Long-term adjustment and community reintegration following spinal cord injury.

This paper presents the results of a 2-year mixed methodology (quantitative and qualitative) study examining the reintegration and quality of life of community-residing adults with spinal cord injury (SCI). A convenience sample of 100 individuals with SCI completed three questionnaires for the quantitative study phase. A self-selected subsample of 34 of the participants then attended qualitative focus groups to further explore adjustment to living in the community after SCI. The results revealed that locus of control is crucial to subjective quality of life, productivity status, satisfaction with performance of daily activities and satisfaction with community integration. Respondents confirmed that substantial adjustments are required after SCI, and that these can be aided through education, involvement in productive activity and participation in social and leisure activities. A dominant finding was that social support and peer mentoring were invaluable. Stable health and appropriate pain management were crucial to subjective satisfaction with community integration, yet for many of the respondents these were elusive. Although the participants valued the rehabilitation process, they felt that "the system" was not client-centred and that the timing of services and information was not always relevant to individual needs. The clinical and practical implications of these results are discussed.

Does vestibular stimulation activate thalamocortical mechanisms that reintegrate impaired cortical regions?

Caloric stimulation induced a transient reversal of multimodal hemispatial cognitive deficits in an 81-year-old woman with an acute left cerebral hemisphere stroke. The patient had unawareness of her right hand (asomatognosia), right-sided visual unawareness (hemineglect), aphasia and right-sided weakness (hemiplegia) prior to the stimulation. Transient improvements in impaired sensory, motor, linguistic and cognitive function developed within 30 s following application of the caloric stimulus and onset of horizontal nystagmus. The effect persisted for 3 min and ceased completely after 5 min. While several recent reports have described the capacity of caloric stimulation to transiently improve or reverse a wide range of attentional, cognitive and motor impairments, most examples are in right-hemisphere-damaged patients with long-standing brain injury. Typically, patients have been tested several months or years after the onset of the deficit. A possible mechanism for the temporary reintegration of multiple cognitive functions in this patient is discussed.

Does money work? Cash transfers to ex-combatants in disarmament, demobilisation and reintegration processes.

This paper analyses the relevance and potential of cash transfers as part of the disarmament, demobilisation and reintegration (DDR) assistance packages provided to ex-combatants in transitions from war to peace. To this end, a theoretical framework is established that permits the identification of the advantages and disadvantages of using cash in DDR. Subsequently, an empirical analysis is carried out to compare selected theoretical assumptions on the use of cash with the reality of lessons learned from recent experience in Sierra Leone and other African countries. The study shows that some theoretical drawbacks commonly associated with the use of cash in DDR processes may indeed bear out in practice. At the same time, though, the paper argues that the utility of cash transfers in DDR is affected by a variety of factors that go far beyond the simple choice of employing cash, most notably decisions on payment location, eligibility criteria and targeting.

A novel illegitimate recombination event: precise excision and reintegration with the Mu gem mutant prophage.

The bacteriophage Mu is known to insert its DNA more or less randomly within the Escherichia coli chromosome, as do transposable elements, but unlike the latter, precise excision of the prophage, thereby restoring the original sequence, is not observed with wild-type Mu, although it has been reported with certain defective mutants. We show here that the mutant prophage Mu gem2ts can excise precisely from at least three separate loci -- malT, lac and thyA (selected as Mal+, Lac+ and Thy+, respectively). This excision occurs under permissive conditions for phage development, is observed in fully immune (c+) lysogens, and is independent of RecA and of Mu transposase. Mu gemts2 excision is invariably accompanied by reintegration of a Mu gem2ts prophage elsewhere in the chromosome. In the case of Mal+ revertants, this prophage is systematically located at 94 min on the E. coli chromosome. Mu gem2ts excision therefore sheds some light on the long-standing paradox of the lack of precise Mu excision.

Targeted reintegration of expatriate brains into developing countries of origin: the EEC-IOM experience in Central America.

"The present article...reviews the major findings of the end-evaluation of the EEC-IOM project in Central America, the Dominican Republic and Panama, a five-year scheme funded by the Commission of the European Communities (EEC) and implemented by the International Organization for Migration (IOM), to promote a selective and development-oriented return of Latin American professionals to Costa Rica, the Dominican Republic, Honduras, Nicaragua and Panama.... The core objectives of this evaluation, undertaken in September-December 1989, were to assess the project's contribution to local socioeconomic, scientific and technological development through the selective supply of expatriate professionals to developing institutions operating in so-defined priority areas; to identify the returnees' personal reintegration patterns at the professional, socioeconomic and family-related levels; and to measure the project's cost effectiveness." Social reintegration of sexual delinquents by a combination of psychotherapy and anti-androgen treatment.

The high frequency of recidivism with convicted sexual perverts is mainly due to the compulsive character of perversion. The classical methods of treatment are seldom satisfactory: Surgery provokes ethical objections, psychopharmaca and oestrogens lead to disagreeable side effects, psychotherapy of any kind is unlikely to be efficacious because of the unfavourable atmosphere the nature of perversion causes it to take place in. However, the climate of both psychoanalytical treatment and suggestive and directive psychotherapy can be enhanced by a combination with an anti-androgen treatment, which eliminates the patient's fear of conviction by inhibiting his sexual urge. Two cases illustrate this combined method. On the basis of a positive disposition, which arises from the improved climate, sublimation of the sex drive and the achievement of a feeling of social responsibility is aimed at. The advantages of the combined therapy are then discussed in comparison with the existing treatments. It is concluded that this combined treatment, next to social guidance and an attempt at solving the fundamental problem of loneliness, can promote the social reintegration of the convicted sexual pervert.

A novel type of unstable homogeneously staining region with a head-to-tail arrangement: spontaneous decay and reintegration of DNA elements into a plethora of new chromosomal sites.

After transfection of amplification-promoting DNA elements into mammalian cells, homogeneously staining regions (HSRs) are formed by high copy numbers of transfected DNA arranged in head-to-tail polymers. Here, we wanted to evaluate the stability of this type of HSR during prolonged cultivation of transfected cells in selective medium. Thymidine kinase-deficient mouse L cells were transfected with pAPR4tk DNA harboring the amplification-promoting element 4 (APR4) linked to the gene for thymidine kinase (TK) or, alternatively, transfected with a DNA construct (pARP4t-PA) carrying, in addition, the expression cassette for human tissue-type plasminogen activator (t-PA). After transfection, one or two HSRs per cell were formed that disintegrated spontaneously after 25-40 wk of continuous cultivation in the presence of selective HAT (hypoxanthine-aminopterin-thymidine) medium. Unexpectedly, plasmid DNA reinserted into a plethora of new chromosomal sites, as revealed by in situ hybridization and Southern blot analysis. Coincidently, secretion of t-PA decreased to 10-20% of its original level. After transfection of pAPR4tk DNA lacking the t-PA expression cassette, HSR decay and reintegration of plasmid constructs into multiple chromosomal sites were also observed, whereas the ptk vector without an amplification-promoting DNA element did not form an HSR after transfection. We conclude that, in contrast to the pattern of known structures with head-to-tail arrangements, the HSR formed by amplification-promoting DNA elements represents a novel type of HSR that disintegrates by transposition into a plethora of new chromosomal integration sites. This process is mediated by the amplification-promoting DNA element itself and can be observed even when selective pressure is maintained.

Reintegration of chronic mental patients into the Malaysian community.

This paper describes a rehabilitation programme for male chronic mental patients, with the aim of reintegrating them into the community. Since in Malaysia about 50% of the work force are stilling earning their living in the agricultural sector, the programme concentrates on training in farming and animal rearing. Since the patients are very institutionalised, resocialisation training is also provided that involves learning to communicate, to mix with others, to know how to use money, to make one's own demands, and to cope with more personal freedom. When a patient is trained well enough to be employed outside, a job is found for him that provides food, lodging and payment. After discharge from the hospital, he is regularly followed up by the social worker.

Children and adolescents with traumatic brain injury: reintegration challenges in educational settings.

Changes in federal law have paved the way for students with traumatic brain injury (TBI) to receive special educational services. Although not all students with TBI need special education, many will need careful planning for reintegration into the classroom. The present article presents information about the range of services that need to be considered for these students and the problems that can affect students' academic and social functioning. Characteristics that distinguish this group of students from peers with special learning needs are highlighted and specific suggestions are made to help educators work more effectively with these students when they return to school. A case study is used to highlight the issues discussed.

Reintegration: public or private responsibility? Consequences of Dutch and Danish policies toward work-disabled persons.

The Scandinavian welfare states are known for their universal social security coverage and high labor-market participation rates for all groups in society. The institutional setup in continental European welfare states, on the other hand, is said to foster a divide between employed persons and persons outside the labor market. In the area of disability policies, The Netherlands and Denmark illustrate this distinction. In The Netherlands, strong ties exist between the employer and the sick-listed worker, both because of solid job-protection legislation and because the financing of sickness and disability benefits is closely linked to the individual employer. In contrast, employer responsibility is limited in Denmark, where an extensive responsibility rests with public authorities. The author investigates the consequences of these differences using panel data on Dutch and Danish long-term sick-listed workers. The findings show that the strong ties in The Netherlands between employers and sick-listed workers enhance labor-market reintegration of long-term sick-listed workers. A significant drawback of the Dutch policy, however, is that employers refrain from employing persons with a high risk of falling ill. As a result, Dutch disability policy leads to a bigger insider-outsider divide than Danish policy.

[Reintegration of health services in the Danube area of the Croatian health care system: introduction of laparoscopic surgery in the Vukovar General Hospital].

After six years of occupation, in the second half of 1997 started the reintegration of the Danube region in the Croatian health care system. Introducing of the laparoscopic surgery in Vukovar General Hospital has been presented in this article. A part of the laparoscopic equipment and reusable instruments was a gift of the Croatian Institute for Health Insurance, and some equipment and instruments were bought. Until now, 170,000 HRK has been spent for laparoscopic surgery. The first laparoscopic operations--laparoscopic cholecystectomies in our hospital were performed on June 25, 1998. From October 1, 1998, laparoscopic operations have been regularly done. Until December 24, 1998, laparoscopic cholecystectomies, hernioplasties, appendectomies and diagnostic laparoscopies were performed. The total number of laparoscopically operated patients is 23. We did not have serious intra or postoperative complications. On November 20-21, the 10th postgraduate course on laparoscopic cholecystectomy was organized in our hospital. In a relatively short time and specific circumstances, laparoscopic surgery has become a standard surgical method in Vukovar General Hospital.

[Reintegration to work in persons having survived a myocardial infarction: current review and ways of development for clinical interventions].

The main purpose of this article is to update the state of knowledge regarding work reintegration following myocardial infarction. Occupational perspectives have greatly evolved during the second half of this century. This can be attributed mainly to a better understanding of the pathology itself and to a development of new therapeutic avenues which are part of a more global vision of the bio-psycho-social characteristics of the patient. The authors did not intend to realize an extensive review of the literature on the subject, but analysed the most significant articles published over the past 20 years. They were selected because of their frequent citation on the scientific literature which seems to confirm their significance and credibility. Following the analysis of the articles selected, the authors conclude that the scientific community has reached a consensus to the effect that the general evolution of the patient post myocardial infarction is greatly determined by psychosocial factors. It follows that comprehensive rehabilitation programs despite their limited impact in an experimental setting offer some stimulating perspectives. They suggest that some research effort be oriented towards the evaluation of the actual long term outcomes of rehabilitative programs especially on quality of life in general and on quality of life at work more specifically.

An analysis of clinicians' verbal behaviors in a community reintegration program for traumatic brain injury survivors.

The authors present an analysis of clinician verbal behaviors in the Community Reintegration Program, a treatment program for traumatic brain injury survivors. Experienced clinicians from different allied health professions were videotaped as they provided group treatment. Each clinician's verbal behaviors were coded using an adaptation of the Clinical Interaction Analysis System, a reliable system for describing verbal behaviors. Clinicians then participated in a structured interview with the investigators; during the interview, clinicians reviewed their treatment tape and stated what treatment goals they were working on during each utterance. The majority of the clinicians' behaviors fell into two categories: clinician-initiated discourse and explanation. Chi-square analyses revealed that these two categories occurred significantly more frequently than other coded categories. These two verbal codes predominated regardless of the goal being addressed and regardless of the professional background of the treating clinician.

[Realities and mirages : psychic and social spaces of reintegration.].

This study, based on a sampling of people who were diagnosed schizophrenic, sought to describe the different types of positive and negative social réintégration. Research combined an anthropological approach with rigid discipline in the gathering of data. The authors examined réintégration from three points of view : social relations, social roles, and utilization of space and time (l'espace-temps). Data shows that people who are constantly rehospitalized feel marginalized and experience rejection, particularly with family and relatives, and that these feelings sharply contrast the relatively normative expectations concerning réintégration. In these cases, the patient's psychiatric environment tends to structure the reintegration process. However, people who have not been rehospitalized are characterized by a dominant type of réintégration that is best described as"posi-tive retreat" ("retrait positif). This characteristic is supported by various psychological, social and cultural hypotheses. Moreover, an intercultural perspective suggests that certain traits in North American societies tend to make people look down on "positive retreat" and view this form of readaptation as negative.

Reintegration for stroke survivors. Home and community considerations.

Plans for home and community re-entry for the stroke patient should begin as soon as medical stability is achieved. The majority of stroke patients will require a comprehensive program of rehabilitation. Patients and their families should be educated about stroke and the impairments associated with it to promote active participation in the rehabilitation process and to solve problems before they happen. The participation of the family and the use of appropriate community health resources for the stroke patient who is in a safe and accessible home and community environment will markedly enhance a successful outcome. A systematic approach is necessary for people with stroke to achieve their optimum functional level and proper reintegration to home and community.
