In vivo activated cytotoxic T cells in the thyroid infiltrate of patients with Hashimoto's thyroiditis.

High proportions of T8+ cells with inverted T4/T8 ratio were found in freshly isolated thyroid lymphocytes from patients with Hashimoto's thyroiditis. In addition, about one third of thyroid infiltrating cells expressed the TAC antigen, whereas in patient peripheral blood (PB) or normal lymphocytes from PB or lymphoid organs the percentage of TAC-positive cells was consistently lower than 10%. Following negative selection with OKT4 or OKT8 monoclonal antibodies and complement, TAC+ T cells were enriched in the T8+ cell population. Thyroid infiltrating T cells from two patients underwent two different cloning procedures. In the first, single T cells were initially activated with phytohaemagglutinin (PHA) and interleukin 2 (IL-2), in the other with recombinant IL-2 (rIL-2) alone. The majority of T cell clones obtained by initial PHA-stimulation (55-65%) had the T8+ phenotype, but the frequency of T8+ clones obtained by stimulating T cells with rIL-2 alone was even higher (78 & 71%, respectively). The majority of T8+ clones elicited by PHA (35/37 & 36/38) and all the T8+ clones (36/36 & 22/22) obtained from thyroid infiltrates with initial stimulation by rIL-2 displayed cytolytic activity. Most of cytolytic T8+ clones obtained from thyroid infiltrates with both cloning procedures, displayed NK activity against human K562 and MOLT-4 target cells, but not against a NK-resistant target, such as Raji cells. These data suggest that in Hashimoto's disease a considerable proportion of thyroid infiltrating T cells are in vivo activated T8+ cytolytic T cells with NK activity, which may be of importance in determining or maintaining the tissue damage of the target gland.

Blood and thyroid-infiltrating lymphocyte subclasses in juvenile autoimmune thyroiditis.

We have studied the distribution of T and B lymphocytes in the blood and in the thyroid lymphocytic infiltrates (obtained by fine needle aspiration biopsy) in sixteen patients with juvenile autoimmune thyroiditis (JAIT). The same cell populations were also tested for cell-mediated immunity (CMI) to thyroid antigen in the leucocyte migration test (LMT). The relative and absolute numbers of blood T lymphocytes were normal (71-76%), as were the numbers of blood B lymphocytes (19%). The thyroid infiltrate contained 48% B lymphocytes, whereas only 40-44% of the infiltrating lymphocytes were T cells. Half of the JAIT patients showed a positive CMI to thyroid antigen with blood leucocytes, but when thyroid-infiltrating lymphocytes of these patients were tested in the LMT, they were negative. Thus, in contrast to what is generally assumed, we were unable to demonstrate T cell-dominated lymphocytic infiltrates or the accumulation of specifically sensitized T lymphocytes within the thyroid gland in autoimmune thyroiditis.

[Immunophenotyping and morphometry of inflammatory infiltrates in variants of basalioma].

The inflammatory infiltrate of 5 histologically different types of basal cell carcinoma (BCC) (n = 22) was characterized by means of the alkaline phosphatase monoclonal antialkaline phosphatase technique (APAAP). We proved a distinct stromal pattern of infiltration, which was focally accentuated; in some cases, the cells infiltrated the tumor itself. On phenotyping these cells, we found clear predominance of T cells (75%), which mostly consisted of T helper/inducer cells (45%) and suppressor/cytotoxic cells (32%); in addition, we found B cells (6%) as well as monocytes and macrophages (15%). Morphea-like and adenoid types of BCC showed some tendency to a less pronounced infiltration. Chronic exposure to sunlight and ulceration did not significantly influence the quality or quantity of the inflammatory infiltrate--except for polymorphonuclear leucocytes and macrophages.

[The extent and pattern of infiltration along the bronchial wall in central type lung cancer].

Gross and microscopic study was made in fifty specimens emphasizing on the bronchial wall of lung cancer. The extent and pattern of infiltration along the proximal bronchial wall in the central type lung cancer were observed. The cancerous tissue extended proximally from less than 1.2 cm in the majority of cases, to a maximum of 1.6 cm from the cancer base. The lesions can be divided into three gross types: infiltrating, polypoid and nodular. In the infiltrating type, the cancerous tissue spread along the bronchial wall far and may involve multiple layers in the wall. In the polypoid type, the infiltration did not reach far as compared with the other types. None in the polypoid type was over 0.4 cm. The nodular type comprising 54% (27/50), was more common than the others. In order to improve the operative result, infiltrating pattern and distance along the bronchial wall are important references for resection of central type lung cancer.

[Significance of local tumor associated infiltration of mononuclear leukocytes for persistent tumor formations following transurethral resection (TUR) of bladder cancers].

In 140 patients with primarily curatively resected tumour of the urinary bladder the inclination to persistence was investigated in relation to the local peritumoural infiltration of leucocytes. 19% of the tumours with cellular infiltration and 33% of the tumours without cellular infiltration showed persistent tumour formations. Altogether 30 (21%) of the 140 tumours were persisting. The following parameters were found in persistent tumour formations: Multifocal tumours have a higher persistence rate than isolated primary tumours. With increasing degree of malignancy (G) the rate of persistence is elevating. It seems that in differentiated transurethral resection (separate resection of the exophyte, the tumour basis, resection of the marginal wall, if necessary, with second look operation) the local depth spreading of the tumour has no influence on the rate of persistence. Tumours without peritumoural infiltration with mononuclear leucocytes show a higher persistence rate than tumours with cellular infiltration, in which case this difference becomes even more significant with advanced tumour formation (T) and degree of malignancy (G).

[Do indications still exist for intra-arterial infiltrations in vascular pathology?].

Intra-arterial infiltrations are a useful therapeutic measure in various vascular diseases where it is important to obtain a lasting and rapid peripheral vasodilatation. In arteritis of the lower limbs, it is paradoxical to match infiltration against surgery. None of these two therapeutic measures can cure completely. Depending on the circumstances, infiltrations can be used at all the stages of arteritis, but the difficulties of applying them limit their use. In certain phlebitis mainly with arterial manifestations, infiltrations can relieve immediately the spasm. In trophic disorders of arterial or venous origin the help the formation of scars. Infiltrations should be used, however, only under maximal technical conditions, which render them harmless.

Plenary lecture: lymphocytic infiltration in relationship to urologic tumors.

In summary, it may be said that, to date, among urologic tumors, seminoma is the only one in which lymphocytic and/or plasmacytic infiltration is known to improve the prognosis, and the available information on tumors of bladder, kidney, and prostate do not yet permit any statement relative to prognostic significance of inflammatory cell infiltration. When studying the relationship of lymphocytic and/or plasmacytic infiltration to tumors, it is essential for one to distinguish between cases in which the infiltration is in intimate relationship to the tumor cells and/or where the tumor cells show definite degenerative changes, e.g., seminoma, from those cases in which the inflammation is present at some distance from the tumor, e.g., chronic prostatis, and from those in which the inflammation is preexisting, e.g., schistosomiasis. Claims made about the prognostic significance of such infiltration should have adequate controls.

Characterization of lymphocyte responsiveness in early experimental syphilis. II. Nature of cellular infiltration and Treponema pallidum distribution in testicular lesions.

The nature of the cellular infiltration and the distribution of Treponema pallidum during rabbit testicular infection were examined by immunofluorescence and light microscopy. Low numbers of treponemes are demonstrable in the perivascular regions on day 3 post-infection. On days 6, 10, and 13, large numbers of organisms are found in the interstitial spaces. The treponemes do not appear to invade the walls or lumina of the seminiferous tubules, although tubular atrophy is obvious. On days 17, 24 and 31, treponemes are no longer identifiable by immunofluorescence in infected testicles. The cellular infiltration, which is apparent on day 6, reaches its peak on day 13, corresponding to the amount of swelling observed grossly. The infiltrate is primarily lymphocytic, but macrophages are also observed during the peak cellular response. These cells are located, as are the treponemes, in the interstitial spaces. The lymphocytes are demonstrated by specific immunofluorescence to be predominantly T cells. The peak T cell infiltration in the infected testicle is followed rapidly by the disappearance of the organisms from that organ. Thus, it is postulated that infiltration by specifically sensitized T cells results in the clearance of large numbers of T. pallidum from infected tissues.

[Immunohistological studies on the identification of T cells and T cell subsets infiltrating in human stomach cancer tissues].

T cells and T cell subsets in human gastric cancer tissues were identified using the immunoperoxidase technique with anti-T cell monoclonal antibodies (Leu-1, Leu-2, Leu-3 and Leu-4), anti-HLA-DR(Ia) monocronal antibody, and anti-Ig sera. The majority of lymphocytes infiltrating into gastric cancer tissues were Leu-1 and Leu-4 positive T cells; they consisted of a Leu-2 positive T cell subset (Tc/s) and a Leu-3 positive T cell subset (Th/i). Leu-2 positive T cells infiltrated the gastric cancer tissues of 6 patients with a moderate or marked degree of lymphocyte infiltration; Leu-3 positive T cells infiltrated these tissues of 11 patients. These results show that the degree of T cell and T cell subset infiltration may effect the host-tumor relationship.

Mononuclear infiltrates in human intracranial tumors as a prognostic factor. Influence of preoperative steroid treatment. I. Glioblastoma.

One hundred ninety-nine glioblastomas were investigated for the presence of mononuclear infiltrates in the light of preoperative steroid treatment and in regard to their prognostic influence. Preoperative steroid treatment does not seem to severely influence the frequency of lymphocytic infiltration in the doses routinely administered by us. The presence of infiltrates corresponds with a better prognosis for the disease, and with a mean postoperative survival time of 8.1 months as compared to 5.6 months in the group without infiltrates, irrespective of adjuvant treatment. As a prognostic factor, infiltrates may be of relevance for a statistical evaluation, but not a reliable indicator in the individual case.

[Peritumoral infiltration of mononuclear leukocytes and clinical course in bladder cancer].

Peritumoral infiltration by mononuclear leucocytes was investigated in 27 recurrent-free cases of bladder cancer and also in 22 cases which had taken an unfavourable course. The investigation was undertaken in all cases after transurethral resection of the tumour and intravesical instillation of adriamycin or BCG. 67% of the recurrence free cases showed peritumoral infiltration by inflammatory cells, whilst the remaining 33% failed to show any such reaction. Of the patients in whom the disease was progressive only 5% showed peritumoral infiltration with mononuclear leucocytes, whereas 95% demonstrated no such cell infiltration. Of 19 patients with tumour-associated inflammatory cell infiltration 95% remained recurrent free, whereas of 30 patients lacking this response only 30% remained recurrence free.

[Relationship between T cell infiltration in breast cancer tissues and prognosis].

The relationship between the degree of T cell infiltration in cancer tissues and prognosis was studied in 26 patients with breast cancer. The degree of T cell infiltration was divided into 4 groups according to the number of infiltrating T cells. There was a good correlation between the degree of T cell infiltration and the survival period or disease-free period. It may be clinically useful to check the degree of T cell infiltration in cancer tissues for predicting the prognosis of patients with breast cancer.

[Dependence of mononuclear infiltration of the liver on hepatocyte proliferation].

Partial liver resection (two-thirds) performed in (CBA X C57BL)F1 mice 2-4 or 24 h after intravenous injection of 2 mg zymozan led to retardation of the formation of mononuclear infiltrates in the liver and the lung. In control sham-operated animals, the first signs of infiltration appeared on the 2nd, whereas in mice with partial liver resection ( PLR ) on the 5th day after zymozan -induced stimulation of liver macrophages. If mice underwent PLR 5 days after zymozan injection, the preformed mononuclear infiltrates experienced the reverse development. PLR did not abolish monocytosis which was observed after zymozan injection. On the other hand, when mice received hydrocortisone in a dose of 125 mg/kg, the zymozan -induced infiltration in the liver as well as monocytosis were blocked. It is assumed that depression of mononuclear infiltration in the liver and the lung after liver resection is linked with a specific effect of proliferating hepatocytes rather than with the stress-induced mobilization of glucocorticoids.

The effect of vasodilator prostaglandins on polymorphonuclear leukocyte infiltration and vascular injury.

Some severe acute inflammatory reactions are characterized by polymorphonuclear leukocyte (PMN) infiltration as well as vascular and tissue damage with hemorrhage. Two types of mediators that may be involved in such reactions are chemotactic factors and prostaglandins. The chemotactic factors can induce PMN infiltration, while some types of prostaglandins cause vasodilatation. We reported previously that injection of soluble, nonphagocytosable chemotactic stimuli, zymosan-activated plasma (ZAP), or C5a des Arg into rabbit skin induced PMN-dependent hemorrhage. Here we investigated whether prostaglandins may modulate the rate of PMN infiltration, measured with 51Cr-labeled leukocytes and the degree of hemorrhage, measured with 59Fe-labeled red cells. Prostaglandin (PG) E1 (0.5 microgram) or E2 (1 microgram) increased ZAP-induced PMN accumulation by 81% and hemorrhage by 400%. A similar potentiation by PGE2 was observed when submaximal concentrations of ZAP were injected. Prostaglandin F2 alpha had no such effect. These results indicate that the degree of PMN infiltration of the tissues may be one factor determining the severity of vascular damage. Furthermore, vasodilatory prostaglandins, generated during neutrophilic inflammatory reactions, may enhance chemotactic-factor-mediated PMN infiltration and increase the extent of vascular injury.

Relationship between natural killer cell activity and histological features of lymphocyte infiltration and partial regression of the primary tumor in melanoma patients.

A group of 194 melanoma patients with Stage I melanoma was studied in an attempt to correlate histological features of prognostic importance in malignant melanoma with the level of natural killer cell (NK) activity in blood mononuclear cells. Histological features examined were lymphocytic infiltrate of the tumor base, lymphocyte infiltration at the tumor margin, and evidence of partial regression in the tumor. NK activity against melanoma cells and non-melanoma cells was studied in the same patients before and after surgical removal of melanoma. Patients with high NK activity against melanoma and Chang cells had less lymphocyte infiltrate at the base of the tumor irrespective of its thickness than did those with low NK activity. Patients with low NK activity and thin tumors had more lymphocyte infiltration at the base of the tumor than did those with high NK activity. Similar results were obtained in certain subsets of cases with respect to lymphocytic infiltration at the tumor margins and to the presence of partial regression. Although the nature of the association between these histological features and NK activity in blood is unknown, it is suggested that previously unexplained associations between histological features and prognosis may be accounted for by their association with NK activity. If this proves to be correct, measurement of NK activity may be an important additional prognostic factor in patients with melanoma.

A retrospective histological study of 669 cases of primary cutaneous malignant melanoma in clinical stage I. 3. The relation between the tumour-associated lymphocyte infiltration and age and sex, tumour cell type, pigmentation, cellular atypia, mitotic count, depth of invasion, ulceration, tumour type and prognosis.

A selected series of 669 primary malignant melanoma of the skin, stage I, was studied. The series included 86 lentigo maligna melanomas, 259 superficial spreading malignant melanomas, 194 nodular malignant melanomas and 130 unclassifiable malignant melanomas. The adjacent lymphocyte infiltration was graded and its prognostic value and its relation to the sex and age of the patient, tumour cell type, pigmentation, cellular atypia, mitotic count, depth of dermal invasion, tumour type and ulceration was studied. There was no significant relationship between lymphocyte response and sex and age of the patient and the tumour cell type. There was a highly significant relationship between a dense lymphocyte infiltration and superficial tumour invasion as far as the papillary-reticular interface in contrast to the weak response associated with deeper invasion. When only tumours with invasion of the papillary-reticular interface were considered, there was no significant relationship between lymphocyte infiltration and pigmentation, cellular atypia, mitotic count, tumour type and ulceration. At the same level of invasion there was no difference in prognosis in relation to the density of lymphocyte infiltration. Nodular malignant melanomas surrounded by a dense lymphocyte infiltration had a significantly worse prognosis than was associated with a simular lymphocyte response against the two other types of melanoma.

[The significance of the lymphocytes and plasma cells infiltration in brain metastases (author's transl)].

The histological slides of 270 consecutive cases of brain metastases have been reviewed to investigate the incidence of lymphocytes and plasma cells infiltration. As shown in a previous study, the overall incidence of the infiltration was found to be significantly higher in metastases than in glioblastomas (respectively, 76 p. cent and 39 p. cent), and similar to, even though slightly less, than that observed in other malignancies elsewhere in the body. Moreover, the infiltration was generally more intense in brain metastases than in glioblastomas (respectively, 50 p. cent and 16 p. cent, of "definite" infiltration), and was characterised by the presence of more plasma cells. These figures once again support the view regarding the brain as a site only partially "privileged" from an immunological standpoint, due to the weakness of the afferent limb of the immunological response. On the other hand, the slight reduction of the infiltration in brain metastases as compared to primary tumors might well represent a host's immune depression during the process of metastasizing.

[Infiltrating T-cells in primary biliary cirrhosis recognize mitochondrial epitopes].

The primary biliary cirrhosis (PBC) is characterized by lymphoid infiltrates in the portal tracts of the liver and occurrence of antimitochondrial autoantibodies (AMA) in serum directed against the pyruvate dehydrogenase complex or other enzyme complexes that are located on the inner mitochondrial membranes. The destruction of the biliary tracts is thought to be mediated by autoreactive liver infiltrating T-cells. In this study we demonstrate the reactivity of peripheral and liver-infiltrating T-cells against a crude preparation of human liver subcellular fractions measured by the tritium-thymidine uptake. Peripheral blood mononuclear cells (PBMC) from 13 of 15 patients with PBC and from 3 of 9 patients with chronic autoimmune hepatitis recognized human liver mitoplasts, i.e. mitochondria depleted of their outer membranes. PBMC from patients with chronic viral hepatitis B and C or with extrahepatic cholestatic icterus and PBMC from healthy controls did not recognize this antigen. Monoclonal antibodies against HLA-class II molecules blocked this proliferative response. Clonal analysis of 115 liver-infiltrating T-cells derived from two diagnostic liver biopsies of patients with PBC revealed a predominance of activated CD4+CD8- T helper cells. Six CD4- positive T-cell clones were reactive to the HLM preparation when the antigen was presented by autologous B cell lines. MAb against HLA-class II or HLA-DR inhibited the response whereas mAb against HLA-DP did not. These clones did not respond to other subcellular fractions of human liver tissue. We conclude that among liver-infiltrating T-cells in PBC autoreactive T-cells exist that recognize some epitopes on the inner mitochondrial membranes.(ABSTRACT TRUNCATED AT 250 WORDS)
