[Xanthogranulomatous pyelonephritis with fatal outcome in a 2-month-old infant].

A 2-month-old infant died of xanthogranulomatous pyelonephritis. Preoperatively pyonephrosis was suspected, because the child presented with a number of inflammatory, septic symptoms. Nephrectomy was performed and histopathology showed the kidney to be affected by xanthogranulomatous pyelonephritis. Postoperatively the child developed persistent attacks of fever and bronchopneumonia that led to his death with signs of pulmonary insufficiency.

[Chemiluminescence of blood and urine in children with pyelonephritis and glomerulonephritis].

It was established during the study of blood serum and urine chemiluminescence in 57 children with pyelonephritis and in 38 children with glomerulonephritis that in the active disease stage, the intensity of overfaint luminescence rises as a result of lipid peroxidation (LPO) activation, accumulation of lipid hydroperoxides and oxygen-containing radicals. Four types of the kinetic curves of urine chemiluminescence were identified. They characterize the correlation between LPO activation and the level of antiradical defence in patients suffering from pyelo- and glomerulonephritis. The measurement of urine chemiluminescence in patients afflicted with pyelonephritis and glomerulonephritis may be one of the criteria for administering drugs possessing antiradical and antioxidant activity and can also be used in the control over the treatment efficacy.

Ureteral involvement in stage I xanthogranulomatous pyelonephritis--(a case report).

A case of xanthogranulomatous pyelonephritis (Stage-I: Nephric) with ureteral involvement is described. The patient had undergone right nephrectomy with the working diagnosis of calculus pyonephrosis and non-functioning kidney. Histopathological examination of the nephrectomy specimen revealed xanthogranulomatous pyelonephritis confined to the kidney and non-contiguous involvement of ureter. Post-operative recovery was uneventful and there had been no evidence of disease recurrence till one year's follow-up.

Emphysematous pyelonephritis. Radiologic and clinical findings in six cases.

Six patients (all women, mean age 59.8 years) with emphysematous pyelonephritis, a rare, severe inflammation causing renal parenchymal destruction and affecting diabetics more commonly than non-diabetics, are described. Four of the patients were diabetics. All underwent ultrasound examination (US), and five computed tomography (CT) of the kidneys. CT was the most reliable diagnostic method. Differentiation between gas and calcification was difficult at US in three patients. Conventional tomography was performed in two patients and showed intra-renal gas. All six patients recovered; three patients had nephrectomy, one as an emergency procedure and two as an elective procedure after two weeks of antibiotic treatment. Infection was eradicated in the other three patients by percutaneous drainage, electrolyte correction and antibiotics. It was concluded that while conventional abdominal radiography may permit the diagnosis of emphysematous pyelonephritis by demonstrating intra-renal gas, CT is the most reliable diagnostic examination. Emergency nephrectomy may not be necessary in all cases.

Chronic pyelonephritis: modulation of host defenses by cyclosporin A.

Chronic experimental pyelonephritis is characterized by a stable level of infection, which persists for many months. Administration of cyclosporin A (CsA) reactivated previously healed renal lesions and caused a marked increase in bacterial numbers in the kidney. Studies were then carried out to compare the effects of CsA, and the nonselective cytodepletive agents irradiation and cyclophosphamide, on both host defenses and the bacteriologic status of chronically infected kidneys. Two different responses were observed. In animals treated with CsA, bacterial numbers increased markedly, although circulating neutrophil numbers were relatively unaffected. This observation was in contrast to the severe ablation of leukocyte numbers and competence needed to achieve an equivalent effect when irradiation and cyclophosphamide were used. One possible explanation for the adverse effect of CsA on the host-parasite balance in chronic pyelonephritis is that CsA affects mediators that control the inflammatory response or induces a qualitative change in a critical cellular defense compartment.

[Xanthogranulomatous pyelonephritis in early childhood].

We report on a two-year-old infant boy in whom a tumorous enlargement of the left kidney which was not imaged in the excretion urogram was discovered by renal sonography after the second urinary tract infection. By synopsis of the clinical data and imaging techniques, but especially by the detection of foam cells in the urinary sediment, we were already able to make the diagnosis of a xanthogranulomatous pyelonephritis before the operation. Since diffuse infiltration of the entire organ was present, total nephrectomy was carried out as the therapy of choice. Besides the case report, in particular the difficulties in the diagnosis of this form of pyelonephritis (especially in infancy) are described.

Pyelonephritis.

Pyelonephritis is a serious infection in pregnancy and is the consequence of undetected bacteriuria. When the diagnosis of pyelonephritis is made, immediate treatment is mandatory in the hope of preventing the potentially serious maternal and fetal sequelae. After successful treatment, careful surveillance and suppression are required for the remainder of the pregnancy. Moreover, owing to the high incidence of recurrences, both postpartum and during subsequent pregnancies, culture and radiologic investigations are warranted after delivery in an effort to detect underlying renal abnormalities.

[Xanthogranulomatous pyelonephritis coexisting with fistula between ureter and sigmoid colon: a case report].

A 63-year-old woman with the complaints of left renal stone and fistula between left ureter and colon was transferred to our clinic by a local doctor. Anemia, gamma-globulinemia, and acceleration of ESR were detected by hematology on admission. Kidney-ureter-bladder X-ray and intravenous pyelography showed left non-functioning kidney with coral stone, and RP revealed a fistula between left ureter and sigmoid colon. She was operated by nephroureterectomy and fistulectomy. The resected kidney appeared pale and parenchyma was almost replaced by yellowish tissue and pus. Histologically, the lesion was confirmed to be xanthogranulomatous pyelonephritis. Meanwhile, the cause of the fistula was considered to be due to extending ureteritis and pelvic infection from the pyelonephritis. Postoperative course was satisfactory.

[Value of the scanner in acute pyelonephritis].

Twenty patients with clinical acute pyelonephritis were investigated by routine CT scan. This examination was positive in 19 cases, demonstrating either a hypodense triangular image reflecting a disorder of blood supply or a mass syndrome. The CT scan can therefore confirm the diagnosis of acute pyelonephritis. It also defines the severity of the lesions and helps to guide treatment.

[The use of ultraviolet-irradiated blood plasma in the treatment of pyelonephritis patients].

Transfusions of ultraviolet-irradiated plasma were carried out in 30 pyelonephritis patients. Subjective sense of improvement, and relief of fever and intoxication were noted. The administration of ultraviolet-irradiated plasma had a controlling effect on lipid peroxidation and blood cortisol, aldosterone and insulin levels, contributed to the normalization of corticosteroid catabolism and stimulated energy metabolism and the antioxidant system. Products of plasma photochemical reaction to ultraviolet irradiation have immunomodulating effects. Treatment of pyelonephritis patients by ultraviolet-irradiated plasma transfusions has shortened the time of hospital stay.

[Plasmapheresis in the complex therapy of patients with acute pyelonephritis and urologic infection].

In 19 urological patients with pyointoxication and urosepsis 49 plasmaphereses for the purpose of detoxication were performed in the complex with other therapeutic measures. The causes for the pyoseptic complications were as follows: urolithiasis in connection with acute pyelonephritis, acute pyelonephritis (among others in pregnancy, cystic renal dysplasia, carcinoma of the urinary bladder), renal insufficiency in the terminal stage. The treatment of these diseases with haemodialysis and haemoperfusion was complicated by a pyosepsis. Two methods of the plasmapheresis were used: the intermitting plasmapheresis with use of a refrigeration centrifuge K-70 (GDR) and the permanent membrane plasma separation with the device A2008 RG of the firm "Fresenius" (FRG). The plasma perfusion was experimentally proved and in 5 cases used on 5 columns with activated charcoal. The efficacy of the plasmapheresis and the plasma perfusion was apart from the clinical condition judged according to the values of the middle molecules in the blood, or urea, creatinine and the normalisation of the hypoproteinaemia as well as of the humoral immunity. To this are added the increase of diuresis, the normalisation of the haematological parameters and the bacteriological findings of blood and urine. Furthermore, several pathogenetic mechanisms of the positive effect of the plasmapheresis were analysed (mechanic removal of bacteria and their toxins, effect of "deplasmation" with tissue dehydration, improvement of the functional state of the kidneys within the first 3-4 days: reduction of the azotaemic intoxication, the DWS-syndrome, improvement of the rheological properties of the blood and of the microcirculation, increase of the antitoxic function of the liver).(ABSTRACT TRUNCATED AT 250 WORDS)

The intravenous pyelogram in acute pyelonephritis.

In a cohort of 67 otherwise healthy patients with acute pyelonephritis that was severe enough to warrant hospitalization and uroradiography, 8% had a genitourinary abnormality that influenced management. Consequently, over 90% of patients had studies that did not alter their care. In an attempt to identify clinical clues that might increase specificity without compromising sensitivity of the intravenous pyelogram in acute pyelonephritis, only the fever curve was statistically useful. Confined to patients who were febrile through 72 hours of appropriate antibiotic treatment, the yield of urography in demonstrating anomalies of immediate clinical significance rose from 8% to 36%. The likelihood of an acutely important abnormality was also increased fivefold in both diabetic patients and patients with a urinary pathogen other than ampicillin-sensitive Escherichia coli, but small numbers precluded statistical significance. Bacteremia was common (27%), but not helpful other than in confirming the microbiological diagnosis. Nonacute structural abnormalities were present in 43% of the patients, three to nine times more frequently than in reported cases without upper tract infection.

[Beta 2-microglobulin in chronic exacerbated pyelonephritis].

The serum and urine beta 2-microglobulin levels were determined in patients with chronic recurrent pyelonephritis and in healthy controls. The urine beta 2-microglobulin level was considerably increased in the patients with chronic recurrent pyelonephritis which is linked with the activity of the disease.

[Diffuse xanthogranulomatous pyelonephritis in childhood].

Xanthogranulomatous pyelonephritis is a rare disease and rare cause of suppurative proliferative pseudotumor in children. An accurate diagnosis is difficult because of its clinical, radiological, sonographical and pathologic-anatomical similarities to renal tuberculosis. After removing the diseased kidney the prognosis is excellent. A 6-year old boy with xanthogranulomatous pyelonephritis is reported.

[Comparative characteristics of the adhesive properties of microorganisms isolated from the urine of children with chronic obstructive pyelonephritis].

The adhesive properties of 215 cultures, including 215 Escherichia coli strains, 43 Klebsiella pneumoniae strains and 60 Pseudomonas aeruginosa strains isolated from the urine of 124 children with chronic obstructive pyelonephritis were studied in the direct hemagglutination test simultaneously with those of 30 E. coli strains and 20 K. pneumoniae strains isolated from the feces of 50 healthy children, as well as 60 P. aeruginosa strains isolated from children with parenteral infections of other localization. E. coli and K. pneumoniae strains isolated from the urine of children with chronic obstructive pyelonephritis were found to have D-mannose-resistant hemagglutinins (68% and 37.2%) and a combination of mannose-sensitive and mannose-resistant adhesins (44.6% and 13.3% respectively). P. aeruginosa strains isolated from the urine of urological patients in the postoperative period showed the presence of mannose-resistant hemagglutinins to a greater extent (76.6%) than those isolated from children with parenteral infections of other localization (45%).

[Study of electrophoretic mobility of urinary lysozyme in the differential diagnosis of chronic glomerulonephritis and chronic pyelonephritis].

Different electrophoretic mobility of urine lysozyme was established in patients with chronic glomerulonephritis and chronic pyelonephritis during electrophoresis in 12% polyacrylamide gel (gel pH 4.3, electrode buffer pH 4.0). The examination of 128 patients has shown that anode position of lysozyme in electrophoretic tubes is observed in 95% of patients with chronic pyelonephritis, and its anode position in 92% of patients with chronic glomerulonephritis. A method of urine lysozyme electrophoresis under the above conditions was proposed as a noninvasive method of differential diagnosis of chronic glomerulonephritides and chronic pyelonephritides.

The effects of cobra venom factor, an inhibitor of the complement system, on the sequence of morphological events in the rat kidney in experimental pyelonephritis.

Acute experimental pyelonephritis has been produced by a combination of mechanical ureteral obstruction and intravenous injection of E. coli (strain IMRU-54). The effects of administration of cobra venom factor, an inhibitor of the complement system, on the sequence of morphologic events in the kidneys have been studied by light and electron microscopy.Pronounced bacterial colonization and suppression of the infiltration of acute inflammatory cells into the kidney were present in the cobra venom factor treated rats on day 2. In these rats, in which the infiltration of polymorphonuclear leukocytes was inhibited, renal structural damage was significantly reduced. The findings appear to indicate that the polymorphonuclear leukocytes infiltrating into the kidney play some role in damaging the renal parenchymal tissue in the early phase of E. coli induced acute pyelonephritis in rats.

[The silent kidney. Evolving ultrasonic picture of emphysematous pyelonephritis: the vanishing kidney. Apropos of a case].

The authors report the progressive disappearing of the sonographical image of a previously visualized kidney in a case of emphysematous pyelonephritis. They have called the sonographic vanishing kidney "the sign of the Deaf Kidney". The non-visualisation of a kidney with advanced emphysematous pyelonephritis is due to the infiltration of the perirenal environment by a gas envelope and the filling of the excretory cavities by gas. The perirenal gas-bag forms a shield against ultrasonic waves. The sonographer must be aware of the eventuality of not being able to individualize a renal structure when wrapped in gas. He must then ask for a zonography of the renal area in order to confirm the gas-containing collection in the kidney.
