Extramedullary hematopoiesis involving the bilateral lacrimal fossae.

Extramedullary hematopoiesis is a complication of a number of myeloproliferative disorders. MR imaging of the orbits was performed in a patient with bilateral orbital swelling with known myeloproliferative disorder and myelofibrosis. The study revealed symmetric, bilateral enhancing masses in the lacrimal fossae extending into the nasolacrimal ducts. Biopsy demonstrated sclerotic extramedullary hematopoietic tumor.

Transitional cell carcinoma of the lacrimal sac.

Lacrimal sac tumor is a rare tumor, for which we do not know the exact incidence. Malignant tumors account for 70% of all cases. Epithelial tumors are the most common pathological type and squamous cell carcinoma is the most common of these, a transitional cell carcinoma is rare and has a poor prognosis. The authors report a rare case of transitional cell carcinoma of the lacrimal sac. A 57-year-old woman had tearing on her right eye. The pre-operative diagnosis was possible nasolacrimal duct obstruction with signs of dacryocele. Intraoperatively the authors found a lacrimal mass and a frozen section showed squamous papilloma with dysplasia. The tumor was completely removed and DCR was done. The subsequent pathological report of the right lacrimal sac was papillary transitional cell carcinoma, so medial maxillectomy and resection of the medial inferior orbit with ethmoidectomy were performed. She received radiation and has been tumor-free for 2 years.

Congenital dacryocystocele with acute dacryocystitis: report of two cases.

Congenital dacryocystocele is an uncommon lesion of the nasolacrimal drainage system of newborns. The lesion often develops secondary infection due to the obstruction of both distal and proximal nasolacrimal ducts. Early recognition by experienced pediatrician might initiate effective therapy to prevent the progression to possible secondary infection. However, there is no standard criteria for optimal treatment. Hence, we review the literature and report two newborns with congenital dacryocystocele in one of which progressed to secondary infection, dacryocystitis.

Morphologic characterization of conjunctiva-associated lymphoid tissue in chickens.

Conjunctiva-associated lymphoid tissue (CALT) in the eyelids of chickens was studied by gross, histologic, and electron microscopic techniques. Structural features were characterized at 1 day of age and at posthatching week (PHW) 1, 2, 3, 4, 6, 8, 12, and 16. Beginning at PHW 1, prominent lymphoid nodules containing a heterogenous population of lymphocytes, lymphoblasts, and macrophages were first observed within conjunctival folds and fissures of the lower eyelid. Nodules contained germinal centers by PHW 2 and plasma cells by PHW 4. The epithelium associated with these nodules was flat, had short, irregular microvilli, contained intraepithelial lymphocytes, and lacked goblet cells. High endothelial venules were located at the base of lymphoid nodules and contained lymphocytes within and below the cuboidal endothelium. In the upper eyelid, CALT was morphologically similar to lymphoid tissue in the lower eyelid, but nodules were smaller and more random, lacked association with epithelial folds and fissures, and were clustered around the opening of the nasolacrimal duct. By PHW 12, CALT was characterized by basal germinal centers outlined by collagenous stroma, suprafollicular plasma cells, columnar epithelium with goblet cells, and fewer intraepithelial lymphocytes. On the basis of these features, CALT in chickens has morphologic characteristics similar to other components of the mucosal immune system and, therefore, may have a role in mucosal immunity.

[Transnasal endoscopic sinus surgery].

The endoscopic sinus surgery is probably the most exciting topics in rhinology. Not only is the technology new, but the approach to sinus disease has changed dramatically. In this paper, we tried to present how sinus endoscopy can be applied to the treatment of sinus diseases and more radical forms of sinus surgery. Three kinds of surgical procedures were performed, they were the functional anterior ethmoidectomy, total ethmoidectomy, and total sphenoethmoidectomy for treatment of chronic ethmoid sinusitis (2 cases), polysinusitis with polyps (16 cases), and pansinusitis with polyps (5 cases). The results showed that the pus disappeared in 14 cases (60.8%), the dizziness and headache improved in 22 cases (95%), the smell improved in 14 cases (70%). There were 4 complications: nasolacrimal duct injury in one, and lamina papyracea injury in three. All of them recovered after management.

The success rate of silicone intubation in congenital lacrimal obstruction.

In our series of 100 patients who had unsuccessful probing for congenital nasolacrimal duct obstruction, all had silicone intubation associated with infracturing of the inferior turbinate. The age range was from 7 months to 4 years at the time of intubation. Sixty patients had tubes in for 1 year, and all but one, a 3-year-old girl, were treated successfully. Of the 40 who had tubes in place for less than 1 year, 38 were treated successfully. The two unsuccessful cases were those of a 3-year-old boy and a 4-year-old boy, whose tubes were removed at 3 and 4 months, respectively. The overall success rate in our series was 97%; all patients younger than two years old at the time of intubation were treated successfully.

Dacryocystorhinostomy at Ramathibodi Hospital.

Two hundred and seventy patients with idiopathic obstruction of the lacrimal passage between the sac and the nasolacrimal duct, who underwent external dacryocystorhinostomy at the Department of Ophthalmology, Ramathibodi Hospital were studied retrospectively. All the patients had the operations between 1983 and 1987; 240 were female (88.9%) and 30 were male (11.1%), with, a ratio of 8 female to 1 male. The ages ranged from 3 to 83 years, with the peak in the 46-55 age group (25.19%). The overall surgical success rate was 86.5 per cent, the definition for success being a relief of the obstruction as evident symptomatically and by the ability to irrigate the saline from the puncta to the nasal cavity at follow-ups of more than 21 days postoperatively. The patients were further subdivided into the "silastic" group and the "non-silastic" group: and the success rate were 75.5 and 95.7 per cent respectively. Significant complications of the dacryocystorhinostomy were recorded in 11 cases; 2 wound infections, 2 lacrimo-cutaneous fistulae, 5 severe operative hemorrhage and 2 ethmoidal sinus involvement.

Alternative pathways for drainage of cerebrospinal fluid in the canine brain.

Although the brain has no formal lymphatic system, a substantial quantity of cerebrospinal fluid (CSF) has nonetheless been shown to drain via cervical lymphatics. To pursue further the issue of alternative drainage pathways for CSF, we infused a solution of Ringer's lactate (RL) into the cisterna magna of the dog brain and monitored both the flow and concentration of total protein of cervical lymph. This maneuver promoted a nearly three-fold rise in intracranial pressure and was accompanied by a rise in cervical lymph flow and fall in its protein content. In addition, a profuse nasal discharge (11.4 ml/hr) developed with a moderately high protein content of the rhinorrhea fluid (1.8 g/dl), along with similar appearance times of Evans blue dye (instilled in the cisterna magna) in both cervical lymph and the rhinorrhea fluid (48-70 minutes after infusion). These findings suggest alternative drainage pathways for CSF besides the arachnoid villi (Pacchionian bodies) including connections with lymphatics in the neck and along the olfactory nerve, and around the cribiform plate to the nasal submucosa, and with proptosis, perhaps also through the aqueous humor-canal of Schlemm and nasolacrimal duct.

Pattern of paediatric ocular problems in north India.

In this study, 3,490 children were examined to know the pattern of ocular problems in children in this part of the country. It showed that heritable diseases were quite prevalent, i.e., 41.49%. In this group refractive error, nasolacrimal duct block, and primary squint were most common, while in the non-heritable group various types of conjunctivitis, trauma, foreign bodies, blepharitis and nutritional diseases were most prevalent.

[Rubinstein-Taybi syndrome--a report of three cases].

The main features of Rubinstein-Taybi syndrome include special facial appearance, broad thumbs, great toes and mental retardation. This syndrome presents many organogenetic and systemic deformities and various congenital ocular abnormalities, such as epicanthus, antimogoloid palpebral fissures, highly arched eyebrows, long eyelashes, obstruction of nasolacrimal ducts, strabismus and iris coloboma. In this paper we report three children with Rubinstein-Taybi syndrome with the above systemic and ocular clinical findings, and think that the lacrimal abnormality should not be overlooked in this syndrome. We brief discuss the genetics of this syndrome and suggest that the search for minimal chromosomal defects by high resolution technique might be useful for etiologic and genetic research.

Dacryocystectomy for the treatment of dacryocystitis patients with Wegener's granulomatosis.

We evaluated a woman with Wegener's granulomatosis who was experiencing recurrent dacryocystitis. Nasolacrimal duct obstruction had been present for many years and epiphora was not a significant concern. Our experience with the development of nasalcutaneous fistulas following dacryocystorhinostomy in several patients with Wegner's granulomatosis led us to perform a dacryocystectomy. The functional result was good, and there were no problems with wound healing. Dacryocystectomy should be considered as an alternative to dacryocystorhinostomy for dacryocystitis in patients with Wegener's granulomatosis and similar disorders in whom epiphora is not a major complaint.

Congenital fistulas of the lacrimal sac.

Four patients with congenital lacrimal sac fistulas are presented. All cases were unilateral and not associated with an occluded nasolacrimal duct. In three cases, the fistula and surrounding skin were surgically removed. The fourth case required no treatment. Microscopic examination revealed that the fistulas were lined with stratified, squamous epithelium with or without keratosis, suggesting that they may have developed as outgrowths from the sac or common canaliculus.

Treatment of the lacrimal excretory system after resection of medial canthal and eyelid tumors.

A simplified method of lacrimal excretory system repair is presented. If part of a canaliculus is resected during removal of an eyelid tumor, the remaining section of the canaliculus can be exteriorized to the lacrimal lake. A silicone tube is threaded into the canaliculus and allowed to remain in place for one to two weeks. If both canaliculi and the common canaliculus are removed during resection for a medial canthal tumor, a silicone tube can be threaded into the nasolacrimal duct and brought out the area of the medial canthal angle. Conjunctiva which is wrapped around the tube can then form a new drainage canal into the remainder of the lacrimal excretory system.

"Silicone-octopus repair" of lacrimal obstructions in children.

Nineteen pediatric cases of lacrimal obstruction due to acquired disorders were repaired by a modification of the conventional silicone intubation procedure. This new operation employs an increased number of silicone strands within the lower excretory system. Sixteen patients had very good results and two were improved; there was one failure. This procedure is indicated in children with combined common canalicular and nasolacrimal duct obstruction.

[Familiary arhinia combined with peters' anomaly and maxilliar deformities, a new malformation syndrome (author's transl)].

A report is given on two sisters with arhinia, hypertelorism, Peters' anomaly and deformities of the maxilla. The lacrimal sacs in both patients were extirpated because of recurrent dacryoadenitis due to bilateral aplasia of the nasolacrimal ducts. One eye showing Peters' anomaly with microphthalmus was enucleated in the elder patient 12 years ago. Differential diagnosis includes frontonasal dysplasia and the different types of holoprosencephaly-syndromes. Probably this new malformation syndrome is a recessive inherited.

[A method of forming a rhinostoma in case of the stenosed lumen of the lacrimal sac].

A new method for the formation of a rhinostoma in patients with stenosed lumen of the lacrimal sac is proposed. It consists in the following: the incision of the soft tissues and the opening in the bone are made as low as possible to be closer to the lacrimal sac, and the nasolacrimal duct is used for the formation of a rhinostoma. Seventy-two operations were performed in patients with stenosed lumen of the lacrimal sac: 32 using sparing dacryocystorhinostomy (group 1) and 40 using the proposed technique (group 2). The results of 66 operations (30 in group 1 and 36 in group 2) were observed in remote periods (from 8 months to 9 years). In group 1 complete effect was attained in 20 out of 30 cases (66.7%), in group 2 in 33 out of 36 (91.6%). Hence, the proposed method for the formation of rhinostoma is highly effective.
