Techniques for transposition of muscles are described when function preservation may be important. Anatomic and clinical data are presented to support the technique of muscle transposition with function preservation.
A modality is presented by which the flexor digitorum brevis muscle, with or without its overlying skin, can be transposed for soft tissue coverage of the heel area. Surgical anatomy, cadaver dissection, operative technique, and our clinical experience in six patients are presented.

This is a retrospective study of 30 children and adolescents who have undergone Butler arthroplasty for correction of an overriding fifth toe. Bilateral operations were performed on six patients, giving a total of 36 operations. Good to excellent results were obtained in 34 of the 36 operations. With attention to detail, this is a simple and safe operation. Full correction is obtainable without postoperative splintage or cast immobilization. This study represents the only reported experience in North America of this little-known procedure.
A 14-year-old boy with linear localized scleroderma had a dramatic improvement in contractures after treatment with N-(3',4'-dimethoxycinnamoyl) anthranilic acid (tranilast, Rizaben). The observation that this anti-allergic drug was effective in localized scleroderma lends further support to the concept that mast cells play a role in increased collagen synthesis in this disease.
This study is part of a work to develop a radiographic method that defines objectively the individual conformation of an equine digit and its appropriate trimming. The authors used isolated distal limbs fastened on a rotation support with variable inclinations to study the influence of induced asymmetric bearings on various angles measured from specific radiographs. The digit responded to asymmetric bearings by 1. deformation of the hoof; 2. rotation of the phalanges; and 3. joint asymmetry or local articular space narrowing. The two last situations occurred mainly in the distal interphalangeal joint. Some of the angles mentioned above allow quantification of interphalangeal articular asymmetry, angular deformity and rotational deviation of a distal limb.

Congenital coalition of the middle and distal phalanges in the fifth toe are common. They have been reported to occur in up to 50% of American and European populations and in up to 73% of people of Japanese descent. Congenital interphalangeal coalition of the other toes is less common. We report two cases of fracture through an interphalangeal coalition following blunt trauma. Diagnosis was delayed in these fractures. One fracture healed only after prolonged immobilization and the other developed a delayed union. Patients with this fracture should be advised of the prolonged convalescence since this may delay their return to preinjury activities.
