[Immunologic monitoring in transplantation of hydrophilic polyacrylamide gel].

There was no significant difference in the level of basic immunological indices between those female patients having received a breast prothesis with making use of the polyacrilamide gel (PAAG) interfall and controls, subjects who had not received a breast prothesis, and group of patients in whom the gel was administered by injection, which fact corraborates the conclusion about immunological tolerance of the gel, with no dependence on the way in which it is used being noted.

Postoperative immunologic monitoring of cancer patients undergoing radical surgery.

Immunologic parameters of cellular immunity in vitro and circulating immunoglobulin levels were evaluated before and after radical surgery in twenty-five patients with localized primary solid tumors. Peripheral blood lymphocyte counts, T-lymphocyte counts and lymphocyte blastogenesis after optimal stimulation with phytohemagglutinin were found significantly depressed at the time of diagnosis. Sequential determinations of immunologic parameters during the postoperative period showed a slight fall of lymphocyte counts and T-lymphocyte counts; subsequently these cell counts progressively returned to preoperative values. The blastogenic response of lymphocytes sharply decreased following surgery and then slowly returned to preoperative levels in one month. No significant alterations of circulating immunoglobulins were detected at diagnosis or during the postoperative period. These findings indicate that patients with localized tumors present impairment of some parameters of cellular immunity preoperatively and that postoperative immunodepression lasts about one month.

[Epidemiological and immunological monitoring of tetanus in Georgia (Republic)].

During the period of 1961-1990 the regular immunological and epidemiological monitoring of tetanus was carried out. At the prevaccination period (1937-1960) 140 cases of tetanus were registered, 77 patients died (morbidity rate being 0.97-0.11 per 100,000 of the population with mortality rate equal to 55.6%). At the period of mass vaccinal prophylaxis (1960-1990) 56 cases were registered and 27 patients died (the corresponding values: 0.09-0.11 with 48.2% mortality). In 1981-1990 altogether 53,213 vaccinees were examined. 4,864 vaccinees (8.9%) proved to be seronegative and 43,615 vaccinees (81.9%) were found to have protective antibody titers. 4,220 pregnant women were examined; 476 women (11.2%) proved to be seronegative and 3,576 women (84.3%) were found to have protective antibody titers. Timely specific prophylaxis proved to be the only way for decreasing tetanus morbidity.

Complex immunological monitoring of breast cancer patients treated postoperatively by electron beam irradiation.

To monitor the electron beam therapy some immunological parameters of breast cancer patients previously undergone surgery were tested before, during and after irradiation. Immune complex levels measured by complement consumption technique were not altered by irradiation. Killer cell activity tested in so-called antibody dependent cellular cytotoxicity (ADCC) capacity assay showed a marked decrease in some cases. Based on the phagocytic capacity of the granulocytes the patients could be divided into two groups: one with declining activity and another with rising activity: The majority of the patients (22/45) were humoral leukocyte adherence inhibition (H-LAI) negative before and during irradiation. Those showed positive H-LAI indices before electron therapy had unchanged (7/45) or decreasing (9/45) tendencies during the observation period. Further study is needed to establish the clinical relevance of these in vitro assays used by us.

Immunologic monitoring of the beneficial effect of one HLA-DR-shared blood transfusion in heart transplant patients.

BACKGROUND: Pre-transplant blood transfusion (BT) results in better graft survival in organ transplant recipients, especially when BT donor and allograft recipient share an HLA-DR antigen. Although the immunologic mechanisms involved are still poorly understood, we wanted to know whether down regulation of donor-reactive T cells play a role. METHODS: In a retrospective study, we analyzed the clinical effects of BT for 45 heart transplant (HTx) patients who had each received 1 BT that shared an HLA-DR-antigen with the recipient, and 55 who had a DR-mismatched BT before heart transplantation. From 30 patients, 15 with DR-shared BT and 15 with DR-mismatched BT, peripheral blood lymphocytes (PBL) were available. From each patient, we analyzed PBL samples taken at the day of transplantation (pre-transplant), and 1 to 2 months, 5 to 7 months, 9 to 14 months, 2 years, and 6 to 7 years after transplantation. Cytotoxic T-lymphocyte precursors (CTLp) and helper T-lymphocyte precursors (HTLp) were measured in a combined limiting dilution assay. RESULTS: Analysis of survival during the first 10 years revealed a significantly (p = 0.016) better survival rate in the group of patients who had received HLA-DR-shared BT compared with the group who had received HLA-DR-mismatched BT. Patients of the DR-shared group experienced significantly (p = 0.042) less acute rejections compared with the patients who received DR-mismatched BT. We found no differences in the development of graft vascular disease. Frequencies of CTLp specific for the organ donor did not change with time after transplantation in the individual patients, nor did we detect any differences between the two BT groups. We found the same for organ donor-specific HTLp frequencies. CONCLUSIONS: These data suggest again that transfusion effect depends on HLA-DR compatibility between the heart transplant recipient and the pre-transplant BT donor. The mechanism that caused better survival rate was not down regulation of the donor-reactive T-cell frequency.

[3-year results of clinical and immunological monitoring of patients with multiple sclerosis treated by copaxone].

Tipe 1 T-helper cells are of importance in development of multiple sclerosis (MS). Copaxone (TEVA, Israel) is one of the preventive drugs, which modifies T-helpers activity. Thirty-three patients with MS were treated by copaxone for 2 years and 13 patients--for 3 years. Clinical and immunological parameters were examined: amount of CD3+, CD4+, CD8+, CD16+, CD22+, CD95+, CD25+ lymphocytes, HLA-D+ cells, IgG, IgA, IgM and phagocytosis. During the treatment course, mean rate of relapses decreased from the pretreatment value from 1.26 +/- 0.11 to 0.59 +/- 0.11 in the first year and 0.28 +/- 0.07 in the second one. After 3 years no relapses were observed in 13 patients. Relapse rate was the highest in the first 3-6 months. But after 3 months there was an increase of CD22+ cells that persisted for all the period of the study. After a year of the treatment, a level of IgM, IgG, IgA and CD95+ cells has raised significantly, with decreasing of a number of CD4+ and CD8+ cells. The authors suggest that immunological shift is related to mechanism of copaxone action determining its positive effect i.e. reducing of frequency of MS relapses.

Neonatal disease in neutral endopeptidase alloimmunization: lessons for immunological monitoring.

Neutral endopeptidase (NEP) alloimmunization has recently been determined to cause severe forms of neonatal disease as a result of the transplacental passage of anti-NEP antibodies. However there is a wide spectrum of neonatal disease variability. We present the medical histories of a large family, specifically of two alloimmunized sisters in their second pregnancy in whom we established the basis of immunological surveillance and therapeutic intervention during pregnancy and after delivery. One mother developed dramatically high titers of IgG1 and IgG4, and was treated with IvIg and one plasma exchange, both of which substantially reduced the anti-NEP Ab titer. However, the neonatal syndrome observed in her infant was severe, partly due to treatment delay. Anti-NEP Ab were also found in the mother's milk and the infant's urine. In contrast, the other mother had a normal second pregnancy and delivered a healthy neonate, which was related to the fact that she only produced the non-complement activating IgG4 subclass of anti-NEP antibodies. Thus, anti-NEP Ab (titer and subclass) seem to be highly sensitive biomarkers of neonatal risk. Interventional strategy aimed at reducing anti-NEP titer, should be started early during pregnancy and, possibly, even before pregnancy in those mothers producing anti-NEP IgG1. Careful monitoring of anti-NEP Ab titer and subclass is mandatory in NEP-deficient mothers during their pregnancies.

[Immunologic monitoring of leptospirosis in the Maritime Territory].

The epidemiological and epizootic situation in Leptospira infections at the Maritime [correction of Primorski] Territory is evaluated on the basis of complex studies carried out in 1984-1989. As revealed in these studies, cases of leptospirosis among humans have a sporadic character and are mainly registered among professional high risk groups of the population. In the immunological structure of persons covered by the survey L. hebdomadis, L. pomona and L. javanica prevail. The anthropourgic foci of leptospirosis caused by L. pomona are of the leading epidemiological importance. Swine serves as the main source of infection in these foci. The study revealed the epidemic danger of the natural foci of leptospirosis caused by L. grippotyphosa and L. javanica in rice fields where the decisive factors of leptospirosis proved to be reed voles and striped field mice serving as reservoirs of this infection, as well as the synanthropic foci of leptospirosis caused by L. hebdomadis with house mice serving as the main carriers.

Heart transplantation model as an immunological monitor.

Various techniques have been described for the heart model, from which we standardized two. These are simple to perform and offer the possibility of a precise determination of rejection times. This is an advantage compared to other models, such as the kidney model, which is still in use, a rejection reaction, the recipient stays alive, making the recipient available for further experiments. One disadvantage of the heart model is that histological examinations are impossible without disturbing the heart function. Thus far, responsible laboratory parameters for rejection reactions could not be differentiated for confirmation of subjective diagnoses. However, we consider this model to be useful in investigating immunological questions.

Pre-transplantation immunological monitoring and graft survival in recipients of renal allografts.

Various immunological parameters were measured pre-transplantation in 82 renal transplant recipients. The results were compared with the clinical course of the recipient post-transplantation and with the results of 40 controls. Only one test C3 inactivation products (C3i) was associated with transplant outcome in that 0/30 patients with no rejection episodes had C3i whereas 9/38 patients with rejection episodes, including 3/12 patients whose graft failed, had C3i.
