Caries experience, periodontal treatment needs, salivary pH, and Streptococcus mutans counts in a preadolescent Down syndrome population.

The prevalence of dental caries and periodontal treatment needs in an institutionalized population with Down syndrome was examined and the relationship between caries prevalence, salivary pH, and salivary levels of Streptococcus mutans was studied. Thirty-two children with Down syndrome, aged 8 to 13, were compared with two control groups of similar age ranges: healthy children and non-Down mentally retarded (MR) children living in the same institution as the Down syndrome population. The gender ratio mixtures in each group also matched the study sample. Caries experience as indicated by decayed, missing, and filled surfaces (DMF-S) showed significantly lower mean scores for the Down syndrome group compared with both control groups. A similar pattern was found when evaluating the mean numbers of decayed surfaces. Streptococcus mutans counts, expressed as number of colony-forming units on mitis salivarius agar plates among the Down syndrome group, were the lowest, although not statistically significant compared with the counts of the healthy children. Both groups had bacterial counts which were significantly lower than those of the MR group. Significant differences between the two institutionalized groups and the healthy group were recorded for the salivary pH levels. The periodontal treatment needs as evaluated by the Community Periodontal Index of Treatment Needs showed significant difference between the MR group versus both the healthy population and the Down syndrome group. On the whole, 84% of the Down syndrome children were cariesfree.

Effects of a memory aid of the three types of conservation in institutionalized retarded children.

Qualitative identity, quantitative identity, and equivalence conservation were assessed in 60 retarded boys and girls in three groups (MAS 5.4,6.3, and 7.5)9 Half of the Ss were provided a memory aid while the other half were not. The memory aid did not facilitate conservation on any of the tasks. The order of task difficulty found was qualitative identity, quantitative identity, and equivalence conservation. These data were discussed in terms of memory deficits and factors contributing to conservation and M.A.

Epilepsy and abnormal calcium metabolism in institutionalized mentally retarded patients.

A group of epileptic and nonepileptic patients from an institution for mentally retarded persons was surveyed for roentgenographic and biochemical abnormalities of calcium metabolism. Osteoporosis, hypocalcemia, and increased alkaline phosphatase were found to be significantly increased in incidence in the epileptic group. The only significant difference among the osteoporotic and nonosteoportic epileptic patients was the increase fo the alkaline phosphatase in the former. Ambulation did not appear to influence the significance of the above parameters.

Secondary enuresis in institutionalized conduct-disordered children.

The incidence of secondary enuresis in 16 institutionalized conduct-disordered children was examined. In contrast to a control group of 16 noninstitutionalized conduct-disordered children, the former showed significantly more enuretic behaviour. Also, enuresis began within 6 mo. of the children's separation from their families, suggesting this problem is very likely related to the emotional trauma surrounding the change in living conditions. However, the small number of subjects limits further interpretation of the findings.

Sex chromatin and cytogenetic survey of 10417 adult males and 357 children institutionalized in Belgian institutions for mentally retarded patients.

A survey of adult male immates of the major institutions for mental patients in Belgium was undertaken in 1965. Out of 10417 males examined 61 had positive sex chromatin (5.8%); 201 had bilateral small testis (1.9%), 4 hypogonad patients carried a D/D translocation. Out of 857 karyotyped children, 2.9% carried an autosomal or sex-chromosomal anomaly other than trisomy G. This prevalence of chromosomal anomalies in children with low I.Q. (less than 50) emphasizes the need for further studies.

Using television to improve the social behavior of institutionalized children.

This field experiment assessed the effects of a "TV diet" composed of ten prosocial programs on the social behavior of eight-to-eighteen year old behaviorally disordered youngsters who were institutionalized in a state psychiatric facility. The influence of a post-viewing group discussion which highlighted the televised positive messages was also examined. The prosocial diet resulted in the youngsters behaving more altruistically, less verbally aggressive, and less destructively relative to those exposed to the control diet containing programs the youngsters typically viewed. The post-viewing discussion reduced the effectiveness of the prosocial programming but mitigated some of the negative effects of the control diet.

Periodontal status and treatment needs of 12-15 year old institutionalized mentally handicapped school children in Ibadan, Nigeria.

Mentally handicapped children are known, to have poor periodontal health. But previous studies lack conformity due to the use of different indices by the different authors. Twenty-five institutionalized mentally handicapped school children in Ibadan, Nigeria were studied with the aim of assessing their periodontal health and per treatment needs using the CPITN which is known to be simple and has international uniformity. The result showed that none of the children had healthy periodontium and all the children will need oral hygiene care with a significant percentage (8%) requiring complex periodontal care. The need for comprehensive preventive dental program is discussed since the hallmark of management of the children is prevention.

Resident-care practices in institutions for retarded persons: a cross-institutional, cross-cultural study.

Institution-oriented and resident-oriented care practices for institutionalized retarded persons investigated in 166 Living units in 19 institutions in the United States and 11 institutions in a Scandinavian country. Living units in the Scandinavian country were found to be more resident-oriented than those in the United States. Large central institutions were characterized by the most institution-oriented care practices, group homes by the most resident-oriented practices, with large and small regional centers falling between these extremes. Within types of institution, care practices were generally homogeneous. Living units for more severely retarded residents were characterized by more institution-oriented practices. Large living-unit size was found to be predictive of institution-oriented practices while cost/resident/day, number of aides/resident, and number of professional staff/resident did not predict care practices. Characteristics of the Child Management Inventory were also examined.

Salivary lactobacilli and yeasts in relation to caries increment. Annually repeated measurements versus a single determination.

The purpose was to compare annually repeated measurements with a single determination of salivary lactobacilli and yeasts in distinguishing subjects with high and low caries increments. The determinations were carried out using Dentocult and Oricult chair-side methods in connection with a 3-year oral health preventive program. The subjects were institutionalized Hungarian children (n = 257), ranging from six to eleven years of age. The data were analyzed by comparing the age-adjusted means of caries increment in subjects with positive and negative microbiological findings. In comparison with a single determination, two or three annual measurements of the tests proved to increase the ability to distinguish subjects with respect to the 3-year caries increment. The analysis of the repeated measurements providing longitudinal information of these microorganisms probably included the time factor required for development of caries.

Effects of combining methylphenidate and a classroom token system in modifying hyperactive behavior.

The combined effects of methylphenidate and a token reinforcement program in controlling a classroom behavior of 16 hyperactive, institutionalized retarded youngsters was investigated. A within-subject, placebo-controlled, double-blind design was employed. The results indicated that the behavior-modification procedures in the presence of placebo medication produced significant increases in work-oriented and related decreased in disruptive behavior. Few effects from the addition of active medication to the treatment program were found on any of the dependent measures. The findings suggest behavior modification to be a viable alternative to drug therapy for hyperactivity in retarded persons.

Evolution of social services for children with emotional disorders.

In this article, the authors examine the evolution of America's varied and often conflicting responses to the needs of its children--more specifically, the historical responses to dependent, neglected, and emotionally disordered children. The authors discuss America's historical ambivalence about whether to institutionalize children or keep them with their families, the relationship between social work professionals and clients, and the lessons that can be learned and applied to current practice. The analysis suggests that social work challenge some of the assumptions upon which prior service and advocacy efforts have been based. The magnitude of the current service system problems warrants a family-centered advocacy stance aimed at improving community-based services. This approach could free social workers to operationalize key values of the profession.

Social and play behaviors of institutionalized mongoloid and nonmongoloid retarded children.

A total of 24 institutionalized retarded children, 12 mongoloid (Down's syndrome) and 12 nonmongoloid, were observed in dyadic interaction with peers in a free-play situation, and a number of specific peer-social and non-social behaviors were recorded as they occurred. Differences between mongoloid and nonmongoloid Ss were most apparent on several social behavior categories which support the stereotypic conception of mongoloids as more sociable, gregarious, etc. Differences were most apparent for the mongoloid boys. The possible influence of tranquilizer drugs and cottage placements on the observed differences was discussed.

Spread of hepatitis-B infections. A seroepidemiologic study.

A solid-phase radioimmunoassay (R.I.A.), allowing simultaneous detection and quantitation either of HB, Ag or of anti-HBs, was applied to check the spread of hepatitis-B virus (H.B.V.) infections in various groups of population. R.I.A. procedures were also used to validate positive reactions and to determine the subtype specificity of sera. The overall prevalence of neutralizing anti-HBs in tested sera was almost 4-fold higher, although generally at rather low titres, than prevalence of HBs Ag. Frequency of HBs Ag and anti-HBs was rather high among healthy individuals from the normal Italian population and was extremely high among individuals from East Africa (Tanzania), as well as among healthy or sub-normal institutionalized subjects, among dialyzed renal patients and unselected blood-donors from 5 Italian towns. 80.2% of HBs Ag and anti-HBs detected in Italy were ay subtype, while most of positive sera from Tanzania (72.3%) were ad, which extends the map of H.B.V. subtypes in Africa. All tested ad antigens, either from Italy and from Tanzania, carried the w secondary determinant. Low-titre neutralizing anti-HBs, predominantly bearing a and d specificity, were also found in almost all examined preparations of standard immunoglobulins, which were imported from U.S.A.

Effects of immediacy of feedback and level of aspiration statements on learning tasks for delinquent youngsters.

The performance of institutionalized delinquent youngsters on paired associate learning tasks was investigated to determine whether level of aspiration (LOA) statements were associated with improved performance under varying feedback conditions. The effects of the feedback conditions were also examined. Forty-eight male adolescents were randomly assigned to the following six treatment conditions: (1) Delayed Feedback, (2) Delayed Feedback with LOA, (3) Immediate Feedback, (4) Immediate Feedback with LOA, (5) No Feedback, (6) No Feedback with LOA. The LOA conditions produced significantly better performance than did the no-LOA conditions. Implications for curricular intervention are discussed.

Collaborative WHO xylitol field studies in Hungary. IV. Saliva levels of Streptococcus mutans.

Salivary Streptococcus mutans was quantified to detect possible age-, sex-, and group-dependent differences at the end of a 3-year field study assessing the value of partial substitution of sucrose with xylitol (X group) in comparison with systemic administration of fluoride (F group) and restorative treatment solely (C group). The material of this substudy consisted of 390 institutionalized children aged 9-14 years. S. mutans was determined through the spatula technique and grouped into categories 0, less than 10(5), 10(5)-10(6), and and greater than 10(6) CFU/ml saliva. Zero to less than 10(5) values included more than 80% of the subjects of the X group. The cross-sectional comparison between groups showed lower S. mutans values in the X group than in the F and C groups. These differences between groups were highly significant. S. mutans was not dependent on age or sex.

Effects of directive vs. nondirective play therapy with institutionalized mentally retarded children.

Institutionalized retarded children were randomly assigned to 11 sessions of directive play therapy (N = 5), nondirective play therapy (N = 5), or no treatment (N = 4). Student nurses were therapists. The Denver Developmental Screening Test was administered before and after treatment. For the Fine Motor and Personal-Social scales, a significant interaction effect (p smaller than .05) was found between treatment group and measurement period (pre vs. post). The interactions tended to support the hypothesis that play therapy was effective in increasing developmental level but not the hypothesis that directive therapy was more effective than nondirective therapy.

Note on locus of control and academic achievement in institutionalized juvenile delinquents.

The previously reported relationship between internal locus of control and academic achievement was explored with the Nowicki-Strickland Locus of Control Scale for Children (Nowicki & Strickland, 1973) and seven measures of academic achievement in a random sample of 45 male residents of seven state learning centers for juvenile delinquents. Correlations between overall locus of control scores and all seven measures were significant, but with intelligence controlled in a partial correlation, locus of control scores were found to add little to the prediction of academic achievement from IQ scores alone. Discussion addressed possible explanations for the current findings in terms of the meaning of the scale for a nonnormal group and the effects of situations on responses of subjects.

Longitudinal epidemiological survey on hepatitis B-antigen carriers among institutionalized juvenile diabetics.

An epidemiological study was carried out in a juvenile diabetics institution, in order to determine the incidence and the significance of the hepatitis associated Australia antigen (HBAg) carriers. By means of counterelectrophoresis and CF an average of 15% HBAg-positive individuals was found during the course of the search (June 1970-January 1973). As a rule the carrier state appeared to be longlasting and it was only occasionally associated with symptoms and/or signs of liver involvement. Isolation measures to inhibit spread of serum hepatitis (the hepatitis most associated with HBAg) failed partially to prevent the spread of the HBAg associated agent suggesting that nonparenteral transmission could play a role in the diffusion of the infection.
