[Periarthritis humeroscapularis (PHS). Indications, technique and outcome of radiotherapy].

BACKGROUND: Radiation treatment for patients with acute or subacute tendonitis or bursitis of the shoulder once commonplace has been mostly supplanted by antiinflammatory drug treatment. PATIENTS AND METHODS: 106 patients with clinically evident disease of PHS were treated between January 1987 and May 1991. 73/106 patients had a continuous follow-up. 16/73 patients showed typical symptoms on both shoulders: therefore a total of 89 shoulders were examined in this investigation. A daily dose of 0.5 Gy was given for 3 times a week and a total dose of 3.0 Gy. After 8 weeks a second treatment course was performed. In follow-up the subjective (pain) and objective (limitation of abduction) symptoms were classified in 3 grade scales. RESULTS: A marked pain reduction and an improved mobility (abduction of at least 20 degrees more than before radiation) could be observed in 72/89 shoulders (81%). 44/89 shoulders (49%) showed an excellent response and were completely free of pain and impairment of motion. CONCLUSION: Radiotherapy of acute or subacute Periarthritis humeroscapularis is a very effective treatment if radiation starts within the first year of symptoms and if it is supported by physiotherapy.

[The cerebral hemodynamics in patients with humeroscapular periarthritis under the influence of decimeter waves and deresinated naphthalan].

110 patients with scapulohumeral periarthritis of vertebrogenic origin were exposed to decimetric waves or received salt-free naphthalan. There were also patients who got combined treatment with decimetric waves and salt-free naphthalan. REG recorded positive shifts in cerebral hemodynamics due to these factors utilization.

Acute calcific periarthritis of the finger joints: a syndrome of women.

We describe a 33-year-old woman with acute calcific periarthritis (ACP) of the interphalangeal joint of the thumb and review 42 reported cases of ACP involving the finger joints. A computer assisted literature search for reported cases of ACP involving the finger joints was performed. Clinical features of our case and those fulfilling the criteria for entry into this study were analyzed.

[Periarthritis humeroscapularis--clinical diagnosis and analysis of the syndrome concept].

Based on the recent knowledge on shoulder pathology the diagnosis of periarthritis humeroscapularis is no longer considered to be adequate. In this article the different pathologies are presented. For all important clinical entities the clinical and functional tests are shown, which are helpful to differentiate the different pathologic features.

Calcific periarthritis of the finger verified by X-ray diffraction of apatite.

The authors describe a 24-year-old woman presenting with swelling and pain of several finger joints. Roentgenograms showed periarticular, calcific deposits at all locations of swelling. One such deposit revealed the presence of hydroxyapatite crystals on X-ray diffraction. After two injections of corticosteroids in and around one deposit its size and density diminished rapidly, whereas the uninjected corresponding site of the other hand showed no initial improvement; after two years no difference was discernible. Under systemic and topic NSAIDs, pain and swelling improved similarly in all joints. In conclusion, periarthritis of the fingers associated with radiopaque deposits responded favourably to NSAID therapy, while local corticosteroid injections showed no additional benefit.

[Osteosynthesis with a rod in crushing injuries of the knee joint and periarthric fractures of the femur and crural bones].

The authors describe the treatment of 14 patients having crushing injuries of the knee joint of periarthric fragmentation fractures of femoral and crural bones with a wide zone of soft tissues damage. Economic resection of the joint with subsequent formation of a juncture between the femoral and tibial bones with a long rod (65--75 cm) seems to be mostly rational.

Acute calcific periarthritis of the hand and elbows in women. A study and review of the literature.

OBJECTIVE: To study the clinical and radiological features of acute calcific periarthritis (ACP) of the hand and elbow in women and to review the literature. METHODS: A retrospective study of ACP of the hand and elbow was conducted in 10 women, and the literature since 1970 reviewed, using Medline data. RESULTS: Ten women fulfilled the criteria of a single focus ACP. Mean age was 36.5 years (range 25-53). X-ray diffraction in 3 patients who underwent aspiration biopsy showed a pattern of calcium hydroxyapatite. The clinical course was short and self-limited. Most cases mimicked septic conditions and were initially misdiagnosed. All patients demonstrated gradual resolution of calcific deposits after 1-3 weeks. CONCLUSIONS: We conclude that calcific tendinitis of the hand and elbow is a self-limiting disease affecting mainly pre or perimenopausal women.
