Causes of morbidity among a sample of elderly hospital patients in Riyadh, Saudi Arabia.

The causes of admission to hospital over a 5-year period of 3539 persons aged 60 years and above in Riyadh, Saudi Arabia have been analysed; 54.2% were males and 45.8% females and 68.5% were aged 65 years and above. The causes of morbidity were chronic degenerative disorders of which cardiovascular diseases were the most frequent followed by acute respiratory problems, diabetes, and digestive and neoplastic diseases. The pattern of disease was very similar to that in the industrialized countries. The median stay in hospital was 10.7 days. Respiratory diseases and diabetes mellitus were significantly higher in females than males (P < 0.02), while cardiovascular diseases, particularly ischaemic heart disease and heart failure, as well as malignant neoplastic diseases mainly of the digestive system, were more prevalent in males.

[Means to improve therapeutic care in the Armed Forces].

The article describes dynamics of diseases of therapeutic field in the Armed Forces in 1991-1996. The analysis of therapeutic sick rate has confirmed, that a major problem of military therapy remains acute pneumonia, which sick rate in 1993-1996 has increased twice. The sick rate of ulcer has increased equally. The main directions of perfection of organization of the therapeutic care in the Armed Forces are etiologic diagnostics and prophylaxis of acute pneumonia in the soldiers, creation of system of prophylaxis, active revealing and centralized high-specialized treatment of the patient with ischemic heart disease among officers, perfection of system of rendering of the specialized care to the patient of endocrinological field, introduction of modern methods of anti-relapse treatment of ulcer, active revealing of the persons, infected by virus of hepatitis, perfection of work of an out-patient department link.

Mechanisms that lead to disease of the endocrine system in animals.

Endocrine glands are collections of specialized cells that synthesize, store, and release their secretions directly into the blood stream. They are sensing and signalling devices located in the extracellular fluid compartment and are capable of responding to changes in the internal and external environments to coordinate a multiplicity of activities that maintain homeostasis. Diseases of the endocrine system are encountered in many animal species and present challenging diagnostic problems. The major pathogenic mechanisms responsible for disturbances in endocrine function include: 1) primary hyperfunction of an endocrine gland; 2) secondary hyperfunction; 3) primary hypofunction of an endocrine gland; 4) secondary hypofunction; 5) endocrine hyperactivity secondary to diseases of other organs; 6) hypersecretion by nonendocrine tumors of hormone-like substances; 7) failure of fetal endocrine function; 8) endocrine dysfunction due to failure of target cell response; 9) endocrine dysfunction resulting from abnormal degradation of hormone; and 10) iatrogenic syndromes of hormone-excess. For each major category, several specific disease problems have been selected to illustrate the morphologic and functional changes that characterize the response of a particular endocrine gland to disruption of function.

[Coenzyme Q--clinical significance].

In the paper the clinical importance is presented of the use of coenzyme Q (ubiquinone) preparation. On the basis of available literature the participation of coenzyme Q was determined in the pathogenesis and aetiology of a number of diseases, e.g. diseases of the circulatory system, locomotor system, diabetes mellitus, obesity or endocrine system diseases.

Diseases of aging.

By definition, diseases of aging become clinically manifested in elderly patients. However, their pathogenetic basis has to be sought earlier in life. The general thread of this presentation relies on the concept of an evolutionary-Darwinian view of the development of age-related diseases. In essence, this concept states that we may have to "pay" for genetic traits that play a beneficial role earlier in life by the later development of diseases since there is no post-reproductive selective pressure that may have eliminated the potential late onset detrimental effects of such genes. Examples for this kind of trade-off are taken from diseases involving the immune system (infections), the endocrine system (andropause), the nervous system (Alzheimer's disease), the locomoter system (osteoporosis), the cardio-vascular system (atherosclerosis) and cancer.

[Diagnosis and therapy of diseases of the endocrine system: new trends in nuclear medicine].

Diagnosis and therapy of endocrine disorders in nuclear medicine has been improved through the implementation of new techniques especially with positron emission tomography (PET). In modern concepts of parathyroid gland surgery an exact anatomic localisation of adenomas is necessary, which may be achieved with MIBI-Scintigraphy being the most sensitive method in primary hyperparathyroidism. The optimal access to localise adenomas is the investigation with combined SPECT/X-CT systems. The use of such systems for diagnosing neuroendocrine tumors of the gastrointestinal system may also be helpful. For neoplasms of the adrenal gland PET systems could be used to differentiate between benign and malignant entities or to detect primary tumours. In case of incidentalomas J131-Norcholesterol and MIGB-scintigraphy has been proven helpful. Indications for nuclear medicine studies to detect abnormalities of the hypothalamic-hypophyseal system are established rarely.

Pharmacologic management of patients with diseases of the endocrine system.

Endocrine disorders are commonly seen in the general population. This article discusses the physiology, pathogenesis, and the medical and dental management of patients with thyroid, parathyroid, and pituitary gland disorders. In addition, the pharmacologic treatment, common side effects, and dental drug interactions of concern are discussed.
