[Argentina as a destination country. Spanish emigration from 1860 to 1930].

"A survey of Spanish immigration in Argentina as compared to other Latin American destinations between 1860 and 1930 is presented here. Statistical sources are examined and compared (inclusions and omissions) and an estimation of migratory balance is presented. Regional differences in emigration flow and variations following shifts in economic trends and in migration policies are considered in [the] search for an explanation [of] the Spanish emigrants' preference for Argentina." Labor migration amongst hierarchically competing and intervening origins and destinations.

"A spatial interaction methodology is developed for modeling flows in a hierarchical system. A competing and intervening destinations framework is employed to model and predict U.S. state-to-state labor migration. This analysis is used to assess the importance of geographic variables in explaining variations in regional labor flows. Empirical findings suggest that U.S. labor migration is largely explained by...size, distance, locational accessibility, and intervening opportunities in a spatial hierarchy. It is also suggested that lagged migration or migrant stock is a product of the combined effect of these forces."

Pathways and destination of some male gland secretions in female Locusta migratoria migratorioides (R&F) after insemination.

The white secretions (WS) from the tubules of the male accessory glands (AG) of Locusta migratoria are composed of peptides and proteins. The WS are transferred during mating to the female's spermatheca. They have been followed to their destinations with immunological and radioactive marker techniques. In the spermatheca, peptides are split off from WS-protein complexes, permeate the spermathecal epithelium via glandular cells, enter the hemolymph and attach to other proteins in various target organs such as the dorsal fat body, the preterminal/terminal oocytes, and the follicular cells. In developing eggs, they concentrate at the posterior pole where sperm enters the egg, and in early embryogenesis they are found in the germ band. These results extend the functions of the spermatheca and the role of the male during the reproductive process.

Determinants of discharge destination following elective total hip replacement.

OBJECTIVE: To identify the factors associated with discharge to an inpatient rehabilitation facility versus discharge directly to home after elective total hip replacement (THR). METHODS: Data were obtained from a medical record review and a cross-sectional survey of a population-based cohort of patients who received elective primary or revision THR in 1995. Postoperative functional status was measured as the ability to walk independently prior to discharge. A multivariate regression model was developed with discharge to an inpatient rehabilitation facility versus directly to home as the dependent variable. The model adjusted for demographic characteristics, socioeconomic features, and several preoperative and postoperative clinical factors. RESULTS: We included 1,276 patients age 65-94 years in the analysis. Of these, 58% were discharged from the acute care hospital to a rehabilitation facility. The cohort had mean age of 73 years; 96% were white; 62% were female; 32% were living alone; 38% had an annual income less than $20,000; and 78% were unable to walk independently in the hospital before discharge. For primary THR patients, multivariate analysis showed a significant association between being discharged to a rehabilitation facility and being unable to walk at discharge, older age, obesity, and living alone. For revision THR patients, multivariate analysis also showed a significant association between being discharged to a rehabilitation facility and being unable to walk at discharge. CONCLUSION: In a large population-based sample, postoperative functional status, age, obesity, and social support all influenced the discharge destination following elective THR.

Fibroblast growth factor and culture in monolayer rescue mesoderm cells destined to die in the developing avian wing.

In an effort to elucidate control mechanisms for developmentally programmed cell death, conditions were sought that rescue the cells destined to die. Three areas of mesodermal cell death in the chick wing were examined: the posterior necrotic zone (PNZ), the opaque patch (OP), and apical mesoderm. The PNZ and OP are areas of normally programmed cell death, whereas the apical mesoderm undergoes cell death only after the overlying apical ectodermal ridge is excised. Cell death in vitro was quantitated using the chromium-release assay. While these tissues undergo apparently normal cell death in organ culture, in monolayer culture almost all are rescued. In addition, the cells are rescued by the addition of fibroblast growth factor to organ cultures. Since fibroblast growth factor is present in decreasing amounts in the limb at this stage of development, normal cell death may occur upon withdrawal of growth factor.

Timing of differential amplification of macronucleus-destined sequences during macronuclear development in the hypotrichous ciliate Euplotes crassus.

The change in copy numbers of macronucleus-destined gene sequences was followed in anlagen DNA during postconjugational development in Euplotes crassus. As noted earlier, copy numbers increase during the polytene stage. During this replication process major differential amplification of different genes is not observed. Instead it is only achieved during or shortly after the fragmentation of the polytene chromosomes. This process is not totally synchronous with respect to different genes. Highly amplified genes are excised earlier than genes with a low final macronuclear copy number. Unexpectedly, the pattern of processing of the newly added oversized telomeres also appears to correlate with the degree of gene amplification. These observations are discussed in terms of a limited replication period after polytene chromosome fragmentation leading to preferential amplification of early excised genes.

Destination, future intentions and views on practice of British-based pharmacists 5 and 10 years after qualifying.

OBJECTIVES: The first ever Pharmacy Workforce Census of pharmacists in Great Britain (GB) was conducted in 2002, and repeated in 2003. Both census surveys aimed to gather empirical data on the employment profile of pharmacists to aid the workforce planning process. The aim of this paper is to compare the work profile and employment destination of two graduate cohorts of pharmacists to explore what changes take place in employment practices and how quickly they occur. SETTING: GB-based pharmacists. METHODS: A two-page postal questionnaire was sent to 38,000 GB-registered pharmacists in August 2003, to provide various data on employment patterns, intentions to work abroad, and desire to practise pharmacy. The pharmacists were contacted using addresses stored on the Pharmaceutical Register, a statutory record of all pharmacists and pharmacy premises held by the Royal Pharmaceutical Society of Great Britain (RPSGB). A sub-set of this large data set--pharmacists who qualified in 1992 and those who qualified in 1997--was selected for comparison. MAIN OUTCOME MEASURE: Sector of employment and strength of desire to practice. RESULTS: A response rate of 75% was achieved for the overall census, while the response rate from the two graduation cohorts was 62% and 58%. Larger proportions of women compared with men, even after only 5 years of working as a pharmacist, are either not working or work part-time. Among the women, these patterns increase significantly for those with a greater number of years on the Pharmaceutical Register. There is evidence that pharmacists move out of the two main sectors of practice (hospital and community) with increasing years on the Register, and some gender differences in job mobility are observed. The School of Pharmacy from which pharmacists graduate appears to have some effect on practice patterns although more research is needed to explore this further. Desire to practise pharmacy is weaker among those practitioners who work in the community sector of practice, and younger pharmacists are more likely to intend working abroad. CONCLUSIONS: Part-time work patterns are implemented fairly quickly after qualifying, particularly among the women pharmacists. Given that female students account for over 60% of all intake onto pharmacy courses current supply problems will continue if work patterns continue along this trajectory. The ability of the profession to meet current, never mind extended roles, is thus called into question.

Origin and destination entropies of U.S. 1965-70 age-sex-specific intercounty migration flows.

"Eight independent pairs of entropy-based rankings of 3,140 U.S. county-level units are obtained. The criteria employed are the homogeneities of the distribution of 1965-70 in- and out-migrants of each county--for eight age-sex-specific groups--over the other 3,139 counties. The most homogeneous counties for males of ages 19-39 in 1970 consistently have major military facilities." The results show that "for those 65 and over, Florida, Arizona and Texas counties have the broadest in-migration, while Northern metropolitan counties have dispersed out-migration. The cross-correlations of entropies--though all strongly positive--are lowest between those 19-39 and those 65 and over, and relatively weak, in general, between those 65 and over and other groups. In all eight demographic groups, destination entropies reach higher values than origin entropies."

Synaptic organization of the cerebello-thalamo-cerebral pathway in the cat. III. Cerebellar input to corticofugal neurons destined for different subcortical nuclei in areas 4 and 6.

To analyze the cerebellar effects on corticofugal neurons destined for different subcortical nuclei, intracellular recordings were made from corticofugal neurons in areas 4 and 6 of the cat. Corticonuclear neurons to the red nucleus (RN) and the pontine nucleus (PN), and pyramidal tract neurons (PTNs) with collaterals to these nuclei were identified by their antidromic responses to the stimulation of these nuclei and the pyramid. Three types of RN-projecting neurons (corticorubral neurons (CRNs), corticopontine neurons (CPNs) with a collateral to the RN and PTNs with a collateral to the RN) and two types of PN-projecting neurons (CPNs and PTNs with a collateral to the PN) were differentiated. Furthermore, these corticofugal neurons were classified as fast and slow neurons on the basis of a critical axonal conduction velocity of 20 m/s. About 80% of 98 RN-projecting neurons in area 4 were PTNs, and among the rest, CPNs were more common than CRNs. A similar tendency of the frequency distribution of 37 RN-projecting neurons was also observed in area 6. In area 4, about 70% of 158 PN-projecting neurons were PTNs (80 fast and 30 slow PTNs) and the rest were CPNs, while in area 6, only 35% of 99 PN-projecting neurons were PTNs (10 fast and 25 slow PTNs). Among the CPNs in areas 4 and 6, slow CPNs were more frequently encountered. Cerebellar effects on these identified corticofugal neurons were investigated, using electrical stimulation of the brachium conjunctivum (BC). In both areas 4 and 6, a substantial number of fast conducting CRNs, CPNs and PTNs projecting to the RN or the PN received short-latency (predominantly disynaptic), large-amplitude EPSPs from the BC, and a considerable number of slow conducting neurons to the RN and/or the PN received longer-latency, smaller-amplitude EPSPs from the BC.

Using computer graphics to map origin-destination data describing health care delivery systems.

An interactive graphic computer program, called FLOWMAP, is used to examine the referral patterns of cancer patients in western Washington between 1974 and 1978. FLOWMAP allows one to produce several types of maps from origin-destination data easily and inexpensively. Several maps show some of the significant geographic components of the referral of more than 7000 cancer patients among 13 counties over a 5-year period. It is shown that cancer care has become more decentralized during this period. Nine other possible applications of FLOWMAP to issues of concern regarding the delivery of health care are described. Maps produced by FLOWMAP can be expected to cost 1/30 to 1/50 of the cost of comparable manual methods. An Appendix gives some technical detail regarding the use of FLOWMAP and its data requirements.

Motoneurons destined to die are rescued by blocking N-methyl-D-aspartate receptors by MK-801.

Motoneurons to soleus muscle die if their axons are injured at birth. We tested the possibility that their death may be caused by toxic effects of excitatory transmitters such as glutamate. Animals that had their sciatic nerves crushed at birth were treated either with a blocker of the N-methyl-D-aspartate receptor, dizocilpine maleate (MK-801), or saline. The number of surviving soleus motoneurons and motor units was assessed 60-90 days later. Treatment of rats with MK-801 rescued a large proportion of injured motoneurons destined to die. Moreover, the weight loss of soleus muscles seen after nerve injury at birth was reduced in animals treated with MK-801. These results suggest that motoneurons axotomized at birth are unable to withstand the excitotoxic effects of glutamate and die. Blocking glutamate receptors in conditions where motoneuron loss occurs could be an effective way of rescuing them.

Ischemic stroke hospital stay and discharge destination.

PURPOSE: Stroke hospitalizations are among the most expensive, with a mean length of stay (LOS) higher than other hospitalizations. This retrospective study assesses factors influencing ischemic stroke LOS taking into consideration the discharge destination of patients. METHODS: Linked hospital separation records between July 1995 and December 1999 were extracted to determine the first admission for ischemic stroke in Western Australia. Multiple hospitalization records for the same patient were screened to obtain the total duration of hospitalization. Demographic characteristics, hospital type, and medical history of patients were also retrieved. In the presence of censoring and without prior assumption on the time-to-discharge distribution, the Cox's proportional hazards model was used to assess the factors affecting LOS. RESULTS: During the study period, 6469 patients with a first-ever admission for ischemic stroke were identified, with average LOS being 28 days (95% CI, 26-30 days). Hospital stays were significantly longer for females and patients directly admitted to hospitals maintaining a specialist stroke unit, whereas patients residing in rural areas had shorter stays. CONCLUSIONS: The impact of stroke severity and placement in nursing homes after discharge need to be understood to manage LOS and the cost of acute care. Accurate diagnosis upon initial presentation would benefit both the efficiency of hospitals and the outcomes of rural ischemic stroke patients.

Left ventricular assist device as destination therapy in doxorubicin-induced cardiomyopathy.

Doxorubicin-induced cardiomyopathy is not uncommon and may progress to end-stage heart failure. Treatment of this condition with heart transplantation, however, requires that the primary malignancy be deemed "cured." We present the case of a 55- year-old woman who had doxorubicin-induced cardiomyopathy and non-Hodgkin's lymphoma. The active status of her lymphoma precluded heart transplantation. She had end-stage heart failure and underwent the insertion of a left ventricular assist device as a destination therapy.

Disease, destination, dose and delivery aspects of ciclosporin: the state of the art.

Since its discovery in 1971, ciclosporin has revolutionized organ transplantation and the treatment of autoimmune disorders. The wide array of applications resulting from its clinical efficacy warrant unique administration strategies and varying doses, times of exposure and extents of distribution, depending on target tissue. The poor biopharmaceutical characteristics of low solubility and permeability makes this uphill task even more challenging for the drug delivery scientist. Efforts underway have explored various body routes employing approaches like emulsions, microspheres, nanoparticles, liposomes, iontophoresis and penetration enhancers. This review attempts a brief holistic view of the "four Ds" (disease, destination, dose and delivery) surrounding this immunomodulator drug.

Same bridge, new destinations rethinking paradigms for mechanical cardiac support in heart failure.

Mechanical cardiac support with ventricular assist devices is an established therapy for a variety of clinical scenarios, including postcardiotomy shock, "bridge to transplant," and "destination therapy." At present, device development, clinical trial design, regulatory approval, and reimbursement decisions for the clinical application of mechanical cardiac support devices continue to be considered in the context of these clinical indications. Although understandable from a historical perspective, these arbitrary divisions are inconsistent with the clinical realities of advanced heart failure therapy. By narrowly focusing on transplant eligibility at a static point in the clinical course, current guidelines impede the broader application of ventricular assist device technology to the growing population of patients who may benefit from this therapy.

Protein targeting to destinations of the secretory pathway in the malaria parasite Plasmodium falciparum.

The secretory pathway in the malaria parasite Plasmodium falciparum has many unique aspects in terms of protein destinations and trafficking mechanisms. Recently, several exciting insights into protein trafficking within this intracellular parasite have been unveiled: these include signals that are required for targeting of proteins to the red blood cell and the relict plastid (known as the apicoplast); and the elucidation of the pathways of the haemoglobin proteases targeted to the food vacuole. Protein-targeting to the apical organelles in P. falciparum, however, is still not very well understood, but available research offers a tantalising glimpse of the system.

Destination of titanium particles detached from titanium plasma sprayed implants.

Small titanium particles may detach from titanium plasma sprayed (TPS) implants during implant insertion, when no preliminary tapping is used, probably for the frictional force between titanium coating and host bone. Aim of this study was to investigate the destination of these titanium particles observed in the peri-implant environment. Twenty-four TPS screws were implanted in tibiae of two sheep. Fourteen and 90 days after implantation the implants with the surrounding bone were removed and processed to be analyzed by light microscope and scanning electron microscope (secondary electron and back-scattered electron probes). Small titanium particles detached from the unloaded TPS implants were observed both in the newly-formed bone matrix and in marrow tissue. Histomorphometric analysis showed that both at 14 and 90 days after implantation the titanium particles appeared more concentrated in marrow tissue than in calcified bone matrix, decreasing by 66.4% over time. In particular, smaller particles (<250 microm(2)) decreased by 81.5%, whereas the larger ones (250-2000 microm(2)) did not show any significant variations over time, suggesting that most of the smaller particles may undergo to ionic dissolution, probably migrating into the peri-implant marrow lacunae. A slight migration of titanium particles from the implant surface towards the more distant peri-implant tissues was also demonstrated over time.
