Study on adequacy of sterilization of air-water syringe tips.

This study examined the adequacy of sterilization of air-water syringe (AWS) tips following contamination during routine dental care treatment procedures. Although the internal chambers of most AWS tips were determined to be contaminated following use, no residual microbes were detected following autoclave sterilization of the AWS tips used during patient treatment procedures. Therefore, AWS tips apparently are readily heat sterilizable by conventional methods following clinical usage.

Effect of syringe filter and i.v. administration set on delivery of propofol emulsion.

Propofol emulsion was studied to determine the effects of filtration and of passage through an i.v. administration set on drug concentration. To study syringe filter effects, propofol emulsion samples (Diprivan Injection, 10 mg/mL) were discharged through either a 5-mm filtering needle or a standard needle. To determine the effects of the i.v. administration set on propofol delivery, a diluted dispersion containing approximately 2 mg/mL was prepared from propofol emulsion and 5% dextrose injection. This formulation was siphoned into a 6-foot length of administration set tubing, from which samples were withdrawn at 0, 30, 60, and 120 minutes. Another 2-mg/mL diluted dispersion of propofol and 5% dextrose injection was placed in a glass reservoir and delivered through a 6-foot i.v. set at a rate of 1.75 mL/min, which corresponds to a low dosage rate of 0.05 mg.kg-1.min-1. Samples of the effluent were collected at 0, 5, 10, 15, 30, 60, 90, and 120 minutes. Samples collected from all three tests were assayed by using high-performance liquid chromatography. The mean propofol concentrations in the filtered and unfiltered samples were 96.05% and 96.09% of label claim, respectively, after discharge through needles; the difference was not significant. In the static study of the i.v. tubing effects, a 31.54-34.74% loss of propofol occurred after 120 min. In the study of propofol flowing through the i.v. tubing, the concentration dropped slightly in the initial sample, which was taken after the propofol was placed in the tubing. The concentration remained essentially constant for the first hour and then declined, with an overall average loss of 7.70%.(ABSTRACT TRUNCATED AT 250 WORDS)

The needleless syringe: efficacy of anesthesia and patient preference in child dental patients.

Delivery of local anesthesia in dentistry is primarily by way of a needle-tipped syringe. This procedure, when used on children, is often stressful and sometimes painful for the patient. The Syrijet is an instrument which delivers anesthesia without the use of a needle. This is accomplished by delivering the anesthetic solution under high compressive forces. The subjects were thirty-four children ranging in age between 5 and 15 years, on whom forty-five dental procedures were completed. There was a statistically significant difference in favor of the instrument, with twenty-five subjects reporting a preference for it. The instrument was completely successful in providing anesthesia in thirty-six of the forty-five procedures.

Intrapartum suctioning of meconium: comparative efficacy of bulb syringe and De Lee catheter.

Intrapartum suctioning of the newborn's pharynx with a De Lee catheter (DL) has reduced the incidence of meconium aspiration syndrome (MAS) in neonates born with meconium staining of the amniotic fluid. However, the bulb syringe (BLB) is used more often for this purpose because of greater technical convenience. In a prospective study, 60 offspring of such deliveries received intrapartum pharyngeal suctioning either by BLB (29 cases) or by DL (31 cases), according to random selection. The presence and amount of meconium in the trachea, incidence and severity of MAS, and mortality from the disease were similar between the two groups. This study suggests that the BLB is as effective as the DL for intrapartum removal of nasopharyngeal meconium in deliveries with meconium staining of the amniotic fluid. Since the BLB is easier to use, less expensive and probably safer, our results suggest that it may be the preferable method.

Acquisition and use of needles and syringes by injecting drug users in Baltimore, Maryland.

Our objective was to determine how injection drug users (IDUs) in Baltimore, Maryland obtain and use needles and syringes (NS) for drug injection, before the opening of a needle exchange program (NEP). The method of this study was a cross-sectional structured interview survey in 1992 of active IDUs in a longitudinal study of human immunodeficiency virus (HIV) infection. For 466 IDUs (94.6% black, 83% male), usual sources of NS were "street" dealers (49.6%), pharmacies (29.8%), diabetics (16.3%), friends/neighbors (2.2%), and "shooting galleries" (1.9%). Half (53.5%) reported pharmacy purchase of NS, and 55.6% had diabetic friends/relatives. Twenty-three percent traded drugs, and 5% traded sex for NS. Eighty-eight and two-tenths percent would use a needle exchange program; 24.6% currently own no NS (median owned = 2.2). NS reuse was common (median = three times). Concern about (55.2%) or history of (33.9%) hassle/arrest for NS possession was typical; 81% kept NS at home, and 67% do not carry NS when purchasing drugs. Pharmacy purchasers (versus "street") were less likely to have been jailed, shared NS, or used shooting galleries during the preceding 6 months. In Maryland, although IDUs can legally purchase NS at pharmacist discretion, possession remains illegal; fewer than one-third of IDUs use pharmacies, and most obtain NS from illegal sources. Most IDUs reuse NS, but discard them after several uses. Current patterns of NS acquisition and use in Baltimore are likely to increase HIV transmission. Increased availability and decriminalization of NS possession could decrease the risk of injection-related HIV transmission.

Impact of new legislation on needle and syringe purchase and possession--Connecticut, 1992.

Human immunodeficiency virus (HIV) and other bloodborne pathogens are transmitted among injecting-drug users (IDUs) through the reuse and sharing of contaminated needles and syringes (NSs). Of the 689 acquired immunodeficiency syndrome (AIDS) cases reported in Connecticut in 1992, 413 (60%) were associated with injecting-drug use. To help reduce IDUs' use of contaminated NSs, Connecticut enacted laws effective July 1, 1992, that allow the purchase without a prescription of up to 10 NSs at one time in pharmacies and the possession of up to 10 clean NSs. Before this date, purchase and possession of NSs without a prescription had been illegal in Connecticut. This report presents preliminary information from the first 5 months of an ongoing evaluation to determine whether the new laws affected pharmacy-based NS sales, IDUs' reported knowledge of the laws and places to obtain NSs, and law enforcement officers' risk for needlestick injuries.

[Manual intrauterine aspiration using a Karmann syringe. Multicenter study in Sonora and Sinaloa, Mexico].

These are the results of a prospective, multicentric study carried out in five institutional hospitals in Sonora and Sinaloa, using Karmann syringe and cannulae for manual intrauterine aspiration in cases of incomplete abortion, dead and retained ovum, hidatiform mola and endometrial biopsy. Management data in 1046 patients, are described; methodology is shown, and demonstrating that it converts uterine emptying is a simple, efficacious and economical procedure, with a very low risk and it allows these patients management in an ambulatory way. From these results, and as it is demonstrated in the study, institutions may increase their programable resources as to amount of disposable beds, for other type of patients, and the offering of attention by surgeon, anesthetist, nurse and medication, as their hospital stay is minimal, and in most of the cases active anesthetist participation is not required. In the same way, it permits the patient a better relationship with the personal of the institution, and so, a more human care and the possibility of reintegrating the patient to a familiar environment, rapidly, and in good psychical and physical conditions.

[The on-line simulation of blood propofol concentration by personal computer transmitting settings of the syringe pump via a digital port].

A program for on-line simulation of blood propofol concentration was developed. Various pharmacokinetic model programs are available for the estimation of intravenous anesthetic concentration. But manual entry of data such as body weight, rate of infusion and the timing of changing the flow rate is mandatory in these programs. This limited the use of these programs for investigational use only. In a new program, the data required such as total dose, flow rate, body weight and time were collected automatically on-line via the digital outlet of the syringe driven pump (Terumo STC525X, Japan and Graseby 3500, UK) to a personal computer (Apple Macintosh Power Book Duo230 or Power Book 520, USA). Based on the obtained data, pharmacokinetic model was solved with personal computer. Calculated blood concentrations of propofol were displayed in a numeric form and a trend graph was obtained. This program provides useful information for maintainance of anesthesia with propofol.

Computerized local anesthetic delivery vs. traditional syringe technique. Subjective pain response.

A study was designed to determine if there are changes in the perception of pain when the flow rate and pressure of an injected anesthetic are precisely controlled. Fifty dentists were given contralateral palatal injections. One side was injected with the Wand injector, a new delivery system that uses a microprocessor and an electric motor to precisely regulate flow rate during administration. The control side was injected using a standard manual syringe, in which flow rate and pressure are operator-dependent and cannot be controlled accurately. The subjects used two subjective scales to describe their perceived pain experience. When their responses were analyzed the Wand injector was found to be two- to three-times less painful than the manual injection. The results were statistically significant (p < .001). The authors conclude that there is an optimal flow rate of anesthetic solution at which the perception of pain during an injection is minimized.

Distribution of neuromuscular junctions in laryngeal and syringeal muscles in vertebrates.

Vertebrates are capable of producing a variable sound spectrum. In mammals, lissamphibia, and reptiles, the larynx is the vocal organ responsible for sound production, whereas in birds it is produced by the syrinx, an avian organ located at the base of trachea. The distribution of neuromuscular junctions responsible for the fine control of laryngeal muscle (LM) and syringeal muscle (SM), although studied with some detail in human LM, remains mostly unknown in other vertebrates. In the present study, we analyzed the distribution of motor end plates (MEPs) in LM/SM of different vertebrate classes using the histochemical detection of acetylcholinesterase: the thyroarytenoid and cricoarytenoid LM of mammal (human, rat, and rabbit) and cricoarytenoid LM of nonmammalian (frog and avian) species and the tracheobronchial SM of rooster and pigeon. In humans and frogs/avians, MEPs were distributed diffusely along, respectively, the thyroarytenoid-cricoarytenoid and the cricoarytenoid LM fibers, whereas in rats and rabbits, MEPs were concentrated in a transverse band located in the middle of thyroarytenoid and cricoarytenoid muscle fibers. In roosters and pigeons, MEPs were distributed diffusely along SM fibers. The highly diffuse MEP distribution along human thyroarytenoid and cricoarytenoid fibers indicates that these muscles can markedly change their degree of contraction, which may contribute for the large range of different sounds produced by human vocal folds. The same rationale was applied to discuss the possible functional significance of the morphological distribution of MEPs along the LM/SM of the other vertebrates analyzed.

Laccase-catalysed iodide oxidation in presence of methyl syringate.

The kinetics of potassium triiodide (KI(3)) formation during fungal laccase action was investigated in presence of methyl syringate (MS). The recombinant forms of Polyporus pinsitus (rPpL), Myceliophthora thermophila (rMtL), Coprinus cinereus (rCcL), and Rhizoctonia solani (rRsL) laccases were used. The triiodide formation rate reached 6.1, 5.5, 6.0, and 2.1 microM/min at saturated rPpL, rCcL, rRsL, and rMtL concentration, respectively, in acetate buffer solution pH 5.5 and in presence of 10 microM of MS and 1 mM of potassium iodide. The triiodide formation rate increased if pH decreased from 6.5 to 4.5. The scheme of laccase-catalysed iodide oxidation includes stadium of MS interaction with oxidized laccase with concomitant production of MS(ox). The reaction of MS(ox) with iodide produced triiodide. The turnover number of MS was 93 and 44 at pH 5.5 for rPpL and rMtL, respectively. The scheme also contained a stadium of reversible reduction of laccase active centre with the mediator explaining the different saturation rate of triiodide production. The fitting kinetic data revealed that the reversibility of the reaction increased for laccases containing lower redox potential of copper type I.

Organization of the zebra finch song control system: I. Representation of syringeal muscles in the hypoglossal nucleus.

Understanding the representation of learned skills in the brain requires that one know the neural substrate for those skills. The avian song control system uses auditory information to establish and modify motor programs, which provide patterns for the excitation of individual muscles. In the present study, a combination of neurophysiological and anatomical techniques was used to map the representation of syringeal muscles in the tracheosyringeal part of the hypoglossal nucleus of adult male zebra finches. Microstimulation revealed that control zones for individual muscles are arranged along the rostrocaudal axis of the nucleus. The ventralis and dorsalis muscles have the largest domains, located at the rostral and caudal ends of the nucleus, respectively. The retrograde tracer fluorogold was applied to the muscles and confirmed this pattern. The muscle map obtained will provide a useful tool for further study of the convergence of muscle representation and sound representation in the more central portions of the song control pathway. This knowledge is essential for understanding how learned sounds are perceived and produced.

Stable transfection of mammalian cells by syringe-mediated mechanical loading of DNA.

We show that mammalian cells can be stably transfected by a mechanical loading procedure in which cells are forced through a small opening in the presence of DNA. A suspension of cells and plasmid DNA in growth medium was passed up and down through a 30-gauge needle attached to a 1-ml syringe. Cells were immediately plated at appropriate densities for subsequent selection for stable expression of a marker gene. Two rodent cell lines, Chinese hamster ovary and mouse Ltk- cells, were successfully transfected with an efficiency of about one transfectant per 5 x 10(4) cells. The human HeLa cell line was transfected with a somewhat lower efficiency. Pluronic F-68, a detergent believed to aid in healing of membrane injuries, had no beneficial effect when present during the loading procedure. Successful transfection was accomplished using three different genes as selectable markers. Southern blotting analysis revealed that transfectants contained one or very few copies of the introduced DNA construct integrated into the genome. Several transfectants were demonstrated to remain stable for more than 20 generations of growth in the absence of selection. This procedure is fast, economical, and of general utility.

Left syringeal dominance in testosterone-treated female canaries.

Male canaries (Serinus canaria) produce most of their song syllable types on the left side of the syrinx, the two-sided vocal organ. Female canaries treated with testosterone propionate begin to sing in a male fashion, a behavior seldom seen in untreated females. To learn whether syringeal dominance occurs in testosterone-treated female canaries, we deactivated either the right or the left side of the syrinx by severing the tracheosyringeal nerve on that side. Recordings of the birds' songs were made before and after the nerve cut, and song analysis was based upon visual comparisons of sound spectrograms. Pre- and post-operative syllable types were identified by their frequency structures and repetition rates. Six of eight birds displayed clear left dominance as assessed by nerve cuts; therefore, we concluded that most, but not all, female canaries generate the majority of their syllable types with the left syrinx.

[Exchange of syringes or apprenticeship? The buses of Bienne and Geneva].

In Switzerland, the health ministry (Office Fédéral de la Santé Publique) has systematically encouraged the evaluation of low threshold services. In this article, we discuss the evaluation of two of these: the buses for syringes exchange in Geneva and Bienne, the implementation of these two services, the success obtained and the contacts established. Even if the design of such an evaluation was relatively complicated, with one monitoring and two specific surveys, the principal aim of this article is not to measure the efficacy as such but to show how an learning process has occurred between the actors: government, administration, police, service's team, drug's users and neighbourhood's inhabitants. The efficacy for a long period of time and the implementation's success are largely linked to such learning processes.
