[Pathogenesis of late radiation necrosis of the brain].

The pathogenesis of late radiation necroses remains still unsolved in the problem of cerebral radiation injuries. The problem of primary site: vessels or medulla is a disputable one. In this case cerebral radiation nectosis developed in a 32-year-old woman 1.5 yrs. after the last course of radiotherapy for hypophyseal adenoma was over. The authors give an irradiation scheme and a detailed histological description of the foci of late radiation necrosis in the temporal lobe and in the substance of the Varolii pons which is not the zone of direct exposure. The histological picture described in the above foci is similar in many details to the process that takes place in experimental allergic encephalomyelites developing in sensitized animals. The time course of histological changes in the foci of radiation necroses shows that the process does not develop steeply but increases gradually affecting the adjoining zones of the medulla that cannot be accounted for by the direct involvement of the medullary tissue only during radiation. Thus it can be assumed that the process develops like an allergic reaction where the role of the allergen is played by the decomposition products of myelin that appear in the affected focus.

Age at onset of puberty following high-dose central nervous system radiation therapy.

OBJECTIVE: To determine if a relationship exists between age at irradiation, sex of the patient, and age at onset of puberty and pubarche in children treated with high-dose radiation to the central nervous system. DESIGN: Case series. SETTING: Tertiary care institutional practices and clinics. PATIENTS: Thirty-six children treated with high-dose irradiation (hypothalamic pituitary dose, 30-72 Gy) by conventional (n = 29) or hyperfractionated (n = 7) schedules. Girls were treated before age 8 years and boys before age 9 years. Twenty-six of the 36 children also received chemotherapy. All tumors were distant from the hypothalamic-pituitary region. MAIN OUTCOME MEASURE: Age at onset of puberty and pubarche. RESULTS: In girls, the median age at onset of puberty was 9.3 years vs 10.9 years for controls (P < .01); pubarche occurred at 9.4 years vs 11.2 years for controls (P < .01). In boys, the median age at onset of puberty--genital II--was 11.0 years vs 11.5 years for controls (P = .30); pubarche occurred at a median age of 10.5 years vs 12 years for controls (P = .25). A censored-data normal linear regression model was used to account for children (n = 6) who had not reached puberty. Age at diagnosis (P < .01) and sex (P = .01) were significant predictors of age at onset of puberty. Body mass index SD score (z score) was inversely related to age at onset of puberty (r = -0.77) and was greater at onset of puberty in girls than in boys. CONCLUSION: In children who have received high-dose cranial radiation therapy, a significant positive correlation exists between age at diagnosis and age at onset of puberty in boys and girls.

Curability of third ventricular region tumours. A multidisciplinary approach to evaluate the places of operative, radiation and chemotherapy.

Between 1976 and 1988, 17 patients suffering from tumours located in the third ventricular region were entered into a retrospective study. There were 11 males and 6 females aged from 2 to 30. These tumours were located in the pineal area (n = 12), in the floor of the third ventricle (n = 3), in the roof of the third ventricle (n = 1) and multiple midline tumours (n = 1). A ventriculoperitoneal shunt was performed in 13 patients. In 8 patients the tumour was operated upon directly. 2 patients did not undergo surgery. A histological examination was performed in 13 cases with identification of 9 germinomas, 2 pinealoblastomas and one astrocytoma. In one case the plasma and cerebro-spinal-fluid level of tumour markers (human béta chorionic gonadotrophin & alpha feto protein) were elevated. All patients received 45 to 60 grays by cobalt megavoltage irradiation on the tumoural volume, 8 of them receiving additional whole brain and spinal irradiation ranging from 30 to 36 grays. One patient had chemotherapy in addition to radiotherapy. The 5 years survival rate was 74% and the mean follow-up period was 5 years. Five deaths occurred: 1 during irradiation, 3 following a relapse and 1 unexplained death later on. There were no neurological sequelae related to irradiation but 7 patients (37%) had pituitary insufficiency. The irradiation dose level required to obtain local control, the methods of spinal prophylactic treatment and the role of chemotherapy are discussed.
