CT assessment of the effect of fluticasone propionate aqueous nasal spray treatment on lower turbinate hypertrophy due to vasomotor rhinitis.

CONCLUSION: Fluticasone propionate (FP) aqueous nasal spray was objectively found to be effective and safe for the treatment of lower turbinate enlargement in patients with vasomotor rhinitis. OBJECTIVE: To assess the efficacy of FP aqueous nasal spray treatment in lower turbinate hypertrophy due to vasomotor rhinitis using CT. MATERIAL AND METHODS: Of 35 patients with hypertrophic lower turbinates due to vasomotor rhinitis, 20 were treated twice daily with FP aqueous nasal spray (200 microg/day) for 3 months continuously and 15 were treated with placebo vehicle as a control group. The local effect of the nasal spray was studied using CT and visual analog scales. RESULTS: Treatment with FP provided significantly greater relief from the symptom of nasal obstruction compared with placebo over the entire 3-month treatment period (p < 0.001). When the change from baseline was compared between the two groups, FP produced statistically significant reductions in the mucosal area of the lower turbinates and in the thickness of the nasal mucosa after 3 months (p < 0.05).

[Study of the changes in microcirculatory bed in children with vasomotor rhinitis based on biomicroscopy of conjunctival blood vessels].

Thirty-eight patients with vasomotor rhinitis, aged 6 to 15 years, were examined by means of biomicroscopy of eyeball conjunctiva vessels. As a result, the following microcirculatory disorders were detected: vessel enlargement, diameter nonuniformity, pathological convolution and red blood cell aggregation. Patients with allergic lesions showed most significant structural changes. The prevalence of functional changes over structural ones in the eyeball conjunctiva of children with neurogenic vasomotor rhinitis can be used as a differential diagnostic indicator.

Ultracytochemical localization of the NADPH-d activity in the human nasal respiratory mucosa in vasomotor rhinitis.

OBJECTIVES: Description of the ultrastructural localization of nitric oxide synthase in the blood vessels of the nasal respiratory mucosa in patients with vasomotor rhinitis. STUDY DESIGN: This research was conducted on seven patients--men and women, ages 20 to 45 years--suffering from vasomotor rhinitis and who had undergone surgical therapy for reduction of the inferior turbinates. METHODS: To study the ultrastructural localization of nitric oxide synthase, NADPH-diaphorase cytochemistry was employed. Samples of the nasal mucosa were obtained from inferior turbinates. RESULTS: The endothelial cells of the arterioles, capillaries, venules and cavernous sinuses revealed a distribution of the enzymatic activity similar to that found in unaffected subjects. A strong enzymatic activity was recognized in the smooth muscle cells of the cavernous sinuses. The smooth muscle cells of arterioles and venules were generally found to be negative to enzymatic reaction. CONCLUSIONS: This study suggests that the vascular disorders of the vasomotor rhinitis depend, at least in part, from nitric oxide synthase induction in the smooth muscle cells of the cavernous sinuses.

Beneficial effects of intranasal applications of capsaicin in patients with vasomotor rhinitis.

Capsaicin, a nonenamide derived from Capsicum plants, has proven useful in patients with vasomotor rhinitis. In the present study, we studied the effects of 15 micrograms capsaicin suspended in 100 microliters solution in patients with known vasomotor rhinitis. Drug was given 3 times/day for 3 days to each patient by means of a spray delivered to the nasal mucosa. Acute effects induced by the drug included painful sensation and secretion of nasal fluid but were no longer observed at the last capsaicin application. Patients recorded their symptoms over a 1-month period. The mean symptom score involving nasal obstruction and nasal secretion was markedly reduced by capsaicin treatment. We advance the hypothesis that the beneficial effect of drug treatment may be due to its specific action on the peripheral endings of primary sensory neurons leading to their functional blockade. Further randomized, double-blind, placebo-controlled trial is now needed.

Local and serum IgE in vasomotor rhinitis.

Total and specific IgE in serum and nasal secretions were measured in twenty five subjects with vasomotor rhinitis without asthma (21 perennial vasomotor allergic rhinitis and 4 seasonal vasomotor allergic rhinitis) after Carbacholine nasal challenge-test. The level of total and specific IgE was lower in the pure, non diluted nasal secretions than in the serum. The diagnostic usefullness of these measurements and the concept of "localized allergy" are discussed.

Acupuncture and phonostimulation in pollenosis and vasomotor rhinitis in the light of psychosomatic investigations.

Patients with vasomotor rhinitis (28) and pollenosis (23) were subjected to psychosomatic examination and treated either by acupuncture or phonostimulation exclusively. Acupuncture was performed after the classical method in 22 patients and phonostimulation in 29. Evaluation of the results was based on laryngological examinations and appraisals entered by the patients in special personal diaries. In pollenosis the condition was unchanged by the treatment. In vasomotor rhinitis, on the other hand, in which psychic factors were of importance, some of the patients usually improved temporarily at the beginning of treatment, whereas a few suffered exacerbation. These effects may be attributable to suggestion.

Cryosurgery on postganglionic fibers (posterior nasal branches) of the pterygopalatine ganglion for vasomotor rhinitis.

Clinical experience with a newly designed device for cryosurgery on autonomic nerves in cases of vasomotor rhinitis is reported. The device consists of a handy spray of liquid nitrogen and a slender probe 2.5 mm in diameter. The posterior nasal branches of the pterygopalatine ganglion were destructed by freezing for 5-8 sec with a jet of supercooled nitrogen gas delivered onto the pterygopalatine foramen under anesthesia with 4% lidocaine. Satisfactory results were obtained by this procedure in 77 of 102 cases of vasomotor rhinitis which had failed to respond to antihistaminics. This original technique of cryosurgery which is not time-consuming and can be performed on an ambulatory basis will prove useful in clinical practice.

[Status of the autonomic nervous system and reflexotherapy in children with vasomotor rhinitis].

Different variants of vegetative nervous system dysfunction were identified during the examination of 86 children with vasomotor rhinitis. Based on the findings of the examination a pathogenetically-based method of acupuncture was proposed for the management of this patients' group. Acupuncture with regard to vegetative dysfunction attenuated the latter's manifestations and corrected associated psychoemotional disorders. As a result the clinical manifestations of vasomotor rhinitis were eliminated.
