HLA system and therapy of psoriasis.

In 192 patients with psoriasis an increased frequency of the antigens HLA-B13 (19.79%, X2 = 32.79, p less than 0.001), HLA-B17 (24.47%, X2 = 64,84, p less than 0.001), and HLA-B27 (19.27%, X2 = 11.07, p less than 0.001) in comparison with the control group of 1,085 clinically healthy persons, was established. After 4 weeks of thalassotherapy 3 groups of patients could be distinguished: (1) those with full clinical remission - 50 (26.8%), (2) those with great or moderate improvement - 116 (60.4%), and (3) those with no improvement - 26 patients (13.6%). In the first two groups there is a statistically significant association with the HLA antigens B13 and B17. In the third group (no improvement) there is no such association with HLA-B13 (p less than 0.01). Consequently, patients in whom psoriasis is associated with HLA-B13 were better influenced by the thalassotherapy. The variability in the therapeutic efficiency means that perhaps there is a genetic heterogenicity in the group of common psoriasis.

Algae-induced occupational asthma in a thalassotherapist.

BACKGROUND: Algae powders have been increasingly used in therapeutic preparations. Their potential as inducers of occupational asthma (OA), however, remains to be demonstrated. METHODS: A 33-year-old woman developed asthma following exposure to algae powder used for thalassotherapy. She had positive patch tests to nickel and cobalt in an evaluation for intermittent skin rash. At work, she had a normal forced expiratory flow in 1 s (FEV(1)) but moderate airway hyperresponsiveness. Bronchial provocation tests (BPT) were obtained. RESULTS: On BPT, an isolated early response was observed after a 1-min inhalation of dry algae powder, with a 50% fall in FEV(1) after exposure; the control BPT with lactose powder was negative. CONCLUSIONS: We report a case of algae-powder-induced OA. Such responses could be due to a sensitization to algae proteins or metal contaminants bioabsorbed by the vegetal particles.

Changes in plasma lactate and pyruvate concentrations after taking a bath in hot deep seawater.

The use of deep seawater (DSW) in thalassotherapy has begun in Japan. To clarify the health effects of DSW on the human body, we investigated the changes in plasma lactate and pyruvate concentrations, or subjective judgment scores, after bathing at rest in 9 healthy young men. Subjects were immersed for 10 minutes in DSW, surface seawater (SSW), and tap water (TW) heated to 42 degrees C. Plasma samples were collected before bathing, immediately after bathing, and 60 minutes after bathing. The scores were obtained by an oral comprehension test. In the DSW bathing, plasma lactate and pyruvate concentrations showed no significant changes immediately after bathing or 60 minutes after bathing. In contrast, subjects who bathed in SSW showed a significant decrease in lactate concentrations 60 minutes after bathing compared with immediately after bathing. Subjects who bathed in TW showed a significant increase in lactate concentrations immediately after bathing compared with before bathing, and they showed a significant decrease in lactate and pyruvate concentrations 60 minutes after bathing. We found no significant change in the thermal sensation score in the DSW bathing, though significant differences were found between before and immediately after bathing in the SSW and TW groups. Moreover, the score decreased significantly 60 minutes after bathing compared to immediately after bathing in the TW bathing. Higher concentrations of salts contained DSW such as sodium, nitrate-nitrogen, phosphate-phosphorus, and silicate-silicon may have a good influence on human health. Although additional studies are needed to support our findings, DSW is the mildest water to the human body among the three kinds of water, since no significant changes in the items measured were found only in DSW.

[Regenerationist Usage of Water's Symbology Between Decaying Spa Resorts Movement and Kneippist Morals].

At the end of the 19th century coexisted in Spain several therapies based on the use of water. Some of them were considered orthodox therapies, some were applied as unorthodox medicines, but all of them relied on the symbolism of water to obtain patients. From the point of view of the orthodoxy, the discourse of medical hydrology, thalassotherapy and hydrotherapy tended to exclude unscientific values. The naturist and kneippist movements, on the other hand, frequently used images of water (especially cold water) as a support for their regenerational projects, using both the material and the moral aspects. Therefore, Kneipp's therapy became a kind of Spanish interpretation of the social catholic reforming attitude.

[Effect of climate exposure on immunologic indices during the treatment of gingivitis in children].

Physical and environmental impacts promoted a substantial improvement in the gingival conditions in children with catarrhal gingivitis. The content of SIgA increased in mixed unstimulated saliva, along with other positive laboratory and functional signs. The data suggest that aero-, helio and thalassotherapy are not only the factors of treatment, but also the natural unspecific stimulators important for the increase of an organism's tissues resistance.

Free fatty acids in the blood serum of psoriatics.

Free fatty acids in the serum of 60 patients with generalized psoriasis were studied by a gas-chromatographic method. The same investigations in 40 healthy persons served for control purposes. It was established with statistical probability (p less than 0.001) that in the serum of psoriatics the levels of palmitic (C 16: 0), stearic (C 18: 0), arachidic (C 20: 0), and docosadienic (C 20: 2) acids were increased, while the levels of the linoleic (C 18: 2) and arachidonic (C 20: 4) acids were depressed. There is no behenic acid (C 22: 0) in the blood serum of psoriatics. Thalassotherapy caused no significant changes in the levels of these free fatty acids in psoriatics.

[Ethics, bioethics and medical sciences].

The aim of bioethics is to define a wise conduct for humans with regard to their environments, whether living or inanimate. However, owing to their diversity, bioethics can only deal with general problems such as biodiversity. Within the framework of bioethics as a whole, different sectorial bioethics must therefore exist to deal with problems specific to certain environments, for example the Oceans and Seas, the Forests. General bioethics and sectorial bioethics have an important contribution to make to medical sciences but official regulations should be proposed only after an attentive investigation has been made. For instance, the preservation of an apparently threatened biodiversity or the revival of a seriously damaged biodiversity must be the subject of a thorough preliminary scientific study and, if legislative decisions are taken, a very careful scientific control of their consequences must be carried out. One example is given: the decree on the protection of Larids and its impact, with regard to an abusive proliferation of certain gull populations having varied effects on public health. Sectorial bioethics can also have obvious consequences on medical sciences. Thus various harmful attacks on coral reefs (contrary to the concepts of thalassoethics) can lead to the death of corals and the appearance of ciguatera. Thalassoethics, by inciting pollution control, should help to improve the conditions of thalassotherapy. Forest ethics, particularly concerning management, can reduce the greenhouse effect and its consequences on health, as well as protecting plant and animal species inhabiting the ecosystem and bringing new chemical bodies to inspire original pharmacological research. Thus the links between general or sectorial bioethics and medical sciences must always be very close.
