Clinical effects of proteinase, sfericase (AI-794), on chronic bronchitis and similar diseases.

The clinical effect and utility of sfericase was compared with an inactive placebo by a double-blind method in three groups of a total 109 patients suffering from chronic respiratory diseases who had complained of difficult expectoration of sputum. A significantly greater improvement of expectoration was obtained in Group A-1 (6 tablets/day). Also in group A-1, more cases showed subjective and objective improvement, i.e., improvement in expectoration, reduction in coughs, decrease in the amount of sputum, and disappearance of râles. However, the sputum and its degree of purulency remained unchanged in most cases in all three groups. Side effects were few. The clinical efficacy of sfericase in the group with 2 tablets, 3X/day (total daily dose of 6 tablets) was shown to be significantly superior to the other two groups. This dosage was established as the optimal dose for treating difficulty in expectoration.

Clinical results of sfericase (AI-794) in chronic parasinusitis.

Forty patients with chronic sinusitis were treated orally with sfericase tablets (each tablet = 10,000 units), a proteolytic enzyme preparation, at a daily dosage of six tablets for adults and three tablets for children in three divided doses after each main meal for 4 weeks. As regards subjective symptoms, very favorable results were obtained after 4-week treatment with sfericase as evidenced by an efficacy rate of 81.6% for postnasal rhinorrhea, 66.7% for rhinorrhea, and 43.6% for nasal obstruction. However, with respect to objective symptoms, the efficacy rate was 28.9% for redness and 30.8% for swelling of the nasal mucosa, with a somewhat lower rate than that obtained for subjective symptoms. According to the patient's and physician's global assessments, sfericase was effective in 65 and 57.5% of the cases. None of the cases showed undesirable reactions to sfericase.

[Separation of high density lipoproteins (HDL) using zonal ultracentrifugation in patients with apolipoprotein AI Milano variant].

Investigations of HDL with zonal ultracentrifugation were carried out in three patients with apolipoprotein AIMilano. HDL in patients with AIMilano apolipoprotein are characterized by the practically total absence of HDL2. The HDL3 peak is broader than normal. In spite of the marked reduction or practically total absence of HDL2, which is believed to be a protective factor against atherosclerosis, our patients show no signs of clinical atherosclerosis.

Characteristics, attitudes and personalities of AI donors.

The first 50 men recruited to an artificial insemination by donor (AID) programme have been assessed for their attitudes and characteristics using a basic questionnaire and a standard well evaluated personality inventory. The results have shown that donors can be characterized as stable and moderately extroverted people who come from a wide range of occupations. Their marital and parental status had no particular relationship to their desire to donate and most of them volunteered for altruistic reasons, financial remuneration being a poor inducement. Most subjects lived within the close environs of the Westmead Centre and preferred to donate during normal working hours. The inconvenience of donating appeared to be the most important factor in considering new approaches to recruiting semen donors.

[Intrinsic cholinergic components in the cholinergic innervation of the auditory cortex in the cat (zone AI)].

Neuronally isolated auditory cortical slabs (zone Al) of cat were studied using histochemical AChE reaction 3 days and 1, 2, 3 weeks after isolation. Following structures were observed: cholinergic long-axon associative neurons responding to the cortex isolation by retrograde degeneration and AChE hyperreaction (Cajal-Retzius cells of layer I and neurons of layer VI whose axons travel to the subcortical layer of the associative fibres); cholinergic short-axon associative cells of layers II-VI, which retain normal structure and moderate AChE activity after isolation; their axon collaterals terminate on neighbouring neurons; short-axon neurons are more numerous in deeper layers of the cortex; on the whole they predominate over the long-axon associative neurons; cholinoceptive cells (pyramidal and stellate) were found with cholinergic terminals on the soma and proximal parts of the dendrites; these terminals were formed by associative cell axons. Cholinoceptive neurons occurred more frequently in deeper layers of the cortex.

Properties of lipid-apolipoprotein association products. Complexes of human apo AI and binary phospholipid mixtures.

The products resulting from the association of human apo A-I with binary phospholipid mixtures of dimyristoyl phosphatidylcholine (DMPC) and either dipalmitoyl (DPPC) or distearoyl (DSPC) phosphatidylcholine have been isolated and characterized. Effective lipid . protein complex formation was found to occur at the onset temperature for melting of the gel state, and equal incorporation of both lipid components of the binary mixture was observed. Two sizes of products were obtained, one containing 2 A-I molecules per complex and the other containing 3; the proportions of these two products depended upon the initial phospholipid/protein ratio employed. these two product species were found to be resolvable by density gradient centrifugation as well as gel filtration, reflecting substantial differences in composition as well as size. The ratio of DMPC to DPPC or DSPC was the same in the isolated complexes as in the initial mixture, suggesting that th protein does not associate preferentially with the fluid phase lipid, but with lipid domains in which the components are randomly distributed. Electron microscopy of recombinant particles containing a 2:1 ratio (w/w) of DSPC to DMPC revealed stacks of discs whose thickness was proportionately greater than for discs containing DMPC alone. Also of significance was the finding that recombinant discs containing 3 A-I molecules possessed a diameter approximately 1.5 times larger than recombinant discs containing 2 A-I molecules. These data are consistent with the model for the recombinant particles described by Tall et al. (Tall, A.R., Small, D.M., Deckelbau, R.J., and Shipley, G.G. (1977) J. Biol. Chem. 252; 4701-4711), in which the phospholipid is found as a circular bilayer, the thickness of which is dependent upon the length of the acyl chain, and around which the protein is distributed as an annulus.
