Urinary 8-oxodeoxyguanosine, aflatoxin B1 exposure and hepatitis B virus infection and hepatocellular carcinoma in Taiwan.

To evaluate the role of oxidative stress and aflatoxin exposure on risk of hepatocellular carcinoma (HCC), a case-control study nested within a community-based cohort was conducted in Taiwan. Baseline urine samples, collected from a total of 74 HCC cases and 290 matched controls, were used to determine by enzyme-linked immunosorbent assays the level of urinary excretion of 8-oxodeoxyguanosine (8-oxodG), a biomarker of oxidative DNA damage and urinary aflatoxin B(1) metabolites, a biomarker of aflatoxin exposure. Multivariate-adjusted linear regression analysis showed that urinary aflatoxin metabolites and gender were significantly associated with level of urinary 8-oxodG among controls. Moreover, after adjustments for potential confounding factors, there was a statistically significant positive dose-response relationship between levels of urinary 8-oxodG and urinary aflatoxin metabolites (P < 0.0001). However, when compared with subjects in the lowest quartile of 8-oxodG, there was a decrease in risk of HCC, with adjusted odds ratios (ORs) of 0.8 [95% confidence interval (CI) 0.3-2.0], 0.7 (95% CI 0.3-2.0) and 0.7 (95% CI 0.2-1.7) for subjects in the second, third and fourth quartile, respectively. The combination of level of urinary 8-oxodG below the median and hepatitis B virus infection resulted in an OR of 11.4 (95% CI 3.9-33.3), compared with those with urinary 8-oxodG above the median and hepatitis B virus surface antigen negative. These results suggest that elevated levels of urinary 8-oxodG may be related to increasing level of aflatoxin exposure but may also indicate enhanced repair of oxidative DNA damage and therefore lower risk of HCC.

Therapy-resistant periodontitis. Psychosocial characteristics.

This study investigated the perspective of a stress system disorder in the pathogenesis of therapy-resistant periodontitis. The goal was to find indications that the stress-behaviour-immune system model holds as an explanatory model for the understanding of periodontal disease. 2 patient-groups were compared: one group classified as responding well to periodontal treatment (responsive-group, R-group, n=11); the other group was classified as responding less well to treatment (non-responsive-group, NR-group, n=11). Somatic and psychological factors were described as obtained by interviews and psychological testings. These findings were related to clinical data documented during the treatment of the patients. An exact logistic multivariate regression analysis was performed on a model based on variables selected by bivariate analysis (variable versus group). The results indicated that the NR-group patients displayed indications of more psychosocial strain and a more passive-dependent personality. The R-group patients displayed a more rigid personality and possibly a less stressful psychosocial situation in the past. The report highlights the possible contribution of stress factors in the context of therapy resistant periodontal disease, and the results seem to be understandable within the context of a stress system disorder perspective.

Eye care utilization patterns in Tehran population: a population based cross-sectional study.

BACKGROUND: The aim of this study is to determine eye care visits which are an indicator of eye care service utilization by Tehran population and its determinants. METHODS: Through a population-based, cross-sectional study, 6497 Tehran citizens were sampled. All participants had complete eye examinations and an interview regarding demographic and socioeconomic status variables, past medical and eye history, and their previous and last eye care visits. RESULTS: Among those sampled, 4565 people participated in the study (response rate of 70.3%). Among these participants, 34.7 % had never visited an ophthalmologist or optometrist (95% confidence interval [CI]: 32.4 to 36.9) and 43.2% had not seen an eye care provider in the last 5 years. Multivariate logistic regression revealed that men (OR = 1.30), younger participants (each year increase in age: OR = 0.98) and the less educated (each year increase in education: OR = 0.93) were more likely, and the visually impaired were less likely (OR = 0.41) to have neglected eye care. CONCLUSION: A large proportion of the population, including those in the high risk group who require eye care, has never utilized any eye care service. These data suggest that efforts have to be made to better understand the causes and to optimize the utilization of the available eye care services in the population.

Toll-like receptor 4 gene Asp299Gly polymorphism is associated with reductions in vascular inflammation, angiographic coronary artery disease, and clinical diabetes.

BACKGROUND: Coronary artery disease (CAD) and, increasingly, diabetes are recognized as having an inflammatory basis. Membrane toll-like receptors (TLRs) activate signaling pathways that up-regulate inflammation. We evaluated whether the Asp299Gly polymorphism in the TLR4 gene, which impairs inflammatory responses, is associated with reduced vascular inflammation (assessed by C-reactive protein [CRP]) and a decreased risk for CAD and diabetes. METHODS: 1894 patients without acute myocardial infarction undergoing coronary angiography were studied. Genotyping was performed by real-time polymerase chain reaction. CAD was defined as >or=70% stenosis. Diabetes was diagnosed by patients' referring physicians. CRP was measured by fluorescence polarization immunoassay. Regression analyses adjusted for traditional cardiac risk factors. RESULTS: Patients averaged 64 +/- 11 years of age, and 69% were male. Asp299Gly genotype frequencies were: AA = 0.911 (n = 1725), AG = 0.086 (n = 164), and GG = 0.003 (n = 5), in keeping with Hardy-Weinberg equilibrium. CRP was lower among G-allele carriers (median = 1.11 mg/dL) than wild-type (AA) subjects (1.23 mg/dL, adjusted P = .044). G-allele carriers also had a lower prevalence of CAD (65% vs. 73%, OR = 0.67, CI = 0.46-0.99, adjusted P = .048) and diabetes (11% vs. 18%, OR = 0.63, CI = 0.42-0.95, adjusted P = .029), which persisted in multivariate logistic regression analyses. 299Gly carriage did not affect clinical outcomes nor interact with statin therapy. CONCLUSIONS: The TLR4 299Gly allele was associated with reduced CRP levels and, in parallel, a decreased risk of angiographic CAD and clinical diabetes. These findings suggest that down-regulation of innate immune responsiveness could beneficially modify CAD and diabetes risk and might provide a novel basis for genetic risk stratification and therapeutic targeting.

Regression models for mixed discrete and continuous responses with potentially missing values.

In this paper a likelihood-based method for analyzing mixed discrete and continuous regression models is proposed. We focus on marginal regression models, that is, models in which the marginal expectation of the response vector is related to covariates by known link functions. The proposed model is based on an extension of the general location model of Olkin and Tate (1961, Annals of Mathematical Statistics 32, 448-465), and can accommodate missing responses. When there are no missing data, our particular choice of parameterization yields maximum likelihood estimates of the marginal mean parameters that are robust to misspecification of the association between the responses. This robustness property does not, in general, hold for the case of incomplete data. There are a number of potential benefits of a multivariate approach over separate analyses of the distinct responses. First, a multivariate analysis can exploit the correlation structure of the response vector to address intrinsically multivariate questions. Second, multivariate test statistics allow for control over the inflation of the type I error that results when separate analyses of the distinct responses are performed without accounting for multiple comparisons. Third, it is generally possible to obtain more precise parameter estimates by accounting for the association between the responses. Finally, separate analyses of the distinct responses may be difficult to interpret when there is nonresponse because different sets of individuals contribute to each analysis. Furthermore, separate analyses can introduce bias when the missing responses are missing at random (MAR). A multivariate analysis can circumvent both of these problems. The proposed methods are applied to two biomedical datasets.

Screening and intervention for illicit drug abuse: a national survey of primary care physicians and psychiatrists.

BACKGROUND: Illicit drug abuse causes much morbidity and mortality, yet little is known about physicians' screening and intervention practices regarding illicit drug abuse. METHODS: We mailed a survey to a national sample of 2000 practicing general internists, family physicians, obstetricians and gynecologists, and psychiatrists to assess their screening and intervention practices for illicit drug abuse. RESULTS: Of 1082 respondents (adjusted response rate, 57%), 68% reported that they regularly ask new outpatients about drug use. For diagnosed illicit drug abuse, 55% reported that they routinely offer formal treatment referral, but 15% reported that they do not intervene. In multivariate logistic regression models, more optimal screening and intervention practices were associated with psychiatry specialty, confidence in obtaining the history of drug use, optimism about the effectiveness of therapy, less concern that patients will object, and fewer perceived time constraints. CONCLUSIONS: Most physicians reported that they ask patients about illicit drug use, but a substantial minority inadequately intervene in diagnosed drug abuse. Initiatives to promote physician involvement in illicit drug abuse should include strategies to increase physicians' confidence in managing drug problems, engender optimism about the benefits of treatment, dispel concerns about patients' sensitivity regarding substance use, and address perceived time limitations.

The validity of a questionnaire on medicines used in health care practice: comparison of a questionnaire and computerized medical record survey.

BACKGROUND: To assess the effect of sociodemographic characteristics, perceived health, chronic disease, and country of birth on use of medicines and the validity of a questionnaire survey on the use of medicines. METHODS: A questionnaire was handed out to all 1776 patients aged 16 years or over who had attended Jordbro Health Center, Haninge municipality, Sweden, during the preceding 6 months (response rate 81%), and the findings were linked to information obtained from the computerized prescriptions of medicines. Three measurements were used: medicine used, prescriptions, and medicines prescribed. RESULTS: The data were analyzed by logistic regression technique in univariate and multivariate forms. Of the respondents 73% reported using some type of medicine, compared to 63% who had been given a prescription by a physician in the Health Center during the past year. The number of medicines used was 3.26 vs. 4.17 prescribed, but after controlling for possible confounders the figures were 3.51 vs. 3.94. Female gender and chronic disease were related to high prevalence of drug use. Old age, female gender, sick-leave or disability pension, and chronic disease were also related to a high prevalence of prescribed medicines, while old age, sick-leave, perceived bad health, and chronic disease were related to the numerous medicines used. Old age, perceived bad health, and chronic disease were related to a large number of prescriptions, while old age and chronic disease were related to a large number of prescribed medicines. CONCLUSIONS: Differences were found between the two methods used to analyze data. The prevalence of medicines used was overestimated, but the number of medicines used was underestimated. This indicates that the prevalence rates based on analysis of the questionnaire are more reliable, while the number of used medicines should be interpreted with caution, and the effect of patient-concordance should be considered in future studies.

Comparison of sestamibi, thallium, echocardiography and PET for the detection of hibernating myocardium.

The detection of hibernating myocardium is important because revascularisation results in improved function and prognosis in patients with hibernation but not in those with non-viable myocardium. The primary aim of this study was to compare the diagnostic accuracy of four techniques with respect to hibernation in the same study population with 6-12 months of follow-up. Twenty-five males underwent rest-stress sestamibi and delayed (>18 h) thallium scintigraphy, high-dose dobutamine stress echocardiography and nitrogen-13 ammonia/fluorine-18 fluorodeoxyglucose (NH(3)/FDG) positron emission tomography (PET). The pre-operative ejection fraction was 36.2% (+/-7.3%). Follow-up was 8.1 (+/-2.8) months. Using postoperative improvement in wall motion on echocardiography as the gold standard, 6/34 dysfunctional vascular territories were hibernating. The mean uptake of all tracers was significantly higher in hibernating than in non-viable territories ( P<0.05). Normal perfusion or mismatch on PET (FDG>NH(3) uptake) and the pattern of response to dobutamine on echocardiography were also predictive of recovery ( P<0.001 and P=0.02 respectively). Univariate logistic regression identified sestamibi, ammonia and FDG as independent predictors of hibernation. FDG-PET was, however, the only independent predictor using multivariate analysis. The nuclear techniques had high negative predictive values (NPV) of >or=95% but lower positive predictive values (PPV) of 45%-75% as compared with echocardiography, which had an NPV of 87% and a PPV of 100%. PET was the most powerful predictor of hibernation although the combination of a technique with a high PPV (echocardiography) and a high NPV (PET or sestamibi) may represent the optimal clinical choice.

Association between atopic sensitization and asthma and bronchial hyperresponsiveness in swedish adults: pets, and not mites, are the most important allergens.

BACKGROUND: Atopic sensitization is a well-known risk factor for asthma and bronchial hyperresponsiveness (BHR). Mites have been regarded as the most important allergens, but the prevalence of sensitization to mites is relatively low in Sweden. OBJECTIVE: The aim of the study was to investigate possible associations between sensitization to various allergens and asthma and BHR in adults. METHODS: A random sample of 1859 subjects, aged 20 to 46 years, was investigated in a cross-sectional study by using a questionnaire, skin prick tests (SPTs), specific and total IgE measurements, and methacholine bronchial challenge tests. Possible associations were analyzed univariately and by using multivariate logistic regression analysis and proportional hazard regression analysis. RESULTS: Positive SPT and specific IgE results were more common in subjects with asthma and BHR than in subjects without these conditions for all allergens. The independent associations between positive SPT responses and asthma and BHR are given as adjusted prevalence ratios (PRRs): pets and asthma, PRR = 3.6; pets and BHR, PRR = 2.0; grass and asthma, PRR = 2.0; grass and BHR, PRR = 1.7; mites and asthma, PRR = 1.4; and mites and BHR, PRR = 1.2. The use of specific IgE measurements instead of SPTs showed essentially similar results. CONCLUSION: Cats and dogs were the sensitizing allergens most closely associated with asthma and BHR. The relationships with sensitization to grass and mites were less pronounced.

Left ventricular circumferential function in patients with essential hypertension.

The aim of the study was to investigate left ventricular (LV) systolic and diastolic circumferential myocardial function using tissue Doppler imaging in patients with essential hypertension. One hundred and two patients and 33 healthy age-matched controls were studied. Each patient underwent echocardiographic study with analysis of tissue velocity curves, which included mean peak systolic (Sm), early (Em) and late diastolic velocity (Am) and isovolumic relaxation time (IRTm). These parameters were expressed as means from the six basal segments of left ventricle reflecting its longitudinal function (L). The same indices estimated in mid-anteroseptal (C-AS) and mid-posterior (C-P) segments in parasternal short-axis view served as a measure of LV circumferential function. Higher value of C-AS-Sm and a similar trend in C-P-Sm suggest increased LV circumferential systolic function in hypertensive patients, whereas lower values of C-AS-Em, C-P-Em, C-AS-Em/Am and C-P-Em/Am indicate impaired LV circumferential diastolic function. Decreased L-Sm as well as decreased L-Em and L-Em/Am reflects LV longitudinal systolic and diastolic dysfunction, respectively. By univariate analysis, positive correlations were demonstrated between C-AS-Sm and LV mass index (LVMI) (r=0.61, P<0.001), interventricular septum thickness (IVS) (r=0.55, P<0.001) and LV posterior wall thickness (PW) (r=0.43, P<0.01) and negative ones between L-Sm and LVMI (r=-0.51, P<0.001) and PW (r=-0.36, P<0.04). By stepwise multivariate regression analysis, LVMI, IVS and age independently predicted C-AS-Sm and LVMI predicted L-Sm. Our study demonstrated in hypertensive patients increased LV circumferential systolic and decreased diastolic function. The former may be a compensatory response to the impairment in LV longitudinal systolic performance.

Structured research training in residency training programs. The impact on the level of resident research activity.

A survey was conducted to construct a profile of structured resident research training and experience in physiatric residency training programs. Resident research activity was quantified, and factors that may influence the level of resident research activity were examined. Questionnaires were sent to all 72 residency training program directors, of which 87% responded. To construct the profile of structured training and experience in research that currently exists, descriptive analyses were applied to the survey responses. Resident research activity was quantified according to projects per resident per year over the last 3 yr. Resident research activity was assigned to level I activity (> or = 0.25 projects/resident/year) or level II activity (< 0.25 projects/resident/year). This point of discrimination was chosen because it represents one project per resident per 4-yr graduate medical education cycle. Nonparametric, univariate analyses were used to evaluate the impact of each characteristic in the research training profile on resident research activity. The following characteristics were found to be statistically significant (P < 0.05): (1) the use of mentors outside of the physical medicine and rehabilitation department; (2) the provision of guidelines to residents for choosing a mentor; (3) the provision of responsibility guidelines to mentors; (4) the presence of a classroom curriculum. Mentorship outside of the department, however, had a negative impact on resident research activity, although the other three characteristics had positive impacts. A multivariate analysis using stepwise logistic regression was applied to further determine the capacity of each characteristic to predict independently the level of resident research activity. Only one characteristic simultaneously and independently remained significant (P < 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

In vivo determinants of energy consumption in electric motor driven artificial hearts.

To identify factors responsible for energy consumption, a retrospective investigation of the in-vivo performance of the 100 ml electric motor-driven left ventricular assist device (ELVAD), and the e-motor 100 ml total artificial heart, was undertaken. Multivariate regression analysis of the device parameters demonstrated that device flow, and estimated outlet pressure, were the most significant independent variables for predicting changes in motor power. Weighted least-square curvefit, using the product of these two variables, showed that changes in energy consumption can be well predicted for the ventricular assist device (r2 = 0.732). However, by applying the same model to the total artificial heart (ETAH), less favorable results were achieved (r2 = 0.422). In this model, device flow seemed to be more important in predicting energy consumption for the ETAH compared to the ELVAD. Therefore, changing the model by using flow to the third order significantly improved the fit (r2 = 0.6706) for the ETAH, and could compensate in part for the greater variability of the values and increased number of outliers in this group.

GEE with Gaussian estimation of the correlations when data are incomplete.

This paper considers a modification of generalized estimating equations (GEE) for handling missing binary response data. The proposed method uses Gaussian estimation of the correlation parameters, i.e., the estimating function that yields an estimate of the correlation parameters is obtained from the multivariate normal likelihood. The proposed method yields consistent estimates of the regression parameters when data are missing completely at random (MCAR). However, when data are missing at random (MAR), consistency may not hold. In a simulation study with repeated binary outcomes that are missing at random, the magnitude of the potential bias that can arise is examined. The results of the simulation study indicate that, when the working correlation matrix is correctly specified, the bias is almost negligible for the modified GEE. In the simulation study, the proposed modification of GEE is also compared to the standard GEE, multiple imputation, and weighted estimating equations approaches. Finally, the proposed method is illustrated using data from a longitudinal clinical trial comparing two therapeutic treatments, zidovudine (AZT) and didanosine (ddI), in patients with HIV.

Early experience with employee choice of consumer-directed health plans and satisfaction with enrollment.

OBJECTIVE: To assess the initial impact of offering consumer-defined health plan (CDHP) options on employees. DATA SOURCES/STUDY SETTING: A mail survey of 4,680 employees in the corporate offices of Humana Inc. in June 2001. STUDY DESIGN: The study was a cross-sectional mail survey of employees aged 18 and older who were eligible for health care benefits. The survey was conducted following open enrollment. The primary outcome is the choice of consumer-directed health plan or not; the secondary outcome is satisfaction with the enrollment process. Important covariates include sociodemographic characteristics (age, gender, race, educational level, exempt or nonexempt status, type of coverage), health status, health care utilization, and plan design preferences. DATA COLLECTION METHODS: A six-page questionnaire was mailed to the home of each employee, followed by a reminder postcard and two subsequent mailings to nonrespondents. PRINCIPAL FINDINGS: The response rate was 66.2 percent. Seven percent selected one of the two new plan options. Because there were no meaningful differences between employees choosing either of the two new options, these groups were combined in multivariate analysis. A logistic regression modeled the likelihood of choosing the novel plan options. Those selecting the new plans were less likely to be black (odds ratio [OR] 0.46), less likely to have only Humana coverage (OR 0.30), and more likely to have single coverage (OR 1.77). They were less likely to have a chronic health problem (OR 0.56) and more likely to have had no recent medical visits (OR 3.21). They were more likely to believe that lowest premiums were the most important plan attribute (OR 2.89) and to think there were big differences in the premiums of available plans (OR 5.19). Employees in fair or poor health were more likely to have a difficult time during the online enrollment process. They were more likely to find the communications very helpful (OR 0.42) and the benefits information very understandable (OR 0.38). They were less likely to feel that they had enough time to make their enrollment decision (OR 0.47). CONCLUSIONS: Employees who were attracted to the new CDHP plan options valued the attributes that distinguished these plans from other choices. The shift to consumer-defined plans and to the electronic provision of information, however, requires a significant increase in the communication support for all employees, but particularly for those in fair or poor health whose information needs are the most complex and individualized.

Diarrhea and respiratory symptoms among travelers to Asia, Africa, and South and Central America from Scotland.

Surveillance using admissions to hospital, while being useful, is a poor indicator of the real incidence of disease encountered by travelers. An alternative is self-reported illness among those who attended at a pretravel clinic prior to their travels. Estimates of incidence and risk factors were determined for attendees at a travel clinic in Scotland using a questionnaire. Analysis for risk factors was carried out for those travelers visiting countries in Africa, Asia, or South and Central America, who had traveled for 1 week or more and had returned between 1997 and 2001 (N= 4,856). Multivariate logistic regression was used to test the hypotheses that time abroad and age-group would be significant for both respiratory and diarrheal symptoms regardless of which of the three geographical areas are visited. From 2006 returned questionnaires (response rate = 41.3%), diarrhea and respiratory symptoms were reported by 44.2 and 16.8% of respondents, respectively; the incidence was significantly greater among travelers to Asia for both diarrheal (55.5%) and respiratory (23.7%) symptoms than among travelers to Africa (36.6 and 12.2%, respectively) or South and Central America (39.5 and 16.2%, respectively). For diarrhea, age was a highly significant risk factor for travelers to Asia, South and Central America, and Africa. Being a self-organized tourist/backpacker, traveling to Asia was associated with increased risk, while for Africa and South and Central America visiting family or friends was associated with a lower risk. For travelers to Asia, traveling to the Indian subcontinent was significantly associated with increased risk. The majority of travelers had an adverse event while traveling abroad, with diarrhea and respiratory conditions being especially common despite attending a travel clinic for advice prior to departure. However, the limitations of this surveillance-based strategy have highlighted the requirement for more research to understand more fully the issues of risk and incidence among travelers to high-risk destinations from Scotland.

Hemodynamic and autonomic changes induced by Ironman: prediction of competition time by blood pressure variability.

We hypothesized that the extreme endurance exercise of an Ironman competition would lead to long-standing hemodynamic and autonomic changes. We investigated also the possibility of predicting competition performance from baseline hemodynamic and autonomic parameters. We have investigated 27 male athletes before competition, 1 h after, and then for the following week after the competition. The Task Force monitor was used to measure beat-to-beat hemodynamic and autonomic parameters during supine rest and active standing. Heart rate (P < 0.001) was increased, and stroke index (P = 0.011), systolic blood pressure (P = 0.004), diastolic blood pressure (P < 0.001), total peripheral resistance index (P < 0.001), and baroreceptor reflex sensitivity (P < 0.001) were decreased after the competition. The 0.05- to 0.17-Hz band of heart rate and blood pressure variability was increased (P < 0.001 and P < 0.001, respectively), the 0.17- to 0.40-Hz band of heart rate interval variability was decreased after the competition (P < 0.001). All parameters returned to baseline values 3 days after the competition. After the competition, the autonomic response to orthostasis was significantly impaired. The 0.05- to 0.17-Hz band of diastolic blood pressure variability before competition and weekly net exercise training, but not the other hemodynamic and autonomic parameters, were related to competition time in multivariate regression analysis (multiple r = 0.70, P < 0.001). The marked hemodynamic and autonomic changes after an ultraendurance race, which are compatible with myocardial depression in the face of sympathetic activation and reduction of afterload, return to baseline after only 1-3 days. Because the 0.05- to 0.17-Hz band of diastolic blood pressure variability contributes to the prediction of competition time, the analysis of blood pressure variability in the frequency domain deserves further study for the prediction of endurance capacity.

The relation between gastroesophageal reflux and respiratory symptoms in a population-based study: the Nord-Trøndelag health survey.

BACKGROUND: In spite of numerous investigations, the relation between respiratory symptoms and reflux symptoms in the general population remains unclear, since population-based studies are few. STUDY OBJECTIVES: To investigate the relation between respiratory symptoms and reflux symptoms in the population. SUBJECT AND METHODS: In a cohort of 65,363 individuals representing 71.2% of the adult population in the Norwegian county of Nord-Trøndelag, 58,596 individuals (89.6%) responded to questions concerning reflux symptoms. The 3,153 persons (5.4%) with severe and recurrent reflux symptoms constituted the case group, and the 40,210 persons (68.6%) without reflux symptoms served as the control group. Odds ratios (ORs) with 95% confidence intervals (CIs) represented relative risks. Potential confounding was tested in multivariable logistic regression analysis. RESULTS: Persons with asthma had reflux to a 60% greater extent than those without asthma after including adjustment for asthma medication (OR, 1.6; 95% CI, 1.4 to 1.9). There was a statistically significant dose-response association between breathlessness and reflux symptoms (p for trend < 0.0001), and the OR of severe breathlessness was 12.0 (95% CI, 9.5 to 15.2). Persons with heavy and wheezy breathing, daily cough, daily productive cough, or chronic cough showed a twofold to threefold statistically significant increase in risk of reflux symptoms. Adjustment for asthma or use of asthma medication did not substantially influence the risk estimates for any of the studied respiratory disorders. CONCLUSIONS: Reflux symptoms commonly coexist with asthma and other respiratory symptoms on a population-based level, seemingly irrespective of asthma medication.

Efficacy of racism-specific coping styles as predictors of cardiovascular functioning.

OBJECTIVE: This study examined the relationship between racism-specific coping strategies and cardiovascular responses to an ethnically relevant speaking task. METHODS: Fifty-seven African-American females participated in a speaking task during which they discussed their experiences with inter-ethnic group racism. Blood pressure and heart rate responses were measured during a pre-speech, speech, and recovery period. Racism-specific coping strategies were assessed with both the Perceived Racism Scale (PRS) and the Ways of Coping Scale-Revised (WCS-R). RESULTS: Multivariate regression analyses indicated that racism-specific coping strategies assessed via the PRS and WCS-R predicted blood pressure and heart rate changes. Passive coping strategies were generally associated with greater changes in systolic blood pressure, diastolic blood pressure, and heart rate during the pre-speech and speech periods, and with incomplete diastolic blood pressure recovery and heart rate recovery. Unexpectedly, the use of more active roping strategies was also associated with greater changes in diastolic blood pressure, and with incomplete heart rate recovery. CONCLUSION: This study provides preliminary empirical evidence that racism-specific coping styles may contribute to blood pressure and heart rate variability in African-American females.
