[Eosinophilic bladder disease].

The authors present ten cases of eosinophil cystopathy and recall the very varied clinical signs of this disease: pollakiuria, haematuria, dysuria and the findings of complementary investigations: possible dilatation of the upper urinary tract, thickening of the bladder wall. Biopsy is essential and reveals lymphocyte and plasma cell infiltration and numerous eosinophils. The treatment administered was either immunosuppressants or dimethyl sulphoxide.

Long-term follow-up after clam ileocystoplasty. Complications and results including some quality-of-life parameters for 32 patients treated for neuropathic and nonneuropathic bladder disease.

The results of clam ileocystoplasty in 32 patients were evaluated. Nineteen patients had a neuropathic (group N) and 13 a nonneuropathic disease (group NN). Mean follow-up was 75 months; 62 months for group N and 95 months for group NN. Mean age was 38 years. Postoperative complications and some quality-of-life parameters including continence, and the need for clean intermittent self-catheterization and an artificial sphincter were assessed. The mean pre-/postoperative bladder volumes were for group N and NN 173/364 ml and 171/246 ml, respectively. Eighty percent of the patient were satisfied with the operation and would undergo a new operation if necessary. The remainder had some expectations that were unfulfilled.

[The lithotriptor "Compact" used in urinary bladder diseases].

The stroke-wave complex "Compact" is the first home-produced lithotriptor with electromagnetic system of stroke-wave generation and ultrasonic diagnosis. During its development it was possible to achieve the optimal parameters of focused waves, good technical characteristics and economic indices. The clinical use of lithotriptor has shown its efficiency in nephrolith and ureterolith remote destruction in the absence of complications connected with negative influence of stroke waves on kidneys and surrounding organs and tissues.

Alterations to the electrophysiology of isolated human detrusor smooth muscle cells in bladder disease.

The aim of the study was to investigate the properties of the inward calcium current (iCa) obtained from isolated human detrusor smooth muscle cells in different pathological conditions using the whole-cell patch-clamp technique. Previous experiments on field stimulated detrusor muscle strips have shown specific differences associated with hypertrophy and detrusor instability. As iCa plays an important role in excitation-contraction coupling in this tissue, it was therefore of interest to determine if iCa was altered in the above disease states. It was found that myocytes from patients with prostatic symptoms had a significantly increased surface area and that the kinetics of iCa were also significantly altered in these cells. Although the surface area of cells from patients with idiopathic instability and detrusor hyperreflexia was also significantly increased, the kinetics of iCa were altered in different ways. These alterations to iCa will alter the action potential configuration and its propagation through the muscle mass.
