Radiostereometric analysis for monitoring percutaneous physiodesis. A preliminary study.

Percutaneous physiodesis is an established technique for treating mild leg-length discrepancy and problems of expected extreme height. Angular deformities resulting from incomplete physeal arrest have been reported, and little is known about the time interval from percutaneous physiodesis to actual physeal arrest. This procedure was carried out in ten children, six with leg-length discrepancy and four with expected extreme height. Radiostereometric analysis was used to determine the three-dimensional dynamics of growth retardation. Errors of measurement of translation were less than 0.05 mm and of rotation less than 0.06 degrees. Physeal arrest was obtained in all but one child within 12 weeks after physiodesis and no clinically-relevant angular deformities occurred. This is a suitable method for following up patients after percutaneous physiodesis. Incomplete physeal arrest can be detected at an early stage and the procedure repeated before corrective osteotomy is required.

Re-attachment of the tuberosities of the humerus following hemiarthroplasty for four-part fracture.

This study evaluated the effect on movement under load of three different techniques for re-attachment of the tuberosities of the humerus using test sawbones. In the first, the tuberosities were attached both to the shaft and to each other, with one cerclage suture through the anterior hole in the prosthesis. The second technique was identical except for omission of the cerclage suture and in the third the tuberosities were attached to the prosthesis and to the shaft. An orthogonal photogrammetric system allowed all segments to be tracked in a 3D axis system. The humeri were incrementally-loaded in abduction, and the 3D linear and angular movements of all segments were calculated. Displacement between the tuberosities and the shaft was measured. The first and second techniques were the most stable constructs, with the third allowing greater separation of fragments and angular movement. Separation at the midpoint of the tuberosities was significantly greater using the latter technique (p < 0.05). The cerclage suture added no further stability to the fixation.

Should we reconsider all-polyethylene tibial implants in total knee replacement?

The role of modular tibial implants in total knee replacement is not fully defined. We performed a prospective randomised controlled clinical trial using radiostereophotogrammetric analysis to compare the performance of an all-polyethylene tibia with a metal-backed cruciate-retaining condylar design, PFC-Sigma total knee replacement for up to 24 months. There were 51 patients who were randomised into two treatment groups. There were 10 subsequent withdrawals, leaving 21 all-polyethylene and 20 metal-backed tibial implants. No patient was lost to follow-up. There were no significant demographic differences between the groups. At two years one metal-backed implant showed migration > 1 mm, but no polyethylene implant reached this level. There was a significant increase in the SF-12 and Oxford knee scores after operation in both groups. In an uncomplicated primary total knee replacement the all-polyethylene PFC-Sigma tibial prosthesis showed no statistical difference in migration from that of the metal-backed counterpart. There was no difference in the clinical results as assessed by the SF-12, the Oxford knee score, alignment or range of movement at 24 months, although these assessment measures were not statistically powered in this study.

The premature failure of the Charnley Elite-Plus stem: a confirmation of RSA predictions.

We performed a clinical and radiological study to determine the rate of failure of the Charnley Elite-Plus femoral component. Our aim was to confirm or refute the predictions of a previous roentgen stereophotogrammetric analysis study in which 20% of the Charnley Elite-Plus stems had shown rapid posterior head migration. It was predicted that this device would have a high early rate of failure.We examined 118 patients at a mean of nine years after hip replacement, including the 19 patients from the original roentgen stereophotogrammetric study. The number of revision procedures was recorded and clinical and radiological examinations were performed. The rate of survival of the femoral stems at ten years was 83% when revision alone was considered to be a failure. It decreased to 59% when a radiologically loose stem was also considered to be a failure. All the patients previously shown in the roentgen stereophotogrammetric study to have high posterior head migration went on to failure. There was a highly significant difference (p = 0.002) in posterior head migration measured at two years after operation between failed and non-failed femoral stems, but there was no significant difference in subsidence between these two groups. Our study has shown that the Charnley Elite-Plus femoral component has an unacceptably high rate of failure. It confirms that early evaluation of new components is important and that roentgen stereophotogrammetric is a good tool for this. Our findings have also shown that rapid posterior head migration is predictive of premature loosening and a better predictor than subsidence.

A constrained acetabular component for recurrent dislocation.

We treated 34 patients with recurrent dislocation of the hip with a constrained acetabular component. Roentgen stereophotogrammetric analysis was performed to assess migration of the prosthesis. The mean clinical follow-up was 3.0 years (2.2 to 4.8) and the radiological follow-up was 2.7 years (2.0 to 4.8). At the latest review six patients had died and none was lost to follow-up. There were four acetabular revisions, three for aseptic loosening and one for deep infection. Another acetabular component was radiologically loose with progressive radiolucent lines in all Gruen zones and was awaiting revision. The overall rate of aseptic loosening was 11.8% (4 of 34). Roentgen stereophotogrammetric analysis in the non-revised components confirmed migration of up to 1.06 mm of translation and 2.32 degrees of rotation at 24 months. There was one case of dislocation and dissociation of the component in the same patient. Of the 34 patients, 33 (97.1%) had no further episodes of dislocation. The constrained acetabular component reported in our study was effective in all but one patient with instability of the hip, but the rate of aseptic loosening was higher than has been reported previously and requires further investigation.

New automated iris image acquisition method.

I propose a new iris image acquisition method based on wide- and narrow-view iris cameras. The narrow-view camera has the functionalities of automatic zooming, focusing, panning, and tilting based on the two-dimensional and three-dimensional eye positions detected from the wide- and narrow-view stereo cameras. By using the wide- and narrow-view iris cameras, I compute the user's gaze position, which is used for aligning the X-Y position of the user's eye, and I use the visible-light illuminator for fake-eye detection.

Calibration of stereo cameras from two perpendicular planes.

We present a novel linear algorithm with which to calibrate stereo cameras from two perpendicular planes. Stereo cameras are two cameras aligned in a special configuration with coplanar image planes and parallel axes that are increasingly more widely used in computer vision tasks. Our objective is to present a more practical and simplified linear algorithm for these special configuration cameras, as traditional linear algorithms usually require too-strong constraints either on three-dimensional scenes or on the camera's motion. We developed the proposed algorithm from a new constraint by exploiting the orthogonality of two planes. The algorithm has much weaker constraints on three-dimensional scenes because two perpendicular planes are commonly found in daily life. We tested the algorithm with synthetic data and real image data. Experimental results show that it is both accurate and practical.

Geometrical effects of positional errors in integral photography.

The effects of misarrangement of elements (elemental lenses and elemental images) that construct three-dimensional (3-D) images in integral photography are presented. If the lens arrays of the capturing system and the display system are not aligned accurately, positional errors of elements may occur, causing the 3-D image to be reconstructed in an incorrect position. The relation between positional errors of elements and the reconstructed image is derived. As a result, it is shown that a 3-D image is separated by local positional errors and blurred by global positional errors. In both local and global positional errors, 3-D images reconstructed far from the lens array are greatly affected.

Joint blind restoration and surface recovery in photometric stereo.

We address the problem of simultaneous estimation of scene structure and restoration of images from blurred photometric measurements. In photometric stereo, the structure of an object is determined by using a particular reflectance model (the image irradiance equation) without considering the blurring effect. What we show is that, given arbitrarily blurred observations of a static scene captured with a stationary camera under different illuminant directions, we still can obtain the structure represented by the surface gradients and the albedo and also perform a blind image restoration. The surface gradients and the albedo are modeled as separate Markov random fields, and a suitable regularization scheme is used to estimate the different fields as well as the blur parameter. The results of the experimentations are illustrated with real as well as synthetic images.

Depth resolution and displayable depth of a scene in three-dimensional images.

The depth resolution and the recordable object depth range, obtainable with parallel, toed-in and sliding aperture camera configurations for multiview image acquisition in the three-dimensional imaging systems, are found by assuming that the camera lens resolution is diffraction limited and the resolution of the recorded image is limited by a pixel pitch of the imaging sensor. The depth resolution for the holographic image is calculated and compared with that of the multiview images for the same parameter values. The influence of the viewer's eye resolution limit on the depth resolution of the multiview images and hologram is also found.

Optimized stereo reconstruction of free-form space curves based on a nonuniform rational B-spline model.

Analytical reconstruction of 3D curves from their stereo images is an important issue in computer vision. We present an optimization framework for such a problem based on a nonuniform rational B-spline (NURBS) curve model that converts reconstruction of a 3D curve into reconstruction of control points and weights of a NURBS representation of the curve, accordingly bypassing the error-prone point-to-point correspondence matching. Perspective invariance of NURBS curves and constraints deduced on stereo NURBS curves are employed to formulate the 3D curve reconstruction problem into a constrained nonlinear optimization. A parallel rectification technique is then adopted to simplify the constraints, and the Levenberg-Marquardt algorithm is applied to search for the optimal solution of the simplified problem. The results from our experiments show that the proposed framework works stably in the presence of different data samplings, randomly posed noise, and partial loss of data and is potentially suitable for real scenes.

Defocus morphing in real aperture images.

A new concept called defocus morphing in real aperture images is introduced. View morphing is an existing example of shape-preserving image morphing based on the motion cue. It is proved that images can also be morphed based on the depth-related defocus cue. This illustrates that the morphing operation is not necessarily a geometric process alone; one can also perform a photometry-based morphing wherein the shape information is implicitly buried in the image intensity field. A theoretical understanding of the defocus morphing process is presented. It is shown mathematically that, given two observations of a three-dimensional scene for different camera parameter settings, we can obtain a virtual observation for any camera parameter setting through a simple nonlinear combination of these observations.

Volume three-dimensional flow measurements using wavelength multiplexing.

Optically distinguishable seeding particles that emit light in a narrow bandwidth, and a combination of bandwidths, were prepared by encapsulating quantum dots. The three-dimensional components of the particles' displacement were measured within a volume of fluid with particle tracking velocimetry (PTV). Particles are multiplexed to different hue bands in the camera images, enabling an increased seeding density and (or) fewer cameras to be used, thereby increasing the measurement spatial resolution and (or) reducing optical access requirements. The technique is also applicable to two-phase flow measurements with PTV or particle image velocimetry, where each phase is uniquely seeded.

Effect of a high fat diet on quantitative features of adipocytes in the omentum: an experimental, stereological and ultrastructural study.

BACKGROUND: Omental adipose tissue specimens of female rats that were fed a high fat (HF) diet were evaluated stereologically and histopathologically. To our knowledge, there is no stereological study on numerical density, nuclear height and volume of adipocytes in omental adipose tissue in the female rat fed a HF diet in the literature. METHOD: 20 female Spraque Dawley rats were used in the study. 10 of the animals were fed HF diet consisting of 30% of calories from fat for 3 months. The remaining 10 rats, the control group, were fed a normal diet. After the experimental procedure, all animals were anesthetized and omental adipose tissues in the same area were dissected and fixed for the histochemical process using a mixture of 3% glutaraldehyde and 1% osmium tetraoxide in 0.1 M phosphate buffer. After embedding of tissues in araldite CY 212, semi-thin and thin sections were cut. The semi-thin sections were stained with toluidine blue. The physical dissector counting method was used for estimation of numerical density and nuclear height of adipocytes. Cavalieri principle was used for the estimation of adipocyte volume; volume fraction approach was applied to find the volume fraction of adipose tissue components. RESULTS: The mean numerical density of adipocytes in the HF diet group was significantly higher than the control. The mean nuclear height of adipocytes was also very high in the HF diet group. The volume fraction of adipose mass was increased whereas the extracellular matrix volume fraction was reduced in the HF diet group compared to the controls. The mean volume of adipocytes in the HF diet group was also significantly higher than in the control group. At the light microscopy level, it was found that adipocytes were enlarged and gaining irregular shape in the HF diet group. Thicker basal lamina and electron dense lipid content were also found in this group at the electron microscopy level. CONCLUSION: Lipid content and number of adipocytes in the adipose tissue of HF diet rats were higher than in the controls. Thus, HF diet induces increase in body weight via both hypertrophy and hyperplasia of adipocytes.

Root surface measurements of mandibular molars using stereophotogrammetry.

Ten mandibular first molars were hemisected and the root surface of each root was measured using stereophotogrammetry and computer analysis. In each tooth the mesial root was determined to have a greater surface area than the distal root. This difference was statistically significant. The fact that the mesial root has the larger surface area must be a consideration when hemisection is planned.

[Endoscopic 3-D imaging--measuring biological surfaces].

The precise endoscopical measurement of topometrical structures of organs will overcome limitations in present minimal invasive surgery and lead to new operation techniques. This paper describes a research project concerning an endoscopical 3D-measurement system based on optical techniques. We will introduce prototype endoscopes for minimal invasive 3D-measurements and the applied photogrammetrical algorithms for acquiring the topometrical structures. The influence of major organ surface properties on the measurement will be discussed and first results will be presented.

Retrieval analyses of implanted biomaterials: light microscopic and scanning electron microscopic analyses of implants retrieved from humans.

We report analyses obtained from 135 implant cases retrieved from humans and submitted to the American Academy of Implant Dentistry Research Foundation--Medical College of Georgia Implant Retrieval Center. The undecalcified samples were embedded in polymethyl-methacrylate and examined with scanning electron microscopy and with routine light via polarized or Nomarski microscopy. Cases included both orthopedic and dental implants as well as entire mandibles obtained at autopsy. Significant numbers of submitted implants had substantial amounts of adhered bone, which permitted evaluation of human bone remodeling to osseointegrated implants. These implants failed because of implant fracture. As has been observed in animal studies, an interdigitating canaliculi network provided communication between interfacial osteocytes and osteocytes deeper within the remodeled osteonal and trabecular bone. Significant numbers of osseointegrated fractured hydroxyapatite-coated dental implants demonstrated the adequate serviceability of these implants prior to biomaterial fracture. In contrast, the hydroxyapatite coating was dissociated from retrieved orthopedic implants, leading to extensive cup loosening and case failure. Caution is advised for the use of hydroxyapatite-coated acetabular implants. This study therefore underscores the need for evaluation of failed human dental and orthopedic implants. Correlations can be drawn between human retrieval and experimental animal studies.
