Effect of the shampoo Ultra Clean on drug concentrations in human hair.

The influence of the special shampoo Ultra Clean (Zydot Unlimited, Tulsa, Oklahoma) on the results of hair analyses was investigated. Hair samples from persons (n = 14) with a known history of drug abuse were collected at autopsy. The hair samples were divided into separate strands which were analyzed both after washing with Ultra Clean and without treatment. Hair analyses were performed by methanol extraction under sonication, purification by solid phase extraction and GC/MS in SIM mode according to routine procedures for tetrahydrocannabinol (THC), cocaine, amphetamine, methylenedioxyamphetamine (MDA), methylenedioxymethamphetamine (MDMA), methylenedioxyethylamphetamine (MDE), heroin, 6-monoacetylmorphine (6-MAM), morphine, codeine, dihydrocodeine and methadone. All drugs originally present in the hair fibers were still detected after a single application of Ultra Clean. However, a slight decrease in drug concentrations could mostly be observed e.g. cocaine (n = 10) -5%, 6-MAM (n = 12) -9%, morphine (n = 12) -26%, THC (n = 4) -36%. The findings clearly demonstrated that drug substances had not been sufficiently removed from human hair by a single Ultra Clean treatment to drop their concentrations below the limit of detection of the analytical method applied.

Effects of meteorological conditions on spore plumes.

Fungal spores are an ever-present component of the atmosphere, and have long been known to trigger asthma and hay fever symptoms in sensitive individuals. The atmosphere around Tulsa has been monitored for airborne spores and pollen with Burkard spore traps at several sampling stations. This study involved the examination of the hourly spore concentrations on days that had average daily concentrations near 50,000 spores/m(3) or greater. Hourly concentrations of Cladosporium, Alternaria, Epicoccum, Curvularia, Pithomyces, Drechslera, smut spores, ascospores, basidiospores, other, and total spores were determined on 4 days at three sites and then correlated with hourly meteorological data including temperature, rainfall, wind speed, dew point, air pressure, and wind direction. On each of these days there was a spore plume, a phenomenon in which spore concentrations increased dramatically over a very short period of time. Spore plumes generally occurred near midday, and concentrations were seen to increase from lows around 20,000 total spores/m(3) to highs over 170,000 total spores/m(3) in 2 h. Multiple regression analysis of the data indicated that increases in temperature, dew point, and air pressure correlated with the increase in spore concentrations, but no single weather variable predicted the appearance of a spore plume. The proper combination of changes in these meteorological parameters that result in a spore plume may be due to the changing weather conditions associated with thunderstorms, as on 3 of the 4 days when spore plumes occurred there were thunderstorms later that evening. The occurrence of spore plumes may have clinical significance, because other studies have shown that sensitization to certain spore types can occur during exposure to high spore concentrations.

Joint Regional Exercise ("JREX") 2000.

Joint Regional Exercise, 2000 (J-REX 2000) was an ambitious, multi-functional, multi-agency exercise conducted October 12-14, 2000. While the main focus of the exercise was centered in Minneapolis MN, and adjacent Fort Snelling, the exercise also involved National Disaster Medical System (NDMS) participating hospitals in Cleveland, Detroit, Duluth, Indianapolis, Omaha, and Tulsa. As such, the exercise presented a unique opportunity to simultaneously and somewhat realistically examine the response capabilities of multiple agencies and institutions within the NDMS and to 'game' several aspects of the Federal Response Plan's Emergency Support Function 8.

Laparoscopic exploration for the clinically undetected hernia in infancy and childhood.

BACKGROUND: Routine contralateral groin exploration in infants and children with a clinically detected inguinal hernia is the subject of much debate. The detection of a patent processus vaginalis by transinguinal laparoscopy has proven advantageous. However, controversy remains regarding the true incidence of a contralateral patent processus vaginalis as well as which of these will actually develop into a clinically apparent hernia. METHODS: From January 1997 through December 1999, 358 infants and children (aged 1 to 157 months, mean 32) were treated in the three University of Oklahoma teaching hospitals in Tulsa, Oklahoma, for inguinal hernia. The findings at laparoscopic exploration of the contralateral side were recorded to determine the incidence of contralateral patency as it relates to a child's age, gender, and side of the initial clinical diagnosis. RESULTS: The overall incidence of a patent processus vaginalis on contralateral examination was 33% (117 of 358). All patent processus vaginalis were repaired. Bilateral inguinal hernia was significantly more common in younger patients (present in 50% if less than 1 year, 45% if less than 2 years, 37% if less than 5 years, and 15% if greater than 5 years of age; P <0.05). In boys, the incidence was 49%, 45%, and 32% in those under 1 year of age, under 2 years of age, and in total, respectively. In girls, the incidence was 59%, 50%, and 37% in those under 1 year of age, under 2 years of age, and in total, respectively. The side of the clinically detected hernia did not influence the laparoscopic findings of a contralateral hernia with 30% (50 of 169) positive findings on left inguinal exploration versus 31% (28 of 90) positive findings on right inguinal exploration. CONCLUSIONS: The high incidence of a contralateral patent processus vaginalis warrants routine laparoscopic exploration in infants and children undergoing unilateral inguinal hernia repair, especially those less than 5 years of age. The use of transinguinal laparoscopic explorations avoids unnecessary open exploration in 66% of infants and children undergoing inguinal hernia repair.

Estimation of primary drainage three-phase relative permeability for organic liquid transport in the vadose zone.

The modeling of transport of organic liquid contaminants through the vadose zone often requires three-phase relative permeabilities. Since these are difficult to measure, predictive models are usually used. The objective of this study is to assess the ability of eight common models to predict the drainage relative permeability to oil in a three-phase system (water-oil-air). A comparison of the models' estimates using data set from Oak [Oak, M.J., 1990. Three-phase relative permeability of water-wet Berea. In: Seventh Symposium on Enhanced Oil Recovery, Paper SPE/Doe 20183. Tulsa, OK, April 22-25] showed that they provide very different predictions for the same system. The goodness of the models does not increase with the amount of data or computation that the models require. Also, the calculations showed how different interpretations of the models and of the terminology associated with them can significantly impact the predictions. Thus, considerable error may be introduced into the simulations of organic liquid transport in the vadose zone depending on the selection and interpretation of the three-phase relative permeability model.

Unmarried black adolescent fathers' attitudes toward abortion, contraception, and sexuality: a preliminary report.

The attitudes of a limited population of unmarried black adolescent fathers toward abortion, contraception, and sexuality were assessed using standardized data collected in Tulsa, Oklahoma, Chicago, Illinois, and in Columbus, Ohio. A majority of the surveyed fathers in each city surveyed, were opposed to abortion. Two-thirds indicated a willingness to share contraceptive responsibility with the female. These data suggest that if family planning agencies offered sex education programs for adolescent fathers, particularly in relation to contraceptive responsibility, the incidence of repeat unwed adolescent fatherhood might be reduced.

A student-directed community project to support sexually abused women veterans suffering from post-traumatic stress disorder.

While awareness of post-traumatic stress disorder (PTSD) and sexual abuse continues to grow, it has only been during the past few years that the military has realized the prevalence and impact of sexual abuse inflicted upon women while on active military duty. Though Veteran Administration (VA) agencies throughout the United States have given concerted attention to this problem, published resources specific to PTSD and military sexual abuse have been limited. In this article the authors present the results of a 2(1/2)-year endeavor to address the problem of PTSD and military sexual abuse at the Tulsa VA Outpatient Clinic. The project started with a research study and the subsequent initiation of a PTSD women veterans support group, and culminated in the development of resource manuals for both professional staff and women veterans.

Radiofrequency neurolysis for facet arthropathy: a retrospective case series and review of the literature.

CONTEXT: Facet arthropathy is a common cause of spine-related pain. Typically resulting from spondylosis, trauma, including surgical trauma or post surgical stress is also a significant cause. Radiofrequency thermocoagulation or neurolysis may be an effective modality providing long-term improvement. OBJECTIVES: To evaluate the success rates for radiofrequency neurolysis for facet arthropathy in a large retrospective case series in a single pain practice setting. DESIGN: A retrospective case series involving chart reviews and patient follow-up visits or telephone contacts of radiofrequency neurolytic procedures performed for facet arthropathy over a 4-year period. SETTING: Private practice pain clinic with academic affiliation in Tulsa, OK. PARTICIPANTS: One hundred forty eight patients with confirmed facet arthropathy refractory to conservative measures underwent 230 radiofrequency neurolysis procedures and were followed for a minimum of 1 year post procedure. For cervical facet procedures: 63 patients (106 procedures); age range F: 27-84 years old; M: 33-65 years old. For lumbar facet procedures: 85 patients (124 facet procedures); age range F: 19-81 years old; M: 20-77 years old. MAIN OUTCOME MEASURES: After the radiofrequency procedure, patients were followed with periodic visits or telephone contacts. Outcome measures were McGill short form pain questionnaire, VAS pain scores, muscle spasm scores, tenderness, range of motion and patient subjective global responses. RESULTS: Subjective patient responses were graded as follows: excellent:greater than 70% improvement, good: 50% to 70% improvement, fair: 30% to 49%, and poor: less than 30%. One hundred six radiofrequency procedures were performed in the 63 cervical cases and 124 in the 85 lumbar cases with those patients who had good to excellent responses undergoing repeat procedures. Of the patients with cervical facet radiofrequency procedures, 38 (37%), 51 (48%), 4 (3%) and 13 (12%) had excellent, good, fair or poor responses, respectively. Of the lumbar facet radiofrequency cases, 37 (30%), 52 (41%), 13 (10%) and 22 (19%) had excellent, good, fair or poor responses, respectively. Excellent responders noted an average duration of 10.8 months (range 3-34 months before dropping below 70% improvement level) for cervical cases and 7.9 months (range 3-20) for lumbar. Good responders noted an average duration of 6.5 months (range 3-22 months before dropping below 50% improvement level) for cervical and 6.8 months (range 3-48) for lumbar radiofrequency procedures. No significant side effects were experienced (short-term neuritis was seen in 2 patients who had cervical and 1 who had lumbar RFTC, but resolved in each case after a few weeks). CONCLUSIONS: In summary, 85% of cervical and 71% of lumbar RFTC cases had at least a 50% improvement in symptoms for extended periods. RFTC of median branches for facet arthropathy is a safe and efficacious modality with the potential for long-term benefit.

Efficacy of bacterial removal from instrumented root canals in vitro related to instrumentation technique and size.

OBJECTIVE: This in vitro investigation assessed the efficacy of removing radioactively labeled bacteria from infected canals with 2 engine-driven rotary nickel titanium instrumentation techniques differing in sequence and apical enlargement size. STUDY DESIGN: A standard quantity of (3)H-thymidine-labeled Enterococcus faecalis (3.70 x 10(4) cpm, 2.0 x 10(7) colony-forming units) was used to inoculate the mesiobuccal canals of 50 extracted mandibular molars. The teeth were incubated for 5 days to allow infection of the surrounding dentin from the canals. Five of the teeth were used as controls to determine the number of cycles of irrigation and drying necessary to reduce the (3)H counts recovered from the canals to baseline levels. After this process, the unbound bacteria in the root canals of the remaining 45 teeth then were washed out with buffer until baseline levels of radioactivity were obtained. The mesiobuccal root of 1 of these 45 teeth was removed, decalcified, and digested, and the total radioactivity released from the root dentin was measured. Of the remaining 44 teeth, 22 then were instrumented with GT and Profile (Dentsply/Tulsa Dental Co, Tulsa, Okla) instruments to apical size #35 (group 1) and 22 teeth with Pow-R instruments (Moyco/Union Broach, York, Pa) to apical size #50 (group 2), in the presence of a standard quantity of phosphate-buffered saline solution placed in the canal. After instrumentation, the medium from each canal was collected with paper points and its radioactivity was counted with liquid scintillation spectrometry. RESULTS: The mean (3)H level recovered with instrumentation of canals in group 1 was 75 cpm (+/- 29, standard deviation) and in group 2 was 123 cpm (+/- 50, standard deviation). A 2-tailed Mann-Whitney test indicated that the radioactivity of samples from group 2 was significantly higher than that of samples from group 1. CONCLUSION: The results suggested that instrumentation to an apical size of #50, as performed with the Pow-R instruments, was more effective in debriding infected root canals than instrumentation to an apical size of #35, as performed with the GT and Profile instruments.

Effect of root resection on the apical sealing ability of mineral trioxide aggregate.

OBJECTIVE: The purpose of this in vitro study was to determine the minimum depth of mineral trioxide aggregate (MTA; ProRoot; DENTSPLY/Tulsa Dental, Tulsa, Okla) required to maintain an apical seal following root resection. Study design. In 10 instrumented teeth, MTA was used to obturate the apical 6 mm of the root canal and was allowed to set for 48 hours. Leakage was determined by means of a fluid filtration method at a pressure of 20 cm H(2)O. Leakage was measured before root resection, and after 3, 4, 5, and 6 mm apical resections. Data were analyzed by means of a Kruskal-Wallis one-way analysis of variance with P <.05. RESULTS: Fluid leakage was shown to increase after each resection, but did not reach statistical significance (P <.05) until 4 mm of the apex had been removed. CONCLUSION: The results indicate that root resection did not significantly affect the sealing ability of MTA when at least 3 mm of the MTA remained. Although there was a statistically significant difference in leakage following the 4 mm resection, it is unknown what the biological difference would be between the 3 mm and 4 mm resections.

A survey of physicians' smoking counseling practices.

Several techniques used in smoking cessation counseling have been shown to be useful in physicians' efforts to help their patients quit smoking. The use of these techniques by Tulsa physicians was assessed through a survey of private practice internists, internal medicine residents, and family practice residents in a university-based community hospital. The majority of physicians (85%) reported they brought up the subject of smoking with their patients. Other techniques were used less frequently, with 34% of physicians never giving their patients self-help materials, 83% never using a quit date contract, and 73% never making appointments mainly to discuss smoking. Private practice physicians reported using more techniques than did the residents. Internists who practice a subspecialty reported using fewer techniques, and this was especially true in the younger physicians. Ten percent of physicians were often satisfied with their efforts, and 14% had formal training in smoking counseling techniques. Some physicians apparently learn how to better use these techniques as their experience increases. Training physicians while they are in residency and early subspecialty practice may accelerate this process and enable more smokers to quit with their physicians' help.

Factors affecting decisions on practice locations.

Questionnaires designed to determine the major factors affecting the choice of practice were mailed to 305 graduates of the University of Oklahoma Tulsa Medical College and their spouses. Sixty-eight percent of the physicians responded. The results showed that graduates reared in large communities chose similar-sized communities as practice sites and placed high priority on their spouses' desires. The lack of career opportunities for spouses, especially those with high levels of education, was a critical factor in not selecting small communities as practice sites.

Outbreak of diarrhea in a day care center with spread to household members: the role of Cryptosporidium.

From August 1 to November 1, 1984, 80 (43%) of 186 children attending a day care center (DCC) in Tulsa, OK, had diarrhea. Seventy-one percent of children less than 3 years of age had diarrhea, compared with 17% of children 3 to 8 years old. The duration of illness was 1 to 42 days (median, 8 days). At least one stool specimen was obtained from 142 (76%) of the 186 children. Children who had had diarrhea were more likely to have a specimen positive for Cryptosporidium (27 of 77, 35%) than were children who had not had diarrhea (8 of 65, 12%) (P less than 0.01). The presence of Giardia, Campylobacter, enterovirus, or rotavirus was not statistically associated with diarrhea. These results suggest Cryptosporidium as a causative agent. The secondary attack rate for diarrhea among household contacts exposed to DCC children with diarrhea was 77 of 204 (38%) compared with 25 of 273 (9%) for household contacts of children in the DCC who did not have diarrhea (P less than 0.001). Stool specimens were obtained from 79 household contacts. Six (23%) of 26 household members exposed to a Cryptosporidium-positive child had a stool specimen positive for Cryptosporidium, compared with 1 (2%) of 53 household contacts exposed to a DCC child whose stool specimen was negative for Cryptosporidium (P less than 0.01). The outbreak resolved within 2 weeks after cohorting children with diarrhea and stressing the importance of handwashing among staff and older children. cryptosporidium can cause outbreaks of diarrhea in DCC settings, can be spread within households and may be more common than previously recognized among DCC attendees and their household members.

Acute myocardial infarction in Oklahoma: are we different?

Data regarding 787 patients admitted to Saint Francis Hospital in Tulsa, Oklahoma, with acute myocardial infarction (MI) were analyzed to determine circadian variation and to assess demographic and anatomic characteristics. The study population consisted of 634 men (81%) and 153 women (19%). This cohort displayed a circadian rhythm that nearly duplicated a previously published national data base. As was seen nationally, our patients had a peak incidence between 6 AM and noon. Of the total population, 448 patients had acute cardiac catheterization allowing definition of the infarct vessel. The majority of myocardial infarctions were from right coronary artery occlusions. Men were more likely than women to have an anterior MI. Both men and women were more likely to have an inferior MI if they were less than 65 years of age, though this was more pronounced for women. As the population aged, they were more likely to have an anterior myocardial infarction.

Three-dimensional mixed-wet random pore-scale network modeling of two- and three-phase flow in porous media. II. Results.

We use the model described in Piri and Blunt [Phys. Rev. E 71, 026301 (2005)] to predict two- and three-phase relative permeabilities of Berea sandstone using a random network to represent the pore space. We predict measured relative permeabilities for two-phase flow in a water-wet system. We then successfully predict the steady-state oil, water, and gas three-phase relative permeabilities measured by Oak (Proceedings of the SPE/DOE Seventh Symposium on Enhanced Oil Recovery, Tulsa, OK, 1990). We also study secondary and tertiary gas injection into media of different wettability and initial oil saturation and interpret the results in terms of pore-scale displacement processes.

Reducing infant mortality rates using the perinatal periods of risk model.

Despite decreases in the last 50 years, infant mortality rates in the United States remain higher than in other industrialized countries. Using overall infant mortality rates to determine the effectiveness of interventions does not help communities focus on particular underlying factors contributing to static, and sometimes increasing, community rates. This study was designed to determine and rank contributing factors to fetal-infant mortality in a specific community using the Perinatal Periods of Risk (PPOR) model. The PPOR model was used to map fetal-infant mortality for 1995 to 1998 in the Tulsa, Oklahoma, Healthy Start Program as compared to traditional calculation methods. The overall fetal-infant mortality rate using the PPOR model was 12.7 compared to 7.11 calculated using the traditional method. The maternal health cell rate was 5.4, maternal care cell rate was 2.9, newborn care cell was 1.9 compared to a 4.1 neonatal death rate calculated using the traditional method, and the infant health cell was 2.4 compared to a 2.9 postneonatal rate calculated using the traditional method. Because the highest infant mortality was in the maternal health cell, intervention strategies were designed to promote the health of women prior to and between pregnancies. The PPOR model was helpful in targeting interventions to reduce fetal-infant mortality based on the prioritization of contributing factors.

Effect of a mandatory geriatric medicine clerkship on third-year students.

A nationwide push has increased geriatric medicine instruction within medical school curricula. Some institutions have proceeded with an integrated 4-year curriculum while others have constructed discrete courses in the third or fourth year of medical school. This paper describes the impact of a new mandatory 4-week geriatric medicine clerkship on third-year students developed by the Donald W. Reynolds Department of Geriatric Medicine at the University of Oklahoma Health Sciences Center. In the first year of implementation, 135 students took the course on both the Oklahoma City and Tulsa campuses. Clinical sites included inpatient, VA extended care unit, outpatient clinics, dementia clinics, home care, long-term care settings, and hospice. Didactic instruction used formal lectures and problem-based learning. The impact of the clerkship on students was assessed in three areas: knowledge, skills, and attitude using a pre- and postknowledge test, student satisfaction survey, and written comments. This article discusses how the clerkship resulted in increased knowledge of geriatric medicine. Student self-report indicates that the clerkship enhanced clinical evaluation and patient assessment skills. Students indicated that the experience was positive and recognized the importance of geriatric medicine in their development as doctors.

Effect of dowel space preparation and composite cement thickness on retention of a prefabricated dowel.

PURPOSE: This investigation examined what effect cement thickness had on retention of prefabricated endodontic dowels luted with a composite resin cement (Panavia 21 OP; J Morita, Irvine, CA) into canals previously obturated with gutta percha and a eugenol-based sealer. MATERIALS AND METHODS: Sixty-four single-rooted teeth were decoronated, filed, cleaned, and sequentially shaped with sizes 2-5 Gates Glidden drills (Dentsply/Maillefer, Tulsa, OK) and 0.12 taper rotary instrumentation. Teeth were then divided into 4 groups of 16 specimens each. All specimens were obturated with gutta percha and a eugenol-based sealer. Dowel space preparation and dowel cementation were completed 1 week after obturation. Ten-millimeter-deep dowel spaces were prepared using dowel drills with 4 different diameters: size 5 Parapost drill (Group 1; Coltene/Whaledent, Mahwah, NJ); size 5.5 Parapost drill (Group 2; Coltene/Whaledent); size 6 Gates Glidden drill (Group 3; Dentsply/Maillefer); size 6 Parapost drill (Group 4; Coltene/Whaledent). Size 5 Paraposts were then cemented with Panavia 21 OP. After 48 hours of storage, specimens were mounted in polyvinyl chloride (PVC) pipe with acrylic, and the dowels were removed in tensile mode using a universal testing machine at 1 mm/min, with data recorded in kilograms. RESULTS: (all values in kilograms) Group 1 (Parapost 5) mean = 15.07, 95% confidence interval (CI) = +/-6.11; Group 2 (Parapost 5.5) mean = 25.60, 95% CI = +/-7.39; Group 3 (Gates-Glidden 6) mean = 43.15, 95% CI = +/-7.81; Group 4 (Parapost 6) mean = 37.75, 95% CI = +/-6.35. Analysis of variance and Bonferroni tests revealed that Group 3 had significantly greater mean retention strength values than Group 1 and Group 2 (p <.05), and that Group 4 had significantly greater mean retention strength values than Group 1 (p <.05). CONCLUSIONS: Paraposts cemented with Panavia 21 OP showed significantly greater retention in oversized dowel spaces compared with dowel spaces prepared with the manufacturers' matched dowel-drill set.
