[A case of multicentric Castleman's disease with multiple pulmonary nodules].

A 55-year-old woman visited our hospital for further examination of abnormal shadows on chest radiographs. Her routine chest radiograph showed two nodular shadows in the right lower lung field. A chest CT scan revealed other nodules, small patchy shadows in both lung fields, and enlargement of the mediastinal lymphnodes (#2, 3). Laboratory data showed polyclonal hyperimmunoglobulinemia. This case was initially considered on the basis of a transbronchial lung biopsy to be a plasma cell granuloma. However, serum gammaglobulin levels gradually increased, and video-assisted thoracoscopic surgery was performed to aid in making a definite diagnosis. Biopsy specimens revealed lymphoid follicles with plasma cells which were stained with both anti-kappa chain and anti-lambda chain antibodies. The patient was treated with prednisolone (50 mg/day), and the serum gammaglobulin level and the shadows on the chest CT were temporarily slightly improved. During the clinical course, her laboratory data and histological specimens were re-examined, and the final diagnosis was multicentric Castleman's disease.

[Multiple pulmonary nodules and amiodarone. KL-6 as a new diagnostic tool].

Pulmonary toxicity is an infrequent but serious adverse event in patients treated with amiodarone. The main problem at present is that we lack the necessary tools to detect this event or predict which patients will develop pulmonary toxicity. Serum Krebs von den Lungen-6 (KL-6) has been previously recognized as a marker for the activity of diffuse interstitial lung disease. We describe a patient with pulmonary toxicity due to amiodarone with increased blood levels of this new marker, and discuss the clinical usefulness of this new diagnostic tool.

Multiple pulmonary nodules in leukemia.

A patient with acute myelogenous leukemia, in clinical remission following chemotherapy, presented with three unusual pulmonary nodules. Needle biopsy showed that these were leukemic cells in the interstitium of the lung.

[Intrapulmonary sarcoid, presenting as tumor or as multiple pulmonary nodules with and without cavities (author's transl)].

Eight cases of nodular, partially cavitary intrapulmonary--mainly acute--sarcoid mimicking metastatic disease or cavitary disease of other etiology are presented to point out the fact that sarcoid can occur in this form. Almost all these patients were young and asymptomatic with the lesions discovered incidentally on the chest films taken for other reasons. The pulmonary nodules and cavities present differential diagnostic problems; with the concomitant--or previous--presence of hilar or mediastinal lymph-adenopathy and the finding of unsharp borders of the nodules somewhat supporting the diagnosis of sarcoid.
