[Comparative evaluation of efficiency of transcutaneous laser irradiation of blood and membrane-stabilizing drug therapy in correction of cell metabolism in patients with acute pyelonephritis].

In patients with acute pyelonephritis, the efficiency has been studied of the blood transcutaneous laser irradiation and of the medicamentous membrane-stabilizing therapy, with n = 31 and n = 27 respectively. In the serous stage of acute pyelonephritis both methods appeared to be efficient while in the transitional stage it is the membrane-stabilizing therapy that has been shown to be superior to transcutaneous blood laser irradiation.

[Acute pyelonephritis in children with minimal or normal urine findings].

BACKGROUND: Over the past years we have seen several children with serious bacterial infections in whom the focus of the infection was found in the kidneys despite of normal or minimal urine findings. MATERIAL AND METHODS: We review the cases of all children hospitalised at our paediatric department from 1995-2000 with normal or minimal urine findings, and in whom acute pyelonephritis was diagnosed by means of computerized tomographic imaging of the kidneys. RESULTS: All 10 children (aged 9 months to 9 years) had focal areas of decreased enhancement in the kidney parenchyma. All received intravenous antibiotic treatment. Vesicouretheral reflux was detected in four patients and required surgical intervention in one. One patient had considerable renal scarring and decreased renal function on dimercaptosuccinic acid (DMSA) scintigraphy. INTERPRETATION: In children with suspected serious bacterial infection, it is important to have the possibility of renal infection in mind in spite of normal urine findings. The importance of establishing the right diagnosis lies in the fact that patients with pyelonephritis need further investigation and follow-up.

[Measurement of kidney function using combined PAH-inulin clearance before and after bactericide treatment of chronic pyelonephritis].

On account of the solution of the problem, whether already after a four weeks' therapy with antibiotics in chronic progressing pyelonephritis a measurable increase of function may develop, the combined PAH-inulin-clearance was performed 35 times. 9 patients did not show a clear restriction of the clearance, wherefore in these cases a control examination was not necessary. In 10 patients of 12 an increase was present, in 7 patients a significant increase of the renal function existed after the therapy carried out. Therefore already after antibiotic impact therapy in patients with chronic pyelonephritis without provable acute episode an improvement of the function can occur, which is to be increased or kept by means of the long-term treatment.

[Effectiveness of sparfloxacin (sparflo) in the treatment of complicated forms of pyelonephritis and prostatitis].

A novel difluoroquinolone drug sparfloxacine has a wide antibacterial spectrum. It is active both against gram-negative and gram-positive flora. Sparfloxacine was tried in 43 patients with pyelonephritis and prostatitis complicated with urolithiasis, diabetes mellitus, nephroptosis, anomalous kidneys, etc. The drug was given according to its pharmacokinetics for 7-14 days in a daily dose 400 mg (day 1) then 200 mg/day. The effectiveness of sparfloxacine in complicated pyelonephritis and prostatitis was rather high: clinical response reached 83.7%, bacteriological one--78.7%.

[Incidence of pyelonephritis on territories with different levels of anthropogenic loading].

Relationship between atmospheric pollution with harmful substances and incidence of pyelonephritis was studied for the populations living in ecologically unfavorable and clean regions of an industrial city. Disease intensity and incidence were significantly higher in polluted regions, the probability of error being no more than p < 0.05-p < 0.001. The correlation between atmospheric nitrogen dioxide concentration and pyelonephritis incidence was the strongest for all years analyzed. Correlation coefficients tend to increase: during the analyzed period they increased from Rxy = 0.42 +/- 0.4 (p < 0.01) to Rxy = 0.64 +/- 0.15 (p < 0.01).

Severe renal hemorrhage caused by pyelonephritis in 7 horses: clinical and ultrasonographic evaluation.

Case records of 7 horses diagnosed with pyelonephritis were reviewed to determine common features that might aid in diagnosis, treatment, and prognosis of this disease. All 7 horses had been admitted for evaluation of hematuria. During cystoscopy of 5 horses, hemorrhage was observed from one or both ureters. Renal biopsy of 1 horse, laboratory analysis of ureteral discharge of 2 horses, and renal ultrasonography of all horses indicated that pyelonephritis was the cause of hemorrhage. Sonographic renal changes included decreased length, increased echogenicity, abnormal outline, loss of corticomedullary distinction, pyelectasia, and focal hypoechoic or hyperechoic cortical defects. Renal hemorrhage in all horses eventually resolved but recurred in 4 of 5 horses that were followed long-term.

[Unusual clinical forms of presentation and infrequent pathologic associations of xanthogranulomatous pyelonephritis].

OBJECTIVE: To describe the unusual clinical forms and rare associations of xanthogranulomatous pyelonephritis seen in a variable proportion of cases. METHODS: Analysis of our series of 83 cases and 2573 cases reported in the literature showed 8 different and less frequent clinical forms and up to 7 rare associations, which are discussed in this study. RESULTS: There are different clinical forms under "renal infection", ranging widely from asymptomatic to severe septic forms that affect patients of all ages--from the newborn to the elderly-, preferentially in association with conditions of immunosuppression, and other more uncommon or rare forms in which xanthogranulomatous pyelonephritis is often incidentally discovered. CONCLUSIONS: These forms are not extremely rare since more than 2500 cases have been reported in the literature. Knowledge about the different forms permits stratification of this condition into different clinical forms of presentation and provides further insight into the associated conditions, some of which are rare. Overall, such forms or associations have been reported to account for 20 to 40% of all cases, according to the studies reported elsewhere. This overall incidence is rather significant and should be taken into account.

[Bacteriostatic shock in acute purulent pyelonephritis].

Fifty-seven patients with acute purulent pyelonephritis (APP) complicated by bacteriotoxic shock (BTS) were treated in Daghestan Medical Academy's urological clinic from 1982 to 1999. The condition was primarily caused by acute occlusion of the urinary tracts. The first step of the treatment was aimed at the recovery of normal hemodynamics. As early as the first hours of BTS, plasma- and hemosorption, hemodialysis, hemodialysis were made. If the patients got better, they were operated to remove the obstruction for urine flow (epicystostomy, nephrostomy). 28 recent cases received adjuvant laser therapy--irradiation of the cavitary renal system and blood. This was performed via light guide inserted into the renal pelvis through the nephrostomic drainage. The guide was connected with the He-Ne optic quantum generator AFL-1. Laser procedure lasted for 8-12 min, a course consisted of 7-8 procedures. Later, laser beam was also directed to the biologically active points corresponding to pyelonephritis. Normalization of renal function and T, B immunity was faster in patients exposed to laser radiation. Lethality in this group was lower.

[L-forms of bacteria in the urine of patients with chronic pyelonephritis].

The authors present the data concerning the study of 200 patients suffering from chronic pyelonephritis; in 28 of these L-forms of bacteria were revealed in the urine. Of 46 L-cultures isolated from these patients 13 reversed into bacterial forms, 8 failed to reverse and were referred to the stable L-forms; the rest 25 L-cultures perished during the 8th--10th passage. This led to a supposition that the relapses and exacerbations of the infectious process in pyelonephritis were associated with the change of the L-forms into bacterial ones, and that the persistence of the L-forms in the kidney tissue promoted the maintenance of the chronic process.

[Renal stone ileus in xanthogranulomatous pyelonephritis. A case report].

A peculiar case of intestinal occlusion caused by a renal stone in a patient with nephroduodenal fistula due to previous xanthogranulomatous pyelonephritis is reported. Only few cases of nephroduodenal fistula are described in the literature, generally as a single case report or in small series. A nephroduodenal fistula as a result of chronic renal inflammatory disease such as xanthogranulomatous pyelonephritis, is usually associated with renal stones, recurrent urinary tract infections or endocrine disorders. Finally, renal stone as a cause of ileus is an event rarely described in the literature. In the case described, a correct preoperative diagnosis was possible with computerized tomography. During the operation a big renal stone was found and removed from the small bowel, but a limited resection was necessary because of the vascular impairment of the tract. At 8-month follow-up from operation, the patient was in good health, and no symptoms of renal or intestinal diseases were found.

[Xanthrogranulomatous pyelonephritis with nephrocutaneous fistula].

The authors report a case of xanthogranulomatous pyelonephritis presenting with nephrocutaneous fistula. This case illustrates all of the typical features of this disease: late diagnosis, non-functioning affected kidney, staghorn calculi, urinary tract anomaly, perinephritis with fistulization. The authors review the diagnostic and therapeutic modalities of xanthogranulomatous pyelonephritis and discuss the other aetiologies of nephrocutaneous fistula. In view of the severity of this disease and its preoperative resemblance to renal cancer, nephrectomy is often the only available treatment option.

[Silver-containing hydrocarbonate-calcium-magnesium mineral water in complex treatment of patients with chronic pyelonephritis].

95 patients suffering from chronic pyelonephritis in active inflammation phase received antibacterial therapy with adjuvant drinking of silver-containing mineral water Serebryany klyuch (21 patients) or without such drinking (17 patients). Spa treatment with mineral water Serebryany klyuch in combined therapy of chronic pyelonephritis is shown to relieve clinical symptoms, decontaminate urine completely in 88.6% cases, to eliminate signs of immunodeficiency and endotoxicosis and to produce a 25.6% decrease in the recurrences for the nearest 2 years.

[Diagnostic errors in purulent pyelonephritis].

Most frequent errors in diagnosis of purulent pyelonephritis have been analysed. It is shown that only a total of data obtained on the disease history, clinical symptoms, results of laboratory tests, ultrasound or x-ray symptoms of purulent pyelonephritis enable making an accurate diagnosis and choice of adequate treatment policy.

[The spectrum and activity of urinary Ig-proteinases as a marker of chronic pyelonephritis].

AIM: To study the activity of Ig-splitting proteinases of various classes in the urine in patients with chronic pyelonephritis. MATERIAL AND METHODS: Clinical, device and laboratory examinations were made in 50 patients (age 20-60 years). Admission and dynamic measurements were also made of the activity of serin, metal-dependent and thiol Ig-splitting proteinases in the urine. RESULTS: Total proteolytic activity in the urine of the patients was higher than in healthy subjects. Thiol proteinase activity was detected in 98% cases. This activity was 100 times higher in the patients than in healthy subjects. Significant differences were registered in the activity of serin proteinases. After the treatment the level of serin proteinases approached the normal value while thiol proteinase activity decreased significantly (2-fold, on the average) but remained high. If this activity was high, the patients (70%) had 1-2 exacerbations annually, in low activity exacerbations were absent. CONCLUSION: The activity of Ig-splitting proteinases of different classes is an important diagnostic and prognostic laboratory criterion of chronic pyelonephritis and can serve a criterion of treatment efficacy.

[Efficacy of using Naftusya type mineral water in pregnant women with cardiovascular diseases and concomitant pyelonephritis].

The present study focuses on blood circulation in pregnant women presenting with impaired renal function. In chronic pyelonephritis (CP), at the expense of increase in volume indicies (stroke index, systolic index) peripheral resistance gets moderately increased, with the hyperkinetic type circulation being mainly formed. The medicamentous method of treatment is but unacceptable more often as not because of its toxic action on the fetus, on account of which fact the complex of sanatorium-and-health resort treatment of pregnant women has been supplemented with the lightly mineralized hydrocarbonate-sulphate Naftusya type mineral water. Efficiency was studied of employment of the above therapeutic alternative in treating 79 pregnant women with cardiovascular pathology concurrent with pyelonephritis.

[A case of emphysematous pyelonephritis with disseminated intravascular coagulation].

A case of emphysematous pyelonephritis with disseminated intravascular coagulation (DIC) is presented. A 54-year-old woman was admitted to our hospital because of unclear consciousness and extremely high blood glucose level. The laboratory data suggested uncontrolled diabetes mellitus (DM) and urinary tract infection with sepsis and DIC. The plain abdominal X-P and abdominal CT revealed the existence of gas in the right renal parenchyma, perinephric tissue and the upper part of the right ureter. Right nephrectomy was performed after the improvement of the patient's condition by the echo-guided drainage of the right kidney and the treatment for infection, DM and DIC. We reviewed 71 cases of emphysematous pyelonephritis in the Japanese literature and the choice of treatment was discussed.

Comparison of ciprofloxacin with netilmicin for the treatment of acute pyelonephritis.

A prospective, randomised trial was undertaken to compare the efficacy of ciprofloxacin and netilmicin for the treatment of acute pyelonephritis. Forty-three patients were enrolled and 34 (29 women) completed the protocol. Fifteen of 17 patients treated with ciprofloxacin and 15 of 17 treated with netilmicin were cured. All patients were treated for five days. One patient relapsed after ciprofloxacin and another had a reinfection, while two relapsed after netilmicin. Five of six patients with a urinary tract abnormality were cured. Side effects were generally mild and rapidly reversible. Patients treated with ciprofloxacin spent a mean of 3.7 days in hospital compared with 5.3 days for those treated with netilmicin. The difference in duration of hospital stay was statistically significant (p less than 0.01). Both drugs proved highly effective and safe for the treatment of severe acute pyelonephritis.

Serum resistance in Escherichia coli strains causing acute pyelonephritis and bacteraemia.

The capacity of Escherichia coli to resist the bactericidal action of serum was examined in 367 clinical isolates obtained from children with acute pyelonephritis (n = 57), adults with acute pyelonephritis (n = 55), non-diabetic patients with bacteraemia (n = 101), diabetic patients with bacteraemia (n = 65) and from the faecal flora of healthy controls (n = 89). The incidence of serum-resistant E. coli strains was significantly higher in pyelonephritogenic strains from children and adults (93% and 82%) as compared to faecal control strains (57%, p less than 0.001 and p less than 0.005 respectively). Strains causing bacteraemia in non-diabetic and diabetic patients were more often serum resistant (72% and 80%) as compared to control strains (p less than 0.05 and p less than 0.001 respectively). The frequency of serum-sensitive strains was similar in diabetic patients with decreased renal function or proteinuria compared to those with normal renal function. There were no significant correlations between serum resistance of E. coli and expression of P fimbriae, type I fimbriae or mannose-resistant haemagglutination, cell surface hydrophobic properties, production of aerobactin, haemolysin or cytotoxic necrotizing factor in 53 pyelonephritogenic strains from adult patients.
