[Relationship between serum insulin, C-peptide in hypertension and syndrome differentiation-typing in TCM].

OBJECTIVE: To study the relationship between insulin-resistance (IR), hyperinsulinemia and TCM Syndrome Differentiation-typing. METHODS: The serum insulin, C-peptide level of the four Syndrome-types of hypertension (30 cases each type) and the control group (30 cases) were determined. RESULTS: The serum insulin level in hypertension patients were significantly higher than that of control group, and there were obvious difference among the four types of Syndrome. The following order was: The abundant phlegm-dampness type > exuberant Liver-Fire type > both Yin-Yang deficiency type > Yin deficiency and Yang-Excess type > control type. CONCLUSIONS: The Excess Syndrome was severe and deficiency Syndrome was mild in hyperinsulinemia. The pattern of change was in accordance with etiology and pathogenesis of TCM. It has the guiding significance to the clinical practice and research of TCM and integrated TCM-WM.

[Observation on therapeutic effect of warming needle moxibustion on knee osteoarthritis of deficiency-cold type].

OBJECTIVE: To compare clinically therapeutic effects of warming needle moxibustion and simple acupuncture on knee osteoarthritis. METHODS: One hundred and twenty cases conformed with the diagnostic criteria of knee osteoarthritis with TCM kidney-yang deficiency and cold syndrome were randomly divided into a warming needle moxibustion group and a simple acupuncture group. Guanyuan (CV 4), Qihai (CV 6), Zusanli (ST 36) and others were selected in the two groups. Their therapeutic effects were assessed with joint pain scale, knee osteoarthritis severity index scale and cold syndrome differentiation factors. RESULTS: Both warming needle moxibustion and simple acupuncture could alleviate pain of knee osteoarthritis, improve dysfunction of the knee joint, reduce symptoms. The clinically basic cured rate and the total effective rate were 32.1% and 96.4% in the warming needle moxibustion group, which were better than 7.4% and 92.6% in the simple acupuncture group. CONCLUSION: Warming needle moxibustion is an effective therapy for knee osteoarthritis with kidney-yang deficiency and cold type.

[Change in serum growth hormone in aged men and effect of kidney-warming and yang-restoring drugs].

Basal and sleeping levels of serum growth hormone (GH) were measured in aged men and young men. GH levels after an intravenous bolus of growth hormone releasing hormone (1 microgram/kg) were also determined in healthy aged men with mild Kidney-Yang deficiency. The effect of Chinese herbs with Kidney-warming and Yang-restoring function was observed. The basal GH value in aged men was not significantly different when compared with young men, but the GH value during sleep evidently decreased (P less than 0.05). The response of GH to GRF was obviously decreased as compared with young people. The GH level following GRF stimulation was significantly different from that of young men. The Kidney-warming and Yang-restoring Chinese herbs could alleviate the symptoms of Kidney-Yang deficiency in aged men. The sleep value of GH and response of GH to GRF were increased after taking Chinese herbs. It suggested that the decrease of pituitary reserve might probably be the pathogenesis of Kidney deficiency. Together with the results of another animal experiments, the authors presumed that the Chinese herbs with Kidney-warming and Yang-restoring characteristics might effect through the increase of hypothalamic dopamine and the subsequent improvement of pituitary hormone reserve.

[Characteristics of traditional Chinese medicine syndromes in both spontaneous and variant angina. An analysis of 21 cases].

An analysis of TCM syndromes is reported in 21 cases with both spontaneous and variant angina as compared with 147 cases with effort angina. The results showed that 3 characteristic features were present, which were as follows: the Biao-Shi syndrome of cold condensation was more than that of the control group in ratio of 42.86% to 3.40%; the Ben-Xu syndrome of Yang deficiency was more and that of Qi deficiency less than those in the control group, and they were in ratio of 33.33% to 6.12% and 33.33% to 72.11% respectively. An absolute reduction of blood supply resulted from coronary spontaneous spasm in both spontaneous and variant angina causes severe chest pain during attacks as a cold condensation type. Hyperfunction of parasympathetic nerves often occurring in coronary heart disease with Yang deficiency is liable to vasoconstriction of the large coronary arteries leading to episodes of both spontaneous and variant angina. The presence of less Qi deficiency type may be related to the less impairment of cardiac function resulted from the short course in these cases and only relatively mild state of an illness, even no marked lesion in coronary arteries in a part of patients with both spontaneous and variant angina. No significant difference in TCM syndromes occurred between spontaneous and variant angina. Both Yang and Yin deficiency, as the Ben-Xu syndromes, were more present in angina of cold condensation type.(ABSTRACT TRUNCATED AT 250 WORDS)

[Abnormality of T lymphocyte subsets in aged persons with kidney deficiency syndrome and its influence on immunologic regulation].

T lymphocyte subsets were investigated in 57 aged persons with Kidney Deficiency Syndrome as well as in 30 normal controls with application of the monoclonal antibody (OKT3, OKT4, OKT8). The results showed that the percentage of OKT3, OKT4 in the aged persons was significantly lower than that of control (P < 0.01), while OKT8 was significantly higher (P < 0.01). The OKT4/OKT8 ratio was significantly lower than that of control (P < 0.01). However, the above-mentioned changes were more evident in the Kidney Yang Deficiency Syndrome than those in Kidney Yin Deficiency Syndrome (P < 0.01). The further analysis indicated that the aged persons with Kidney Yang Deficiency appeared as marked positive correlation (r = 0.64, P < 0.001) between the OKT4/OKT8 ratio and IgA, and they also appeared as marked negative correlation (r = -0.89, P < 0.001 and r = -0.665, P < 0.001 respectively) between the OKT4/OKT8 ratio and C3 or CIC. The results suggested that the imbalance of the cell mediated immunity expressed by the T lymphocyte subsets in aged persons with Kidney Deficiency might be one of the essences of senility.

[The efficacy of electromyographic biofeedback treatment on cardiovascular disease and the correlation of type A behavior and traditional Chinese medicine syndrome differentiation].

Using the method of trilineal relaxation training, electromyographic biofeedback technic (EMGBT) in the treatment of 400 cases of cardiovascular disease was reported. There were 274 (68.5%) cases of hypertension and coronary heart disease, more than other diseases. After treatment, average systolic blood pressure reduced 1.85 Kpa (13.86 mmHg), and average diastolic blood pressure reduced 1.29 Kpa (9.73 mmHg). The treatment was more effective for the type A behavior patients, so was it in the treatment of various extrasystoles (75%). To analyse the correlation between TCM syndrome differentiation and behavior pattern, it was found that there were 90.66% of Heart Yin deficiency and Liver Yang ( ) associated with Phlegm among type A behavior patients and 82.5% of Heart Qi ( ) Yang deficiency and Qi Yin deficiency among non-type A behavior patients. By means of relaxation training, EMGBT might reduce sympathetic tone, dilate blood vessels, so as to reduce blood pressure and treat arrhythmia.

[Study on relationship between insulin-resistance and syndrome differentiation typing in hypertensive patients].

OBJECTIVE: To find the relationship between insulin-resistance and Syndrome Differentiation typing (SDT) in hypertensive patients. METHODS: Two hundred and nine patients of early stage hypertension with no complication of heart, brain or kidney were selected and classified into 4 types according to SDT, the Liver-Fire exuberant type (A), the Phlegm-Dampness abundant type (B), the Yin-Deficiency and Yang-Excess type (C) and the both Yin-Yang Deficiency type (D). Their insulin sensitivity were examined and compared with that of 40 healthy subjects. RESULTS: (1) Compared with healthy subjects, all hypertensive patients had apparent insulin resistance (P < 0.05). If the insulin sensitivity of healthy subjects was defined as 1, that of patients of type A, B, C and D were 0.54, 0.58, 0.65 and 0.80 respectively. (2) Comparison between the insulin sensitivity of type A, B and C showed no significant difference, while they were compared with type D, the difference was significant (P < 0.05), that is, the insulin sensitivity of type D was close to that of healthy subjects. (3) The fasting blood insulin of type D was obviously lower and the insulin sensitivity of type D was obviously higher than that of the other three types (P = 0.0001). (4) After adjusting age, sex and body mass indexes, multivariate regression analysis demonstrated that insulin sensitivity was closely correlated with SDT (P = 0.0001). CONCLUSION: Insulin resistance is one of the pathological basis for SDT in hypertension.

[Experimental study on effect of bushen shengjing decoction on kidney yang and testicular dysfunction in rats].

OBJECTIVE: To explore the therapeutical mechanism of Bushen Shengjing Decoction (BSSJD) in treating infertility. METHODS: Fed Wistar male rats with adenine 30 mg/100 g.d continuously for 30 days. The rats manifested the symptom which was similar to the human Kidney Yang Deficiency, the genitality dropped, the function of sperm-producing in the testis was disturbed, the contents of luteinizing hormone (LH), testosterone (T) and compound F (F) in blood were all lower than rats of normal group, P < 0.01. The model rats took the BSSJD 3 ml/100 g.d (each ml contains crude drugs 2 g) continuously for 10 days, interval for three days, then medicated again and total medication of BSSJD was 30 days. RESULTS: The symptom of Kidney Yang Deficiency in rats was improved obviously, the genitality, sperm quality and function of sperm producing in testis were all enhanced obviously, so did the hormone level of LH, T, F in blood. Comparing to untreated control group, the difference was significant (P < 0.05). CONCLUSION: BSSJD has the function of reinforcing Yang and replenishing essence and regulate the endocrinological function.

Comparative observations on the types of atrophic gastritis and biopsy pathology of gastric mucosa.

300 cases of chronic atrophic gastritis were classified into 3 types of qi-deficiency of spleen and stomach, yang-deficiency of spleen and stomach, and yin-deficiency of spleen and stomach according to syndrome differentiation of traditional Chinese medicine. The biopsy specimens of gastric mucosa were examined histopathologically and for the presence of pyloric spirobacteria. The results showed that the type of yin-deficiency was the most serious in the degree of pathologic changes, or the glandular atrophy and the interstitial inflammatory cellular infiltration, while the rate of pyloric spirobacteria presence was comparable in all types.

[Effect of Weikangfu granule on ultrastructure of gastric mucosa in patients of precancerosis with spleen deficiency syndrome].

OBJECTIVE: To observe the effect of Weikangfu Granule (WKFG) on the ultrastructure of gastric mucosa. METHODS: WKFG was used to treat 61 patients with gastric mucosa intestinal metaplasia or atypical proliferation, and histopathological and ultrastructural examination on gastric mucosa of antral region obtained through gastroscopy were conducted before and after treatment. RESULTS: After treatment, the histopathology and intestinal metaplasia subset of antral focal region, and the background lesion in ultrastructure of the mucosa in the nonfocal region were all improved in the 4 groups, The effects of Spleen-Qi Deficiency syndrome group and Spleen-Yang Deficiency group were superior to Ying-Deficiency group as well as Spleen-Deficiency and Qi-Stagnation group (P < 0.05, P < 0.01), and approached to those in the healthy control group. CONCLUSION: WKFG improves the quantitative balance of internal bioactive substances to maintain the homeostasis, promotes the ultrastructural normalization of mucosa so as to cure the clinical symptoms and reverse the intestinal metaplasia and atypical proliferation of mucosa.

[Clinical observation and experimental study of the treatment of aplastic anemia by warming and tonifying the spleen and kidney].

84 cases observed were divided into 4 types according to principles of diagnosis and treatment based on an overall analysis of symptoms and signs, namely: Yang-deficiency of the Spleen and Kidney (62 cases, 73.8%), Yin-deficiency leads to internal heat (6 cases), deficiency of both the Heart and the Spleen (7 cases), and prostration of Qi after loss of blood (9 cases). The patients were treated with the method of warming and tonifying the Spleen and Kidney by using Er-Xian decoction of warming the Kidney. The three other types were also treated with the method after being relieved with the methods of tonifying the Heart and the Spleen, of nourishing Yin to relieve internal heat and cooling blood, and of strengthening Yang to stop chronic hemorrhage respectively so that function of the blood and Qi would promote each other and be improved. The total effective rate was 84.5%, and remission rate was 47.6% among 84 cases, but 91.9% and 50.0% in the type of Yang-deficiency of the Spleen and Kidney alone. Among the effective cases there was a remarkable improvement in the hemogram after treatment (P less than 0.001, less than 0.01), and the long-termed curative effect was also exciting. The mechanism of Er-Xian decoction of warming the Kidney was investigated through the nourishment of the hemopoiesis stem cell. The experiment showed that the decoction could increase CFU-S and GM-CFU in the bone marrow under the lower hemopoietic function of the bone marrow, and increase GM-CFU and CFU-E in the bone marrow of normal rats (P less than 0.001, less than 0.01, less than 0.05). The histological findings showed that there was a reduction in hemorrhage and hyperemia in the bone marrow between the decoction group and the control group, and the recovery of hemopoietic function was better than the latter.

[Relation of changes in plasma cAMP, cGMP and the clinical conditions, pathology and the type of traditional Chinese medicine in 50 cases of chronic severe icteric hepatitis].

Plasma of cAMP and cGMP in 30 cases of chronic active hepatitis (group of cholestatic patients) as well as in 20 cases of chronic hepatitis with submassive or massive necrosis (group of chronic severe hepatitis patients) were studied with 125I-marked radioimmunoassay. 50 cases with their serum bilirubin levels higher than 171 mumol/L were selected as subjects for this study. Among them, 42 were diagnosed as hepatitis B, 6 as coinfection of hepatitis A and B. According to differentiation of symptoms and signs of TCM, 24 were diagnosed as simple hepatitis due to blood stasis and blood heat, 17 as hepatitis due to blood stasis and blood heat accompanying symptoms of Yang deficiency of the Spleen and Kidney. 20 healthy persons were selected as controls. The results were as follows: cAMP was 56.82 +/- 25.54 ng/L and 80.32 +/- 20.73 ng/L, and cGMP was 9.07 +/- 6.56 ng/L and 19.49 +/- 9.34 ng/L in the group of cholestatic patients and in the group of chronic severe hepatitis patients respectively. Both were higher than those in the control group whose cAMP was 14.12 +/- 3.25 ng/L and cGMP was 5.87 +/- 1.44 ng/L (P less than 0.01). Increase in cGMP and decrease in the ratio of cAMP and cGMP in the cases accompanying symptoms of Yang deficiency of the Spleen and Kidney were much higher than those in the other two types of hepatitis patients (P less than 0.01 and 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Study on the pathological basis of classification of spleen deficiency in chronic gastritis.

BACKGROUND: Spleen in Traditional Chinese Medicine (TCM) is not actually the spleen in the anatomic sense designated in western medicine because its functions basically belong to the physiological category of digestive system in modern medicine, and it represents a macroscopic concept of digestion, absorption and nutrition metabolism. Spleen deficiency syndrome refers to the clinical phenomena such as hypofunction of digestion, absorption and nutrition metabolism. By integrating TCM with modern medicine, this paper is intended to explore the pathological basis of classification of spleen deficiency in chronic gastritis. METHOD: By means of optical microscope, scanning electron microscope (SEM), transmission electron microscope (TEM) and histochemical staining, we conducted histopathological and subcellular ultrastructural (nuclei and mitochondrial) analysis of gastric mucosa of 188 patients of spleen deficiency, and that of 42 voluntary blood donors without clinical symptoms. RESULTS: The gastric mucosa of patients with spleen Qi deficiency (SQD) and spleen yang deficiency (SyangD) could either be affected by organic lesion (type G-occurring on the basis of chronic superficial gastritis (CSG), chronic atrophic gastritis (CAG)) or unaffected (type F-chiefly belonging to functional indigestion); spleen yin deficiency (SyinD) and spleen deficiency with Qi stagnation (SDQS) both occurred on the basis of CSG and CAG; and the degree of mucosa inflammatory cells infiltration, the degree of decrease in glands propria, and the incidence of IMIIb in CSG and CAG were more serious than those of G-SQD and G-SyangD, P < 0.05 - 0.01. CONCLUSION: Spleen deficiency syndrome is likely to occur on the basis of organic lesion of gastric mucosa (disease with symptoms of both CSG or CAG and spleen deficiency symptoms), as well as on the basis of inorganic lesion of gastric mucosa (nondisease with symptoms, which is, despite spleen deficiency symptoms, there is no CSG or CAG). Besides, the clinical phenomenon of disease without symptoms (despite CSG or CAG, there is no spleen deficiency symptoms) occurres because of such factors as genetic diathesis and compensation. The lesion degree of CSG or CAG and the incidence of IMIIb of SyinD and SDQS are more serious than those CSG and CAG of G-SQD and G-SyangD.

[Study on syndrome-type in TCM and its correlation with superoxide dismutase and malonyldialdehyde in patients with non-insulin dependent diabetes mellitus].

OBJECTIVE: To explore the possible relationship between Syndrome-Type in TCM and the superoxide dismutase (SOD) as well as malonyldialdehyde (MDA). METHODS: Serum SOD and MDA were measured in 61 patients with non-insulin dependent diabetes mellitus (NIDDM) and 20 normal subjects. RESULTS: SOD activity of the NIDDM patients blood were clearly lower than those of normal subjects, but MDA were markedly elevated. From the Syndrome-Type of Yin Deficiency with Hyperactivity of Heat, to both Qi and Yin Deficiency, to both Yin and Yang Deficiency, or from without hemostasis to with the appearance of hemostasis in order. The SOD activity decreased and meanwhile MDA increased gradually. CONCLUSIONS: SOD activity and MDA levels in patients with NIDDM were associated with Syndrome-Type in TCM, it might be responsible for the diagnosis of Syndrome-Type in TCM.

[Histopathology of spleen deficiency syndrome of chronic atrophic gastritis and its ultrastructural changes].

Pathologic change of gastric mucosa in 247 CAG patients of Spleen Qi (Yang) Deficiency (QYD) and Spleen-Stomach Yin Deficiency(YD) Syndrome were observed. The surface of gastric antrum mucosa of 26 patients with scanning electron microscope as well as the gastric mucosa of 34 patients with transmission electron microscope was observed for their intracellular structure. It was found that the CSAG and CAG I of Spleen QYD amounted to 165 cases (66.8%), and CAG II and III of Spleen-Stomach YD amounted to 34 cases (64.2%). It denoted that the Spleen QYD belonged to mild case, while the Spleen-Stomach YD belonged to severe case. The mitochondria of all kinds of gastric antrum mucosa cells showed retrograded degeneration, the pathologic basis of Spleen Deficiency of CAG, those of Spleen-Stomach QD had sparse broken and swollen mitochondria (82.3%), while the 53.3% of Spleen-Stomach YD displayed vacuolation as its predominant change. The different severity of retrograde degeneration and damage of mitochondria yielded various stage in atrophy of gastric mucosa which was the basis of Syndromes evolution from Defi. to QYD and then YD. Inflammatory change of the gastric mucosa was predominant in Spleen-Stomach YD (45.3%), and far less in QYD (27.1%), P < 0.01.

[Diphasic regulatory effect of lishen injection on sex hormones in senile female patients with coronary heart disease of kidney deficiency type].

Effects of Lishen injection on serum estradiol (E2), testosterone (T) and the E2/T ratio in 30 cases of postmenopausal patients with coronary heart disease and of Kidney Deficiency type were observed. Results showed that: (1) Serum E2 and the E2/T ratio were lower (P < 0.01) in the group of Kidney-Yin Deficiency type and higher (P < 0.01) in the group of Kidney-Yang Deficiency type as compared with normal control. (2) After treatment with Lishen injection the levels of serum E2 and the E2/T ratio were elevated in the former (P < 0.01) and lowered in the later (P < 0.01). Both were approaching to normal level (P > 0.05). (3) The levels of T in both groups were unaffected (P > 0.05) before and after treatment. These results suggested the imbalance of Kidney-Yin and Kidney-Yang is related to the disturbance of sex hormones and Lishen injection could not only regulate the Kidney-Yin and Kidney-Yang, improve the symptoms of Kidney Deficiency, but also could convert the abnormal change of sex hormone, it has a good diphasic regulatory effect on E2, and is beneficial on balance of sex hormones of both sex in the inner environment of human body.

[Observation on 104 senile chronic renal insufficiency patients treated with integrated traditional Chinese and Western medicine].

A clinical study on 104 cases of senile chronic renal insufficiency (SCRI) was conducted. In order to analyze the clinical features of SCRI and the efficacy of integrated TCM-WM treatment, all cases were treated by this combined therapy, except for the treatment of primary diseases and removing the aggravating factors. It was concluded that the effective rate reached 57% it had no significant difference with 61% of non-senile group, P > 0.05. The senile patients featured Kidney Yang Deficiency in TCM Syndrome Differentiation. It could guide the clinical treatment and thus the senile patient's Yang should be strengthened at usual time. When Kidney Qi was insufficient and frequent nocturia appeared, sufficient water supply as well as Warming Kidney Yang drugs should be given and that Yang Qi should be protected all the time. Also diuretics and purgatives should not be misused which might damage Yin and Yang.
