Pancreatobiliary or pancreatodigestive fistulae are rare during the course of intraductal papillary mucinous tumors. The mechanism of the fistula is either malignant involvement or mechanical. We report the case of an 81-year-old patient with intraductal papillary mucinous tumor complicated by fistulae between the main pancreatic duct and common bile duct, duodenum and stomach. These fistulae were treated endoscopically and radiologically by several metallic stents.

Somatostatin and its synthetic analogues have important physiological roles in human. These compounds antagonize the endocrine and exocrine secretion of pancreas, which provide theoretical ground for their use in patients with pancreatic diseases. For a quarter of a century many researchers have investigated the effect of somatostatin and octreotide in animal models of experimental pancreatitis and in patients with acute pancreatitis, and in those with complications of pancreatic operations, fistulas and pseudocysts. In most of these studies the use of synthetic octreotide with longer duration proved to be more efficient. In experimental and acute pancreatitis somatostatin and octreotide were less effective than expected, however, they proved to be helpful in preventing complications of pancreatic operations and in the therapy of fistulas and pseudocysts.

OBJECTIVE: The authors, thanks to experience obtained in an Unit for the treatment of digestive fistulas, discuss the possibility of a conservative treatment for the anastomotic fistulas. MATERIAL AND METHODS: From 2000 to 2003 were treated thirty-five patients with post-anastomotic gastroenteric fistulas marked according to their localization, way end output (51.5% high, 42.8% moderate and 5.7% low). The treatment is based on an aspiration system, sometimes integrated with an irrigation system. A semi-permeable barrier was created over the fistula by vacuum packing a synthetic, hydrophobic, polymer covered with a self-adherent surgical sheet. This system create a vacuum chamber equipped with a subathmospheric pressures between 262.2 and 337.5 mmHg (350-450 mmbar), integrated with a continuous irrigation using antibiotic solutions or 3% lactic acid. RESULTS: The AA. obtained the resolution in 30 patients (85.7%), 3 patients needs the surgery (8.6%), 2 died, one for sepsis and the other one for malnutrition. The mean time for the closure was 45 days (from 20 to 90). A part of digestive external fistulas goes to spontaneous resolution so comes the idea that the creation of particular condition is the basis of their closure.
BACKGROUND: The open abdomen, or laparostomy, is becoming increasingly used in the management of critically ill surgical patients. METHODS: The published work on laparostomy is reviewed, in the light of personal experience, with particular attention to the history and pathophysiology associated with laparostomy. RESULTS AND CONCLUSION: The combination of an inert plastic sheet in contact with the viscera, and the application of subatmospheric pressure on the wound, is an effective combination to maximize the prospects of delayed primary wound closure while minimizing the chance of fistula and ventral hernia.
It is important to find a method for promoting spontaneous closure of gastrointestinal fistulas with fibrin sealant. Twenty-seven patients presented with gastrointestinal fistulas underwent the selective fibrin sealant occlusion method under fistulography. Fistula occlusion was obtained within 2 weeks in 25 of the 27 patients, and there has been no sign of recurrence over a follow-up period of 1-17 months (average, 5.6 months). It is concluded that selective occlusion under fistulography is highly effective for gastrointestinal fistulas.
