Cardiac function in dialysis patients evaluated by Doppler echocardiography and its relation to intradialytic hypotension: a new index combining systolic and diastolic function.

AIM, PATIENTS AND METHODS: Cardiac function of 40 patients on maintenance dialysis was examined using a new Doppler index combining systolic and diastolic function. Immediately before dialysis, ultrasonic pulsed Doppler was used to measure right and left ventricular inflow and outflow waveforms. The sum of the isovolumetric contraction time (ICT) and isovolumetric relaxation time (IRT) was calculated by subtracting the ejection time from the interval between the end of an inflow waveform and the start of the next inflow waveform. The new index was obtained by dividing the sum of the two isovolumetric times by the ejection time. Cardiac function was evaluated in dialysis patients and healthy controls using new indices of the right and left heart systems. RESULT: Indices of the right and left heart systems in dialysis patients were significantly higher than those in healthy controls. With respect to hypotension during dialysis, patients were divided into two groups, a group with decreased blood pressure (group A: 27 patients) and a group with normal blood pressure (group B: 13 patients). Only the left heart system index in group A was significantly higher. There were no significant differences in other M-mode Doppler indices between the two groups. CONCLUSION: The new index was a more sensitive evaluator of cardiac function and predictor of hypotension during dialysis compared to standard echocardiographic indices.

Effects of loss-of-function and gain-of-function mutations of c-kit on the gastrointestinal tract.

Protooncogene c-kit encodes a receptor tyrosine kinase, KIT. Interstitial cells of Cajal (ICCs) that are important for the autonomous movement of the gastrointestinal tract essentially require the normal function of the KIT for their development. Therefore, germline loss-of-function mutations of the c-kit gene cause deficiency of ICCs that results in disturbed gastrointestinal movement. On the other hand, somatic gain-of-function mutations of the c-kit gene induce gastrointestinal stromal tumors (GIST) that are considered to originate from ICCs. Moreover, germline gain-of-function mutations of the c-kit gene are a cause of familial development of multiple GISTs.

The assessment of functional impairment in fibromyalgia (FM): Rasch analyses of 5 functional scales and the development of the FM Health Assessment Questionnaire.

OBJECTIVE: Functional assessment by self-report questionnaire plays an important role in most rheumatic conditions, but psychometric properties of questionnaires have not been studied in fibromyalgia (FM), particularly by Rasch analysis, which allows for examining adequacy of the questionnaire scale. To assess currently used instruments, we examined the Fibromyalgia Impact Scale (FIQ), 4 versions of the Health Assessment Questionnaire (HAQ), and the Medical Outcome Survey Short Form (SF-36). METHODS: More than 2,500 patients from 4 sites (3 US, 1 Israel) completed the FIQ. The HAQ questionnaires were completed by 1438 patients participating in the US National Data Bank for Rheumatic Diseases. Seven hundred sixty patients from Wichita, Kansas, completed the SF-36. Rasch analysis was applied separately to each of these data sets. RESULTS: The FIQ systematically underestimated functional impairment by its handling of activities not usually performed. All questionnaires had problems with non-unidimensionality and ambiguous items when applied to patients with FM. In addition, scales were found to be non-linear. Because of these findings we used the 20 item HAQ questionnaire as an item bank to develop a new questionnaire more suitable for use in FM, the fibromyalgia HAQ (FHAQ). This questionnaire fits the Rasch model well, is relevant, is linear, and has a long, well spaced scale. CONCLUSION: No available functional assessment questionnaire works well in FM. A new questionnaire, the FHAQ, was developed. It has appropriate metric properties and should function well in this condition. Since the FHAQ is a subset of the larger HAQ questionnaire, a new questionnaire is not required; only a different method of scoring is needed. Additional studies regarding sensitivity to change are required to fully validate the FHAQ.

[Cerebral vasoreactivity and functional response in stroke: a study with functional MR].

INTRODUCTION: Lacunar infarcts are usually associated with anatomical and possibly functional changes in the walls of small blood vessels (penetrating arteries). The functional effect varies and is accompanied by cerebral adaptive/reorganizational changes. BOLD contrast, originated in the microvasculature (especially with ultra high magnetic fields) depends, in the end, on haemodynamic changes and is useful for exploring patterns of cerebral activation using fMR. OBJECTIVES: To compare the temporal behaviour of the BOLD signal and the distribution of activation between a group of patients with no functional sequelae following a stroke and a control group, by using fMR at 3T. RESULTS: The stroke group showed a smaller number of voxels activated, but this was not statistically significant. The patterns of activation, size of the sensor motor area (SM or ASM) and index of laterality were similar in both groups. In SM the BOLD response was 85% slower (p< 0.01) and 25% less (not statistically significant). CONCLUSIONS: We found no differences in the patterns of activation of the two groups. This may be explained by the minimal lesions of the stroke group, which were insufficient to produce reorganizational/adaptive changes or by the great variety of responses. The temporal response of the BOLD response was different in the stroke group, probably as a result of the alteration in the haemodynamic response in relation to the underlying processes which damage the functional properties of the vascular wall. This difference in the BOLD response should be interpreted with caution, and borne in mind when interpreting activation in persons with vascular pathology. Further studies are necessary for better understanding of its significance.

[Effect of aging on male sexual function in 93 patients using international index of erectile function].

OBJECTIVES: To investigate the influence of aging on male sexual function. METHODS: The study selected 93 ED patients, aged from 23 to 64, who responded to the International Index of Erectile Function (IIEF) questionnaire. The questionnaire includes 15 items related to male sexual activity, which are organized into 5 domains, namely, erectile function (EF), orgasmic function (OF), sexual desire (SD), intercourse satisfaction (IS) and overall satisfaction (OS). For statistical analysis, ANOVA with DUNCAN test was conducted, and statistical significance was set at P < 0.05. Some other risk factors of ED such as hypertension, diabetes etc. had been excluded. RESULTS: According to the age, the subjects were divided into 5 groups. With age increasing, the proportion of moderate and severe in each group increased from 16.17% to 57.14%, whereas EF score decreased from (19.50 +/- 4.64) to (15.27 +/- 5.64), OF score decreased from (6.93 +/- 2.86) to (5.62 +/- 2.94), SD score decreased from (6.33 +/- 1.63) to (4.50 +/- 2.94), IS score decreased from (10.17 +/- 1.94) to (6.93 +/- 2.90), OS score decreased from (5.00 +/- 0.89) to (3.15 +/- 1.84). The tendency took on linearity (P < 0.01). Aging was negatively correlated to above mentioned scores (r = 0.98, P < 0.01). CONCLUSION: Aging could be thought as a risk factor of ED, which is negatively correlated with male's EF, OF, IS, OS and SD scores, furthermore. IIEF questionnaire is a useful tool assessing epidemiology of ED.

[Three-dimentional organization of synapses and astroglia in the hippocampus of rats and ground squirrels: new structural and functional paradigms of the synapse function].

The literature data and our own data on the synaptic plasticity and remodeling of synaptic organelles in the central nervous system are reviewed. Modern techniques of laser scanning confocal microscopy and serial thin sectioning for in vivo and in vitro studies of dendritic spines, including the relationship between morphological changes and the efficacy of synaptic transmission, are discussed using, in particular, a model of long-term potentiation. The organization of dendritic spines and postsynaptic densities of different categories as well as the role of filopodia in spine genesis were analyzed. It was shown that the method of serial ultrathin sections is the most effective for unbiased quantitative stereological analysis and 3D reconstructions. By using the refined method of serial ultrathin sections with subsequent three-dimensional reconstructions, the presence of giant mitochondria in hippocampal neuronal dendrites was demonstrated. It was shown that smooth endoplasmic reticulum forms a unified continuum with the outer membrane of the mitochondrial envelope within dendrites. It was suggested that this continuum provides calcium tunneling, which makes possible intracellular signal transduction during synaptic transmission. Evidence is presented indicating the presence of gap junctions ("electrical synapses") in the synapses of mammalian brain, as well as between glial processes, and between glial cells and neurons. Our data and the data of other authors show that glial cell processes form a structural and functional glial network, which modulates the functioning of the neuronal network. The connection of dendritic spines with the glial network is shown on 3D reconstructions by analyzing the neuropil volume in CA1 hippocampal area of ground squirrels in three functional states: normothermia, provoked arousal, and hibernation when brain temperature falls below 6 degrees C. The own data of the authors are discussed indicating the formation of more than five presynaptic boutons (multiple synapses) on both CA1 mushroom-like dendritic spines and CA3 thorny excrescences. On the basis of the analysis, new ideas of the organization and functioning of synapses were suggested.

[Functional asymmetry of the brain and effectiveness of sensorimotor function of oral cavity organs].

It has been established that in healthy persons functional brain asymmetry positively influences the effective performance of sensomotor function of oral cavity organs. Individual brain asymmetry is of considerable importance, the higher it is, the quicker the final results of sensomotor function are achieved. The character of brain asymmetry (right hand or left hand) does not practically influence the performance of sensomotor functions.

Functional and non-functional liver blood flow in patients with liver cirrhosis measured by indocyanine green.

Indocyanine green administration was used in a group of ten patients with liver cirrhosis with portal hypertension to estimate hepatic extraction, liver blood flow and the degree of blood shunting. Extraction coefficients obtained by the hepatic catheterization were well correlated with the pharmacokinetic method. Values of the total liver blood flow measured by the classical method based on Fick's principle were always higher than those evaluated by the pharmacokinetic analysis estimating the functional liver blood flow only. Estimates of the degree of shunting varied between 32.0-78.9% of the total liver blood flow. A statistically significant correlation (r = 0.79) was found between the percentage of the functional fraction of the total liver blood flow and albuminemia. It is suggested that in liver cirrhosis with portal hypertension the reduction of the functional fraction of total liver blood flow is combined with partial impairment of hepatic cell metabolism in the remaining functional areas of the liver. In such a case neither the "intact" nor the "sick hepatocyte" hypotheses are appropriate in their strict sense for the interpretation of the findings.

Changes over years in gross motor function of 3-8 year old children with cerebral palsy: using the Gross Motor Function Measure (GMFM-88).

BACKGROUND: In the developing child the nervous system undergoes a maturation process. The development and organization of any motor ability is the naturally adopted preference among the possibilities and constraints. The motor behavior of children with cerebral palsy is a personal automatic preference based on such constraints. One of the clinical measures designed for measuring the function of children with CP is the Gross Motor Function Measure. Motor development curves for children with CP have been established based on the GMFM instrument and Gross Motor Function Classification System. OBJECTIVES: To examine the change over time in gross motor function for children with CP attending a special education school for handicapped children in Israel. METHOD: We conducted a retrospective review of the medical records of children at various ages and with varying degrees of severity who were being treated by a multidisciplinary team. The study population comprised 106 children aged 3-8 years with CP who were attending the school of special education at Assaf Harofeh Medical Center. The GMFM-88 test was performed annually for the study children over a 7 year period (1995-2001). RESULTS: During the study period the GMFM scores improved significantly. The rate of improvement and top achievements over the years differed according to the severity of the motor impairment. The gross motor development reached a plateau at the age of 6-7 years. CONCLUSIONS: The changes in gross motor development of the study population were similar to the profile of changes in the developmental process of children who develop normally. The nature of the curves of gross motor change for the children with CP should be borne in mind when designing individual treatment goals and strategies for a child.

[Sequential protocols with thymoglobuline and delayed introduction of calcineurin: effects on renal function in renal transplant recipients with non immunological risk and low risk of delayed graft function].

Induction therapy with thymoglobuline significantly decreases the risk of acute rejection, particularly in high immunological risk patients, in combined transplantations and in pediatric renal transplantation. After the introduction of cyclosporin in the 1980's and the recognition of its potential nephrotoxicity, induction therapy with delayed introduction of cyclosporin (sequential protocols) have became very popular in many transplant centers and are now used even in low-immunological risk patients. The aim of the present study was to assess the recovery of renal function in low-immunological risk patients receiving a sequential protocol versus those receiving a calcineurin inhibitor at day 1. Among patients receiving their first transplant in our center between January 1999 and June 2000, we selected 72 recipients with no immunological risk and no risk of delayed graft function (DGF). 35 patients (group I) have received a sequential protocol whereas 37 patients (group II) have received a calcineurin inhibitor (Neoral or prograf) at day 1. We analysed creatinine reduction ratio, 24-hour creatinine excretion on post-transplant day 2, and serum creatinine and creatinine clearance at day 15, 30, 45, 60 and 90 post-transplant. There was no difference between groups concerning demographic, immunological or surgical parameters. The percentage of patients with immediate graft function was similar between the two groups (10 versus 9). The number of patients requiring dialysis in the first post-transplant week was also similar (8/35 versus 6/37). The day serum creatinine reached 200 micromol/l was 15 +/- 11 versus 14 +/- 12 in groups I and II respectively. Serum creatinine and creatinine clearance were similar at all time intervals. CMV disease was significantly higher in the group 1 (42% versus 18.5%; p < 0.005). Our data suggest that in patients with low immunological risk and low-risk of DGF, introduction of calcineurin inhibitors as early as the post-transplant day 1 is not deleterious for renal function recovery. These data should be confirmed by a prospective randomised trial.

A comparison between older persons with down syndrome and a control group: clinical characteristics, functional status and sensorimotor function.

The increase in life expectancy within the general population has resulted in an increasing number of elderly adults with intellectual disability, and this is reflected in the increased life expectancy in persons with Down syndrome, currently about 56 years. The aim of this study was to study the clinical characteristics, the functional status and sensori-motor function of 10 older persons with Down syndrome (mean age 59 years), 13 younger persons with Down syndrome (mean age 44 years) and compare them with 38 adults with intellectual disability without Down syndrome and a control group of people without intellectual disability. All the persons with Down syndrome and intellectual disability resided in two residential living centres in Israel, while the 31 older persons without intellectual disability (mean age 75 years), who served as the control group, lived in an independent living facility. The study considered demographic data, medical backgrounds, physical and functional tests. The results showed that the older persons with Down syndrome in the study were more obese, shorter and had more medical problems than both the older persons with intellectual disability and the control group. The functional performance of the older adults with Down syndrome was more impaired in comparison with both other groups. It is postulated that their slower responses may be explained by a less physically active lifestyle, that may accelerate the onset of disease, resulting in symptoms associated with aging that are detrimental to health.

Nose: aesthetics and function. Rhinoplasty and functional problems related to anterior ethmoid.

Aesthetic nasal surgery has progressed in the last years, as concerning both surgical techniques and surgical instruments, which allowed the finding of new and more sophisticated surgical solutions. Clinical practice led to observe sometimes functional surgical failures, due rather than an inaccurate surgical technique, to an incomplete diagnostic approach to the patient. It has been observed that modifying the external conformation of nasal pyramidis inner-nose volumes and spaces will be subsequently reduced, giving as a result a condition which is only aesthetically but not functionally valid. 32 subjects, selected for a rhinoplasty and presenting nasal respiratory obstruction and anterior ethmoid abnormalities, confirmed by nasal endoscopy and nasal CT, were evaluated and subdivided into two groups: the first (A) group underwent only to an aesthetic rhinoplasty, the second group (B) was operated of a rhinoplasty combined with functional endoscopic sinus surgery (FESS) in order to correct the above mentioned anatomical abnormalities. The nasal airflow, and though the nasal cavities patency, was evaluated pre- and post-operatively in the two groups of patients, referring to rhinomanometric conductance values. Patients of group A reported decreased post-operative conductance values, patients of group B reported increased post-operative conductance values, showing though the functional involvement of anterior ethmoid in nasal obstruction and the necessity of correcting its anatomical abnormalities, in order to reach not only a new nasal profile, but also a better respiratory performance.

[Study of Qingciguo oil's function on blood lipid and other functions].

OBJECTIVE: To study Qingciguo Oil's function on blood lipid and other functions. METHODS: 84 male SD rats were randomly divided into seven groups (control group, high lipid group, low dose group, middle dose group, high dose group, combined dose group and combined control group) based on the level of TC. We respectively feed each group of rats with the normal food, high lipid high cholesterol food, Qingciguo Oil's 4 ml/kg BW food, Qingciguo Oil's 8 ml/kg BW food, Qingciguo Oil's 12 ml/kg BW food, Qingciguo Oil's 12 ml/kg BW + 0.8% soybean PL + 0.05% VitE food & 0.8% soybean PL + 0.05% VitE food ninth week. RESULTS: At the fourth week, the TC of each group rats (except lower dose group) began to decreasing, and at sixth week, the TC of middle dose group was significantly lower than the high lipid group. At sixth week, the TG of low, middle and high dose group was significantly lower than the TG of high lipid group, the HDL of high lipid group and middle dose group decreased significantly, and the HDL of combined control dose group significantly higher than high lipid group. At the ninth week, the level of SOD of all experimental groups significantly higher than the high lipid group and the MDA of each group have no significant differents between each other. In vitro, we test the effect of different dose Qingciguo oil and fish oil on platelet aggragation of each blood by tuibidimetry, the results show that the platelet aggregation of Qingciguo oil high dose group was significantly lower than the control group. CONCLUSION: Under our condition, Qingciguo oil has certain functions of decreasing the level of blood lipids modulating the function of antioxidants and restraining the platelet aggregation in vitro.

Functional fitness: maintaining or improving function for elders with chronic diseases.

OBJECTIVE: This study's objective was to prove or disprove four hypotheses, three addressing physical function and a subjective measure of self-perceived well-being from participating in an elder functional fitness program. METHODS: Participants included 17 chronically ill residents from an assisted-living center in Maine. Measures on mobility, metabolic equivalent estimation (MET) levels, resting heart rate, blood pressure, oxygen saturation, muscular strength, flexibility, and body weight were conducted at three intervals-baseline, 6 months, and 1 year. SAS software was used for analysis of means and paired t test. RESULTS: Increases in or maintenance of function proved significant. The subjective hypothesis also proved significant. DISCUSSION: Research on elder exercise focuses on one or two components, rather than the benefits of a thorough fitness program. This study illustrated improvements in physical function and well-being from participating in a comprehensive elder fitness program.

[Tests for autonomic functions: sudomotor function].

Sudomotor function tests provide information on the functional state of the sympathetic system innervating skin, the postganglionic fibers of which are cholinergic. Various methods for sweat elicitation and its qualitative or quantitative detection are described here, but most practical combinations vary depending on the purpose of testing and possible pathophysiology of each patient; 1) hyperhidrosis vs. hypo- or anhidrosis, 2) central vs. peripheral or postganglionic lesions, 3) generalized vs. focal disturbances. When these are unknown, thermal sweating of the whole body is most informative. Comparison of thermal and pharmacological sweating enables one to estimate the extent and possible site of sympathetic disturbance. In evaluating the results of sudomotor function tests, factors due to employed methods, age, sex, posture, medication, etc must be taken into consideration. In general, hyperhidrosis is more difficult than hypohidrosis to estimate its underlying mechanism, since the functional relation between the facilitatory hypothalamo-spinal pathway and other central structures still remains largely unknown.

[The role of podocytes in normal glomerular function and in the pathogenesis of glomerulonephritis. Part I. Phenotypic and functional characteristics of podocytes during their differentiation and maturity].

Podocytes play an important role in glomerular function. Together with endothelial cells of the glomerular capillary loop and the glomerular basement membrane they form a filtration barrier. Podocytes cooperate with mesangial cells to support the structure and function of the glomerulus. With regard to its structure, the podocyte can be divided into three structural and functional segments: cell body, major processes, and foot processes, the latter playing a crucial role in the pathogenesis of proteinuria. Podocytes are end-differentiated cells with specific phenotypic features associated with their function. The compensatory ability of these cells is limited to hypertrophy. Under normal conditions a podocyte cannot restart the cell cycle. Attention has recently been drawn to the importance of slit diaphragms in the pathogenesis of proteinuria. Structural damage to each of the slit diaphragm proteins as well as disturbances in their mutual interactions can result in insufficiency of the filtration barrier.

[Abnormalities of respiratory function in civil defence firefighter-submarine divers. Respective role of diving and occupational exposure related to occupational firefighter's functions].

Lung function was studied in 20 firemen-submarine divers (mean age 36 +/- 1.2 years) of the French civil defence undergoing the medical check-up compulsory for professional divers (lung function tests are not systematically performed in ordinary firemen). Compared with the CECA standards: (1) vital capacity (VC) was increased, residual volume (RV) was decreased and total lung capacity (TLC) was unchanged; (2) with the exception of peak respiratory flow, all expiratory flow values (FEV1, MEF50, MEF25) were decreased; (3) the permeability factor (KCO) was decreased. These functional abnormalities were moderately worse in subjects who smoked. Some abnormalities (increased VC, decreased RV) are typical of diving activities, but the deterioration of effort-dependent expiratory flow values and alveolar-capillary diffusion must be ascribed to specific nuisances (fumes, polluants, toxic substances) associated with fireman's activities. Monitoring lung function in all professional firemen therefore seems to be necessary, if not indispensable.

[Histologic comparison of a functioning and non-functioning filtration membrane with the Molteno implant].

In two patients with a Molteno implant, the filtration membrane was excised 2 and 21 months postoperatively because of uncontrolled elevation of intraocular pressure. This tissue was examined by light and electron microscopy and the findings compared with those of a third functioning Molteno filtration membrane obtained from an enucleation specimen. The most conspicuous differences were the high density and cellularity of the connective tissue of the non-functioning membranes and the frequent cell pyknosis and necrosis in the functioning membrane. In addition, one of the non-functioning membranes showed a condensed cellular layer on the inner surface. There were no inflammatory cell infiltrates in any of the three specimens. We conclude that the variable results of Molteno implants are related to the quantities of tissue components of the filtration membrane.
