Telemetric monitoring of intracavernous pressure in freely moving rats during copulation.

Changes in intracavernous pressure (ICP) were recorded in freely moving rats by means of a catheter implanted in the corpus cavernosum and connected to a pressure transducer located subcutaneously. Intracavernous pressure changes and copulatory events (mounts, intromissions and ejaculations) were recorded during several overnight testing sessions with 12 rats in the presence of receptive females. Video recordings of sexual behavior in the implanted and nonimplanted rats revealed that the ICP device did not impair copulatory behavior. Comparison between copulatory behavior and ICP changes revealed that ICP profiles could identify the number of intromissions and ejaculations as well as the timing of these events. The results showed that during mounts, intromissions and ejaculations, ICP increases were 36.9 mm. Hg, 62 mm. Hg and 106.3 mm. Hg in 4 rats. The increases in ICP during mounts and intromissions appeared higher in sexually experienced as compared with sexually naive rats. These results demonstrate the reliability of the telemetric measurement of ICP in freely moving rats under natural physiological conditions. The model provides quantitative data for research on the neurophysiology of penile erection and evaluation of oral treatments for impotence.

Efficacy of an epidural combination of morphine and detomidine in alleviating experimentally induced hindlimb lameness in horses.

Amphotericin B-induced synovitis of the left tarsocrural joint was used to create a grade 3 of 4 lameness in 11 horses. Caudal epidural catheters were placed and advanced to the lumbosacral region. Baseline heart and respiratory rates were recorded and horses were videotaped at a walk and trot. Morphine sulphate (0.2 mg/kg) and detomidine hydrochloride (30 micrograms/kg) were administered to treated horses (n = 8) through the epidural catheter; an equivalent volume of physiologic saline solution was administered to control horses (n = 3) through the catheter. At hourly intervals after epidural injection for a total of 6 hours, heart and respiratory rates were recorded, and horses were videotaped walking and trotting. At the end of the observation period, video recordings were scrambled onto a master videotape. Lamenesses were scored by three investigators unaware of group assignment or treatment time. Lameness scores, heart rates, and respiratory rates were compared between groups using repeated measures analysis of variance. There was a significant decrease in lameness score after treatment with epidural morphine and detomidine (P = .0003); average lameness scores of treated horses were less than grade 1 at each hourly observation for 6 hours after drug administration. Early in the observation period, heart rates significantly increased in control horses and decreased in treated horses (P = .03). A similar trend occurred for respiratory rates (P = .07). Results of this study demonstrate that epidural administration of a combination of morphine and detomidine is capable of providing profound hindlimb analgesia in horses.

Correlates of gay and lesbian couples' relationship satisfaction and relationship dissolution.

A sample of committed gay and lesbian cohabiting couples engaged in two conversations after being apart for at least 8 hours: (a) an events of the day conversation and (b) a conflict resolution conversation. Physiological data were collected during the conversations and a videotape record was made. Couples viewed the videotapes and rated their affect during the interaction. The video records were coded with a system that categorized specific affects displayed. Models derived from physiology, from the perception of interaction, and from specific affective behavior were related to relationship satisfaction, and to the prediction of relationship dissolution over a 12-year period. Results supported previous findings that satisfaction and stability in gay and lesbian relationships are related to similar emotional qualities as in heterosexual relationships.

Effects of an Internet-based voucher reinforcement program for smoking abstinence: a feasibility study.

The present study tested the feasibility of an Internet-based method to obtain objective evidence of smoking abstinence and to deliver vouchers for evidence of abstinence. Four heavy smokers participated in this 4-week study. Twice daily, participants made video recordings of themselves providing a breath carbon monoxide (CO) sample with a Web camera. The video was sent electronically to the smoking clinic. Participants could earn vouchers for gradual reductions in breath CO during an initial shaping condition, and then for achieving abstinence (CO < or = 4 ppm). Vouchers could be exchanged for merchandise at select Internet vendors. Relative to baseline conditions, participants substantially reduced their smoke intake, and 3 achieved sustained periods of abstinence. The study suggests that an Internet-based voucher reinforcement program is a feasible method to promote abstinence from cigarette smoking.

Anxiety in self-confrontation: detection and measurement.

This paper summarizes an attempt to measure and detect "anxiety" or "arousal" in self-confrontation in a university teaching methods training programme. Self-confrontation arousal is caused by viewing CCTV recordings of short micro-lectures. Arousal detection is important as the use of video-recordings is an important element in the training provision made for new members of the teaching staff. Using ECG detection the heart rate of staff viewing themselves teach was found to increase on initial confrontation, but to decrease as the initial cosmetic effect became more acceptable. The paper suggests that ECG detection and measurement is a suitable tool for investigating confrontation in training programmes that use CCTV recordings.

The think aloud method: a guide to user interface design.

OBJECTIVES: Clinical use of computer systems has been hampered by their poorly designed user interfaces. The objective of this study was to design a user interface for a pediatric oncologists' computerized patient record with great consideration of their working behavior and of human computer interfacing principles so as to contribute to oncologists' efficiency and satisfaction in interaction with the system. METHODS: The think aloud method was used in combination with video recording to get a deep understanding of the way in which four pediatric oncologists searched through the paper-based patient record in preparing a patient visit. Protocol and video analyses was used to develop a cognitive task model reflecting pediatric oncologists' task behavior. This model was input for a prototype user interface, which was subsequently evaluated by eight other pediatric oncologists. RESULTS: The resulting computerized medical record system proved to meet pediatric oncologists' information needs and task behavior patterns. The design of the user interface minimized pediatric oncologists' work load and was highly efficient in supporting the pediatric oncologists in preparing their patient visits. The pediatric oncologists were very much satisfied with the computer system. CONCLUSIONS: It is argued that early involvement of cognitive engineering methods in the system design process may be of great help in designing systems that fully support health care professionals' work practices. The think aloud method, if applied under prescribed conditions, is a valuable information source of human task-behavior and as such a useful technique for requirements analysis in designing clinical computer systems.

Epithelial healing rates with topical ciprofloxacin, ofloxacin, and ofloxacin with artificial tears after photorefractive keratectomy.

PURPOSE: To determine whether corneal epithelial healing differs after the use of topical ciprofloxacin alone, topical ofloxacin alone, or topical ofloxacin with artificial tears in patients having photorefractive keratectomy (PRK). SETTING: Department of Ophthalmology and Visual Science, The University of Texas Health Science Center at Houston, Houston, Texas, USA. METHODS: Eighteen patients (6 women, 12 men) with moderate myopia (-1.50 to -6.00 diopters [D]) had standardized PRK. Patient age ranged from 25 to 62 years. The 28 eyes (16 right, 12 left) were randomized into 3 treatment groups: ofloxacin alone, n = 9 eyes; ciprofloxacin, n = 9 eyes; and ofloxacin with Refresh Plus, n = 10 eyes. The drugs were administered immediately after surgery and then every 6 hours. Video recordings of the corneal wounds stained with fluorescein were performed at 8:00 AM and 4:00 PM using a video slitlamp camera with a cobalt-blue light until the wound completely healed. The videotaped images were recorded and analyzed by a computer planimetry program. Wound areas were recorded and compared among the 3 drugs. The square-root transformation was applied to the wound area to obtain a constant healing rate. Statistical comparisons were analyzed using an analysis of variance test. RESULTS: Mean recovery time was 82.67 hours +/- 14.42 (SD) in the ofloxacin eyes, 120.89 +/- 34.05 hours in the ciprofloxacin eyes, and 76.80 +/- 19.30 hours in the ofloxacin with Refresh Plus eyes. Mean healing rate was 0.66 +/- 0.17 hours, 0.54 +/- 0.16 hours, and 0.67 +/- 0.15 hours, respectively. The healing rate was significantly higher in the ofloxacin with Refresh Plus eyes than in the ciprofloxacin eyes (P < .0001). There was no significant difference between the ofloxacin eyes and the ofloxacin with Refresh Plus eyes (P = .42). CONCLUSION: Ofloxacin with Refresh Plus and ofloxacin alone had a more positive effect on epithelial healing than ciprofloxacin. The ciprofloxacin eyes were significantly more prone to impaired or delayed wound healing and to the development of corneal haze.

Neuroethology application for the study of human temporal lobe epilepsy: from basic to applied sciences.

The aim of this investigation was to apply neuroethology to the study of human temporal lobe epilepsy (TLE). For this purpose, 42 seizures in 7 patients recorded during video/EEG monitoring (1997-1998) were analyzed by means of a behavioral glossary containing all behaviors. Video recordings were reobserved, and all patients' behaviors were annotated second-by-second. Data were analyzed using Ethomatic software and displayed as flowcharts including frequency, mean duration, and sequential statistic interaction of behavioral items (chi2 > or = 10.827, P<0.001). Flowcharts of (1) a group of seizures from a single patient, (2) the sum of four seizures per patient of two patients with right and five patients with left TLE, and (3) the comparison of left versus right TLE are shown. Well-established data in the literature were confirmed, such as aura (especially epigastric), contralateral lateralization value of dystonia and version, consciousness and language alterations in ictal and postictal periods, mostly with respect to dominant hemisphere involvement, among others. Less well established data such as awakening seizures in TLE patients, lateralization value of facial wiping (ipsilateral to the focus), statistically significant associations between behavioral pairs (dyads), and new behavioral sequences in TLE were also observed. We suggest that neuroethology also has great potential in the study of human epilepsy semiology. This work had an important role in method standardization for human epilepsy, setting the basis for the development of future clinical studies including correlation with other diagnostic methods (EEG, magnetic resonance, and SPECT). The next step will be the comparative study of seizures of patients with left and right TLE, with a greater number of patients, and the development of a digital video library.

Is pseudosulcus alone a reliable sign of gastroesophago-pharyngeal reflux?

To investigate if laryngeal pseudosulcus (bilateral infraglottic oedema) is a reliable sign of pharyngeal reflux, the video recordings of 59 patients and 18 controls were examined off-line by two phoniatricians. Eleven video segments were duplicated for calculation of intrarater reliability. The frequency of pseudosulcus was correlated to the results of 24-h double-probe pH monitoring. The intrarater reliability was moderate (K = 0.582; P = 0.005). The sensitivity of pseudosulcus in the diagnosis of pharyngeal reflux among the patients was 30% and among the controls it was 0%. The positive predictive value (PPV) of pseudosulcus for patients with pharyngeal and laryngeal symptoms was 67%, while for the controls it was 0%. The negative predictive value (NPV) of pseudosulcus for patients with pharyngeal or laryngeal symptoms was 32% and for the controls it was 64%. The single finding of pseudosulcus is not a very sensitive predictor of pharyngeal reflux but if the patient has laryngeal symptoms and a pseudsulcus, the probability that the patient has pharyngeal reflux is almost 70%.

Effect of vagal nerve stimulation in a case of Tourette's syndrome and complex partial epilepsy.

We report on a 30-year-old man with Tourette's syndrome (TS) and medication-refractory epilepsy whose tics improved after implantation of a vagal nerve stimulator (VNS). To verify the patient's observation, we performed a blinded video assessment using the modified Rush video-based tic rating scale. The patient underwent two separate video recordings (VNS on and VNS off). A rater, blinded to patient's VNS status, evaluated the videos with the modified Rush video-based tic rating scale. There were improvements in total tic score and motor and phonic tic frequency. If verified by controlled clinical trials, this observation may provide insights into the pathophysiology of tics and may lead to a novel therapy for patients with severe TS.

Evidence for aggression-modulating pheromones in prepuberal pigs.

A series of behavioral bioassays were conducted to determine the aggression-influencing properties of urine and other fluids. Subjects were prepuberal castrated male and female domestic pigs from commercial stocks. In the behavior assay, pigs were painted with a test fluid and grouped for a videotaped 90 min observation period. Experiment 1 validated use of videotape recording by showing that duration of aggressive behavior registered live was correlated with that obtained from video records (R = .98). In experiment 2, urine and plasma collected from actively aggressive pigs reduced the durations of aggressive behavior of test pigs compared with the effects of urine and plasma collected from socially stable, handled pigs. In Experiment 3, a new set of test pigs confirmed that urine from fighting pigs reduced the duration of attack by test pigs compared with urine from nonfighting, handled pigs. In addition, the suggested reproductive pheromone, 5 alpha-androst-16-en-3-one, substantially reduced the duration of attack. The effects of gender and aggressive state of urine-donor pigs on test pigs was determined in Experiment 4. Again, urine from castrated male and female aggressive pigs reduced attack by test pigs compared with the level of attack shown by test pigs coated with urine from handled castrated males and females. Urine from fighting and nonfighting intact males had similar effects on test pig aggression. In Experiment 5, urine was obtained from nonhandled, socially stable pigs in their home pens and again from the same pigs after they had been regrouped (aggressive). These urine types had no significant influence on test pigs' aggression over the entire 90-min observation. However, during the first 30 min nonhandled, nonfighting pigs' urine induced less aggression in test pigs than did urine from fighting pigs. Results indicate that urine and blood plasma from aggressive pigs reduces aggression by test pigs compared with the effects of urine from handled pigs. Handling may increase the aggression-promoting properties of urine, and aggression may inhibit this aggression-promoting property.

The schedule for the assessment of drug-induced movement disorders (SADIMoD): inter-rater reliability and construct validity.

The Schedule for the Assessment of Drug-Induced Movement Disorders (SADIMoD) is a newly developed instrument, consisting of a compilation of rating scales, to measure the severity of drug-induced movement disorders: dystonia, dyskinesia, Parkinsonism, akathisia, ataxia, and several types of tremors. The inter-rater reliability and the construct validity of this scale were investigated. Six investigator teams were trained by means of a standard package of instruction material to such an extent that a single member of the team could represent the entire team. Thirty-one patients [20 male/11 female; 57.1+/-6.5 yr (mean+/-S.D.)] with a variety of movement disorders were recorded on videotape according to the SADIMoD Schedule. Single representatives of all six teams scored these video recordings. To this set the existing SADIMoD ratings of 82 patients were added to form the so-called 'total data set'. These patients were examined by 6 different researchers, who rated 4, 8, 10, 14, 18 and 28 patients, respectively, mostly in the context of a research protocol. A specific subset consisted of 12 patients that were examined three times with a two-weekly interval without any change of their medical condition or treatment. The 6 ratings of the 31 individual patients correlated to a highly significant degree, with Kendall's Coefficients of Concordance of 0.436 to 0.891 (median 0.717). The same was true for the 6 ratings of the 7 SADIMoD subscales (median 0.578, range 0.462-0.715) Considering the total data set, the homogeneity of the various subscales was good (Cronbach's alpha = 0.81-0.94, median: 0.87). The SADIMoD dyskinesia and dystonia subscales showed a highly significant mutual correlation. The Parkinsonism subscale correlated highly significantly with the rest and postural tremor subscales and to a lesser extent with the akathisia and ataxia subscales. An analysis of variance showed that the three ratings in the subset of 12 patients were not significantly different for any scale. Also Scheffé tests for homogenous subsets did not reveal any significant differences. When investigated under 'real world' circumstances, the inter-rater reliability of the SADIMoD was found to be satisfying. The instruction material, that was developed and used in this study, fully comes up to the requirements. The construct validity of the SADIMoD is more than sufficient.

Validity of the detection of wheelchair propulsion as measured with an Activity Monitor in patients with spinal cord injury.

STUDY DESIGN: Validation study. OBJECTIVES: An accelerometry-based Activity Monitor (AM) has proven to be a valid instrument to quantify mobility-related activities (lying, sitting, standing, walking, cycling, general (noncyclic) movement). The aim of this study was to assess whether, additional to other activities, wheelchair propulsion (hand-rim wheelchair propulsion and handbiking) can be validly detected by the AM in patients with spinal cord injury (SCI). SETTING: Rehabilitation center. METHODS: In all, 10 patients with SCI (aged 19-63 years; five patients with poor triceps strength and five patients with good triceps strength) participated. Patients performed a series of representative daily life activities (involving wheelchair propulsion and nonwheelchair propulsion activities), according to a standard protocol, in a seminatural setting. Continuous registrations of signals from body-fixed accelerometers were made and the AM output (after automatic analysis) was compared with visual analysis of simultaneously made video recordings (reference method). Validity scores (agreement, sensitivity, specificity) between the output of the AM and the video analysis were calculated. RESULTS: Agreement, sensitivity and specificity for the detection of wheelchair propulsion were overall 92 (range, 87-96)%, 87 (76-99)% and 92 (85-98)%, respectively. Sensitivity was smaller in patients with poor triceps strength compared to patients with good triceps strength; 81 (76-89)% and 95 (89-99)%, respectively (P<0.01). Mean overestimation in duration of wheelchair propulsion by the AM was 3.9% (P<0.05). CONCLUSION: Besides already validated other activities, wheelchair propulsion (hand-rim wheelchair propulsion and handbiking) can be validly detected by the AM in patients with SCI, both with good and poor triceps strength. Therefore, the AM offers the possibility to obtain objective and detailed information on all major mobility-related activities performed by patients with SCI. Analysis of the characteristics of competitive badminton.

OBJECTIVE: To describe the characteristics of badminton in order to determine the energy requirements, temporal structure, and movements in the game that indicate performance level. To use the findings to plan training with greater precision. METHODS: Eleven badminton players (mean (SD) age 21.8 (3.26) years) with international experience from four different countries (France, Italy, Spain, and Portugal) were studied. Two of the Spanish players were monitored in several matches, giving a total of 14 samples, all during the 1999 Spanish International Tournament. Blood lactate concentration was measured with a reflective photometer. Maximum and average heart rates were recorded with a heart rate monitor. Temporal structure and actions during the matches were determined from video recordings. All variables were measured during and after the game and later analysed using a descriptive study. RESULTS: The results confirmed the high demands of the sport, with a maximum heart rate of 190.5 beats/min and an average of 173.5 beats/min during matches over 28 minutes long and performance intervals of 6.4 seconds and rest time of 12.9 seconds between exchanges. CONCLUSIONS: The results suggest that badminton is characterised by repetitive efforts of alactic nature and great intensity which are continuously performed throughout the match. An awareness of these characteristics, together with data on the correlations between certain actions such as unforced errors and winning shots and the final result of the match, will aid in more appropriate planning and monitoring of specific training.

The effect of towing a range of relative resistances on sprint performance.

The aim of this study was to compare sprint performance over 10 and 20 m when participants ran while towing resistances, weighing between 0 and 30% of body mass. The sample of 33 participants consisted of male rugby and soccer players (age 21.1 +/- 1.8 years, body mass 83.6 +/- 13.1 kg, height 1.82 +/- 0.1 m; mean +/- s). Each participant performed two sets of seven sprints over 20 m using a Latin rectangular design. The times were recorded at 10 and 20 m using electronic speed gates. The sprints of 13 players were video-recorded to allow calculation of stride length and frequency. For both sprints, a quadratic relationship was observed between sprint time and resistance as sprint time increased from 2.94 s to 3.80 s from 0 to 30% resistance. This relationship was statistically significant but considered not to be meaningful for performance because, over the range of resistances used in this study, the quadratic model was never more than 1% (in terms of sprint time) from the linear model. As resistance increased, the stride length shortened, with mean values of 1.63 +/- 0.13 m at 0% body mass and 1.33 +/- 0.13 m at 30% of body mass. There was no significant change in stride frequency with increasing resistance. The results show that in general there is an increase in sprint time with an increase in resistance. No particular resistance in the range tested (0 - 30%) can be recommended for practice.

Description, validation, and performance characteristics of a new computer-automated sperm motility analysis system.

A computer-automated sperm motility assay (CASMA) system has been developed that provides a rapid and accurate analysis of multiple sperm movement parameters and a new measure of linearity, the linear deviation angle. CASMA provides objective, unbiased sampling and accurate quantitation of the movement characteristics of 200 sperm cells in 20 min. It consists of a microscope-mounted video camera, a high-resolution video disk recorder, a video digitizer/memory board mounted in an IBM 9000 microcomputer, and newly developed software. After manual recording, at 60 frames/s, of the video sequences (takes) of sperm suspensions, each take is automatically played back frame by frame, digitized, and stored in video memory. The software searches each frame, recognizes sperm cells, randomly selects a preset number for analysis, and traces each cell through the sequence to generate sperm "tracks" that are then stored in disk memory. This process is repeated for each take. Analysis of the stored tracks of each take yields the mean +/- SEM of the standard sperm motility parameters: percent motile (%M), curvilinear velocity (VC), net velocity (Vn), position-averaged velocity (Va), linear index (Vn/Va), progressiveness ration (Vn/Vc), and curvilinear progressiveness ratio (Va/Vc). Additionally, CASMA allows measurement of the linear deviation angle, a more direct measure of the linearity of sperm movement. For statistical comparisons, multiple takes can be considered either as replicates or separate experimental determinations. Finally, for more detailed analysis, each individually stored track, with its associated parameters, or histogram distributions of all sperm for each parameter can be displayed and printed. The performance of CASMA was evaluated by comparison of CASMA-determined movement parameters with manually determined values derived from the same sperm cells in the same video sequence and by comparison with published values determined using microcinematographic techniques. In each case, the CASMA values were essentially identical to those determined by manual measurements. Finally, CASMA accurately quantitates the linearity of sperm movement, a characteristic previously determined only by much more time-consuming methods. CASMA is a rapid and accurate system for measuring washed bull sperm motility and has reliably analyzed monkey and elephant sperm. The system has the potential to quantitate motility equally well with sperm from any species that have similar sperm head size.

The use of video in anesthesia record keeping.

Although formal comparisons of record keeping systems are difficult, the authors are persuaded that a video system provides substantial advantages over AARK in several (though not all) of its aims and at a substantially reduced cost. The video system might be viewed as a parallel rather than a replacement system in some cases. The authors have been struck by certain capabilities such a video system offers. Not the least of these is the advantage of having a multiple OR monitoring system for use by the floor supervisor to check all rooms easily. The implications of a video technology in clinical research also are important. At present, a clinical trial involving 40 cases is being organized and will require that physiologic parameters be recorded in detail. Plans to use the ubiquitous multichannel strip recorder for logging the parameters have been changed. Video will be used instead, since it is more than adequate to meet most research needs of the department and greatly simplifies the recording of such data at reduced cost.
