Second intention healing after Mohs micrographic surgery.

BACKGROUND: Although second intention healing has been used for many years in Mohs micrographic surgery, it has lost popularity. This is due to the long process and functional alteration of important anatomical units if certain wound healing factors are not carefully taken into consideration. OBJECTIVE: To review indications, contraindications, complications, advantages, disadvantages, as well as basic concepts of the wound healing process, wound care and antibiotic prophylaxis in the management of open wounds after Mohs surgery. METHODS: A short but comprehensive review of studies published in the literature dealing with second intention healing as a reconstruction alternative option in certain situations. CONCLUSIONS: Second intention healing is a simple and cost-effective method for reconstruction after Mohs micrographic surgery in well-defined cases. It allows adequate tumoral control with good to excellent functional and cosmetic results.

Intention to breastfeed in low-income pregnant women: the role of social support and previous experience.

BACKGROUND: The purpose of this study was to describe the relationship between breastfeeding intention among socioeconomically disadvantaged pregnant women and maternal demographics, previous breastfeeding experience, and social support. METHODS: A cross-sectional, convenience sampling strategy was employed for data collection. Low-income women (n = 1001) in a public hospital completed a six-page questionnaire about their infant feeding plans, demographics, and social support. Simple regression analyses were conducted to compare maternal breastfeeding intention with the hypothesized correlates. RESULTS: Breastfeeding intention was positively correlated with older maternal age, higher education, more breastfeeding experience, Hispanic ethnicity, and hearing about breastfeeding benefits from family members, the baby's father, and lactation consultants, but not from other health professionals. Health professionals' attitudes were less influential on women's infant feeding decisions than the attitudes and beliefs of members of women's social support networks. When controlling for breastfeeding experience (none vs any), some findings, varied, indicating a need for breastfeeding interventions tailored to women's level of experience. CONCLUSION: Use of peer counselors and lactation consultants, inclusion of a woman's family members in breastfeeding educational contacts, and creation of breastfeeding classes tailored to influential members of women's social support networks may improve breastfeeding rates among low-income women, especially those with no breastfeeding experience, more effectively than breastfeeding education to pregnant women that is solely conducted by health professionals.

Attitudes, subjective norm, perceived behavioral control, and intentions related to adult smoking cessation after coronary artery bypass graft surgery.

The purpose of this study was to examine the relationships between intention, attitudes, subjective norm, and perceived behavioral control related to smoking cessation in adults after initial coronary artery bypass graft surgery (CABG). The theoretical framework for the study was derived from Ajzen's Theory of Planned Behavior. Intention, the global and belief-based measures of attitude, subjective norm, and perceived behavioral control were measured with the Determinants of Adult Smoking Cessation (DOASC) Questionnaire developed by the investigator. Thirty-two adult smokers completed the questionnaire 2 to 3 weeks following hospital discharge. Four weeks after the questionnaire completion, a follow-up telephone call was used to determine the participants' current smoking status. The study results indicated that there was a statistically significant relationship between the intention to quit smoking after CABG and the global measure of attitude, and perceived behavioral control. This study highlights some of the beliefs about the outcomes of quitting smoking permanently after CABG which may underlie attitudes, subjective norm, and perceived behavioral control in this population. Implications for theory, practice, and research are discussed.

Biological processes, quantum mechanics and electromagnetic fields: the possibility of device-encapsulated human intention in medical therapies.

The general hypothesis that quantum mechanics (QM) and thermodynamic concepts relate to biological systems is discussed and applied to the biological influence of: (1) electromagnetic fields (EMFs); and (2) EMFs that have been exposed to human intention. We illustrate our hypothesis with experiments involving four simultaneous treatments: exposure to ambient EMFs in the laboratory environment (C), exposure in a Faraday cage (F) and exposure in a Faraday cage with either: (i) an electronic device (IIED) which had been exposed to a specific human intention (d,j); or (ii) a non-exposed, physically identical, device (d,o). Experimental systems were fitness and energy metabolism in Drosophila melanogaster, in vitro enzyme activity and molecular concentration variability over time. Results indicated that shielding from ambient EMFs via a Faraday cage (F) made a significant difference relative to the unshielded control (C). Further, (d,o) had a significant lowering effect in the shielded environment. Finally, there was a strong 'intention' effect with the IIED (d,j) producing significant and positive effects in comparison to (d,o) in each experimental system.

[Intention-to-treat].

What is meant by intention-to-treat? Why should data be analyzed in controlled trials in a way that all participants are included in the group to which they were randomly assigned, regardless of whether they completed the intervention given to the group? In this Tutorial, the logic of the intention-to-treat principle is outlined. It is shown that study results may be biased by excluding patients post hoc thus producing spurious effects that do not exist in the population under study. The intention-to-treat strategy avoids this bias.

Intentions to use an automated external defibrillator during a cardiac emergency among a group of seniors trained in its operation.

OBJECTIVE: By decreasing the time to defibrillation, automated external defibrillators (AEDs) provide an opportunity for lay people to improve survival in out-of-hospital sudden cardiac arrest. We examined how beliefs, expectations, and actual performance are related to intentions to use an AED during a future heart emergency among a group of seniors. DESIGN AND OUTCOME MEASURES: One hundred fifty-nine seniors who had been previously trained in the operation of an AED were tested on their AED skills and asked about their perceptions regarding their AED skills; their expectations that an AED would save the life of a cardiac arrest victim; and their intentions to use an AED during a future cardiac event. RESULTS: Logistic regression analyses showed that greater self-perceived ability to use an AED better actual performance on skills assessment but not expectations regarding the efficacy of AED treatment were independently associated with positive intentions to use an AED in a future heart emergency. CONCLUSIONS: The likelihood that an elderly lay bystander will actually use an AED during a cardiac event may be closely tied to perceptions of his or her ability to operate an AED.

The use of second-intention healing for partial-thickness Mohs defects involving the vermilion and/or mucosal surfaces of the lip.

BACKGROUND: Partial-thickness defects involving the vermilion and mucosa of the lip are difficult to repair surgically. OBJECTIVE: The purpose of this study was to evaluate the cosmetic and functional effectiveness of second-intention healing in the repair of partial-thickness defects confined predominantly to the vermilion and mucosa of the lip. METHODS: Thirteen patients with squamous cell carcinoma of the lip underwent Mohs micrographic surgery, which resulted in partial-thickness defects predominantly confined to the vermilion and/or mucosa of the lip. All defects were allowed to heal by secondary intention. RESULTS: Twelve patients had good to excellent cosmetic results, and functional impairment was not noted in any of the patients. One patient whose defect extended to the cutaneous portion of the lip had deformation of the vermilion border. CONCLUSIONS: Second-intention healing yields good to excellent cosmetic and functional results for the repair of partial-thickness defects involving the vermilion and/or mucosa of the lip. Defects involving more than 2 mm of the cutaneous lip or defects extending deeper than the superficial portion of the orbicularis oris muscle may result in cosmetic or functional deformity or both if allowed to heal secondarily.

Eye movements during intentional car following.

Does intentional car following capture visual attention to the extent that driving may be impaired? We tested fifteen participants on a rudimentary driving simulator. Participants were either instructed to follow a vehicle ahead through a simulated version of London, or were given verbal instructions on where to turn during the route. The presence or absence of pedestrians, and the simulated time of the drive (day or night) were varied across the trials. Eye movements were recorded along with behavioural measures including give-way violations, give-way accidents, and kerb impacts. The results revealed that intentional car following reduced the spread of search and increased fixation durations, with a dramatic increase in the time spent processing the vehicle ahead (controlled for exposure). The effects were most pronounced during nighttime drives. During the car-following trials participants were also less aware of pedestrians, produced more give-way violations, and were involved in more give-way accidents. The results draw attention to the problems encountered during car following, and we relate this to the cognitive demands placed on drivers, especially police drivers who often engage in intentional car following and pursuits.

Dynamical basis of intentions and expectations in a simple neuronal network.

Selection of behavioral responses to external stimuli is strongly influenced by internal states, such as intentions and expectations. These internal states are often attributed to higher-order brain functions. Yet here we show that even in the simple feeding network of Aplysia, external stimuli do not directly specify which motor output is expressed; instead, the motor output is specified by the state of the network at the moment of stimulation. The history-dependence of this network state manifests itself in the same way as do intentions and expectations in the behavior of higher animals. Remarkably, we find that activity-dependent plasticity of a synapse within the network itself, rather than some higher-order network, mediates one important aspect of the change in the network state. Through this mechanism, changes in the network state become an automatic consequence of the generation of behavior. Altogether, our findings suggest that intentions and expectations may emerge within behavior-generating networks themselves from the plasticity of the very processes that generate the behavior.

Induced arousal and orienting tasks as determinants of intentional recall.

In this study, the author examined the effects of intention to learn, noise, and different types of orienting tasks on short-term and long-term recall for 15 Hindi paired-associates. Intention to learn improved recall, as did encoding of semantic features, and noise impaired both short-term and long-term recall. The analysis of short-term recall scores indicated that there were no significant interactions between these factors, but the analysis of long-term recall scores indicated that there were significant interactions between noise and orienting tasks and between intentionality and orienting tasks.

Neuromotor synergies as a basis for coordinated intentional action.

Although neurally based units of action (neuromotor synergies) have often been proposed as a possible basis for coordinated intentional as well as automatic actions, the idea has rarely been translated into sets of testable hypotheses. This essay examines four issues which should facilitate the development of such hypotheses: (a) definitions of neuromotor synergies, (b) criteria for recognizing and comparing synergies in automatic and intentional actions, (c) problems in representing systems of synergies, and (d) models for generating intentional actions from sets of neuromotor synergies. Limitations of, and support for the neuromotor synergy hypothesis are discussed, both in general and for the specific cases of postural synergies and cervico-spinal reflexes. Although current data do not provide conclusive support for or against the neuromotor synergy hypothesis, the problem can be formulated in ways open to experimental investigation.

The relationship between differential inequity, job satisfaction, intention to turnover, and self-esteem.

This study was an examination of differential inequity or underreward in working conditions, originating from the discrepancy between individual working conditions and those of comparative referents. In its exploration of the outcomes of inequity in working conditions, the study fills a gap in the literature because most such studies have been primarily devoted to investigations of pay inequity. Empirically, it is an investigation of elements of differential inequity as antecedents of job satisfaction and intentions to turnover and of self-esteem as a moderator of inequity-criteria relationships. Significant relationships between system and age inequity and job satisfaction and between company inequity and intention to turnover were found. Self-esteem significantly moderated the global inequity-job satisfaction and global inequity-intention to turnover relationships.

Evaluation of a rapist as a function of expressed intent and remorse.

Two studies were conducted to examine the effects of expressed versus denied intent and remorse on evaluation of a rapist. In both studies, the rapist was evaluated more negatively and was assigned a longer prison sentence when expressing rather than denying intent. The rapist received more favorable evaluations when expressing rather than denying remorse. Manipulation of the rapist's expressed remorse did not significantly affect the recommended prison sentence. However, the recommended prison sentence did correlate significantly with participants' perceptions of the rapist's remorse. A multiple regression analysis indicated that recommended prison sentence could be best predicted by attributions of cause, intent, and remorse. The results were related to a growing body of research on accounting strategies, responsibility, and excuse making.

Shortage specialties: changes in career intentions from medical student to newly qualified doctor.

There is currently a shortage of doctors in Britain, especially in general practice, psychiatry and pathology. This study aimed to examine whether career intention, especially in the shortage specialties, changes between undergraduate level and graduation. The 234 participants were Senior House Officers and Pre-Registration House Officers, who had provided information about their career intentions in their fourth year of medical school. Participants completed a postal questionnaire about their current career intentions. 38.9% of medical students rising to 63.3% of doctors definitely intended to pursue a particular specialty. While the numbers of people who definitely wanted to pursue general practice and psychiatry increased, in line with nearly all other specialties, the overall attractiveness of these shortage specialties fell. Early career advice and support during medical school and immediately after graduation may help doctors to be confident in pursuing shortage specialties to which they were originally attracted.

Sources of stress, burnout and intention to terminate among rugby union referees.

We examined sources of stress, burnout and intention to terminate among rugby union referees. Questionnaires were returned by 682 of 1175 referees (58%) from Wales, Scotland and England. Exploratory and confirmatory factor analyses revealed four correlated sources of stress factors (performance concerns, time pressure, interpersonal conflict and fear of physical harm), which accounted for 49.6% of the variance. Referees believed the first three factors to be mildly related to their stress. They believed fear of physical harm to be unrelated to their stress. A structural model hypothesizing that time pressure and interpersonal conflict predict burnout, and that age and burnout predict intention to terminate refereeing, fit the observed data. The goodness-of-fit index for this model was 0.96. Our results and those of other studies indicate that interpersonal conflict and time pressure are common sources of stress for sport officials and are frequently related to their burnout experiences. Also, age and burnout are frequently, although only marginally, related to intention to terminate.

Smoking, weight loss intention and obesity-promoting behaviors in college students.

OBJECTIVE: To examine whether college smoking was associated with trying to lose weight and other weight-related behaviors. METHODS: We surveyed 300 students at the University of Kansas about smoking (ever, current, and amount), weight loss intention (y/n), weight-related attitudes, and eating and exercise behavior. Weight, height, and body fat were measured. RESULTS: About half the students (49%) self-identified as having ever smoked while 53 (17.6%) self-identified as current smokers. After controlling for sex, age, and ethnicity, ever smoking was not related to weight loss intention but was associated with greater pressure to maintain a healthy weight (p = 0.05), and having engaged in mild exercise on more days in the previous year (p = 0.05). Compared to nonsmokers, current smokers ate more at restaurants serving high calorie foods (p < 0.05) and ate more frequently in front of the TV (p < 0.01). Amount smoked was related to diminished use of exercise facilities (p = 0.03) and more frequent eating at restaurants serving high calorie foods (p < 0.05) and in front of the TV (p = 0.01). CONCLUSIONS: Current smoking among college students was related to weight loss intention. Despite wanting to lose weight, current smoking was concomitant with obesity-promoting behaviors such as eating higher calorie foods and eating in front of the TV. College-based interventions to prevent smoking initiation or promote smoking cessation should include a focus on healthy eating, exercise and healthful ways to lose or maintain weight.

Genetic screening for Alzheimer's disease: what factors predict intentions to take a test?

The authors investigated factors that predict intention to take a genetic test for Alzheimer's disease (AD). The 449 men and women were surveyed in two groups: (a) those told that a positive result meant a 90% chance of developing AD (increased certainty) and (b) those told that a positive result meant a 50% chance of developing AD (decreased certainty). Participants completed measures of the Theory of Planned Behavior (TPB), anticipated regret, risk perception, likelihood of taking a genetic test for cancer, and AD risk factors. Just over 50% of the variance in intentions was related to TPB variables, likelihood of taking a genetic test for cancer, number of people the participants knew who had AD, experimental condition, and anticipated regret. The subjective norm was the strongest predictor of intention in the increased certainty group, whereas positive belief was the strongest predictor in the decreased certainty group.

Crack smokers' intention to use condoms with loved partners: intervention development using the theory of reasoned action, condom beliefs, and processes of change.

Prevalence rates of HIV infection acquired through heterosexual contacts have risen steadily since 1982. Crack cocaine smokers are at particular risk of HIV infection due to heterosexual exposure. HIV risk reduction interventions seeking to increase condom use among drug users have met with minimal success, and there is a need for interventions to be strongly grounded in psychosocial models of behaviour change. This study presents the results of an investigation of predictors of intention to use condoms and related therapy processes among heterosexual drug users. Data were analyzed from 586 crack smokers recruited in Washington, DC, Miami, Florida, and Collier County, Florida who reported having both primary and casual sex partners. Participants responded to items derived from the theory of reasoned action, the theory of planned behaviour and the transtheoretical model of change. Condom use beliefs and therapy processes used to initiate and maintain condom use were assessed. Outcome expectancies and normative beliefs were the strongest predictors of intention to use condoms with a primary sexual partner. In turn, beliefs that condoms inhibit sexual romance and decrease sexual pleasure strongly predicted outcome expectancies. Therapy processes found to be associated with these constructs included: self-liberation, counter conditioning and stimulus control/reinforcement. Results suggest that HIV risk reduction interventions using a group format and targeting condom beliefs related to sexual romance and pleasure will decrease negative outcome expectancies about condom use. Also, reinforcing attempts to use condoms with intimate partners should increase positive outcome expectancies and intention to initiate or maintain condoms with a primary sexual partner.
