[The relationship between loading on the TMJ and occlusal splints]

The primary purpose of this study is to estimate loading conditions on the TMJ with five different occlusal splint.We developed a model that simulated dynamic relationship between dental occlusion,masticatory muscles and TMJ.It is demonstrated that loading on the condyle increases whatever kinds of splints are used.The anterior splint brings about larger loading on the condyle than any other splint.When the ratio force of the muscle on the non pivot is1,left-side pivot split makes loading on left condyle decrease and right increase.It was change on both condyles when posterior teeth are missing including loading, contact and unilateral mastication.

Treatment for flexion contracture of the knee during Ilizarov reconstruction of tibia with passive knee extension splint.

Joint stiffness is one of the complications of limb procedure. It developes as a result of failure of knee flexors to lengthen in tandem with the bone, especially when there is inadequate physical therapy to provide active and passive mobilization of the affected joint. We are reporting four patients who developed fixed flexion contracture of their knees during bone lengthening procedure for the tibia with Ilizarov external fixator. Three of them were treated for congenital pseudoarthrosis and one was for fibular hemimelia. None of them were able to visit the physiotherapist even on a weekly basis. A splint was constructed from components of Ilizarov external fixator and applied on to the existing frame to passively extend the affected knee. Patients and their family members were taught to perform this exercise regularly and eventually near complete correction were achieved. With this result, we would like to recommend the use of this "Passive Knee Extension Splint" to avoid knee flexion Contracture during limb lengthening procedures with Ilizarov external fixators.

Use of a prefabricated splint for segmental augmentation of the mandibular alveolar ridge with hydroxyapatite.

Hydroxyapatite is a biocompatible material for augmentation of edentulous ridges to improve patient comfort and denture stability. The problems associated with hydroxyapatite are generally related to the amount of material introduced and the site of implantation. This article describes the use of a prefabricated splint that permits localization of the implant material to selected sites of the mandible and restricts the amount of material used. Fabrication of this splint under the guidance of the prosthodontist and oral surgeon permits the most effective use of this technique.

Sequence-dependent hydrolysis of RNA using modified oligonucleotide splints and RNase H.

To cleave RNA molecules using RNase H in a site-specific manner, a short deoxyoligonucleotide (3-5mer) joining with 2'-O-methyl oligonucleotide(s) was designed as a DNA splint to be used. Model experiments were carried out using ribooligonucleotide substrates (9 and 18 mer). It was found that the use of this type of splints (9 mer) causes a unique cleavage by RNase H. For example, when 3'm (GA)d(AGAA)m(GGU)5' was used as a hybridization strand, 32pUCUUUCUUCUUCCAGGAU was cleaved specifically between U11 and C12 to yield 32pUCUUUCUUCUU. This method will have a variety of applications for the study of RNA.

[Stenosis of biliobilary anastomoses and its prevention by ring splinting outside the lumen--results of an animal experiment study].

The causes of the stenosing and their prevention were investigated by means of a ring splinting outside of the lumen in 19 dogs. The dogs were relaparotomised 3, 6, and 12 months after division and reanastomosis of the supra-duodenal choledochus. In contrast to the therapy group (n = 6) the control group shows a statistically significant stenosis of the anastomosis and a dilatation of the proximal bile passages. The stenosis of the anastomosis is caused by a circumscribed cicatricial contraction. This can be held up effectively by a ring splinting outside of the lumen.

Bone scanning in the adductor insertion avulsion syndrome ("thigh splints").

Shin splints is a defined clinical entity resulting from extreme tension on muscles inserting on the tibia, resulting in periosteal elevation which is detectable by bone scanning. The clinical equivalent in the thigh has been described. We found scintigraphic changes in the femurs of seven short, female, basic trainees at the Fort Dix Army base, most of whom were referred for stress fractures elsewhere in the lower extremities. The scan findings were generally noted in the upper or mid femurs, always involved the anteromedial cortex, and were bilateral in five of the seven subjects. The abnormalities were linear and suggested periosteal elevation, and did not have the typical appearance of stress fracture. Since the findings correspond to the insertion of one or more adductor muscle groups, the descriptive term "adductor insertion avulsion syndrome" or "thigh splints" is proposed for this entity.

The specific scintigraphic pattern of "shin splints in the lower leg": concise communication.

The clinical entity, "shin splints," is now being recognized, and more specifically characterized by the findings of exercise-induced pain and tenderness to palpation along the posterior medial border of the tibia. In this prospective study, ten patients with this syndrome were evaluated using three-phase bone scintigrams, and a specific scintigraphic pattern was determined. Radionuclide angiograms and blood-pool images were all normal. On delayed images, tibial lesions involved the posterior cortex, were longitudinally oriented, were long, involving one third of the length of the bone, and often showed varying tracer uptake along that length. Obtaining both lateral and medial views was crucial. The location of activity suggested that this entity is related to the soleus muscle. These scintigraphic findings can be used to differentiate shin splints from stress fractures or other conditions causing pain in the lower leg in athletes.

[Internal intestinal splinting with a soluble tube for the prevention of recurrent adhesion ileus].

An absorbable, non-toxic, easy gliding, cross-linked gelatine tube has been developed to prevent and to treat chronical adhesion ileus. The tube is patterned upon the interior intestinal splinting with long tubes. By doing a small enterotomy it was possible to splint the whole intestine in pigs or endangered parts within a few minutes. The flexible gelatine tube does not kink and is melting off from the surface. It prevents the intestine from kinking and does not cause any lesions even on previously damaged intestinal walls. Possible complications seen by insertion and extraction of conventional tubes were not observed.

Microsurgical tubal reanastomosis--the role of splints.

Tubal reanastomosis was performed in 32 patients using microsurgical techniques. In 25 women, occlusion was due to prior sterilization. A splint was not used during or after repair. Intrauterine pregnancy occurred in 21 (84%) and fetal loss in 6% of 17 potentially viable pregnancies. The mean midluteal serum progesterone level of previously sterilized patients (13.4 +/- 1.0 ng/ml) was similar to the level in normal partners of infertile men (12.9 +/- 1.5 ng/ml) and in women with other tubal causes of infertility (14.3 +/- 1.1. ng/ml). Seven patients with occlusion due to disease achieved postoperative patency; there were 4 (57%) intrauterine pregnancies and 1 tubal gestation. The author concludes that a splint is not necessary in performing tubal reanastomosis and that there is no clinical or laboratory evidence of luteal insufficiency in this group of previously sterilized women.

[A new motorized dynamic splint for the hip joint].

The authors present a new mechanical exercise splint for the hip. The appliance carries out movements in three dimensions and is used early in the postoperative period following surgical treatment of the hip and surrounding structures for continuous passive motion (CPM) treatment. When the hip is flexed, the knee bends at the same time. It is possible, however, for the part of the leg cradle that holds the lower leg to be locked in the extended position to avoid any problems for patients with arthrodesis in the knee. It is also advisable for knee movement to be blocked when the patient does exercises involving spreading and closing of the legs and inward and outward rotation. In this case the leg concerned should be bent about 10 degrees at the hip to avoid friction losses against sheets and mattress. The main indications for use of the new splint are internal fixation following acetabular fractures, periacetabular fractures, dislocation, joint fractures with dislocation of the same joint and fractures or pseudarthrosis at the upper end of the femur. Other appropriate forms of treatment are synovectomy, surgery to improve the form of the cup, including pelvic osteotomy, all kinds of transformation osteotomy, especially for hip necrosis or osteochondrosis dissecans, soft tissue surgery such as capsulotomy, tenotomy or resection following ectopic bone formation and flaccid paralysis.(ABSTRACT TRUNCATED AT 250 WORDS)

Compartment syndrome and shin splints of the lower leg.

OBJECTIVES: The objective of this article is to review and categorize the current knowledge on compartment syndromes (CS) and shin splints (SS), with specific importance relegated to the diagnosis, differential diagnosis and management of these conditions. DATA SOURCES: The bibliographic data sources reviewed are limited to the English language and human content and are from medical and scientific journals, as well as chiropractic and medical texts. A mini-Medline version of Index Medicus was utilized. Terms for indexing included compartment syndromes, shin splints and stress fractures. The bibliographies of the journals selected were then evaluated and, where appropriate, the specific journal or text references regarding diagnosis and management were then reviewed. This information was then included in this article, where useful, to further clarify or reference statements made. CONCLUSION: Differential diagnosis of the acute CS from chronic CS and SS requires clinical methods and imaging procedures. The pathogenesis of the acute CS of the lower leg is associated with external pressure or internal hemorrhage. If the tissue pressure were to rise above 30-40 mm Hg for 4-12 h, irreversible muscular damage would result. Emergency surgical intervention is the only appropriate form of treatment in acute CS. In chronic CS, where elevated pressures exist on a transient basis, influenced by activity, conservative management procedures are felt to be effective. However, if these methods are not helpful, surgical intervention may be necessary. The etiology of pain associated with SS is not associated with compartmental pressure elevations, but rather, results from periostitis occurring along the tibia caused by muscular and tendinous strain associated with inflammation. Conservative management is most appropriate for this disorder, with surgical intervention being an uncommon treatment approach. Although clinical findings are useful in the diagnosis of these disorders, fluid pressure findings may be necessary to fully differentiate acute CS from other disorders. Up until recently, common methods of obtaining pressure measurements of compartments included the use of a needle manometer. More recently, however, a hand-held miniature fluid pressure monitor has been developed that produces reproducible measurements of interstitial fluid, making testing potentially practical for the clinician.

[In vitro perfusion of so-called endoprothesis. An experimental study of incrustations on internal splinting plastic tubes (author's transl)].

We have used the method of internal splinting of the ureter (endoprothesis) for the past ten years. Because we observed partial obstruction of the lumen by incrustations and/or fibrin coagula in cases in which the splints had been in place for long periods, we conducted in vitro experiments into the problem. Under constant and variable conditions, urine was perfused through plastic tubes of different materials. Organic and inorganic urine substances and urine pH were analyzed. The materials used were examined with special scanning electron microscopic techniques, and various depositions, especially in comparisons of in vivo and in vitro conditions, were found.

[Inner pyelovesical splinting in pyeloplasties (author's transl)].

A total of 112 cases of pyeloplasty were analyzed and the advantages of inner pyelovesical splinting as opposed to external drainage are outlined. Long-term results especially favor the former procedure. Scanning electron microscopic studies were done on the splinting material.

A temporary-permanent splint.

A method of temporary permanent splinting has been presented which enables the clinician to stabilize anterior teeth aesthetically and rigidly without resorting to cast restorations. The method utilizes an acid-etch composite technique with T.M.S. Pins and has shown itself to be a rapid, cosmetic and durable alternative to either the temporary intra-coronal wire ligation or the more permanent gold prosthesis.

Management of auricular hematoma using a thermoplastic splint.

Several methods have been described in the literature for the management of acute auricular hematoma. Herein, we review the pathogenesis of auricular hematoma, describe the various treatment modalities, and report a case in which a thermoplastic splint was used to prevent recurrence of hematoma in a patient with immune thrombocytopenic purpura.

Perthes' disease: a comparison between two methods of treatment, Thomas' splint and femoral osteotomy.

The results of twenty-four hips treated by femoral derotation varus osteotomy and sixty-six hips treated by Thomas' splint are compared. The average follow-up period was eight years with in a range of two to twenty years. All diagnoses were classified into Catterall's four groups according to the degree of epiphyseal alterations. Other factors affecting the outcome, such as subluxation of the femoral head, age at onset and stage at treatment were studied. Osteotomy improved the results remarkably in Group 3 and Group 4 hips. Femoral osteotomy would be indicated for those cases which show extended epiphyseal necrosis and subluxation of the femoral head. Even osteotomy has limitations in obtaining good results in older children with extended necrosis and in an advanced stage in which the femoral head is already deformed and regenerated.

[Occlusal splint therapy in reciprocal TMJ clicking. A critical observation within a follow-up study].

During post-treatment examination the effect of splint therapy on 14 temporomandibular joints (TMJ) with anterior disk displacement were examined using clinical and instrumental findings and magnet resonance imaging (MRI) to judge the therapeutical success of the treatment from an orthodontic perspective. A stable reposition of the TMJ disk was achieved in four joints, which showed no relapse over a two-year period of observation. The success of the therapy was dependent on the extent of the tissue damage to the TMJ and also on patient compliance. The MRI yielded therapeutically valuable information, which is not revealed in the clinical-instrumental analysis, on the tissue structure of the disk and the ligaments. In certain cases the use of MRI is necessary to confirm the clinical diagnosis.

Correction of prominent ears with a new splinting technique.

Correction of prominent ears represents one of the main applications in plastic surgery. Apart from its cosmetic nature, this operation is important because of the psychological distress that the deformity causes patients. In this study, 40 patients who underwent treatment for prominent ears in our department were evaluated. The classical surgical technique described by Chongchet followed by a new splinting procedure was performed. The advantages of this technique are described in comparison with traditional methods.
