Intraforaminal O(2)-O(3) versus periradicular steroidal infiltrations in lower back pain: randomized controlled study.

BACKGROUND AND PURPOSE: Reports about steroids and oxygen-ozone therapy to treat lower back pain have been increasing. The purpose of our study was to compare the clinical outcomes in patients treated with infiltrations of O(2)-O(3) gas or steroids at short-, medium-, and long-term follow-up. METHODS: A total of 306 patients (166 with primarily disk disease, 140 with nondisk vertebral disease) with acute or chronic low back and sciatic nerve pain received a CT-guided intraforaminal infiltration of an O(2)-O(3) gas mixture or an periradicular infiltration of steroids. Neurologists unaware of the type of treatment assessed the patients. RESULTS: At 1-week follow-up, most patients had a complete remission of pain, regardless of the treatment. At 6-month follow-up, differences in favor of O(2)-O(3) treatment were significant in patients with disk disease (P = .0021) but not in those without disk disease (P = .0992). Clinical outcomes were poor in 13 (15.1%) of 86 patients receiving O(2)-O(3) infiltration and in 18 (22.5%) of 80 patients receiving steroid injection (P = .2226). Among patients without disk disease, six (8.6%) of 70 patients receiving O(2)-O(3) infiltration but 21.4% of the patients receiving steroid injections had poor outcomes (P = .0332). CONCLUSION: Oxygen-ozone treatment was highly effective in relieving acute and chronic lower back pain and sciatica. The gas mixture can be administered as a first treatment to replace epidural steroids.

[The distribution and viability changes of the Agrobacteria in the vacuum infiltrated plants of Chinese cabbage].

To understand further the mechanism of agrobacteria transformation by vacuum infiltration, the distribution and viability changes of the agrobacteria in infiltrated Chinese cabbage( B. rapa ssp. chinesis) plants were investigated by histochemical staining and bacteria plating techniques. Results revealed that there were different quantities of Agrobacteria in different organs, among which the most quantity occurred in flowers, especially in ovules. The Agrobacteria lied in the space between cells and concentrated in the vascular bundle no matter the type of the organs examined. The CFUs (colony forming unit) of the Agrobacteria derived from the flowers were always the most in every experiment. The viabilities of the agrobacteria in the plants declined dramatically along with the elongation of the time after plant infiltration in all organs, but there were still 10(3) CFUs per gram fresh tissue in the flowers after 15 days of infiltration. These results may explain the fact that the ovules were the targets for the Agrobacteria mediated vacuum infiltration transformation.

Nonpalpable ductal carcinoma in situ versus infiltrating carcinoma of the breast--can they be differentiated by mammography?

The radiologic patterns of 193 consecutive women with nonpalpable breast carcinoma were evaluated. The results were correlated with patient age and with the histopathologic diagnosis. Of the 193 women in the study, 135 (70%) were older than 50 years; 102 women had ductal carcinoma in situ, and 67 (62%) of them were over age 50; the remaining 91 women had infiltrating carcinoma, and 73 (80%) of these patients were older than 50 years. Of the 58% of women who presented with microcalcifications, 75% had ductal carcinoma in situ and 25% infiltrating carcinoma. Of the 36% of patients who presented with a mass, 87% had infiltrating carcinoma and 90% were over age 50. Six percent presented with a mass with microcalcifications; 75% of these masses were found to be infiltrating carcinoma. Most of the masses were infiltrating carcinoma, and of these 90% were high-density masses. The small number of masses representing ductal carcinoma in situ were proportionately of low density.

[Solid neoplasms localised within the parameningeal region and CNS infiltration].

INTRODUCTION: Solid neoplasms primarily localised within parameningeal region constitute a significant diagnostic and therapeutic problem. Solid tumours developing in the primary site are soft tissue sarcomas (STS) and lymphoepithelioma. AIM OF STUDY: Evaluation of the incidence of CNS infiltration, histological type of tumour and results of treatment. MATERIAL AND METHOD: The study enrolled 14 patients being treated for solid tumours primarily localised within the perimeningeal region, in the Department of Paediatric Haematology and Oncology of the Medical University of Lublin between 1992 and 2004. The age of patients ranged from 2 up to 17 years (mean 8.5): 4 girls and 10 boys. RESULTS: Soft tissue sarcoma was diagnosed in 10/14 children: rhabdomyosarcoma (RMS) embryonale in 8 patients, RMS alveolare in 1, leiomyosarcoma in 1, and lymphoepithelioma in four children. The stage of disease was III degree in all children (without CNS infiltration). Secondary infiltration of CNS was diagnosed in 2/14 children with STS. CONCLUSION: Secondary CNS infiltration was diagnosed in 2/14 patients. Despite of combined treatment, results of treatment are still poor due to local recurrance with CNS infiltration.

Overexpression of oncogene products can cause tumor progression without parenchymal infiltration in the rat brain.

Tumor cell progression and parenchymal infiltration play important roles in the pathogenesis of gliomas. Using retrovirally marked rat 9L gliosarcoma cells, which when injected in the CDF rat brain form noninfiltrating tumors which grow only surrounding existing blood vessels, we were able to investigate elements of tumor growth and progression within the substance of the brain. Transfection of 9L by oncogenes associated with tumor progression and expressed in gliomas resulted in alterations in the in vivo phenotype of these cells, with the production of faster growing, well-vascularized, large solid tumors. However, diffuse infiltration was not seen. Moreover, coinjection of 9L or its transfectants together with the infiltrative C6 cell line resulted in a mixed tumor of noninfiltrating 9L cells in an environment of infiltrating C6 cells, suggesting that the infiltrative ability of C6 cells is controlled on the individual cell level.

[Expression of the genes FHIT, Bcl-2 and Bax in breast infiltrating ductal carcinoma and clinicopathological significance thereof].

OBJECTIVE: To investigate the expression of the genes fragile histidine triad (FHIT), Bcl-2, and Bax, biological markers of breast infiltrating ductal carcinoma in this carcinoma and clinicopathological significance thereof. METHODS: The clinical data of 100 patients with breast infiltrating ductal carcinoma, all females, aged 435 (28 - 92), were collected. Immunohistochemistry was used to detect the protein expression of FHIT, Bcl-2 and Bax in the carcinoma tissues resected during operation. RESULTS: The protein expression rates of FHIT, Bcl-2 and Bax in the tumor tissues were 73%, 50%, and 34% respectively. The disease free survival (DFS) and overall survival (OS) of the FHIT positive patients were 81.8 months and 123.6 months, both significantly longer than those of the FHIT-negative patients (27.7 months and 74 months, both P < 0.05). The DFS of the Bcl-2-positive patients was 83 months, significantly longer than that of the Bcl-2-negative patients (45 months, P < 0.05). The mean DFS of the Bcl-2-positive patients who received postoperative adjuvant chemotherapy was 54.8 months, significantly longer than that of the Bcl-2-negative patients who received postoperative adjuvant chemotherapy (41.6 months). The mean DFS and OS of the Bcl-2-negative patients receiving CAF regimen were 55 months and 58.8 months respectively, both longer than those of the Bcl-2-negative patients receiving other regimens (27 months and 36 months respectively). However, the expression of Bax failed to show correlation with the prognosis of breast infiltrating ductal carcinoma. CONCLUSION: Expression of FHIT and expression of Bcl-2 are positively correlated to the DFS and OS of the breast infiltrating ductal carcinoma. Bax is not predictive to the prognosis of breast infiltrating ductal carcinoma.

Interstitial cellular infiltration detected by fine-needle aspiration biopsy in nephritis.

Fine-needle aspiration biopsy (FNAB) was used to detect renal mononuclear interstitial inflammation in 56 patients with various types of nephritis (20 IgA nephropathy, 8 focal necrotizing glomerulonephritis, 7 interstitial nephritis, 6 non-classifiable chronic glomerulonephritis, 5 mesangial proliferative (non-IgA) chronic glomerulonephritis, 4 focal glomerulosclerosis, 6 normal histology, who were examined for microscopic hematuria, and 7 controls). Regular renal biopsies for histological and immunofluorescence studies were simultaneously obtained, and available for comparative analysis (not controls). Differential counts of mononuclear infiltration and subtyping of T-cell infiltration into T-helper (T-h) and T-suppressor-cytotoxic (T-s-c) cells, as detected by immunoperoxidase stains from FNAB, were correlated to clinical manifestations and renal function tests. Generally, our results indicated increased mononuclear cell infiltration (monocytes, lymphocytes, and/or activated lymphocytes) in FNAB of patients with IgA nephropathy, interstitial nephritis or focal necrotizing glomerulonephritis (especially monocytes in IgA nephropathy and interstitial nephritis, p less than 0.05 compared with controls, lymphocytes in focal necrotizing glomerulonephritis, and non-classifiable glomerulonephritis, p less than 0.02 and 0.05, respectively). The number of infiltrating activated lymphocytes was significantly increased in focal necrotizing glomerulonephritis, interstitial nephritis and focal glomerulosclerosis, p less than 0.05, less than 0.01 and less than 0.01, respectively. FNAB was at least as sensitive as histological examination for the quantification of interstitial cellular infiltration, and it allowed for cytological differential counts. Patients had decreased T-h and increased T-s-c cell counts, which were accentuated in FNAB compared with peripheral blood, although there were strong positive correlations between local and peripheral counts (p less than 0.0001).(ABSTRACT TRUNCATED AT 250 WORDS)

[Pulmonary infiltration with eosinophilia possibly induced by cefotiam in a case of steroid-dependent asthma].

A 37-year-old female with refractory asthma taking betamethasone orally (1 mg/d), showed a fever of 38.5 degrees C, productive cough and dyspnea. The chest X-ray demonstrated diffuse infiltration in the left lower lung field. The WBC count was 16,000/mm3 with 6% eosinophils. She was treated with intravenous drip infusion of antibiotics (Cefotiam 2 mg/d and Sisomicin 150 mg/d) for 2 weeks, and her symptoms and the chest X-ray findings improved. However, at the end of the therapy eosinophilia was noticed. Sixteen days after the completion of antibiotic therapy, she again experienced fever, cough and dyspnea. The chest X-ray again demonstrated diffuse infiltrations in the left lower lung field. The total IgE level, histamine and circulating immune complex titers were elevated. The WBC count was 14,700/mm3 with 34% eosinophils. Although a sputum culture yielded no organisms, many eosinophils were observed in the sputum. There were no clinical or laboratory findings compatible with allergic broncho-pulmonary aspergillosis. After the administration of oral prednisolone (40 mg/d), the patient showed rapid improvement with resolution of all symptoms and normalization of the IgE, histamine and circulating immune complex levels. The chest X-ray revealed marked regression of the pulmonary infiltrations. A microscopic examination of a transbronchial biopsy specimen demonstrated moderate eosinophilic infiltrations. It was compatible with the diagnosis of pulmonary infiltration with eosinophilia. Treatment was performed with prednisolone. The result of a lymphocyte stimulation test was positive for Cefotiam. An in vitro test was performed to evaluate the diagnosis of drug allergy in this case.(ABSTRACT TRUNCATED AT 250 WORDS)

[Cutaneous blood flow following subcutaneous infiltration of lidocaine with and without the addition of adrenaline or ornipressin].

The addition of ornipressin to local anesthetics increases the duration of regional anesthesia. A dose of 0.1 unit/ml produces an increase in nerve block duration comparable to adrenaline 5 micrograms/ml. In order to suppress intraoperative bleeding higher concentrations of ornipressin have been used in infiltration anesthesia. This study was designed to examine whether an increase in ornipressin concentration above 0.1 unit/ml causes a further reduction in cutaneous blood flow. Twelve volunteers took part in the study. They received intracutaneous infiltrations of 4 ml lidocaine 0.5% in the volar aspects of both forearms (Fig. 1). The solution was either plain (= Lido) or contained adrenaline 5 micrograms/ml (Lido + A), or ornipressin 0.1 unit/ml (Lido + P1) or 0.2 unit/ml (Lido + P2). Blood flow was measured with a laser Doppler flowmeter; before the infiltration the skin was locally heated to 40 degrees C in order to obtain maximal perfusion (Flow 100); this temperature was maintained throughout the measuring period. The minimal flow after infiltration was calculated as a percentage of Flow 100. Lido caused a significant decrease in flow to 59 +/- 25% (M +/- SD). Lido + A, Lido + P1 and Lido + P2 caused further significant flow reductions, to 19 +/- 21%, 23 +/- 16% and 26 +/- 23%, respectively (Figs. 3, 4). The latency of maximal flow reduction did not differ between the four solutions (Fig. 5). The results show that in infiltration anesthesia an increase in ornipressin concentration above 0.1 unit/ml does not improve superficial vasoconstriction.

Relationship between the tuberculin-type and Jones-Mote-type hypersensitivities: suppression of basophil infiltration by mycobacterial adjuvant.

Guinea-pigs immunized with bovine gammaglobulin (BGG) in incomplete Freund's adjuvant (IFA) showed the typical Jones-Mote-type hypersensitivity (JMH) reaction when tested 5 days later. This is characterized by prominent basophil infiltration. After pretreatment with complete Freund's adjuvant (CFA) 16 days before immunization with BGG in IFA, the lesions resembled the JMH reaction macroscopically in their evolution over time and in the absence of a positive macrophage migration inhibition (MIT) test. However, histologically, the lesions resembled classical tuberculin-type hypersensitivity with prominent mononuclear cell infiltration without any basophils. The pretreated animals, which failed to show basophil infiltration, were able to transfer JMH reactions with basophil infiltration into normal animals. In contrast, pretreatment of recipients with CFA or Corynebacterium parvum prevented the passive transfer of the characteristic effect on the JMH reaction when given shortly before skin testing. We postulate that macrophages activated by CFA may play an important role in regulating basophil infiltration in the effector phase of the delayed hypersensitivity reaction.

Depletion of T-cells with the CD4+CD45R+ phenotype in lymphocytes that infiltrate subcutaneous metastases of human melanoma.

We studied the phenotypes of lymphocytes extracted from 22 specimens of human melanoma, 11 s.c. metastases and 11 lymph node metastases, by two-color flow cytometry. Lymphocytes extracted from s.c. metastases were characterized by a significantly reduced ratio of CD4+ to CD8+ T-cells, as compared with peripheral blood lymphocytes from the same patients. Ten of 11 tumor-infiltrating lymphocytes from s.c. metastases, but only 1 of 11 peripheral blood lymphocytes, had a CD4/CD8 ratio of less than 1.0. This alteration was not observed for lymphocytes obtained from nodal metastases. Furthermore, almost all of the CD4+ T-cells in s.c. metastases expressed the antigen CD29w and were negative for the complementary antigen CD45R. In contrast, the CD29w/CD45R ratio of tumor-infiltrating lymphocytes from lymph node metastases was similar to that of matched peripheral blood lymphocytes. Thus tumor-infiltrating lymphocytes from s.c. metastases have the phenotype associated with true helper or antigen-committed T-cells, which could reflect their sensitization to tumor antigens, while tumor-infiltrating lymphocytes from lymph node metastases may represent merely an expanded residua of lymph node lymphocytes. Since tumor-infiltrating lymphocytes expanded in vitro are being tested as therapy for patients with advanced cancer, these observations may have important therapeutic implications.

Pathogenesis of central nervous system infiltration in acute leukemia.

The distribution pattern of leukemic infiltrates was studied in 31 cases of acute leukemia with CNS involvement. Dura mater involvement was found in 93% (29/31) of the cases; arachnoid, 71% (22/31); perivascular cuffing, 37% (17/31); parenchymatous, 16% (5/31). Dura matter infiltrates were the sole manifestation in nine cases; Infiltration of the arachnoid in the absence of dural infiltration was rare (9%). The anatomic evidence supports the concept that leukemic cells infiltrate by way of perivenous adventitial tissue connecting the dura mater and subarachnoid space. It is likely that this pathway leads directly from the bones of the skull into the brain parenchyma.

[Microscopic analysis of gingival inflammatory infiltrate in periodontal disease].

Twenty surgically treated patients (aged 14-67) were subject of analysis aiming to found localisation, intensity and cellular structure of gingival inflammatory infiltrate. Results of our research showed following characteristics of inflammatory infiltrate: location--30% superficial, 85% deep (focal) and 30% diffused infiltrate; intensity--15% weak, 40% moderate and 45% strong infiltrate; structure--60% mainly plasmocits, 20% plasmocitis and 20% mixed inflammatory infiltrate. In sulcular epithelium were very often found large amounts of granulocits and lymphocits. It can be concluded that periodotal type of gingival inflammation had local immunologic reaction. This process was mainly located in deep parts of gingival tissue and was also predominantly alterative.

[The role of leukemic conversion in the development of lymphoma infiltration of the nervous system].

On the basis of neuropathologic studies in 22 patients died from ML it was found that lymphoma cells in the brain and spinal cord were in the form of leukostasis, infiltrations usually limited to perivascular spaces, infiltrations in hemorrhagic foci and their vicinity. In cerebrospinal meninges, spinal roots and peripheral nerves we observed leukostasis and intraparenchymatous infiltrations. It was found that the most important role in the development of lymphoma infiltrations in the nervous system was played by hematogenic metastases whose development in the brain and spinal cord was promoted by leukemic conversion with high leukocytosis. In contrast, marked dissemination of lymphoma cells to peripheral blood is not indispensible for the development of infiltrations in the cerebrospinal meninges and peripheral nervous system.

The infiltration of bile duct carcinoma along the bile duct wall.

The infiltration of extrahepatic bile duct carcinoma along the bile duct wall was studied in 29 resection cases. Histologically, spreading took place by mucosal infiltration in one case, mucosal and transmural in three, and transmural in 25. The average microscopic infiltrating distance from the gross margin of the tumor (ID) was 16.8 mm towards the liver, 6.5 mm towards the duodenum. The ID of the infiltrating type in the gross appearance of the tumor and the ID of the tubular adenocarcinoma in the histological cellular type tended to be long. The severer the invasion of the carcinoma to the lymphatic vessels, the longer the ID. The result indicated that most bile duct carcinomas have a tendency for long infiltration along the wall. An exceedingly wide resection involving hepatectomy is advisable for treatment.

Conjunctival eosinophil infiltration evoked by histamine and immediate hypersensitivity. Modification by H1- and H2-receptor blockade.

The present histological studies have demonstrated that histamine causes dose-dependent eosinophil infiltration into the conjunctiva. A highly directional movement toward the conjunctival epithelium was observed, and the presence of large numbers of degranulating eosinophils appeared to result in epithelial cell damage and goblet cell discharge. Blockade of H2-receptors by systemic cimetidine pretreatment significantly inhibited the eosinophil infiltration elicited by an intermediate histamine dose, whereas the H1-receptor blockade produced by systemic pyrilamine pretreatment markedly reduced the response to all histamine doses. The pyrilamine-insensitive residual eosinophil infiltrate was not affected by administering a cimetidine-pyrilamine combination. In animals presensitized to ovalbumin, antigen challenge evoked extensive and directional emigration of eosinophils toward the conjunctival epithelium with resultant exfoliation and depletion of goblet cell populations. In conjunctival immediate hypersensitivity, neither cimetidine nor pyrilamine alone produced an inhibitory effect, but a cimetidine-pyrilamine combination caused a significant reduction in the number of infiltrating eosinophils and prevented epithelial damage and goblet cell depletion. These results suggest that histamine may participate in the recruitment of eosinophils during immediate hypersensitivity reactions. The differential effect of pyrilamine on the eosinophil infiltration evoked by histamine or immediate hypersensitivity may, perhaps, reflect the importance of increased microvascular permeability in facilitating eosinophil emigration.

Mononuclear cell subpopulations and infiltrating lymphocytes in erythema dyschromicum perstans and vitiligo.

Erythema dyschromicum perstans (EDP) and vitiligo are two cutaneous pigmentary dermatoses of unknown etiology. In the present study, the leukocyte infiltrates in the affected skin of EDP and vitiligo patients were studied using the avidin-biotin (ABC) immunoperoxidase technique and monoclonal antibodies which recognise the following mononuclear cell subgroups: T-suppressor/cytotoxic (CD8-Leu-2), T-helper (CD4 = OKT4), T-suppressor + macrophages (Leu-15), Pan T (CD3 = Leu-4), macrophages (Leu-M3) and Langerhans cells (CD1 = Leu-6), and other cellular markers such as Ia antigens and the Interleukin-2 receptor (CD25 = TAC). The immunocytochemical analysis showed a selective accumulation of CD3+, CD8+, Leu-15-, T-cytotoxic cells in the epidermis of both EDP and early lesions of vitiligo. In addition, an increase in the number of epidermal Langerhans cells (CD1+) was observed in some cases of EDP and vitiligo. The CD4/CD8 ratios in affected and uninvolved skin for both disorders were not significantly different, although values lower than unity were only observed in the infiltrates of affected skin. Ia antigen positivity was observed in the dendritic cells of the dermis and epidermis, as well as in most of the lymphoid cells within the infiltrates for both diseases. Macrophages (Leu-M3) in EDP dermal infiltrates were generally found adjacent to extracellular melanin pigment. Lymphocytes expressing TAC (CD25) surface antigens were also present in the dermal infiltrates. These morphological observations suggest a possible immune cell participation in the dyschromia of such cutaneous disorders.

Carcinomatous meningitis associated with infiltrating lobular carcinoma of the breast.

The records of 365 patients with advanced carcinoma of the breast were examined to identify those with CNS involvement. Nineteen (5.2%) developed parenchymal cerebral deposits and 10 (2.7%) developed meningeal infiltration during the course of their disease. Parenchymal cerebral deposits were almost exclusively associated with infiltrating duct carcinoma (95%) and meningeal infiltration (carcinomatous meningitis) was almost exclusively associated with infiltrating lobular carcinoma. Meningeal infiltration occurred late in the course of advanced disease and was associated with diffuse involvement of the bone marrow and abdominal structures.
