Epidemiology of end-stage renal failure in Reunion Island (results from the registry of the Indian Ocean Society of Nephrology).

BACKGROUND: End-stage renal disease (ESRD) on long-term dialysis is a substantial problem in Reunion because of the high incidence and prevalence of this disease due to non-insulin-dependent diabetes mellitus (NIDDM) and systemic arterial hypertension. SUBJECTS AND METHODS: In 1996 the renal study group of the Indian Ocean Society of Nephrology established a regional registry of end-stage renal failure (ESRD) on long-term dialysis. The present report summarizes data obtained from this registry. RESULTS: In 1996, there were 125 patients who were initiated on long-term dialysis, 657 patients on dialysis with a mean age 52 +/- 17 years, and 110 patients with a functioning kidney graft. The incidence rate of ESRD was 188 per million population (p.m.p.) and the prevalence rate of this pathology was 1155 p.m.p. The sex ratio (F/M) was 1.4/1. The two most common causes of ESRD were NIDDM in 33.6% and systemic arterial hypertension in 27.5%. The mean Kt/V value was 1.47 +/- 0.23 and the mortality rate was 8.1% per year. CONCLUSION: The results demonstrate high incidence and prevalence rates of ESRD mainly as a result of NIDDM and systemic arterial hypertension.

Contrasting Patterns of Connectivity Between Populations of Euphotic and Mesophotic Hydroids in Reunion Island Support the Deep Reef Refuge Hypothesis.

In the context of coral reef decline, mesophotic coral ecosystems (MCEs, 30-150 m) offer hope for the recovery of degraded euphotic reefs. The Deep Reef Refuge Hypothesis (DRRH) postulates the potential of mesophotic reefs to reseed euphotic reefs. This hypothesis needs to be further tested by estimating connectivity along the depth gradient. Mesophotic data are lacking worldwide, particularly in the southwestern Indian Ocean (SWIO). Here, using a total of 2218 samples collected at depths ranging from 10 to 103 m, we estimated the connectivity of 7 hydroid species sampled at euphotic, upper, and lower mesophotic depths around Reunion Island using a multi-species comparative framework. Population genetic analyses using 8-17 microsatellite markers per species (80 markers in total) as well as Bayesian inference were performed to estimate population structure and contemporary migration rates to highlight connectivity patterns and directionality of gene flow between depths. The results revealed three main genetic patterns depending on the species: a horizontal stepping stone pattern between areas around the island, a vertical stepping stone pattern between adjacent depths, and a quasi-panmictic pattern. Each species showed some specificity within these patterns, but overall, at least 4 of the 7 species support the assumption of vertical connectivity from the Deep Reef Refuge Hypothesis, highlighting the importance of studying multiple species. The existence of vertical connectivity between euphotic and mesophotic depths in the southwestern Indian Ocean confirms the importance of mesophotic coral ecosystems for conservation efforts and our global understanding of coral reef ecosystem dynamics.

A splicing mutation in intron 16 of the cystic fibrosis transmembrane conductance regulator gene, associated with severe disease, is common on Reunion Island.

In addition to the frequent delta F508 and Y122X mutations on cystic fibrosis (CF) chromosomes of patients from Reunion Island, one splicing mutation, 3120+1G-->A is observed relatively frequently (12.5%) in this group, in comparison with the French metropolitan population (<0.001 %). This mutation, disrupting the 5' splice donor site of intron 16, can be detected easily by restriction enzyme BstNI digestion. In 2 CF patients homozygous for this mutation, the clinical pattern was severe, with both pancreatic insufficiency and respiratory symptoms.

Importance of competition mechanisms in successive invasions by polyphagous tephritids in La Reunion.

Understanding the strength and modes of interspecific interactions between introduced and resident species (native or previously introduced) is necessary to predict invasion success. We evaluated different mechanisms of interspecific competition among four species of polyphagous fruit flies (Diptera: Tephritidae) from the island of La Reunion: one endemic species, Ceratitis catoirii, and three exotic species, C. capitata, C. rosa, and Bactrocera zonata, that have successively invaded the island. Larval competition experiments, i.e., co-infestations of the same fruit, and behavioral interference experiments measuring the ability of one female to displace another from a fruit, were performed among all pairs of the four species. We observed asymmetric and hierarchical interactions among species in both larval and adult interference competition. In agreement with the hypothesis that invasion is competition-limited, the competitive hierarchy coincided with the temporal sequence of establishment on the island, i.e., each newly established species tended to be competitively dominant over previously established ones.

[Chikungunya virus: its recent spread to the southern Indian Ocean and Reunion Island (2005-2006)].

Chikungunya virus (alphavirus, Togaviridae) is transmitted by mosquitoes of the Aedes genus and responsible for a dengue-like acute disease characterized by severe arthralgias sometimes persisting during months and eventually years. Its geographical distribution is large, including west, central, east and southern tropical Africa, India and South-eastern Asia. Since 2005 february, Chikungunya disease invaded a number of islands in southern Indian Ocean, namely Comoro, Mauritius, Seychelles and Reunion islands. In this French department, it was responsible for 110000 to 200000 infections, neurological disorders, many deaths and some congenital infections not previously observed during preceding epidemics.

[Epidemiology of suicide attempts in Reunion Island].

In order to specify the epidemiological characteristics of attempted suicides on Reunion island, a quantitative study was carried out, using Emergency Room registers of all the hospitals on the island. This enabled collection of 9526 files. The results revealed a high proportion of suicide attempts (335/100,000/year), particularly prevalent in rural areas to the south of the island. There was a tendency towards over representation of women, though less distinct than in Europe, as well as an abnormally frequent use of pesticides or herbicides. Medication and violent means were not used as often as in France. These outcomes, in particular the very high frequency (especially in the rural areas of the south), along with the significance of medico-psychological factors, should be taken into account by anthropologists, linguists, clinical specialists, in order to help determine preventive measures.

Recombinant chromosome 9 possibly derived from breakage and reunion of sister chromatids within a paracentric inversion loop.

Chromosomally unbalanced offspring resulting from the recombination of parental paracentric inversions are uncommon. We report on a 20-month-old boy with a partial duplication of 9p due to the recombination of a paternal paracentric inversion. The patient's recombinant chromosome was designated rec(9)(p13-->p24::p12-->p24::p12-->qter). The patient's father and paternal aunt have a paracentric inversion of chromosome 9:inv(9)(p13p24). Although several mechanisms have been proposed to explain the chromosome imbalance generated from paracentric inversions, none of the previously described mechanisms can account for the structure of the recombinant chromosome observed in the propositus. We propose an unusual mechanism of formation involving breakage and unequal reunion of sister chromatids within the inversion loop to explain the structure of the patient's recombinant chromosome.

Simultaneous detection of the two prevalent mutations in the cystic fibrosis gene in Reunion Island.

A rapid method for the diagnosis of the most frequent cystic fibrosis mutations in the Reunion Island is described based on a coamplification polymerase chain reaction (PCR) followed by a single digestion using MseI. We have used this strategy to detect the two most frequent mutations in this area: delta F508 (in exon 10) and Y122X (in exon 4). These two mutations account for 70% of the CF chromosomes. This diagnosis method, which is rapid, easy, direct, and inexpensive, allows adult and neonatal carrier screening in this population.

Breakage--reunion and copy choice mechanisms of recombination between short homologous sequences.

To study recombination between short homologous sequences in Escherichia coli we constructed plasmids composed of the pBR322 replicon, M13 replication origin and a recombination unit inserted within and inactivating a gene encoding chloramphenicol resistance. The unit was composed of short direct repeats (9, 18 or 27 bp) which flanked inverted repeats (0, 8 or 308 bp) and a gene encoding kanamycin resistance. Recombination between direct repeats restored a functional chloramphenicol resistance gene, and could be detected by a simple phenotype test. The plasmids replicated in a double-stranded form, using the pBR322 replicon, and generated single-stranded DNA when the M13 replication origin was activated. The frequency of chloramphenicol-resistant cells was low (10(-8)-10(-4] when no single-stranded DNA was synthesized but increased greatly (to 100%) after induction of single-stranded DNA synthesis. Recombination between 9 bp direct repeats entailed no transfer of DNA from parental to recombinant plasmids, whereas recombination between 18 or 27 bp repeats entailed massive transfer. The presence or length of inverted repeats did not alter the pattern of DNA transfer. From these results we propose that direct repeats of 9 bp recombine by a copy choice process, while those greater than or equal to 18 bp can recombine by a breakage-reunion process. Genome rearrangements detected in many organisms often occur by recombination between sequences less than 18 bp, which suggests that they may result from copy choice recombination.

A mycological and bacteriological survey on feed ingredients and mixed poultry feeds in Reunion island.

A survey was carried out in Reunion island to obtain data on the occurrence of fungi, aflatoxigenic strains of Aspergillus flavus, aflatoxins, total aerobic bacteria and salmonellae of 150 samples of mixed poultry feeds and raw materials. These were collected at five farms over a 3-month period during the warm rainy season. White corn and Braizlian soybean meal seemed to present a better microbiological quality than yellow corn and US soybean meal. Mixed poultry feeds presented a high total mold count reflecting the mold flora of raw materials. The most frequent and abundant fungi were Aspergillus flavus. A. glaucus group, Fusarium spp., Penicillium spp., A. candidus, Mucor spp., A. restrictus, Scopulariopsis spp., Cladosporium spp. and A. versicolor. Of the 118 A. flavus strains screened, 42 (35.6%) were aflatoxigenic. Yellow corn samples were the most frequently contaminated with aflatoxigenic strains (54.5%), followed by mixed feeds (44%). Of the 66 samples tested, 24 (36%) contained aflatoxins (traces to 22 ng/g). A good correlation seemed to exist between presence of at least one aflatoxigenic strain per sample and presence of aflatoxins.

Effect of CPT on the calf thymus Topoisomerase I-mediated DNA breakage-reunion reaction: optimal conditions for the formation and reversal of the CPT trapped Topoisomerase I cleavable complex.

The effects of CPT on the calf thymus Topoisomerase I-mediated DNA breakage-reunion reaction were studied at an enzyme concentration range proper for evidencing, at the same time, both DNA relaxation and DNA cleavage/religation. Some of the requirements and the optimal conditions for the formation and reversal of the CPT-trapped Topoisomerase I-DNA cleavable complex are also characterized. We conclude that: 1. Calf thymus (100 kDa) Topoisomerase I requires, for maximal DNA cleavage activity, specific and characteristic reaction conditions. 2. CPT does not affect these optimal conditions, but only stabilizes the normal enzyme-DNA intermediate. In this way, the drug lowers the religation process, becoming responsible for the relaxation inhibition. 3. The optimum of monovalent salt concentration for cleavable complex formation is found between 30 and 70 mM. These values are lower than those required for the relaxation activity optimum (75-125 mM NaCl). 4. The addition of 0.5 M monovalent salt causes reversal of the reaction, and shifts the equilibrium distribution between cleavable intermediate and closed relaxed DNA in the direction of DNA resealing. Therefore, it is suggested that salt affects the cleavage but not the religation reaction.

The induction of chromatid deletions in accord with the breakage-and-reunion hypothesis.

In the present experiments it has been possible to study large numbers of X-ray induced chromatid deletions, or breads, in Chinese hamster chromosomes and to discern whether or not a sister chromatid exchange also occurs at the point of breadage. Chromatid deletions are only infrequently associated with a sister chromatid exchange. This is contrary to the expectations derived from the exchange hypothesis of Revell. Pn the basis of this hypothesis, in which chromatid deletions are considered to be incomplete exchanges that occur in the necks of little loops in the chromosomes, 40% of the chromatid breaks are expected to be associated with sister chromatid exchanges. The present data are in accord with the conclusions drawn from the earlier autoradiographic experiments of HEDDLE AND BODYCOTE, and show that chromatide breaks can be accounted for on the basis of the breakage-and reunion hypothesis, with the majority being simple breaks and some being incomplete exchanges between two such breaks.

Trypsin inhibitor and maternal reunion increase plasma cholecystokinin in neonatal rats.

Intragastric soybean trypsin inhibitor increased plasma CCK bioactivity by 87% in nondeprived, 9-12-day-old rat pups. Reunion with the dam for 1 h after overnight maternal deprivation also increased plasma CCK significantly. These results demonstrate that CCK can be released from the small intestine of rats as early as postnatal day 9.

Separation and reunion behaviors as criteria of attachment to mothers and fathers.

Twenty infants were observed 4 times between 7 and 13 months of age in a short-term longitudinal study. Responses to separations from and reunions with mothers and fathers are reported. None of the measures showed a preference for either parent at any age. Other studies confirm that even young infants are attached to both parents. It is argued that greater attention must be paid to the family's role in social and personality development.

Prevalence of Type 2 diabetes and central adiposity in La Reunion Island, the REDIA Study.

La Reunion Island, a French overseas department in the Indian Ocean provides an example of rapid urbanisation and drastic changes in the way of life--from traditional to "westernized" lifestyle--over a few decades. To study the impact of this epidemiologic transition, a diabetes prevalence study was performed in 1999-2001. Fasting capillary blood glucose (cBG) and glycated hemoglobin (HbA1c) were systematically measured in a random sample of 3600 subjects aged 30-69 years. Weight, height, waist and hip circumferences were also measured to assess body mass index and waist-hip ratio. Diagnosis was assessed using an oral glucose tolerance test according to the World Health Organization recommendations in 363 subjects who had a cBG value > or =6.1 mmol/l, and/or a HbA1c value > or =6%. The overall diabetes prevalence rate was 17.7% for men and 17.3% for women, and the standardized diabetes prevalence rate was 20.1% (95% confidence interval: 18.7-21.4%). The most important morphological factor linked to Type 2 diabetes mellitus was waist-hip ratio, a marker of central adiposity, especially in women. This study confirms that Type 2 diabetes is increasing dramatically in societies in epidemiologic transition and is strongly linked to nutritional status.

Fourth International Workshop on immunology of pre-eclampsia, December 2004, Reunion, France.

The Fourth International Workshop on 'Reproductive Immunology, Immunological Tolerance and Immunology of Pre-eclampsia' was held in the Island of La Reunion in December 2004. Besides intense sharing of immunological data, it included also epidemiological studies from South Africa, USA and Australia, as well as genetic studies from South Africa and Mauritius, discussions on the future of graft transplant tolerance, implications of inositol-phosphoglycans in the pathogenesis of pre-eclampsia (PE), pathways to the inflammatory syndrome and other topics. Participants shared the belief that we may be witnessing significant steps forward in our comprehension of PE as an immunological event, with a prominent role for deregulation of the innate immune system, probably controlled by T cells and cytokine networks at the feto-placental interface. There was a growing consensus for an NK cell (KIR)-dependent event, with regulation exerted by T cells, as well as an important role for HLA-C presentation/recognition and HLA-G in mediating inflammatory cytokine imbalance, with an emphasis on IL-12 and IL-16 It is hoped that the next Workshop in 2006 will prove the immune involvement by dissecting the NK/Treg/Ts cell and HLA-CI HLA-G circuits leading to cytokine and vascular dysfunction.

Preliminary crystallographic analysis of the breakage-reunion domain of the Escherichia coli DNA gyrase A protein.

The 64 x 10(3) Mr N-terminal breakage-reunion domain of the Escherichia coli DNA gyrase A protein was purified from an over-expressing strain. When complexed with the gyrase B protein, this truncated A protein has all of the enzymic properties of the full-length counterpart, although with reduced efficiency in some cases. The 64 x 10(3) Mr protein has been crystallized in several forms, a number of which were too small for crystallographic analysis. However, two forms grew to sufficient size for preliminary X-ray analysis. Both forms were tetragonal with a primitive lattice. One form (type I) had cell dimensions of a = b = 170 A, c = 145 A a space group of either P41212 (P43212) or P42212, and diffracted to 6 A resolution. The type II crystals had cell dimensions of a = b = 177 A, c = 175 A, a space group of P41212 (P43212) or P42212, and diffracted to at least 4.5 A resolution. Both crystal forms apparently contained four subunits (possibly a tetramer) in the asymmetric unit. We are attempting to increase the size and quality of these crystals.

Behavioral, cardiac and cortisol responses to brief peer separation and reunion in cattle.

Behavioral, cardiac, and adrenal responses of heifers to short-term isolation and to subsequent reunion with familiar or nonfamiliar conspecifics were measured. Two groups of heifers were studied according to their different social reactivity: Aubrac heifers (n = 12) reared under suckler conditions and Friesian heifers (n = 12) reared under dairy management. Because these two groups could also react differently to human beings, testing social isolation was realized by removing pen mates without handling the subject. Moreover, heifers were tested in confinement to avoid an alteration of the cardiac response to isolation by an excessive motor activity. Although physical restraint can influence the reactions, this effect is assumed to be weak because heifers had been exposed to the experimental procedures, including confinement for 3 days before isolation test, in addition to brief periods of physical restraint occurring regularly according to rearing practices. Results show that social separation induced struggling and large increases in vocalization, heart rate, and plasma cortisol concentrations in all heifers. Except for vocalization, these effects were more severe in Aubrac than in Friesian heifers. For all heifers, isolation-induced distress was positively correlated with the duration of social contacts they engaged with the pen mates prior to separation. Behavioral responses, i.e., struggling and vocalization, decreased when conspecifics were brought back, independently of their familiarity to the subject. In contrast, the heart rate decline induced by the entrance of conspecifics was more pronounced in response to reintroduction of pen mates. These findings indicate that social isolation is a severe psychological stress in cattle and that the mere sight of conspecifics reduces behavioral distress regardless of peer identity. The isolation-induced distress depends on the genetic and rearing backgrounds of the heifers without allowing to differentiate their respective effects.
