Ritleng intubation set: a new system for lacrimal pathway intubation.

Silicone tubing of the lacrimal pathway is used after lacrimal pathway injuries or canalicular stenosis. This procedure is often complicated by the so-called accordeon phenomenon. Far the last 3 years we have used a new system for lacrimal intubation: the Ritleng intubation set. It consists of a silcone tube which is connected to a polypropylene tube and a specially designed stainless probe for lacrimal insertion. Due to the smooth connection of the polypropylene tube and the silicone tube intubation can be performed easily and without the so-called accordeon phenomenon. In case of congenital or acquired lacrimal stenosis probing and silicone intubation can be performed in one step. We used this set in 19 lacrimal lacerations of the lacrimal pathway and in 7 cases of congenital and 12 cases of acquired lacrimal stenosis. With this specially designed lacrimal intubation set lacerated canaliculi can be quickly reapproximated. It allows probing of the lacrimal system and intubation in one procedure. In this report the technique and indications will be described and discussed.

Intubation of the lacrimal pathways under endoscopic control.

Described herein is a new modification of the lacrimal pathway intubation in children with efferent lacrimal pathway obstruction. For removal of the intubation set probe without olive tips from the inferior nasal meatus, a special titanium loop is used. An endoscopic technique allows us a basically more considerate performance of this procedure and to speed it up. The risk of iatrogenic damage to the mucous membrane of the inferior meatus is, thus, reduced too. From April 1994 to June 1998, lacrimal pathway intubation was performed in 235 children 283 times. Infraction of the inferior concha was performed 27 times. Insertion of a probe submucously into the inferior nasal meatus was found 35 times and the probe was released by focused incision of the mucous membrane.

[20-MHz ultrasound of pre-saccular lacrimal ducts].

BACKGROUND: In clinical practice, ultrasound examination of the lacrimal drainage system is not able to represent the lacrimal canaliculi with a high quality but the high-resolution ultrasound with 20 MHz transducer is a better possibility to get images of the lacrimal canaliculi. PATIENTS AND METHODS: A total of 20 patients suffering from diseases of the lacrimal drainage system were examined ultrasonographically. Detection and localisation of the lacrimal canaliculi was made by a 20 MHz sector scanner in combination with the ultrasound diagnostic system I. After patient positioning, the high-resolution ultrasound investigation was done via immersion connection (methylcellulose). Instillation of viscoelastic substances (e.g. Healon) was necessary to detect the lacrimal canaliculi. RESULTS: In all investigated patients, the high-resolution ultrasound (transducer frequency of 20 MHz) allowed reproducible images of the lacrimal drainage system. Without viscoelastic substances, a good echographic visualisation of canaliculi structures was obtained from patients with chronic canaliculitis or dislocated punctum plugs. In chronic canaliculitis the ultrasonic images showed an ectatic canaliculus (diverticulum) and high-reflective structures (concrements). CONCLUSION: The high-resolution ultrasound (transducer frequency of 20 MHz) seems to be well suited for investigations of the lacrimal drainage system. In comparison with ultrasound biomicroscopy the resolution is lower but the depth of penetration is higher. The use of 20 MHz ultrasonic examination can be helpful in the clinical diagnostic of chronic canaliculitis.

Update on punctal plugs.

Punctal plugs have offered a safe and often reversible treatment for aqueous-deficient dry eye for over three decades. However, they represent only one tool in our armamentrarium to help patients with dry eyes, and plugs do have limitations. This article briefly reviews the history of occlusive treatments for aqueous tear deficiency and provides an update of recent advancements in punctal and canalicular occlusive materials and techniques.

[Eosinophilic adenoma of the nose and ethmoid bone simulating a malignant tumor].

A 82-year old woman patient was first diagnosed to have a dacryocystitis and was treated at the Department of Ophthalmology. Clinical examination and x-ray of the paranasal sinuses including tomographs led to the suspicion of a malignant, infiltratively growing tumour in the region of the left cheek and of the left ethmoidal cells. After endonasal removal of the tumour that obstructed the left nasal cavity and ethmoidal cells, histological diagnosis revealed an eosinophilic adenoma. Dacryocystitis had been caused secondarily by the impairment of lacrymal flow. Now, one year later, the patient is free from any recurrence and clinical or subjective signs and symptoms.

[Surgical treatment of the lacrimal apparatus--technics and results].

After a short review of anatomy the clinical and radiological diagnosis of stenoses of the lacrimal duct system are discussed in detail. Microcatheterdacryocystography and high resolution CT are very important in determining the indications for surgery. Microsurgical endonasal dacryocystorhinostomy is preferred for treatment of postsaccal stenoses. It is also the method of choice for one stage definitive therapy of acute lacrimal sac empyema. Surgical reconstruction in cases of presaccal stenoses is difficult and should be handled individually. For intubation of the canaliculi we have developed a special modified pigtail-hollow-probe (Draf-Haag 1981), which reduces damage to the epithelium. If reconstruction of the natural lacrimal duct system is not possible, a conjunctivorhinostomy by Walter's technique is indicated. The different surgical concepts and results are discussed.
