Abdominoperitoneal approach to management of the high, short vagina in the adrenogenital syndrome.

We describe a combined transtrigonal, abdominoperineal approach to the surgical management of the female pseudohermaphrodite with the adrenogenital syndrome and a high, short vagina. Most of our patients had undergone previous clitoral reduction surgery and, therefore, clitoral shaft skin was not available for exteriorization of the vagina. Our operation involves partial mobilization of the vagina through an inverted U flap from the perineum followed by detachment from the urogenital sinus from above by splitting the trigone. Exteriorization is accomplished by mobilizing 2 lateral skin flaps from the perineum and joining them with the inverted U flap to reach the vagina. Cosmetic results have been excellent, although anastomotic stenosis occurred in 1 young infant.

Clitoroplasty for clitoromegaly due to adrenogenital syndrome without loss of sensitivity.

For preservation of postoperative sensitivity of the glans clitoris, clitoroplasty for clitoromegaly due to adrenogenital syndrome is reported. The operative technique includes the removal of ventral corporal tissue, taking care to avoid damage to the dorsal neurovascular bundle. A small lump of glandular tissue, which is connected with the neurovascular bundle, is left over and affixed to the pubic bone. Postoperative appraisal of patients in whom this technique has been applied is satisfactory. However, precise assessment of glandular sensation is not possible because of the patients' young ages. Follow-up investigation at puberty is necessary for a final, proper appraisal.

True prostatic tissue in 46,XX female with adrenogenital syndrome.

A case is presented of a seventeen-year-old phenotypic male with empty scrotum, hormonal studies consistent with adrenogenital syndrome, a 46,XX karyotype, and a normal prostate on rectal examination. Histochemical evidence of the true nature of the prostatic tissue is presented, and the developmental aspects of the case are discussed.

Reconstruction of the external genitalia in the adrenogenital syndrome by means of a personal one-stage procedure.

The techniques for correction of the external genitalia in the adrenogenital syndrome, as far as known by the author, are reviewed. The goals of repair and timing of surgery are discussed. The author's one-stage technique, first published in 1974, is described with intraoperative illustrations, and the results obtained in nine patients are discussed. The advantages of the technique are preservation of a clitoral glans with erogenous sensation based exclusively on the deep dorsal neurovascular bundle. The glans of the megaloclitoris is obliquely reduced in size at its base toward the ventral surface and by resection of up to two-thirds of the ventral segment. It is relocated at its anatomic female position. The labia minora and the introitus vaginae are reconstructed at the same stage with the skin of the megaloclitoris displaced in posterior direction after a cutback incision. Dispareunia is prevented by total excision of the corpora cavernosa, including the crura.

[Psychosexual development and sex life of women with congenital adrenogenital syndrome].

OBJECTIVE: The aim of the study was to examine the psychosexual development and sexual life of 13 adult women with congenital adrenogenital syndrome. DESIGN: A clinical study. SETTING: Psychiatric clinic, Gynecological clinic and Sexological institute, Medical Faculty of the Charles University in Prague. METHODS: The examination was carried out using a structured interview and three tests: MF (Masculinity and feminity), SFW (Sexual function of women) and SAI (Inventory of sexual arousibility). RESULTS AND CONCLUSION: It was ascertained that only about one third of the patients were coitally active and that the first intercourse was delayed. The results of the questionnaire SFW were in 66% subnormal and an adequate sexual arousability was found only in 40% of examined persons (the test SAI).

[Balanced translocation 8:10 and adrenogenital syndrome. Family study (author's transl)].

An 8:10 translocation was found in a 15-day-old female child affected with adrenogenital syndrome. Some other members of the family also showed the same chromosomal anomaly. By G-, Q- and T-banding, break and fusion points were determined at the level of 8q21 and 10q24 bands. The hormone and enzymatic tests carried out in all the members of the family revealed a 21-hydroxylase deficiency only in the proposita. Possible relationship between the manifestation of syndrome and chromosomal anomaly are discussed.
