[Human rights and procreation].

Assisted procreation raises several problems in relation to Human Rights. The first owes to gamete donation which introduces foreign genetic material into the family. In this respect, conflicting rights of the child, the parents and the donor, are diversely perceived in different cultures. Another difficulty concerns the status of the human embryo cultured in vitro. An analysis based on a relational definition of the human person indicates that the embryo cannot reasonably become endowed with legal rights, if it does not represent the project of having a child. Assisted procreation, moreover, raises the question of the right to parenthood, not only of sterile couples but also of female homosexuals and women without partners. It also reactivates the debate on Human Rights in relation to technocratic power.

[The use of clomiphene and menotropins as inducers of multiple follicular growth in a program of in vitro fertilization].

In our IVF & related techniques program, clomiphene 50 mg was used from days 2 to 6 of the cycle in 20 patients and 20 cycles. (17 IVF patients and 3 GIFT patients) menotropins were also used in 2 different schemes: a) 2 ampules in alternated days (12 cases) and b) one ampule daily (8 cases). The patients with the daily scheme required lesser total dose of menotropins and developed higher estradiol levels, although this had none statistic significance, and they also developed a greater number of follicles. There were 16 laparoscopies for ovum capture and 53 oocytes were retrieved (30.2% mature) 10 oocytes were from the GIFT patients. 16 oocytes were fertilized and 8 cleaved. There was only one pregnancy from and IVF patient with the alternated scheme that ended with a first trimester abortion.

In vitro fertilization: an overview.

The advanced reproductive technologies are an acceptably safe but invasive method to treat infertility. The decision to proceed with IVF/ET should be based on 1) a complete infertility investigation, 2) an understanding by the couple of the emotional, physical and financial aspects of the process and 3) an understanding by physicians of the real advantages and disadvantages of the procedures.

[The role of anxiety as a psychological co-factor in invasive treatment of sterility].

To prove the importance of anxiety as a disturbing factor during invasive sterility therapy 87 women of an IVF- and GIFT-series were investigated by a standardized anxiety inventory. We could not find any direct relations between breaking off stimulation and a striking level of anxiety. The role of anxiety as a co-factor in the different kinds of sterility management will be discussed.

Ovum donation.

Recent advances in assisted conception and reproductive endocrinology have provided novel avenues for the treatment of couples previously thought to be irreversibly sterile.

[Diagnostic value of the hypo-osmotic swelling test in pathologic ejaculation findings].

304 ejaculates with oligozoospermia from 227 partners of women suffering from sterility have been investigated by hypoosmotic swelling test, the detection of the motility rate and sperm motile efficiency. In a comparison between selected parts of the group and 104 semen samples of 15 fertile donors from the AID-program is was possible to show, that the swelling test has a better discrimination between fertile and infertile ejaculates than the other two methods. In spite of that the motility rate and the swelling test had a positive correlation (r = 0.73) in the whole group. Because of the high standard deviation of the sperm motile efficiency the diagnostic value of it is doubtful. In borderline cases of pathologic ejaculate factor the hypoosmotic swelling test is useful as a selection criterium for IVF and related techniques.

Autosperm antibodies in treated, pregnant and non-pregnant immunologically infertile patients.

Twenty-two immunologically infertile couples (male partners had proven autosperm antibodies-positive mixed antiglobulin reaction test [MAR] and direct immunobead test [d-IBT]) were treated with washed spermatozoa used either in the gamete intrafallopian tube transfer (GIFT) or artificial insemination (AIH) procedures. Sixteen of the 22 couples (72.2%) fell pregnant with an ongoing pregnancy rate of 54.5% (12/22). The pregnant and non-pregnant groups were compared with regards to the sperm antibodies detected on the spermatozoa (MAR, d-IBT and sperm cervical mucus contact [SCMC] test) and in the serum and seminal plasma of the male partners (tray agglutination test [TAT], indirect immunobead test [i-IBT], and enzyme-linked immunosorbent assay [ELISA]). The semen parameters, motility, forward progression, count/ml and normal morphological forms were also compared. Statistical analysis showed no difference between the two groups (pregnant and non-pregnant) with regards to the antibody tests performed on semen, serum and seminal plasma. No difference was also seen between the semen parameters of the two groups. The washing of spermatozoa for the GIFT or AIH procedures may therefore be a successful method of treatment for immunologically infertile couples. The results also indicate no difference in the fertility prognosis for the two groups since antibody levels and semen quality were not different between the pregnant and nonpregnant group.

[The value of fertilization in vitro and derived technics in oligo-astheno-teratospermia].

The authors report the advantage of fertilization in vitro and ancillary techniques in hypofertility by male component. In their fertilization in vitro program, a male indication is individually found in 10% of the cases, but also in female indications, even those of tubal origin. In summary, sperm anomalies are reported in 25% of couples entering a program of fertilization in vitro. Therefore they explain their current approach toward one or two cycles of diagnostic FIV, and then propose the most appropriate technique of assisted procreation for each case.

The role of the new reproductive technologies including IVF and GIFT in endometriosis.

In the context of the pathophysiology of endometriosis, the technology of IVF-ET and GIFT appears to offer a promising alternative for infertility associated with endometriosis in situations refractory to conventional medical or surgical therapy. The success rate of IVF with respect to follicular response, oocyte recovery, fertilization rate, and pregnancy rate in mild-to-moderate (stages I to II) disease is comparable to patients with tubal disease. However, compromise of oocyte recovery and fertilization appears to be present in severe-extensive cases (stages III to IV) implying an impairment of reproductive potential in advanced disease. In all stages of endometriosis, prior therapy, either medical or surgical, offers the best prognosis for an IVF or GIFT attempt. Surgical therapy with operative endoscopy at the time of oocyte retrieval offers a reasonable prognosis for treatment-independent pregnancy in cases of active endometriosis. The GIFT procedure bypasses an unfavorable peritoneal environment in stages I to II endometriosis without anatomic distortion or compromise of fallopian tube function. Pretreatment with GnRH analogues prior to ovulation induction is valuable in suppression of active disease as well as in inducing improved homogeneity of follicular response in IVF or GIFT cycles.
