimeters) were associated with a 33 per cent incidence of non-union (four non-unions of twelve grafts) while the longer grafts (twelve to twenty-five centimeters) had a 32 per cent rate of non-union (nine non-unions of twenty-eight grafts). The incidence of fatigue fractures in the longer grafts (58 per cent) was much greater than that in the shorter grafts (17 per cent). The grafts decreased in density during the first six months but gradually regained their mass and were generally comparable to normal cortical bone at two years. As the patients became functiona, most (55 per cent) of the the grafts became more dense than normal, some (34 per cent) remained the same, and a few (11 per cent) became less dense. Similarly, some (32 per cent) hypertrophied, most (58 per cent) remained the same size, and a few (9 per cent) atrophied. There was little morbidity (three of forty patients) associated with graft procurement. In twelve patients an additional graft was implanted experimentally, labeled with tetracycline, and subsequently removed at the time of a secondary procedure. These grafts were analyzed to determine if human grafts were repaired in the same fashion as grafts in experimental animals. The studies showed that human grafts are repaired in the same fashion, but that the sequence takes approximately twice as long as it does in the dog.

[Clinicopathological study of twenty-two cases of benign chondroblastoma (author's transl)].

Benign chondroblastoma, a rare bone tumor clinically and histologically resembles giant cell tumor. We have experienced twenty-two cases of histologically confirmed benign chondroblastoma over the 25 year period from 1952 to 1978. The age of the patients ranged from 10 to 46 years, with an average of 20 years. Fifteen cases were in the second decade. Males were predominant to females with a ratio of 2.7:1. Location of the tumor varied widely. The tumor was located in the long bones in 18 out of 22 and limited to the epiphysis in 15. Detailed histological examination in twelve cases with hematoxylin-eosin, alucian blue, toluidine blue, silver impregnation and safranin-O staining, revealed positive staining with alucian blue and PAS. PAS positive granules wee found within cytoplasm and identified as glycogen granules by electron microscopy. However, metachromasia was not always positive and none of these cases stained positive with safranin-O. Rich reticulin fibers encircled each tumor cell and each group of cells. Electronmicroscopic study of 4 cases revealed two types of giant cells. The cell membrane had many microvilli-like protrusions, and the nuclei of the tumor cells were irregular in appearance and had large indentations. Sometimes many fine filaments and electron dense fine granules were observed in intercellular chondroid space. Morphological study of cultured cells was done in two cases. Morphology of these tumor cells proved to be different from fibroblasts, cultured chondrocytes and macrophages. Tumor cells moved rapidly and had undulating membranes at the pseudopodia. From these findings, we speculated that the origin of the benign chondroblastoma cell may be histiocytic cells. Surgical treatment of this tumor was carried out as follows. Curettage was performed in 16 cases, with bone graft in 15, resection 4 and amputation in two. Recurrence was noted on two cases, although no further recurrence occurred after reoperations for nine and twelve years respectively.

Impending and actual pathological fractures in patients with bone metastases of the long bones. A retrospective study of 233 surgically treated fractures.

OBJECTIVE: Analysis of short-term and long-term complications after cemented osteosynthesis for pathological fractures. DESIGN: Retrospective study. SETTING: South Municipal Hospital and the Daniël den Hoed Cancer Centre, Rotterdam, The Netherlands. SUBJECTS: 199 patients consecutive surgically treated between 1978 to 1990 for 233 fractures (161 actual and 72 impending) caused by metastatic lesions of the femur, humerus and tibia. INTERVENTIONS: Local resection of the tumour was followed by endoprostheses (n = 52) and by internal plate osteosynthesis (n = 167); 14 fractures were treated with intramedullary nails. Bone cement was added in 211 cases (91%). MAIN OUTCOME MEASURES: Pain relief, mobilisation, complications. RESULTS: Pain relief was achieved in about 90%. 145 (76%) who were treated for fractures of the lower extremity were able to walk again. There were 13 local complications: 26 (11%) implanted devices failed (cumulative probability 40%, after 60 months). In 11 cases the fixation failed after 7 weeks. The failure rate was 16% in the subtrochanteric region treated with an angled blade (probability 70% after four years). The patients' survival rate was 55% after six months and 20% at two years. CONCLUSION: Despite the poor life expectancy, our results indicate that hemiarthroplasty or osteosynthesis with bone cement for treatment of pathological (impending) fractures is a safe way to restore limb function and to improve quality of life.
