The free autogenous gingival graft is an excellent surgical procedure for treating gingival extension. It is a procedure that can be performed rapidly with little postoperative trauma to the patient. Its greatest feature is the high degree of predictability of success in producing a specific dimension of masticatory mucosa. However, we have a moral and ethical obligation to our patients to be more selective in its use in treating gingival recession. Too many patients have been subjected to unnecessary surgery, especially in cases in which the recession has been of long duration and is stable. In the maintenance of such cases, surgical intervention should be elective to solve problems of maintenance of the dentoginginval junction as they arise. As a result of the innovations from the 1970s, the gingival graft procedure appears to be extremely versatile. It will probably undergo further innovative developments in the future as a procedure to be extremely helpful to the restorative dentist. We have already seen its use in new attachment procedures, for improving the edentulous ridge for denture prosthesis, and in other restorative procedures.
Dental practitioners have an important role in the dissemination of information to their patients and their advice is often reinforced with written health education literature. Unfortunately, this material is often of limited value because of its level of readability. This study covered two aspects of the problem: first, a group of 140 secondary school children were asked to explain the meaning of a selection of words commonly used in dental education and second, the readability of some health education literature was assessed and compared with that of texts in magazines and newspapers. Many of the words were not understood by the children: for instance, over 80% were unsure of the meaning of fluoride tablets, gum disease, gingivitis and oral hygiene. As for readability, the test showed that the material reflects the reading capacity of the designers rather than the ability of the potential target group. These findings pinpoint the need to ensure that dental health education literature is not unduly complex and, for dentists, to take more time explaining the dental vocabulary in lay terminology. The methods used in the study can easily be extrapolated to other areas of health.
The granuloma of pregnancy (angiogranuloma) has been studied by light-and electron microscopy. It has been revealed that granuloma of pregnancy consists chiefly of capillaries and undifferentiated connective tissue. It is a hyperplastic proliferative growth developing on the hormonal basis. In the case reported under the basement membrane presence of an electron lucent layer of fibrillar structure has been observed. Authors suppose that this phenomenon later will successfully be used in morphological differential diagnostics.
