[Applications for disability benefits before and after the 1991 restrictions].

In 1991, the eligibility criteria for disability benefits were restricted in Norway. Some effects are described in the present evaluation. Based on documents, first time applicants in 1990 and in 1993 in two counties were analysed according to social and medical variables. "Social security careers" before application are described, and proposals from physicians and the local social security office are compared with the decisions made by the county social security administration. Over a three-month period, applications decreased from 2.2 per 1,000 inhabitants in 1990 to 1.4 in 1993, a 39% decrease. About the same decrease was observed in all social and diagnostic groups. The proportion on vocational rehabilitation before application increased from 19% to 23% (p = 0.02). The certifying physicians proposed refusals in 9% and 8% in the two samples, and the local social security office did so in 12% and 13%. The refusal rate increased from 8% in 1990 to 21% in 1993. Refusals were mostly given to women, the middle-aged, those living alone, those with short education, and applicants with medically unclear conditions. It is pointed out that the restrictions on disability benefits in 1991 had the greatest impact on applicants with few resources.

Vaginal applicators (ovoids) for local control and alleviation of rectal complications of cervical cancers treated by brachytherapy.

PURPOSE: To assess the effects of vaginal applicators (ovoids) on dose distribution, local control and rectal complications in cervical cancers treated by brachytherapy. METHODS: From 1984 to 1992, 41 patients (15 stages I+II; 26 stages III+IV) were treated by high-dose-rate brachytherapy (20 Gy in 4 fractions) after external beam irradiation (36-56 Gy). Twenty-three patients were treated by standard application of both intrauterine tandem and ovoids, and 18 patients were treated by non-standard application. RESULTS: The five-year, local-relapse-free rates by standard application and tandem alone were 69 and 65% overall; 83 and 100% for stages I+II; and 62 and 49% for stages III+IV. Local control was related to tumor response following the external beam irradiation and initial tumor size by multivariate analysis. Rectal complications seen in patients followed for more than 1 year were 33% after standard application and 22% for tandem use alone. The rectal doses given for standard application (24 Gy) and tandem use (16 Gy) were significantly different. CONCLUSIONS: The ovoid sources did not always contribute to local control, and occasionally led to rectal complications. The optimization of brachytherapy was dependent on patients' anatomy, tumor size, and tumor response.

Notice inviting applications for new award for fiscal year 1999. The Office of the Assistant Secretary for Planning and Evaluation (ASPE), DHHS.

The Office of the Assistant Secretary for Planning and Evaluation (ASPE) announces the availability of funds and invites applications for research into the status of applicants and potential applicants to the Temporary Assistance to Needy Families (TANF) program, individuals and families entering the TANF caseload, and individuals and families who leave TANF. Approximately four to six States or large counties will receive funding that will enable them to monitor and conduct research into the progress of individuals who apply for TANF benefits and their families. ASPE is particularly interested in targeting those applicants who apply for cash assistance but are never enrolled because of non-financial eligibility requirements, participation in up-front job search or other diversion programs, or failure to complete the application process. Proposed studies of new entrants onto the TANF program and of individuals leaving welfare also will be given consideration. Research topics could fall into the broad categories of employment and earnings, participation in government assistance programs, and child and family well-being. Grant applicants may choose any method for their proposed studies, including the linking of administrative data, surveys, or other methods as appropriate. The funds could either support a newly designed project or could be used to add new data sources and analyses to an existing project.

Session management for web-based healthcare applications.

In health care systems, users may access multiple applications during one session of interaction with the system. However, users must sign on to each application individually, and it is difficult to maintain a common context among these applications. We are developing a session management system for web-based applications using LDAP directory service, which will allow single sign-on to multiple web-based applications, and maintain a common context among those applications for the user. This paper discusses the motivations for building this system, the system architecture, and the challenges of our approach, such as the session objects management for the user, and session security.

Depth of cutaneous analgesia after application of a eutectic mixture of the local anesthetics lidocaine and prilocaine (EMLA cream).

BACKGROUND: EMLA cream, a eutectic mixture of lidocaine and prilocaine, is a topical anesthetic, frequently used to avoid pain during venipuncture and superficial surgery. However, the depth of analgesia needs further exploration. OBJECTIVE: Our purpose was to investigate the depth of cutaneous analgesia after application of EMLA cream. METHODS: In a single-blind crossover study, EMLA cream was applied under occlusion to the left thighs of 16 subjects. Eleven had application times of 60 and 120 minutes. Five subjects had application times of 3 to 4 hours. Approximately 10 minutes after removal of the EMLA cream, a skin biopsy punch (diameter 4 mm) was inserted to a depth of 1 mm into the skin and the subject rated the pain intensity on a visual analogue scale. If there was no pain or if pain was assessed as acceptable, a new biopsy punch was inserted at a new site to the depth of 2 mm. In this manner, the insertion depth was gradually increased in steps of 1 mm down to a maximum of 6 mm. No skin specimens were removed. RESULTS: After 60 and 120 minutes of EMLA application the mean insertion depths with acceptable pain were 2.9 and 4.5 mm (P <.01), respectively. After 3 to 4 hours of application, 6-mm deep insertions were made with acceptable pain in all 5 subjects. CONCLUSION: Skin biopsy punch insertions in steps of 1 mm appear adequate for assessing the depth of cutaneous analgesia. Biopsy punch insertions with acceptable pain can be made to depths of 1 to 2 mm after 60 minutes, to 2 to 3 mm after 120 minutes, and to 6 mm after 3 to 4 hours of EMLA cream application.

Effects of cold application on isometric strength and endurance of quadriceps femoris muscle.

This study was carried out to investigate the effect of cold therapy on isometric strength and endurance of the quadriceps femoris muscle group. Sixty volunteer healthy normal subjects participated in the study, 30 were males and 30 were females. They were aged between 20 and 27 years. The subjects were screened for previous knee injuries or abnormalities through history taking and physical examination. The baseline isometric strength and endurance of the quadriceps was tested before application of cold on the thigh. Immediate post and 10-minute post cold application isometric strength and endurance were measured using an adapted cable tensiometer. The result of the study showed that cold increased the isometric strength of the quadriceps in all the subjects immediate post and 10-minute post cold application (P < 0.05) and the immediate post cooling endurance index of male and combined male and female subjects (P < 0.05). No significant increase was seen in the immediate post cooling endurance for female subjects (P > 0.05). Also no significant difference was seen in 10-minute post cooling endurance of all the subjects when compared with the pre-cooling endurance. This study therefore concluded that cold application increased isometric muscle strength and endurance post cold application. It was therefore recommended that application of cold on muscle should be employed while rehabilitating an individual with musculoskeletal pathology or deficit particularly while training for muscle endurance, strength and restoration of muscle function.

The application of molecular biology.

Molecular biology methods have tremendous value not only in the investigation of basic scientific questions, but also in application to a wide variety of problems affecting the overall human condition. Disease prevention and treatment, generation of new protein products, and manipulation of plants and animals for desired phenotypic traits are all applications that are routinely addressed by the application of molecular biology methods. Because of the wide applicability of these methods, they are rapidly becoming a pervasive--some would argue invasive--aspect of our technologically based society. The public concerns that address the application of these methods should be addressed by informed public discussion and debate. While scientists can be extremely critical of the quality, interpretation, and significance of experimental results, they have a rather remarkable tendency to be non-judgmental of the relative social merits of many applications of scientific research. It remains a public responsibility to be sufficiently well-informed to critically assess the merits of applied science research and participate in a communal decision-making process regarding the extent to which a new technology will be allowed to affect society.

[Endocavitary ablation for arrhythmias. New modalities of radiofrequency applications. New energy types].

Radiofrequency remains the reference energy type for catheter ablation of rhythm disorders. In the classic indications, which are atrial flutter or tachycardia, nodal re-entry and Wolff-Parkinson-White syndrome, this energy source has the best cost-efficiency-safety ratio, subject to strict conditions of use. Some new modalities of application have further improved performance, especially active irrigation of the electrode which allows induction of deeper lesions which is very useful for the ablation of difficult atrial flutters, epicardial fascicles of Kent and ischaemic ventricular tachycardias. The only emerging alternative energy type, in the framework of classical ablation, is cold, for which the principal advantages are the homogenous and slightly thrombogenic character for the lesion involved, and the possibility of reversible applications tests which are especially useful in the ablation of structures at risk. The situation is more open-ended concerning research on ablation for atrial fibrillation or the so-called new energy types, such as ultrasound and laser, whilst recognising a renewal in interest, especially for circumferential ablation of the pulmonary veins to isolate the ectopic venous foci. Mechanical energy such as luminous energy is emitted across a catheter balloon deployed at the orifice of the vein, perpendicular to its axis, aiming to reach a continuous circumferential lesion with a minimum of applications. Equally radiofrequency has been undergoing significant evolution for this application, such as by the development of porous catheter balloons with a liquid electrode, as well as by the development of deployable circumferential catheters. Ablation is use for atrial fibrillation, by endocavity atrial segmentation remains a field of research in which radiofrequency retains an important place. It is delivered via multi-electrode catheters according to the new application modalities, either pulsed or by phase interval, which secure better efficacy by better continuity of the line of block. Research is equally underway on the use of microwaves and cold in this application.

[Changes in serum concentration of podophyllotoxin after its topical application with liposome on rat skin].

OBJECTIVE: To investigate the changes of serum concentration of podophyllotoxin after topical application of liposome podophyllotoxin suspension on rat skin. METHODS: SD rats were used in this study, which were divided into test group (n=48) to receive application of liposome podophyllotoxin (0.5 % ) suspension and control group (n=48) treated with 0.5 % podophyllotoxin alcohol solution. Blood samples were obtained from the heart at 1, 2, 4, 6, 8, 10, 12 and 24 h respectively after drug application, and the serum concentration of podophyllotoxin was determined by spectrofluorometry. RESULTS: The area under the curve of plasma drug concentration of the control group was 2.3-fold greater than that of the test group. Eight hours after drug application, the serum concentration of podophyllotoxin reached the peak in the test group, while in the control group, only two hours was needed to reach the peak. The peak serum level of podophyllotoxin in the test group were significantly lower than that of the control group (166.395 +/- 14.634 ng/ml vs 378.603 +/- 26.105 ng/ml, P<0.001). CONCLUSION: The systemic absorption of podophyllotoxin in rats after its topical application in liposome suspension is significantly lower than that after application of 0.5 % podophyllotoxin alcohol solution, therefore the systemic toxicity may be reduced.

Distribution of MCA-coated grits in maize fields after high wheel tractor application for disrupting orientation of Diabrotica virgifera virgifera LeConte.

High wheel tractor applications of 4-methoxycinnamaldehyde (MCA)-coated corn granules ('grits') were conducted in Ruski Krstur (Serbia) in summer 2001 in a 5 ha corn field. Grits are a by-product after corn is harvested and separated from the cob and used as a carrier medium to disseminate MCA into the corn field. MCA is a kairomone mimic derived form Cucurbita maxima (Duchesne) used to disrupt orientation of Diabrotica virgifera virgifera LeConte towards different MCA and pheromone baited traps. The ultimate goal is to investigate the use of MCA as a mating disruptant. MCA was dissolved in an organic solvent and mixed in a cement machine with the grits. Grits were applied at rates of 17.39, 17.1 and 12.45 kg/ha on July 4th, July 19th and August 3rd. Before the impact of MCA as a disruptant can be addressed, the distribution patterns of MCA coated grits have to be thoroughly investigated. They were evaluated by counting girts deposited in 16 or 20 plastic dishes of 30-cm diameter positioned along 2 rows through the field directly after the grit application by tractor. Additionally, grits deposited on corn plant surface such as leaves, leaf axils and corn cobs were counted. Total number of grits collected in plastic dishes revealed even application rates at the first and second application but not on the third application date. Number of grits collected on plant surfaces were significantly different from each other regarding each application date. Altogether, grit distribution in the dishes as well as on the plant surface was variable. However, distribution patterns achieved so far hold promise to disseminate MCA coated grits into corn fields for orientation disruption or mating disruption of D. virgifera virgifera.

[Effects of forms and application rate of nitrogen fertilizer on yield and qualities of tobacco in southeast Tibet].

Field experiments in southeast Tibet were conducted to study the effect of nitrogenous fertilizer supply and its forms on tobacco. The result indicated that the yield, production value, and the contents of nicotine, potassium oxide and total nitrogen of the cured leaves were positively correlated to the amount of N application, and significantly and negatively correlated to the percentage of superior-medium class leaves of tobacco and contents of deoxidize sugar. The highest yield and best quality of tobacco were obtained by applying 75 kg N per hectare. Inorganic nitrogenous fertilizer could significantly improve the yield and quality of tobacco, compared with organic nitrogenous fertilizer. Although there was no significant difference among ammonium N, nitrate N and nitrate-ammonium N application, the effects of ammonium N, nitrate N, and nitrate-ammoniun N application were significantly higher than that of urea application. The effect of ammoniun N application on tobacco yield and quality was better than that of nitrate N application, and the important reason was the great difference in the process of absorption and assimilation of ammoniun N and nitrate N.

The regulation of home diagnostic tests for genetic disorders: can the FDA deny a premarket application on the basis of the device's social impacts?

This article discusses the Food and Drug Administration (FDA) authority to regulate home use diagnostics for genetic disorders based on the social effects of the devices in the premarket approval process (PMA). It begins with a discussion of the potential social impacts of home use genetic diagnostics, focusing particularly on the psychological effects of the devices. The article then discusses the FDA's past experience in regulating home use diagnostics for HIV and for drugs of abuse testing under its PMA authority. In approving PMA applications for home tests for HIV and drugs of abuse, the FDA has considered various social effects of the devices and tailored its approval to these considerations. However, the agency did not deny approval because of social considerations. The author argues that the FDA's past experience in approving PMA applications indicates both a willingness to consider social considerations in the PMA process and that the agency may deny a PMA application based on social considerations in appropriate circumstances. The author also argues that the FDA may deny application if the social impacts and other health risks of the device greatly outweigh the therapeutic benefits of the device. For example, in applications involving late onset genetic disorders for which no therapeutic options currently exist, the social risks of the device may be found to outweigh the benefits to health and therefore the FDA may deny a PMA application in these situations.

[The regression analysis of the in hospital medical staffs infection rate and the application of the isolation measures including the emergency-isolation radiology information system].

OBJECTIVE: To analyze the relationship between the rate of the in-hospital medical staff SARS infection and the measure of isolation which including the application of the emergency-isolation radiology information system (EIRIS) in polyclinic. METHODS: The groups were classified according to the application of the prevention and protection measure. Group A referred to the stage before the declaration of infectious area was made. Group B was defined as the stage in which the application of common measures of the prevention and protection were made, but the isolation measure was not taken (the suspected or diagnosed cases had the chest X ray film examination in the department of radiology). Group C was the stage after the startup of the isolation measure including the application of EIRIS. the number of the diagnosed cases, the medical staffs exposed, and the diagnosed cases of medical staffs were recorded. RESULTS: In group A, the infectious rate of in-hospital medical staffs showed a marked ascending trend with the regression coefficient of 0.000545, P = 0.04. The rate showed a little descending trend in Group B compared with that in Group A with the regression coefficient of 0.000164, P = 0.140. While marked descending trend compared with group A was demonstrated in group C with the regression coefficient of 0.000037, P = 0.02. CONCLUSION: The trend of the in-hospital medical staffs infection rate changed with different style of prevention and protection measures. With the application of the common measure of the prevention and protection, the rate was significantly lower than that in group A. After the application of the isolation measures including EIRIS, the rate showed the descending trend much further.

[Study of the recovery rate of non-NMDA desensitized receptors of hippocampal neurons using paired and rhythmic application of glutamate].

The new technique has been used to realize paired short-term application of glutamate to whole dialyzed neuron. Desensitization of receptors via first application of glutamate resulted in incomplete recovery of the current evoked by the second application if interval between both applications was less than 160 ms. The recovery time constant of the current was found to be 44 ms at single exponential approximation. It is close to the mean desensitization time constant of the current evoked by single prolonged application of glutamate. Rhythmic application of glutamate resulted in a rapid fall of the mean current amplitude at frequencies over 10 Hz. Data obtained by the authors confirm the known concept of desensitization of non-NMDA receptors as a relatively slow conformation of the receptor-channel complex.

The effects of weekly mupirocin application on infections in continuous ambulatory peritoneal dialysis patients.

Application of mupirocin to the nares or catheter exit site and frequency of mupirocin administration in continuous ambulatory peritoneal dialysis (CAPD) patients remain controversial. The objective of our study was to evaluate, using a historical control group, the efficacy on CAPD-related infections of once-weekly application of mupirocin at the catheter exit site. We instructed 18 CAPD patients, who did not initially use prophylactic antibiotic treatment, about once-weekly application of mupirocin ointment to the exit site as part of their exit-site care. We recorded the incidence of catheter-related infections, the causative micro-organisms, and the rate of catheter loss. We observed 17 acute exit-site infections (AESIs: 0.45 episodes/patient-year) before mupirocin treatment and 2 AESIs (0.06 episodes/patient-year) after treatment. The relative rate of AESI reduction was 86%. Before application of mupirocin, 52% of AESIs were attributable to Staphylococcus-aureus; after mupirocin administration, no AESIs were staphylococcal. Peritonitis episodes were also reduced from 21 before mupirocin treatment (0.56 episodes/patient-year), to 9 after mupirocin administration (0.29 episodes/patient-year). The relative rate of peritonitis reduction was 48%. Once-weekly application of mupirocin to the exit site resulted in a reduction in exit-site infections and peritonitis episodes comparable to those obtained with daily application.

Superiority of water application to water sealing in burn wound healing.

BACKGROUND: Recently there have appeared on the market many external dressings for wound management. They all use water-sealing materials to maintain wounds in a moist state. However, in our daily clinical observations, a water-sealing material sometimes results in poor wound healing. Although a water-sealing material does in fact keep the wound moist, the quality of the water is far from that of the optimum extracellular fluid. METHODS: A second-degree burn wound was made in the rat by contact with a metal stick. Experiment 1 was to evaluate the influence of delayed primary wound treatment on burn-wound depth progression. We used a time lag procedure. Each group was treated with a water-sealing application (hydrocolloid sheet). Experiment 2 was to investigate the progression-preventing effect of normal saline on a burn allowed to dry for 3 hours, we compared a group that received a water sealing (non-moistening and incomplete moisture) dressing with one that received a water-supplying (moistening) dressing with using chi-square test. RESULTS: In the Experiment 1 (time lag and water sealing test), the early group showed quite normal wound healing at 7 days after onset. The slough was thin (about 10-20% of full dermal thickness). The delayed groups showed the poor healing characteristic. The slough was relatively thick (about 30-60% of full dermal thickness). Three hours' delay seemed to bring about poor healing. In the Experiment 2 (water application test), the moistening (water application) group showed relatively good healing after seven days. The slough was as thin as in the early group in the Experiment 1. The non-moistening group showed a poor healing process like the delayed group in the Experiment 1. For the moistening group, the healing percentage of 77.8% was significantly greater than the 41.3% observed in the non-moistening group (chi-square test, p < 0.05). CONCLUSIONS: All the materials in wound dressing are water-sealing. When the component water under the sheet is far from the extracellular fluid, these applications bring about poor healing. Delayed application and incomplete moisture should be corrected by external water-application.

Effect of application mode on bonding performance of self-etching adhesives.

PURPOSE: To investigate effects of static vs. dynamic application of self-etching adhesives on bond strengths to enamel and dentin and marginal adaptation in dentin cavities. METHODS: Products: AC Bond experimental (ACB), AQ Bond (AQB), Fluorobond (FLB), and One-Up Bond F (OUB). Shear bond strengths (SBS) to enamel and dentin were determined on human molars. Primer applications: 1. undisturbed, 2. brush agitation, 3. ultrasonic agitation (Cavitron, lowest setting), or 4. ultrasonic agitation (5 seconds). A 3.5 x 2 mm mold was mounted for application of the resin-based composite Charisma. Six specimens for each condition were stored for 24 hours and loaded until failure. STATISTICS: ANOVA and Scheffé test (P= 0.05). Marginal adaptation was determined in cylindrical dentin cavities (4 x 1.5 mm) 15 minutes after curing of Charisma. Eight specimens/adhesive were produced following application modes 1, 2, and 4. Kruskal-Wallis and Mann-Whitney showed a P= 0.05. RESULTS: Application modes showed no differences in shear bond strengths (SBS) on enamel and dentin by adhesive. SBSs on enamel were not different from those on dentin. Average SBSs for ACB, AQB, FLB, and OUB were 21.0, 21.9, 20.8, and 13.7 MPa. Ranking of adhesives for SBS on enamel and dentin was: AQB, ACB > ACB, FLB > OUB. Application modes had no influence on marginal adaptation. From the 24 cavities each, all ACB, 21 AQB, 13 FLB, and 1 OUB were gap-free. Mann-Whitney ranking: ACB, AQB, FLB > OUB.

Effect of prolonged application times on resin-dentin bond strengths.

PURPOSE: To evaluate the cumulative loss of mass of two-step solvent-based adhesives, examine the effect of prolonged application time on the resin-dentin microtensile bond strength (microTBS), and correlate the microTBS with the evaporation rate of adhesive systems. MATERIALS AND METHODS: An ethanol/water based (Single Bond: SB) and an acetone-based system (One Step: OS) were employed. Approximately 16 microl of each product was transferred to a small glass container and placed in an analytical balance. The cumulative loss of mass (CLM) was determined in every minute for 5 min to the nearest 0.0001 mg and again after 20 min (n = 4). For the microTBS test, a flat superficial dentin surface was exposed in 24 human third molars by wet abrasion. The adhesives were applied to a delimited area of 52 mm2 on wet surfaces, for 40, 90, and 150 s. Four teeth were assigned to each experimental group. Composite crowns were constructed incrementally. After storage in water at 37 degrees C for 24 h, teeth were sectioned to obtain sticks with cross-sectional areas of 0.8 mm2 to be tested in tension (0.5 mm/min). The CLM (%) for both adhesives was compared using a Student-t test at each minute (alpha = 0.05). The microTBS values were analyzed by two-way ANOVA and Tukey's test (alpha = 0.05). RESULTS: The CLM for OS was higher than for SB during the first 5 min, but these values were only statistically different at minutes 3, 4, and 5 (p < 0.05). A CLM of approximately 50% was observed for both adhesive systems after 20 min. The factor application duration and the interaction between application duration and adhesive were significant (p < 0.05). The increase in the application duration led to an increase in microTBS for both systems; however, the differences were only statistically significant for OS over the 150 s period (p < 0.05). A strong correlation was found between the microTBS and the cumulative loss of mass of adhesives. CONCLUSION: Prolonged application times can increase the resin-dentin microTBS of two-step adhesive systems, but the application duration is dependent on the solvent present in the bonding system.
