A cross-cultural study of suicide intent in parasuicide patients.

As part of the WHO/EURO Multicentre Study on Parasuicide, the level of suicide intent among female and male parasuicide patients from various European regions was compared. From nine regions, a total of 1,212 parasuicide patients, 752 females and 460 males, were included in the study. Although some statistical significant differences in level of suicide intent between the regions and genders were found, the effect sizes of these relationships were so small that the differences have neither theoretical nor practical significance. As far as level of suicide intent is concerned, the WHO study has succeeded in recruiting a relatively homogeneous group of self-harming patients across borders of region, culture, and country.

Acculturation and breast-feeding intention and practice in Hispanic women on the US-Mexico border.

OBJECTIVES: To explore the extent to which acculturation indicators predict both breast-feeding history and intentions among Mexican-American mothers having their first births, and among those having subsequent births. DESIGN: Cross-sectional survey in a hospital post-partum unit. METHODS: 3,036 Hispanic women were interviewed post-partum in their hospital room. A survey was administered in English or Spanish, and included questions about prenatal care, diet, work exposures, contraceptive use, and breast-feeding history and intentions. For the purposes of this study, acculturation was measured using a series of indicators including language spoken at home, language ability, country of birth, and country in which last schooling was received. RESULTS: Previous breast-feeding was significantly associated with educational attainment, speaking both English and Spanish at home, having had prenatal care during the previous pregnancy, and with both country variables (country of birth and country where finished school). Women with less education, women who were single, and women who did not receive any prenatal care were less likely to intend to breast-feed than were women with a college education, women with a partner, and women who received any prenatal care. Women born in Mexico (for multiparous women), or having finished school in Mexico (for primiparous women), were more likely to intend to breast-feed. CONCLUSIONS: Acculturation is associated with breast-feeding history and intention to breast-feed. Acculturation is a complex construct and traditional measures of acculturation based on language preference may not be as useful on the US-Mexico border. It is recommended that further study be conducted to determine what factors prevent women from breast-feeding, even though they intend to do so, especially in multi-cultural communities like those around the US-Mexico border.

Pregnancy after intentional cryopreservation of percutaneous sperm aspiration specimens and intracytoplasmic sperm injection. A report of two cases.

BACKGROUND: Microsurgical epididymal sperm aspiration (MESA) and percutaneous epididymal sperm aspiration (PESA) are two methods of obtaining spermatozoa from patients with irreparable obstructive azoospermia. Intentionally using frozen-thawed spermatozoa obtained from MESA had been reported to be successful. With minimal invasiveness, intentionally cryopreserved PESA specimens and intracytoplasmic sperm injection (ICSI) were carried out in two cases. CASES: Two cases of irreparable obstructive azoospermia received PESA, and the spermatozoa were cryopreserved intentionally. Successful ICSI was performed later, utilizing frozen-thawed spermatozoa. CONCLUSION: PESA and ICSI are promising methods for these patients. The major advantages are minimal invasiveness and flexibility for further treatment.

Scale construction for measuring attitude, beliefs, perception of control, and intention to exercise.

BACKGROUND: Using the Theory of Planned Behavior as a framework, the Fitness Attitude, Expectations of Others, Perceived Behavioral Control, and Intention to Exercise Scales were developed. Professionals in exercise physiology generated items for the instruments. Experts in clinical psychology and exercise physiology established content validity. METHODS: Each of the scale items is phrased in a rating scale format. Both unipolar and bipolar scales were developed with seven response choices offered. Following the pilot testing and subsequent revisions, 19 items were retained in the Fitness Attitude Scale, seven items were retained in the Expectations of Others Scale, three items were retained in the Perceived Behavioral Control Scale, and 11 items were retained in the Intention to Exercise Scale. RESULTS: Correlation coefficients for the total instruments were significantly positive for stability and internal consistency, ranging from a = 0.75 to a = 0.87. Exercise specialists may wish to implement the use of these scales in their practices to develop intervention techniques for the promotion of positive fitness attitudes, good perception of control over the ability to exercise, and good intentions to exercise. CONCLUSIONS: The positive results in this study s sample suggest that these scales are ready for application in the field, but they should be pilot-tested with each group to check reliability with the particular population.

Mental health, job satisfaction, and intention to relocate. Opinions of physicians in rural British Columbia.

OBJECTIVE: To determine the prevalence of depression and burnout among family physicians working in British Columbia's Northern and Isolation Allowance communities. Current level of satisfaction with work and intention to move were also investigated. DESIGN: Cross-sectional, mailed survey. SETTING: Family practices in rural communities eligible for British Columbia's Northern and Isolation Allowance. PARTICIPANTS: A random sample of family physicians practising in rural BC communities. Initial response rate was 66% (131/198 surveys returned); excluding physicians on leave and in temporary situations and those who received duplicate mailings gave a corrected response rate of 92% (131/142 surveys returned). MAIN OUTCOME MEASURES: Demographics; self-reported depression and burnout; Beck Depression Inventory and Maslach Burnout Inventory scores; job satisfaction; and intention to leave. RESULTS: Self-reported depression rate was 29%; the Beck Depression Inventory indicated 31% of physicians suffered from mild to severe depression. About 13% of physicians reported taking antidepressants in the past 5 years. Self-reported burnout rate was 55%; the Maslach Burnout Inventory showed that 80% of physicians suffered from moderate-to-severe emotional exhaustion, 61% suffered from moderate-to-severe depersonalization, and 44% had moderate-to-low feelings of personal accomplishment. Depression scores correlated with emotional exhaustion scores. More than half the respondents were considering relocation. CONCLUSION: Physicians working in these communities suffer from high levels of depression and very high levels of burnout and are dissatisfied with their current jobs. More than half are considering relocating. Intention to move is strongly associated with poor mental health.

[Risk of deep infection in first-intention total hip replacement. Evaluation concerning a continuous series of 790 cases].

OBJECTIVE: Deep infections of the operative site is a rare but serious complication of first intention total hip arthroplasties. French data on infections are sparse. We studied the incidence, characteristics and potential risk factors for these infections in a consecutive series of 790 total hip arthroplasties performed as first intention implantations in a University Hospital in France. PATIENTS AND METHODS: All patients undergoing first intention total hip arthroplasty surgery between November 1995 and May 1999 were included in this study. Demographic, clinical, therapeutic and surgical data were collected. Deep infection was defined as proven presence of microorganisms in at least two preoperative samples during revision surgery. Patients were followed for 1 month to 4 years. Potential risk factors were identified with univariate analysis. Chi square and exact Fisher test were used. RESULTS: The overall incidence of deep infections was 1.11 per 100 implantations (95% CI 0.84-2.70). Delay to development ranged from 14 days to 32 months. Eleven infections were proven within the first year and 3 after one year. Two risk factors were identified: absence of systemic antibiotic prophylaxis (RR = 4.74, p = 0.03) and drainage discharge after 48 hours (RR = 3.62, p = 0.02). Other variables associated with infection with a relative risk greater than 2 were obesity, corticosteroid therapy, development of a hematoma, or difficult postoperative wound healing. CONCLUSION: The incidence observed in this series is slightly higher than generally reported in the world literature. It would be important to write an antibiotic prophylaxis protocol with prescription of systemic antibiotics for all operated patients in order to reduce this incidence. Other measures that could improve the quality of care should also be taken: re-evaluation of preoperative skin preparation protocols, systematic screening of patients carrying Staphylococcus aureus before implantation and proper care for these patients.

[Intentional neck-plastic incomplete clipping followed by coil embolization for broad-necked distal BA aneurysms].

OBJECTIVE: We report three patients with broad-necked distal basilar artery (BA) aneurysms treated with intentional incomplete clipping followed by endovascular occlusion using Guglielmi detachable coils. METHODS: The location of the aneurysms was BA bifurcation in one patient and BA-superior cerebellar artery (SCA) in two. One patient presented with acute subarachnoid hemorrhage and two patients had incidental aneurysms. In two patients, endovascular treatment was thought to be difficult considering the morphology of the aneurysms and surgical treatment was performed as the first choice of treatment. One patient with a BA-SCA aneurysm underwent endovascular treatment using a remodelling technique first. However, it was impossible to place the coil preserving SCA, so surgical treatment was performed. In all patients, the attempt to pursue complete clipping was considered to be accompanied with high risks of morbidity, so neck-plastic incomplete clipping was performed intentionally. One to six days after the surgery, coil embolization was performed. RESULTS: In all patients, complete occlusion of the aneurysms was achieved and all patients had excellent clinical outcomes. CONCLUSION: Intentional neck-plastic incomplete clipping followed by endovascular coiling may be a useful treatment option for patients with broad-necked distal BA aneurysms.

The intentionality model and language acquisition: engagement, effort, and the essential tension in development.

The purpose of the longitudinal research reported in this Monograph was to examine language acquisition in the second year of life in the context of developments in cognition, affect, and social connectedness. The theoretical focus for the research is on the agency of the child and the importance of the child's intentionality for explaining development, rather than on language as an independent object. The model of development for the research is a Model of Intentionality with two components: the engagement in a world of persons and objects that motivates acquiring a language, and the effort that is required to express and articulate increasingly discrepant and elaborate intentional state representations. The fundamental assumption in the model is that the driving force for acquiring language is in the essential tension between engagement and effort for linguistic, emotional, and physical actions of interpretation and expression. Results of lag sequential analyses are reported to show how different behaviors--words, sentences, emotional expressions, conversational interactions, and constructing thematic relations between objects in play--converged, both in the stream of children's actions in everyday events, in real time, and in developmental time between the emergence of words at about 13 months and the transition to simple sentences at about 2 years of age. Patterns of deviation from baseline rates of the different behaviors show that child emotional expression, child speech, and mother speech clearly influence each other, and the mutual influences between them are different at times of either emergence or achievement in both language and object play. The three conclusions that follow from the results of the research are that (a) expression and interpretation are the acts of performance in which language is learned, which means that performance counts for explaining language acquisition; (b) language is not an independent object but is acquired by a child in relation to other kinds of behaviors and their development; and (c) acquiring language in coordination with other behaviors in acts of expression and interpretation takes work, so that acquiring language is not easy.

Intentional poisonings in children below 15 years of age in the city of Lodz.

Data on self poisonings in children < l5 y of age from all over Poland are not available. An analysis of intentional poisonings among juveniles < 15 y of age in the city of Lodz in 1991-1995 was done. Intentional poisonings accounted for over 14% of total poisonings in children < 15 y of age. Self-poisonings in 9-11 and 12-15 y old, accounted for 5.2% and 86.8%, respectively, of the total juvenile intentional poisonings. They were more frequent in girls (80.8%) than boys (19.2%). Drugs were the most common agents (93%), of which 56% were sleep-inducing and psychotropic drugs. The dominating reasons for attempts were family and school problems, and impaired social relations. The high number of these poisonings indicates impaired parent-child relationship and resulting difficulties in social adaptation. The wide use of sleep-inducing and psychotropic drugs by adults makes these medicines more accessible.

Fertility intentions and actual behaviour. A longitudinal study: 1974, 1976, 1979.

Fertility intentions and actual fertility behavior are compared for a sample of young women with one child for the years 1974 through 1979 in France. The author "analyses interviews with some 2,000 women who were asked questions about their intended family size in 1974 and who were re-interviewed in 1976 and 1979. This has made it possible to compare their actual behaviour with their intentions and to discuss the reasons which may have led them to revise their intentions."

Analyzing migration decisionmaking: relationships between residential satisfaction, mobility intentions, and moving behavior.

"The author examines the role of residential satisfaction and mobility intentions vis-a-vis structural variables in migration decisionmaking with the aid of data drawn from the 1985-1989 waves of the American Housing Survey. A conceptual model is derived which is based on behavioral theories developed in social psychology, namely the theories of reasoned action and planned behavior. Several previously ignored links among structural variables, mobility predispositions and moving behavior are incorporated. Empirical analyses show that, although satisfaction and intentions are important predictors of mobility, most of the structural variables that are commonly known to be related to migration also have direct effects on subsequent moving behavior, independent of satisfaction and intentions."

[Migration from East to West Germany: intention and realization. A sequential probit model with unobserved heterogeneity controls].

"This study investigated migrations from eastern to western Germany. Using a sequential decision model we first examined which persons expressed the intention of wanting to move from eastern to western Germany. Then, using unobserved heterogeneity controls we determined at the same time which persons of those willing to move actually did so later.... Once the intention to move has been declared...those who are well-qualified, short-time workers and persons who feel threatened by unemployment as well as persons with relatives in western Germany more often than average do in fact realise this intention. Commuters also have a very high rate of realisation." (SUMMARY IN ENG AND FRE AND RUS)

Latina adolescents: predicting intentions to have sex.

This study utilized the theory of reasoned action to examine how Latina adolescents' intentions to have sex in the coming month were influenced by their general attitude toward having sex and their perceptions of general social norms about having sex. Eighty-four Mexican American and Central American adolescent females ages 14 to 19 participated in telephone interviews. One group of 41 Mexican American adolescents was obtained through a telephone survey and the second group of 43 pregnant or parenting Mexican American and Central American adolescents was recruited from health clinics. Path analysis conducted separately for the two groups indicated that, for the clinic group, perceptions of general social norms, but not general attitude, predicted intentions to have sex; and for the survey group, whether they were sexually active directly predicted perceptions of general social norms and intention to have sex. Adolescents' perceptions of what significant others desired of them, but not their motivation to comply, predicted perceptions of general social norms about having sex. Results underscore the importance of social norms for Latina adolescents when they are considering whether or not to have sex.

Negative Feelings Exacerbate Hostile Attributions of Intent in Highly Aggressive Boys.

Tested the hypothesis that aggressive boys' tendency to attribute hostile intentions to peers is exacerbated in a negative emotional state. Twenty-nine highly aggressive boys in special education, 12 moderately aggressive boys in regular education, and 16 nonaggressive boys in regular education inferred peers' intentions in 8 vignettes concerning ambiguous provocation by peers. Mild negative emotions were induced by unjust loss of a manipulated computer game. Half the vignettes were completed in this negative emotional state. After completion of all vignettes, the game was played again and won to reinduce positive feelings. Self-ratings of feelings obtained throughout the study showed the manipulations consecutively induced negative and positive feelings. Negative feelings increased hostile attribution of intent in the highly aggressive group. Highly and moderately aggressive boys responded more aggressively than nonaggressive boys.

[Medical care of injuries caused intentionally by domestic violence].

OBJECTIVE: To describe and analyze the causes of emergency care services for intentional injuries, especially those caused by domestic violence, at four public hospitals in Mexico City. MATERIAL AND METHODS: A cross-sectional study was conducted between January and April 1998, which included variables related with the victim, the aggressor, and the medical care provided to the victim. A questionnaire was applied to individuals who had been injured intentionally. Statistical analysis of data consisted of simple frequencies, the chi 2 test, and odds ratios (OR) with 95% confidence intervals (CI). A logistic regression model was also used to adjust for variables associated with the injury requiring emergency medical care. RESULTS: A total of 598 cases of intentional injuries were analyzed, 16% of which were due to domestic violence. Females were the most frequent victims (76%), followed by young people between 15 and 29 years old (46%). Variables associated with medical care due to injuries by domestic violence were: age 30 or older (OR 2.36, 95% CI 1.13-4.90), female gender (OR 8.60 95% CI 4.25-17.40), history of injuries (OR 4.93 95% CI 2.03-11.95), home as place of occurrence (OR 36.25 95% CI 16.59-79.18), and low education level (OR 2.33 95% CI 1.03-5.26). CONCLUSIONS: Study findings are consistent with those from other studies and call for enforcement of the Mexican Official Norm for Medical Care of Domestic Violence (Norma Oficial Mexicana para la Atención Médica de la Violencia Familiar) established in March 2000.

Treatment of locoregional recurrence after intentional curative resection of pancreatic cancer.

In order to analyze the results of treatment of patients with locoregional recurrence after intentional curative resection of pancreatic cancer, a retrospective study was performed. During the period 1978-1988, 108 patients underwent an intentional curative resection fo the pancreas. In 34 patients locoregional recurrence occurred, all within a period of three years (cumulative recurrence rate 56%). Sixty-eight percent of the patients presented with upper abdominal pain, and 62% with weight loss. Survival was significantly better (p = 0.02) in the group of 18 patients without distant metastases (1-year survival 22%) than in the 16 patients with distant metastases (1-year survival 0%). Five patients without proven distant metastases were treated by resection or chemotherapy. The mean survival was 33 months (range 6-74) in the treated group, and 4 months (0.4-7 months) in the untreated group, p = 0.002. In this retrospective study the longest survival was seen after radical resection of locoregional tumor recurrence. We therefore recommend that patients with locoregional recurrence without distant metastases after intentional curative resection of pancreatic cancer be treated.

[Intention of purchasing generic prescription drugs on the part of consumers in Asturias, Spain].

BACKGROUND: Ascertaining how consumers perceive the risk related to the use of generic prescription drugs and those factors which have the greatest impact on the intention to request a generic drug from the prescribing physician and/or the pharmacist for the purpose of determining any possible barriers or hindrances to the acceptance of generics and to gather information to aid healthcare managers in their decision-making processes. METHODS: Study on prescription drug use revolving around the degree to which patients are willing to request an EFG. In this quantitative transversal study, a total of 542 individuals were individually surveyed upon exiting a healthcare center or pharmacy in Asturias. A scale for measuring the perceived risk involved in the purchase of a prescription drug including 15 attributes grouped into five aspects was included in the questionnaire. Information was also gathered regarding the intention of using generic prescription drugs and on the demographic and socioeconomic characteristics of those surveyed. For the analysis of the results, a factorial confirmational analysis, multiple regression and univariate analysis were used. The data was processed using the EQS and SPSS statistics programs. RESULTS: Mean perception of the risk (scales 1-7): functional: 2.75; physical: 2.68: financial: 2.19; psychological: 1.99; social: 1.42. Factors having a bearing on the intention of requesting generic prescription drugs from their physician: psychological risk (p = 0.000). On requesting the same from their pharmacist: psychological risk (p = 0.000) and social risk (p = 0.020). CONCLUSIONS: The agents interested in the development on the EFG market should target their communication efforts on putting the functional and financial aspects of the manufacturer's specialties and generic specialties on the same level, but should not leave out psychological and social aspects of the consumers' purchasing behavior.

Knowledge, attitudes and intention of high school students towards the nursing profession in Riyadh city, Saudi Arabia.

OBJECTIVE: To determine the knowledge, attitudes, and intention among the Saudi high school students towards the nursing profession. In addition, the study aims to identify students' perception of causes preventing them to become nurses. It also aims to determine the factors influencing the Saudi high school students' choice of nursing profession. METHODS: We used the descriptive analytical research design. Stratified random sampling procedures were employed to represent the Saudi high school students, 3 male and 3 female schools scattered in all areas of Riyadh city were selected. A total of 600 questionnaires were distributed and 503 questionnaires were returned, of which, 479 questionnaires were valid for analysis (79.8% response rate). The data collection started from November 2002 to January 2003. Descriptive and inferential statistics were used to analyze the collected data. RESULTS: High school students scored a reasonable level on the knowledge dimension, but did not achieve high scores on the attitude dimension, however, they achieved very low scores on the intention of being a nurse in the future (5.2% of them indicated nursing as their preferred future job). Inferential data analysis showed that attitude, having or not having a nurse friend and knowledge was found to have significant positive influence on high school students' intention, while long working hours and high work load compared to other jobs were found to have a significant negative influence. CONCLUSION: Results indicated that the more knowledge and positive attitude on nursing, the more likely the student would be attracted to the nursing profession. Accordingly, it could be concluded that Saudi health decision-makers need to increase the positive attitude on the nature and encouraging characteristics of modern nursing and its increasing respect as a skillful career for high school students. Reconsideration of salaries and benefits were recommended in order to attract more students to the nursing profession.
