Oral acyclovir to suppress frequently recurrent herpes labialis. A double-blind, placebo-controlled trial.

OBJECTIVE: To determine whether oral acyclovir reduces the incidence of recurrent herpes labialis in otherwise healthy patients with proven frequently recurrent disease. DESIGN: Randomized, double-blind, placebo-controlled, crossover trial. SETTING: Outpatient facility of the Clinical Center, National Institutes of Health, Bethesda, Maryland. PATIENTS: Fifty-six otherwise healthy adults who reported frequently recurrent herpes labialis (> or = 6 episodes/y) were enrolled into the study. During a 4-month observation period, 22 patients had herpes labialis two or more times and were eligible for study treatment. INTERVENTIONS: Twenty-two patients were randomized to receive either acyclovir, 400 mg twice daily, or matched placebo for 4 months. After the first treatment period, patients were given the alternate treatment for another 4 months and were then taken off study medication to observe the first post-treatment recurrence. Recurrent outbreaks were determined by examination and by viral culture. RESULTS: Twenty patients completed blind treatment with both acyclovir and placebo. The median time to first clinically documented recurrence was 46 days for placebo courses and 118 days for acyclovir courses (P = 0.05). The mean number of recurrences per 4-month treatment period was 1.80 episodes per patient during placebo treatment and 0.85 episodes per patient during acyclovir treatment (P = 0.009). The mean number of virologically confirmed recurrences per patient was 1.40 with placebo therapy compared with 0.40 with acyclovir (P = 0.003). CONCLUSIONS: Oral acyclovir, 400 mg twice daily, is effective in suppressing herpes labialis in immunocompetent adults confirmed to have frequently recurrent infection. Treatment with acyclovir in this study resulted in a 53% reduction in the number of clinical recurrences and a 71% reduction in virus culture-positive recurrences compared with placebo therapy.

[Intravesical instillation of epirubicin in the prophylactic treatment of recurrent superficial bladder cancer].

A retrospective study was performed to determine the efficacy of intravesical instillation of epirubicin in the prophylactic treatment for recurrent superficial bladder cancer when compared with no treatment controls. Seventeen patients with recurrent superficial bladder cancer received epirubicin (30 mg/40 ml in normal saline) seventeen times after transurethral resection (TUR). Historical controls consist of sixteen patients with recurrent superficial bladder cancer who received no adjuvant treatment after TUR. In the epirubicin group, 1-, 2- and 3-year recurrence free rates were 64.7%, 41.2% and 5.9%, respectively. In controls all of them recurred and 1- and 2-year recurrence free rates were 17.6% and 0%, respectively. The recurrence-free rate was significantly higher in the epirubicin group than in the controls (p < 0.05). We investigated whether intravesical instillation of epirubicin prolonged recurrence-free intervals when compared with recurrence-free intervals without adjuvant treatment after TUR in each patient. The 1- and 2-year recurrence free rates after initial TUR were 25% and 12.5%, respectively, and those with epirubicin treatment after TUR were 62.5% and 37.5%, respectively. Recurrence-free intervals were significantly prolonged by intravesical instillation of epirubicin as compared with those without adjuvant treatment in each patient. This study demonstrated the efficacy of intravesical instillation of epirubicin in the prophylactic treatment for superficial recurrent bladder cancer.

[Recurrent varicosity of the lesser saphenous vein. A 5-year follow-up].

UNLABELLED: Between January 1st, 1992 and December 31st, 1996 we performed 1189 variectomies, 127 (11%) of these patients with recurrent varicosis. Only 5 (4%) of them exhibited an isolated perforator vein varicosis and 15 (1%) an isolated varicosis of lateral branches of the lesser saphenous vein. In 66 cases (52%) we noticed, as it was expected, a recurrent varicosis of the saphenous vein. The 41 patients (32%) with recurrent varicosis of the lesser saphenous vein were analysed exactly in detail. We used the classification described by Fischer. According to this we divided them up as follows. Type I: insufficiency of the lesser saphenous vein stump or from lateral branches of the lesser saphenous stump. Type II: recurrence by the femoro-popliteal vein. Type III: recurrence from the lateral branches vein. We obtained from type I: 27 patients (65%), from type II 6 patients and 8 patients (20%) from type III. The recurrences from the stump of the lesser saphenous vein dominated in male patients with 25 of 33; in female patients the recurrences from branches of the lesser saphenous vein with 4 of 8. The lesser saphenous vein was removed in 33 cases completely, and partly in 8 cases at the first operation. The ligation of the termination of the lesser saphenous vein in patients with a type I recurrence was done 16 times in fascial level and 8 times above the fascial level at the first operation. All the 27 type I patients were undergoing a subfascial ligation of the termination of the lesser saphenous vein at the second operation. We noticed the following complications: 2 haematomas, 2 lymphatic fistulas, 1 paraesthesia and 1 ilio-femoral venous thrombosis. After the recurrent operation we noticed no type I and no type II recurrence during follow-up, 17 out of the 26 patients examined again were satisfied. The value of the different investigations, indications, technical details, modifications and the results are discussed. CONCLUSIONS: The results demonstrate that most of the saphenous parvous vein stump recurrence would have been avoided by a sufficient ligation of the saphenous parvous stump at the first operation. It is obvious that an exact indication, a sophisticated operation technique and an experienced surgeon are necessary for successful therapy.

[Advanced breast cancer: an evaluation of the cost of recurrence].

In the context of a medicoeconomic study of the adjuvant treatment of breast cancer, we evaluated the cost of the recurrence. This cost was assessed from the medical records of 146 patients having presented either distant metastases, or a local recurrence followed or not by metastases between 1983 and 1990. We checked according to published data that the frequency of the metastatic risk is negligible if beyond 5 years after the local recurrence. Costs are expressed in 1995 French Francs (FF), with the French Social Security point of view. From the medical records, we calculated the mean cost of each type of recurrence using medical costs (visits, drugs and treatments, assessments, tests, hospital care, outpatient services.) and non medical costs (patient transportation). The costs are 175,168 FF (standard deviation or SD: 127,972) for metastatic recurrence, and respectively 287,582 FF (SD: 142,280) and 115,705 FF (SD: 78,677) for local recurrence followed or not by metastases. There is a significant difference between these figures (p < 0.001). The hospitalization costs are around 66% of the total cost of each type of recurrences and they are significantly higher (p < 0.005) when metastatic disease occurs after a local recurrence. The mean cost of isolated local recurrence added to metastatic recurrence, 290,873 FF, is not different from that of local recurrence followed by metastases, 287,582 FF (p = 0.15). These results will be integrated in a model in order to evaluate the long-term economic consequences of an adjuvant strategy in the treatment of breast cancer and presented in other publications.

Predictive factors for intrahepatic recurrence after percutaneous ethanol injection therapy for small hepatocellular carcinoma.

BACKGROUND: Percutaneous ethanol injection therapy has been used widely for small hepatocellular carcinoma. This study was undertaken to determine factors predictive of local recurrence or new nodular recurrence in patients with small hepatocellular carcinoma treated with percutaneous ethanol injection. METHODS: The authors studied 73 nodules treated with percutaneous ethanol injection in 49 patients with small hepatocellular carcinoma. The usefulness of predictive factors for recurrence was assessed with the Kaplan-Meier method. The clinicopathologic variables examined included age, gender, Child-Pugh classification, number of tumors (single vs. multiple), tumor size, degree of tumor differentiation, ultrasonographic findings such as peripheral hypoechoic band (so-called 'halo'), intratumoral echo pattern, tumor staining on enhanced computed tomography, combination therapy with transcatheter arterial embolization, and serum alpha-fetoprotein level. RESULTS: The local recurrence rates were 19%, 27%, 33%, 33%, and 33%, respectively, and the new nodular recurrence rates were 19%, 51%, 74%, 83%, and 83%, respectively, at 1, 2, 3, 4, and 5 years after percutaneous ethanol injection therapy. The frequency of local recurrence was associated with the histologic differentiation of more than moderately differentiated (P < 0.001), presence of a sonographic halo (P < 0. 005), an intratumoral heterogeneous echo pattern (P < 0.001), and positive tumor staining on enhanced computed tomography (P < 0.01). Multivariate analysis showed that the presence of a halo and an intratumoral heterogeneous echo pattern were the most important variables for predicting local recurrence. The frequency of new nodular recurrences was related to the presence of multiple tumors (P < 0.01) and a high serum alpha-fetoprotein level (P < 0.001). Multivariate analysis showed that a high serum alpha-fetoprotein level was a reliable predictor of new nodular recurrence. CONCLUSIONS: This study showed that the presence of a halo and an intratumoral echo pattern on ultrasonography were useful predictors for local recurrence after percutaneous ethanol injection therapy for small hepatocellular carcinoma, and that a high serum alpha-fetoprotein level was associated with a higher frequency of new nodular recurrences.

Loss of heterozygosity on chromosome 6p21.2 as a potential marker for recurrence after radiotherapy of human cervical cancer.

Cervical carcinomas develop as a result of multiple genetic alterations, and specific alterations lead to specific clinical behavior. However, the effect of such alterations on the recurrence of cervical cancer after radiotherapy remains unknown. Chromosome arm 6p is one of those most frequently involved in a loss of heterozygosity (LOH) in patients with cervical carcinoma. The aim of this study was to identify the correlation between the LOH on chromosome 6p21.2 and the recurrence of cervical cancer after radiotherapy. A total of 62 patients with cervical cancer (stage I, 4 patients; stage II, 9 patients; stage III, 37 patients; and stage IV, 12 patients) were included in this study. All patients were treated with definitive radiotherapy. We analyzed specimens from the tumors and venous blood of all patients. Tumors and normal DNA were analyzed by PCR for genetic losses at three polymorphic microsatellite loci (D6S276, D6S1624, and D6S1583). Chromosome 6p21.2 is involved in the LOH in 46.8% (29 of 62) of the informative carcinomas. Ten patients had a local recurrence, 4 had distant metastases, and 13 had both local recurrence and distant metastases after radiotherapy. To evaluate the relationship between the recurrence after radiotherapy and LOH on chromosome 6p21.2, we divided the patients into those with cancer recurrence (n = 27) and those without recurrence (n = 35). LOH on chromosome 6p21.2 was correlated with recurrence after radiotherapy (P = 0.006). The tumors in patients with recurrence were significantly larger than those in patients without recurrence (P = 0.003). However, there was no correlation between the sizes and stages of tumors and the LOH on chromosome 6p21.2. In addition, both overall survival and relapse-free survival were significantly worse for the patients with LOH as compared with those without LOH (P = 0.02 and P = 0.002, respectively). The results of this study suggest that LOH on 6p21.2 is correlated with recurrence of cervical carcinoma after radiotherapy.

[Multi-center study of recurrent nasal sinus polyposis: prognostic factors and possibility of prophylaxis].

Today, surgery is the treatment of choice for nasal sinus polyposis. Nevertheless, although meticulous surgery does "per se" reduce the percentage of recurrences, there are cases where even the most painstaking removal of the entire pathology cannot prevent recurrence. Therefore recurrences do not appear linked to the type of surgery; rather onset appears linked to intrinsic, only partially recognizable factors responsible for the primary and secondary polypogenesis. In order to identify negative prognostic factors which might be implicated in recurrences, the present study extrapolated the data from forms on 181 patients who had undergone surgery for nasal sinus polyposis and subjected it to multivariance analysis. These patients were recruited during the course of a multicenter study with the participation of 12 ENT Centers in Piemont and Liguria. The recurrence rate was 13%. In analyzing unfavorable factors prognosticating recurrence, thirteen parameters were examined. Nine of these (age, sex, severe deviation of the septum causing restriction, severe turbinate hypertrophy, surgery or repeat surgery for recurrence, type of macro-micro endoscopic surgery, allergy to seasonal inhalants, allergy to perennial inhalants, mixed allergies) did not prove to have any significance in recurrences. The presence of bilateral involvement of the sinus system presented a negative trend as regards recurrences while involvement of more than one subsite (anterior ethmoid, posterior ethmoid, maxillary sinus, sphenoid), ASA and NSAID intolerance and abundant eosinophilic infiltration in the mucous chorion proved statistically significant (p < 0.05 for all three parameters) for recurrence. Post-operative topic prophylactic treatment with steroids (beclomethasone) or anti-H1 drugs (azelastin, HCl) did not appear to affect the onset of recurrence although it did have a positive effect on subjective symptoms.

Risk of recurrence of seizures following single unprovoked idiopathic seizure.

A prospective study was conducted to look for various factors that could predict the risk of recurrence of a single unprovoked idiopathic seizure. Seventy six patients with a history of single episode of seizure ultimately completed the study and the data regarding age, sex, duration of seizure, time of occurrence of the ictus, interval between onset and referral, family history of seizure and alcohol consumption were analysed. All patients of symptomatic epilepsy and those with an abnormal scan were excluded. The patients were randomized into two groups, one of which received anti epileptic medication and the other did not. All patients underwent electroencephalography (EEG). Twenty two (M=16, F=6) of the 76 patients (M=56, F=20) had a recurrence of seizure. The duration of seizure at initial presentation was 10.1 +/- 5.2 min. in the recurrence group and 6.5 +/- 4.1 min. in the non-recurrence group. Twelve of the 16 patients with an abnormal EEG had a recurrence whereas only 10 of the 60 patients with a normal EEG had a recurrence (p <0.001). Of the treated cases (n=36), only 4 had a recurrence compared to 18 of the untreated cases (n=40) (p <0.002). Eighteen of the 22 cases having a recurrence did so within three months. Six of the cases with family history of seizure (n=10) had a recurrence, whereas only 16 of the cases without family history of seizure (n=16) had a recurrence (p <0.05). Patients of a single unprovoked idiopathic seizure with a normal CT scan are less likely to have a recurrence if the duration of seizure at presentation is short, EEG is normal, more than 3 months have passed since the first seizure and if treatment has been started. Family history of seizures does have a moderately significant bearing, but alcohol intake does not increase the chances of seizure.

Influencing factors for recurrence of hepatocellular carcinoma treated with radiofrequency ablation.

Radiofrequency ablation (RFA), a new local therapy, has recently been developed for hepatocellular carcinoma (HCC). In this study, we have checked for the factors influencing the recurrence of HCC following RFA. We gave special emphasis to complete coagulation. The study population was comprised of 47 patients (80 tumors) with HCC with tumor size of <3 cm in maximal diameter. The patients were observed for a period of 2-3 years (average 865 days). The local recurrence rate was 19% at the end of 1 year, and 21% by the end of 2 years. The patients with local recurrence received significantly fewer RFA sessions (P<0.05) compared to patients with no recurrence. The frequencies of complete coagulation were significantly less (P<0.05) in patients with local recurrence than patients without local recurrence. The distant recurrence rate was 38% at 1 year, and 60% at 2 years. Patients with distant recurrence had significantly increased number of tumors (2.0+/-1.4) (P<0.05) compared to patients without distant recurrence (1.2+/-0.4). In conclusion, obtaining complete coagulation is an important factor to prevent local recurrence and the number of tumors predicted the distant recurrence in patients with HCC undergoing RFA.

Correlation between occult neoplastic cells in the lymph node sinuses and recurrence in patients with Dukes' C colorectal cancer.

The aim of this study was to investigate whether it was possible to select patients with Dukes' C primary colorectal cancer who had a high risk of metastasis and recurrence. Among 434 patients who underwent surgical resection of primary colorectal cancer, 115 patients (26.5%) had Dukes' C cancer. Among them, 35 patients (30.45%) suffered from metastasis/recurrence postoperatively. Thirty-two patients with recurrence for whom complete follow-up was possible (recurrence group) and 32 patients who survived for at least 5 years without recurrence (non-recurrence group) were compared by immunohistochemical staining of resected lymph nodes for cytokeratin using two antibodies (AE1/AE3 and CAM 5.2). AE1/AE3 staining was positive in 93.85% of the recurrence group versus 68.85% of the non-recurrence group (p=0.0250), while CAM 5.2 staining was positive in 84.45% vs. 53.15%, respectively (p=0.0152). The occult neoplastic cell count (mean +/- SD) in the peripheral lymph node sinuses was 6.28+/-5.17 vs. 2.38+/-3.03 (p=0.0002) in the AE1/AE3-positive patients from the recurrence and non-recurrence groups, while it was 5.13+/-4.84 vs. 1.53+/-2.37 (p=0.0003) in the CAM 5.2-positive patients in each group. Accordingly, the immunohistochemical positivity rate for both AE1/AE3 and CAM 5.2, as well as the occult neoplastic cell count, were significantly higher in the recurrence group. These results suggest that patients with Dukes' C primary colorectal cancer who have a higher risk of recurrence can be selected by immunostaining of resected lymph nodes for cytokeratin.

Correlation between occult neoplastic cells in the lymph node sinuses and recurrence in patients with curatively resected Dukes' B colorectal cancer.

This study investigated whether it is possible to detect patients who have a high risk of metastasis and recurrence after resection of stage II Dukes' B primary colorectal cancer. Among 434 patients who underwent curative resection of primary colorectal cancer, 167 (38.5%) had Dukes' B cancer. Among them, 19 patients (11.4%) suffered from postoperative metastasis or recurrence. In 17 patients with recurrence who could be followed-up completely (recurrence group) and 17 other patients who survived for at least 5 years without recurrence (non-recurrence group), immunohistochemical staining of resected lymph nodes for cytokeratin (AE1/AE3 and CAM 5.2) was performed. AE1/AE3 was positive in 76.5% and 47.1% of the patients from the recurrence and non-recurrence groups, respectively, while CAM 5.2 was positive in 52.9% and 17.6%, respectively. There were no significant differences of either AE1/AE3 or CAM 5.2 positivity between the groups. However, the occult neoplastic cell count (mean +/- SD) floating in the lymph node sinuses was significantly higher in patients from the recurrence group who were positive for AE1/AE3 or CAM 5.2 than in patients from the non-recurrence group (6.12+/-6.00 vs. 0.59+/-0.71; p=0.0019 and 3.94+/-5.06 vs. 0.29+/-0.69; p=0.0098, respectively). These results suggest that patients with Dukes' B primary colorectal cancer who have a higher risk of recurrence can be selected by immunostaining of resected lymph nodes for cytokeratin.

[Recurrent hepatocellular carcinoma after hepatic resection].

A total of 125 patients with hepatocellular carcinoma (HCC) treated by hepatic resection in our department from 1970 to 1989 were reviewed to determine recurrent factors, recurrent modes of HCC and to assess the treatment for recurrent HCC. Seventy-five of 125 patients had recurrent tumors after the first hepatic resection. The 1-, 2-, and 3-year cumulative recurrent rates after hepatic resection were 25%, 52% and 67% respectively. The size of the tumor, intrahepatic metastasis, portal vein involvement, clinical stage and DNA ploidy pattern were judged as useful predictive factors for recurrence of HCC after hepatic resection. In the patients with intrahepatic metastases, the bilateral lobes of the remnant liver were the most frequent recurrent sites. The treatment for recurrent HCC was divided into 3 groups such as re-resection, transcatheter arterial embolization (TAE) and palliative treatment. The survival curves of patients receiving re-resection and TAE were significant better than those of patients receiving palliative treatment. Patients treated by re-resection for recurrent HCC had the longest survival. The 1-, 3- and 5-year cumulative survival rates after re-resection were 84%, 60% and 48% respectively. It is concluded that the early detection of recurrent HCC is important and re-resection or TAE is effective treatment for recurrent HCC.

Recurrence patterns after curative resection of colorectal cancer in patients followed for a minimum of ten years.

BACKGROUND/AIMS: To investigate the recurrence patterns and interval from initial surgery in patients with curatively resected colorectal cancer followed for a minimum of 10 years. METHODOLOGY: We retrospectively reviewed 418 patients who had undergone curative resection for colon cancer (n = 246) or rectal cancer (n = 169). Follow-up periods ranged from 10 to 23 years. Main outcome measures were interval until recurrence, site of first recurrence, and influence of adjuvant chemotherapy. RESULTS: 26 (6%) had been lost to follow-up by 10 years and 143 (34%) had died. The most common site of recurrence was liver in colon cancer and locoregional in rectal cancer. The cumulative recurrence rate in colon cancer was 100% at 4 years. In rectal cancer, it was 89% at 5 years, 98% at 7 years and 100% at 10 years. The interval until recurrence was longer in rectal cancer (26.0 +/- 24.2 months) than in colon cancer (17.1 +/- 11.0 months) (p = 0.03). It was also longer in patients receiving than in those not receiving adjuvant chemotherapy (p < 0.01). The interval until lung metastasis was longer than that until liver metastasis in colon cancer (p = 0.04), and longer than that until locoregional recurrence in rectal cancer (p = 0.03). The interval until recurrence in the colon cancer was shorter for stage III than for stage II (p = 0.02). CONCLUSIONS: Surveillance for recurrences, particularly for relapses in the liver and lung, should be performed for at least 4 years in colon cancer patients. Patients with rectal cancer should be followed for a longer period than those with colon cancer, focusing on locoregional, liver and lung recurrence. It is particularly noteworthy that adjuvant chemotherapy may prolong the interval until recurrence and the interval until lung metastasis is relatively longer.

[Analysis of risk factors for epithelial ovarian cancer recurrence].

BACKGROUND & OBJECTIVE: Advanced ovarian cancer shows a high recurrence and poor survival rate. It is important to identify the risk factors for the recurrence of ovarian cancer, due to the lack of reliable methods for the detection of early disease. This study was designed to explore the risk factors for the recurrence of epithelial ovarian cancer. METHODS: Factors that might be related to the recurrence of 109 epithelial ovarian cancer patients including age, pathological type, FIGO stage, operation type, chemotherapy regimen, neoadjuvant chemotherapy, postoperative chemotherapy cycles, first operation status, and size of residual tumor were analyzed by logistic regression model using SPSS 10.0 statistical software. RESULTS: Of 109 patients with ovarian cancer, 36 (33%) relapsed after a combination of surgery and chemotherapy, and the median recurrence-free interval was 19 months. The 5-year survival rates were 62.7% and 43.3% for the 109 patients and the recurrent patients, respectively. The survival rates of pelvic recurrence (50.1%) and extra-pelvic recurrence (36.1%) were not statistically different. Univariate analysis showed that the risk of recurrence in mucous adenocarcinoma or FIGO stage I patients were lower than that of other pathology type or FIGO stage (beta=-1.565 and -1.799,P=0.0120 and 0.026 ) while it was higher in the patients given above 8 cycles chemotherapy than those given 1-4 or 6-8 cycles(beta=-3.591 and -1.500,P< 0.001 and =0.038). Multivariate analysis showed that histological type, FIGO stage and postoperative chemotherapy cycles had remarkable influence on the relapse of epithelial ovarian cancer independently(RR=3.473, 4.713, and 6.140, respectively, P< 0.05). CONCLUSION: Histological type, FIGO stage, and postoperative chemotherapy cycles influence the recurrence of epithelial ovarian cancer. Since FIGO stage is a remarkable risk factor for the recurrence, early diagnosis should be the key point to decrease it. Reasonable chemotherapy plan is essential for the therapy of ovarian cancer, but excessive chemotherapy is not good for effectiveness.

[Early recurrent factors of ovarian cancer after treatment].

BACKGROUND & OBJECTIVE: Recurrence of ovarian cancer is an important factor that leads to therapeutic failure. At present, the treatment in ovarian cancer is not sufficiently effective. This study was designed to investigate the factors of early recurrence after treatment in ovarian cancer in order to reduce its recurrence. METHODS: The clinical data of the ovarian cancer patients admitted to Affiliated Hospital of Guangxi Medical University during January 1986 to May 1996 who recurred within one year after treatment were analyzed retrospectively. The recurrent rates were compared according to their different clinical stages, pathological types, and residual volumes of the disease as well as the number of courses of pre and postoperative chemotherapy received. Chi-square test was used for statistic analysis. RESULTS: Eighty-two of 373 cases recurred within one year after treatment; the overall recurrent rate was 22.0%. The 1-year recurrent rate in stage I-II patients was lower than that in stage III-IV patients (2.7% versus 30.3%, P< 0.01). The recurrent rate in the patients with undifferentiated adenocarcinoma was higher than that with other pathological type. The recurrent rate in the patients with residuals >or=2 cm was higher than that with residuals < 2 cm (33.8% versus 13.7%, P< 0.01). The recurrent rate in the patients with preoperative chemotherapy was lower than that without it. The recurrent rate in the patients who had received only 1-3 courses of postoperative chemotherapy was higher than that having received 4-6 or >6 courses (67.0% versus 7.7%, 67.0% versus 7.0%, respectively, P< 0.01). CONCLUSION: Early (1-year) recurrence in the patients with ovarian cancer seems to be closely related to late stage of the disease, incomplete surgery, and insufficient chemotherapy courses.

Patterns of recurrence in patients treated with photodynamic therapy for intraperitoneal carcinomatosis and sarcomatosis.

The purpose of this study was to evaluate the patterns of recurrence in patients treated with Photofrin-mediated intraperitoneal photodynamic therapy (IP PDT). Sixty-six patients with gastrointestinal cancers, ovarian cancers, and sarcomas have been enrolled to date and 51 patients underwent IP PDT. Photofrin, 2.5 mg/kg, was administered intravenously 48 h prior to surgical debulking and intraoperative light treatment. Forty-five, and 49 patients were evaluable for response rates, and patterns of recurrence, respectively. Response to treatment was evaluated by CT or MRI scans of the abdomen and pelvis every 3 months. Patterns of recurrence were determined by evaluating the abdomen as a combination of different treatment regions. Of the 51 patients enrolled and treated with IP PDT two are alive without evidence of recurrence. Eleven of 45 patients showed no evidence of recurrence 3 months after treatment. No evidence of recurrence was noted in 7/17 sarcoma patients, 2 of 13 ovarian cancer patients, and 2 of 15 gastrointestinal cancer patients. The most common site of recurrence as determined by radiographs was the pelvis, which was noted in 19 of 49 (39%) patients. The presence of gross residual disease before light treatment (as determined by the attending surgeon) did not affect the site of recurrence. When studying those patients who had only locoregional recurrence, 9 of 33 evaluated radiographically and 10 of 24 evaluated operatively recurred only in peritoneal areas not previously involved with gross disease. The pelvis was the site with the highest rate of recurrence after IP PDT. A significant minority of patients recurred only in sites not previously involved with gross disease. Patients with gross residual disease before light therapy had similar recurrence rates to those without gross residual disease. Since sites involved with gross residual tumor often received boost doses of light, this could suggest a dose-response relationship for IP PDT.

[Long-term follow-up observation of clinical therapy for laryngeal carcinoma recurrence and cervical metastasis].

OBJECTIVE: To observe the long-term effects of clinical treatment for laryngeal carcinoma recurrence and cervical metastasis, and discuss the operative therapy method for laryngeal carcinoma recurrence and cervical metastasis. METHOD: Retrospective analysis the 70 patients with laryngeal carcinoma recurrence and cervical metastasis who was cured in our hospital from 1990 to 1998, including 31 local recurrence, 15 stomal recurrence and 24 cervical lymph node metastasis. RESULT: Sixty-four cases were observed over 3 years and 41 cases were studied over 5 years. Twenty-eight patients who received the second-time operation survived over 5 years. In 31 patients with local recurrence, 19 patients received total laryngectomy and 15 patients of them are now living, but 12 patients who did not accept surgical therapy were died in 1-3 years separately; 9 patients with stomal recurrence received the second-time operation and 5 of them lived 5 years longer, however, the 6 patients with stomal recurrence who did not accept the second-time operation all died in 18 months; In 24 patients with cervical lymph node metastasis, 15 patients received the neck dissection and 8 of them are now alive, but the rest patients who did not accept the operation died in 2 years. CONCLUSION: Regular re-examination and systemic clinical follow-up studying are very important for early detection and early treatment in laryngeal carcinoma recurrence and cervical metastasis; Second-time surgical operation is an effective therapeutic method for laryngeal carcinoma recurrence and cervical metastasis; Radiotherapy and chemotherapy play a limited role in clinical therapy for laryngeal carcinoma recurrence and cervical metastasis.

[Recurrence risk factors of platinum-sensitive epithelial ovarian cancer].

BACKGROUND & OBJECTIVE: The prognosis of platinum-sensitive ovarian cancer patients was better than that of chemoresistant ones. However, platinum-sensitive ovarian cancer patients still have a high recurrence rate, which affects their prognosis. This study was designed to analyze clinical features and recurrence risk factors of platinum-sensitive epithelial ovarian cancer. METHODS: Factors that might relate to recurrence of 90 platinum-sensitive epithelial ovarian cancer patients, admitted in Cancer Center of Sun Yat-sen University from 1993 to 1999, with complete remission of more than 6 months, were assessed. Univariate analysis was performed using Chi(2) test; while multivariate analysis was carried out using Cox proportional hazard model. RESULTS: Among the 90 patients, 36(40.0%) relapsed with the median recurrence-free interval of 20 months. Pelvic cavity (18/36, 50.0%) was the most frequently involved. The 3-and 5-year survival rates of all patients were 79.6% and 69.5%; while those of the recurrent ones were 62.3% and 39.6%. Univariate analysis showed that the early FIGO stage group, mucinous type group, and no neoadjuvant chemotherapy group had lower recurrence rates than advanced FIGO stage group, non-mucinous type group, and neoadjuvant chemotherapy group, respectively (P=0.001, P=0.002, and P=0.025). Cox multivariate analysis showed that only FIGO stage was the independent risk factor of recurrence of ovarian cancer (risk ratio=1.771, P=0.003). There was no significant difference in recurrence rate between CBP and other postoperative chemotherapy regimen groups. More cycles of chemotherapy could not reduce the recurrence rate. CONCLUSION: Since FIGO stage is an independent recurrence risk factor of platinum-sensitive epithelial ovarian cancer patients, early diagnosis is the key point to decrease the recurrence rate.
