Microvascular tissue transfer. Perioperative nursing considerations.

Microvascular tissue transfer is a growing subspecialty in reconstructive surgery. Its use is increasing, and it has been used to solve difficult wound problems. The use of microvascular surgery has placed new demands on the perioperative nurse to remain current on perioperative implications of this type of surgery. Operating room nurses must continue to expand their knowledge of this area through ongoing education from journal review, specialty seminars, and increased operative experiences.

From theory to perioperative practice with Parse.

The operating room nurse today is often thought of as a task skilled, highly technical, process oriented, and efficient member of the operating room team. Too often her/his nursing education and preparation in practice, theory and discipline are overlooked. Perioperative nursing as a specialty is an important, vital factor in the success of the planned surgical intervention. Perioperative nursing practice involves caring for the patient; it is patient-centered rather than task oriented. The RN combines both the physiologic and psychosocial aspects of nursing in delivery of care. This is reflective of the theoretical model used to plan effective nursing care. The RN interacting with the preoperative patient has a focus on being in true presence with the patient (Parse, 1987). The interrelationship sets in motion utilization of theory, reflecting nursing knowledge and practice.

The impact of preoperative assessment on intraoperative nurse performance.

A study was undertaken to test the feasibility and effectiveness of preoperative assessment by operating room nurses. The study addressed whether preoperative assessments influenced the circulator's intraoperative nursing care, what side benefits accrued, and what problems were generated. Nurse performance was evaluated in 100 cases. Although nurses who had performed preoperative assessments had higher total scores and mean, the difference was not statistically significant. Preoperative assessments could not be justified on the basis of improved circulator performance due to prior knowledge of the patient.
