[Staphylococcal infection after intramuscular injection].

A 74-year-old woman and a 68-year-old man admitted to hospital for backache and somnolence both showed growth of Staphylococcus aureus in blood cultures. One patient died from septic shock. In both patients septicemia was secondary to a gluteal abscess after intramuscular injection of antirheumatic drugs. Diagnosis was particularly difficult due to lack of local signs. Local pain hardly differed from that of the original "rheumatic" backache. The literature is reviewed and diagnosis and treatment are discussed.

[The measurement of the psychomotor capacities involved in the technic of the intramuscular injection].

This study is to verify the psychomotor skill using psychomotor capacity measure. The authors describe two tests that will be used by nurses denominated "electronic psychomotor" and "perferation". Those tests are applied in the first stage of this study to analyse the teaching-learning process of intramuscular injection. Three groups of undergraduate nursing students in different stages of evolution of the course, were analysed and were verified that there was homogeneity among groups in relation to psychomotor capacity.

Plasma, urinary and biliary residues in cattle following intramuscular injection of nortestosterone laurate.

The synthetic androgen 19-nortestosterone (beta-NT) has been used illegally as a growth promoter in cattle production in the European Union. Elimination of beta-NT and its metabolites in plasma, urine and bile was studied in three cattle with cannulated gallbladders following intramuscular injection at a single site of 500 mg of the laurate ester (NTL) containing 300.5 mg beta-NT. Using enzyme immunoassay quantification, plasma Cmax of free beta-NT was 0.5 +/- 0.15 microgram/L (mean +/- SEM). Concentrations of free beta-NT in plasma were consistently greater than the assay limit of quantification (0.12 microgram/L) for 32.7 +/- 13.42 days. Mean residence time for the beta-NT in plasma was 68.5 +/- 20.75 days. Following sample preparation by immunoaffinity chromatography, high-resolution GC-MS was used to quantify beta-NT and alpha-NT in urine and bile. beta-NT was detected irregularly in urine from two of the three animals post injection. The principal metabolite present in the urine, alpha-NT, was detected for 160.3 +/- 22.67 days post injection. Cmax for alpha-NT in urine was 13.7 +/- 5.14 micrograms/L. Mean urinary AUC0-183 days for alpha-NT was 845.7 +/- 400.90 (microgram h)/L. In bile, alpha-NT was the only metabolite detected for 174.3 +/- 8.67 days post treatment. Cmax for alpha-NT in bile was 40.8 +/- 12.70 micrograms/L and mean biliary AUC0-183 days for alpha-NT was 1982.6 +/- 373.81 (microgram h)/L. Concentrations of alpha-NT in bile samples were greater than those in urine samples taken at the same time. The mean ratio of biliary:urinary AUC0-183 days was 3.0 +/- 0.72. It is concluded that bile is a superior fluid for detection of alpha-NT following injection of NTL, owing to the longer period during which residues may be detected after administration.

Long-term correction of obesity and diabetes in genetically obese mice by a single intramuscular injection of recombinant adeno-associated virus encoding mouse leptin.

The ob/ob mouse is genetically deficient in leptin and exhibits a phenotype that includes obesity and non-insulin-dependent diabetes mellitus. This phenotype closely resembles the morbid obesity seen in humans. In this study, we demonstrate that a single intramuscular injection of a recombinant adeno-associated virus (AAV) vector encoding mouse leptin (rAAV-leptin) in ob/ob mice leads to prevention of obesity and diabetes. The treated animals show normalization of metabolic abnormalities including hyperglycemia, insulin resistance, impaired glucose tolerance, and lethargy. The effects of a single injection have lasted through the 6-month course of the study. At all time points measured the circulating levels of leptin in the serum were similar to age-matched control C57 mice. These results demonstrate that maintenance of normal levels of leptin (2-5 ng/ml) in the circulation can prevent both the onset of obesity and associated non-insulin-dependent diabetes. Thus a single injection of a rAAV vector expressing a therapeutic gene can lead to complete and long-term correction of a genetic disorder. Our study demonstrates the long-term correction of a disease caused by a genetic defect and proves the feasibility of using rAAV-based vectors for the treatment of chronic disorders like obesity.

Lymphotropic delivery of cyclosporin A by intramuscular injection of biodegradable microspheres in mice.

Biodegradable microspheres containing cyclosporin A (CsA), an immunosuppressant, enabled the lymphotropic delivery of the drug over the long term by parenteral administration in mice. The CsA encapsulated in the microspheres of polylactic acid polymer (Mw, 9000) was released over 30 days in vitro. The intramuscular injection of the microspheres containing CsA in the femoral site of mice maintained the drug at a high level in the inguinal lymph nodes over 1 month, although only low blood levels of the drug were observed.

The role of drug lipophilicity in release from intra-adipose and intramuscular injection sites.

Release rates from intramuscular and intra-adipose injection sites are dependent upon several including injection depth. Little is known about the relationship between drug lipophilicity and transport rate of drugs through adipose and muscle tissue. The principal objective of the present study was to investigate to what extend drug lipophilicity affects release and release rate from adipose tissue. Nine pigs were given intravenous (0.1 mg/kg), intramuscular (0.2 mg/kg) and intra-adipose (0.2 mg/kg) injections of propranolol, alprenolol, carazolol, metoprolol and atenolol. The fraction not-absorbed versus time plots after intramuscular and intra-adipose injection showed a biphasic decline for all model compound with the exception of atenolol being the most hydrophilic drug. This biphasic decline indicates that two different mechanisms may be involved in drug release. Initial release rates after intra-adipose injection were negatively correlated (Kendall's rankorder test) with fat-buffer distribution constants. The extent of release after 24 h could be considered as a characteristic parameter for the second release phase. The amounts released after 24 h were lower for propranolol, alprenolol, carazolol and metoprolol than for atenolol. Incomplete release at 24 h can be explained by the decrease of the solvent drag after absorption of the vehicle is complete.

[Choosing an intramuscular injection needle according to body type].

The sample of this study consisted of 360 patients with asthenic, sthenic or ortho-sthenic type. The thickness of hypodermic fat at dorsogluteal site was measured and the effect was compared. The result suggested that needle should be selected according to different habitus before intramuscular injection. The commonly used No. 6. 5 and No. 7 needle are not suitable for elderly females patients with ortho-sthenic type or male and female patients with sthenic type. For these kind of patients, a needle with a length of 35 mm, 40 mm or 45 mm should be used respectively in order to reach muscular tissues, have good effect, and reduce local pain and tissue hardening.

[Comparative analysis of diuretic activities of human ANP gene injected intramuscularly and intravenously in adriamycin-induced nephrotic rats].

In order to explore the feasibility of gene therapy strategy based on the human atrial natriuretic peptide (hANP) gene delivery for the treatment of nephropathy and compare the diuretic activities of the hANP gene injected intramuscularly(i.m.) and intravenously(i.v.), the naked retroviral vector DNA harboring the hANP cDNA under the control of retroviral 5' long terminal repeat at a dose of 5 mg/kg body weight was injected i.m. or i.v. into the nephrotic model rats induced with adriamycin(ADR) injected i.v. at a dose of 7.5 mg/kg body weight. A single injection of the hANP gene resulted in a marked elevation in plasma level of hANP 5 days after gene delivery and a significant increase in the ratio of urine volume to body weight and the diuretic effect continued for more than 15 days. In addition, there was a significant rise in the body weight of treatment groups as compared with that of negative control group and no difference in the concentrations of electrolytes in urine between groups. There was no significant differences in total effects resulted from the two routes of gene delivery and the way of gene delivery through the skeletal muscle is simpler and easier. These results suggest that somatic gene delivery of the hANP gene could enhance the renal functions in nephrotic rats significantly and would be a potential strategy for the treatment of renal disorders.

Muscle granulomas following intramuscular injection.

An 8-month-old child underwent a muscle biopsy because of hypotonia. The biopsy showed centronuclear myopathy and, in addition, an unusual granulomatous inflammatory infiltrate. The child died of respiratory complications and a complete autopsy failed to reveal further evidence of granulomatous disease. The lesion was reproduced in experimental animals by intramuscular injection of diphtheria and tetanus toxoids and pertussis vaccine. Granular material present in histiocytes in these lesions was shown histochemically to be an aluminum salt.

[Cutaneous leishmaniasis: treatment with cryotherapy and intramuscular injections of meglumine antimonate (glucantime)].

A 37-year-old female patient with cutaneous leishmaniasis of the helix of the left ear is reported. The diagnosis was based on her past history, clinical appearance and histological examination (detection of Leishman-Donovan bodies). Initial cryotherapy failed to cure the lesions completely. Treatment with intramuscular injections of meglumine antimonate (Glucantime) was successful. The discussion focuses on the pathophysiology, diagnosis and therapy.

[Photosensitization after intramuscular injections of hematoporphyrin. 3 cases].

Haematoporphyrin is a synthetic porphyrin the photosensitizing properties of which are used for the detection and treatment of some neoplasias. Three cases of phototoxicity from Hemedonine, an "anti-asthenia" drug composed of pure haematoporphyrin, are reported. Following exposure to the sun, painful urticarial plaques developed at the sites of intramuscular injections, and spread to the lumbar and abdominal regions, leaving a brownish pigmentation which persisted for years. Photobiological exploration was performed in one of these patients, demonstrating photosensitivity to Hemedonine in vivo and showing that long UVA rays were essentially responsible for the lesions. Histological examination elicited the initial vascular changes of a phototoxic reaction and suggested a new mechanism of photosensitization: "photo-induced immune vasculitis". The phototoxic effects of haematoporphyrin are primarily due to the destruction of intracellular organelles consecutive to oxidative denaturation of membrane systems. The initial phase of the phototoxic reaction might consist of complement activation and degranulation of dermal mast cells.

Acute intramuscular injection of oils or the oleic acid component protects mice against paraquat lethality.

Although selenium or vitamin E deficiencies or changing from cereal-based to purified diets augments paraquat toxicity, the action of other dietary components in normal animals fed nutritionally adequate diets is not clear. Upon injection of mice with antiinflammatory agents, a protective action of the corn oil vehicle against paraquat lethalities was noted. This preventive action of a large parenteral administration of unknown components in oils served as the basis for this study. Intramuscular injection of various vegetable oils protected similarly, indicating that in mixtures, the degree of lipid saturation did not seem to be an important factor. Injection of the monounsaturated fatty acid oleic acid decreased oral paraquat lethalities in mice, but linoleic, gamma-linoleic or linolenic acids were not protective in either male or female mice. Measurement of paraquat concentrations in various tissues at various times after administration indicated no effect of corn oil on paraquat distribution. Although the exact mechanism of the complex nature of oil protection against paraquat toxicity in mice is still unknown, this study provides evidence for in vivo oxidant protection by a monounsaturated fatty acid.

A comparison of the analgesic effect of oxymorphone by rectal suppository and intramuscular injection in patients with postoperative pain.

The relative analgesic potency of oxymorphone by rectal suppository and intramuscular injection was evaluated in a double-blind, twin-crossover comparison of graded single doses in 136 patients with postoperative pain. The time-effect curves of the two routes of administration differed substantially; rectal resulted in lower and more delayed peak analgesia and a longer duration of action than intramuscular administration. When both duration and intensity of analgesia are considered (total effect), rectal oxymorphone was 1/10 as potent as the intramuscular form; in peak effect, it was only 1/16 to 1/20 as potent. However, because intramuscular oxymorphone is nine to ten times as potent as intramuscular morphine, 5 to 10 mg oxymorphone by suppository provides analgesia comparable to that provided by the usually used doses of parenteral narcotics. Rectal oxymorphone produced no more, and perhaps somewhat fewer, side effects than doses of intramuscular oxymorphone producing equivalent total analgesic effect. None of the patients objected to the rectal route of analgesic administration. This study demonstrates the feasibility of well-controlled analgesic assays employing the double-dummy technique to compare suppositoreis with oral or parenteral analgesic dosage forms. Our observations also suggest that the rectal route is an acceptable and practical way of administering potent analgesics and is probably being underutilized by physicians in the control of moderate to severe pain.

Absorption of sodium benzylpenicillin from the equine uterus after local Lugol's lodine treatment, compared with absorption after intramuscular injection.

Plasma concentrations of sodium benzylpenicillin following intrauterine infusion were increased by reducing the volume of solution and expelling air from the vagina after infusion. Instillation of 10 per cent Lugol's iodine solution into the uterus before penicillin infusion further increased the absorption rate, although peak plasma levels of penicillin were less than half those which resulted from intramuscular injection of the same dose.

A phase II trial of interleukin-2 by continuous infusion and interferon by intramuscular injection in patients with renal cell carcinoma.

Fifteen patients with advanced, measurable renal cell carcinoma entered a Phase II clinical trial of interleukin-2 (IL-2) (Teceleukin, Hoffmann-La Roche Inc., Nutley, NJ) and interferon (IFN) (Roferon A, Hoffmann-La Roche Inc.). IL-2 was administered by continuous infusion daily for 4 days and IFN was administered by intramuscular injection daily for 4 days; therapy continued for 4 weeks. Eight men and seven women were treated in this trial (median age, 61 years). Toxicity was moderate to severe with fatigue, nausea, vomiting, hypotension, and elevated blood urea nitrogen bunion and creatinine levels seen in all patients. Two patients achieved a complete remission and two patients achieved a partial remission. The median duration of response was 18 months. IL-2 and IFN is an active combination in the treatment of renal cell carcinoma and warrants further investigation.

Hormone profiles after intramuscular injection of testosterone enanthate in patients with hypogonadism.

To examine hormone levels after androgen replacement therapy (ART) in Japanese male patients with hypogonadism, nine Japanese male patients with hypogonadism (serum total testosterone (tT) or free testosterone (fT) levels of < or = 2.7 ng/mL or < or = 10 pg/mL, respectively; average age, 59 years) were enrolled. They were treated with 125 mg of testosterone enanthate by single intramuscular injection. Blood samples were collected on the morning of the day of treatment, pre-ART, as well as on days 1 to 7 and day 14 after administration. Serum levels of tT, fT, estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and sex hormone-binding globulin (SHBG) were determined. On day 1 after administration, the mean serum levels of tT and fT were 7.62 ng/mL and 23.22 pg/mL, respectively. Serum levels of tT and fT on day 14 after administration were lower than their pre-ART values. One patient exhibited abnormally high serum tT and fT levels of 19.6 ng/mL and 44.4 pg/mL, respectively. Serum levels of LH and FSH began to decrease gradually on day 5 after administration. Serum levels of SHBG did not change throughout the observation period. Serum levels of E2 increased 1.7 times on day 1 after administration but returned to its pre-ART value by day 14 after administration. The dose of testosterone enanthate for male patients with hypogonadism requiring ART should be determined carefully because some patients exhibited high serum levels of androgen beyond the physiological range and gonadotropin was suppressed in all treated patients.
