Clearance of radioactive xenon in evaluating microcirculation in patients with ischaemic heart disease.

In 158 patients with chronic ischaemic heart disease (IHD), the state of microcirculation was investigated, using local 133Xe clearance. With growing severity of the disease, muscular blood flow decreased. Local disturbances were accompanied also by changes in central haemodynamics (decrease in cardiac index, slow-down of blood flow in the pulmonary and systemic circulations, increased peripheral resistance), which were established by radiocardiography using 131I-labelled albumin. A distinct relationship was found between muscular blood flow and the intensity of intravascular microcirculatory disturbances (microthrombosis, intravascular thrombocyte aggregation, arteriolo-venular anastomoses). The authors recommend the use of 133Xe clearance for microcirculation study in patients with chronic IHD.

[Systemic hemodynamics and regional microcirculation in diffuse peritonitis].

A complex study of the central and peripheral hemocirculation in 134 patients and direct control of the microcirculation bed by the method of contact microscopy in 15 experiments in dogs were used in order to study the character of disturbances of microcirculation depending on the phase of diffuse peritonitis. It was established that a reactive of peritonitis is characterized by an active spasm of microvessels changed by their paralytic dilatation followed by stasis, aggregation of formed elements, edema and infiltration of tissues.

[Pulmonary microcirculation after distant gamma-therapy of breast and thyroid cancer (perfusion pulmoscintigraphy data)].

A study was made of an effect of irradiation on pulmonary microcirculation using perfusion pulmoscintigraphy indicators. Altogether 13 breast cancer patients (9 with left-side and 4 with right-side involvement) and 9 thyroid cancer patients were examined before and after radiation therapy. Absorbed doses in breast cancer were determined by phantom measurements, for thyroid cancer by calculation. Perfusion pulmoscintigraphy with labeled albumin macroaggregate preceded and followed radiotherapy. Accumulation of the radioactivity of the drug in different pulmonary zones was taken into account. Slight changes of the microcirculation index found after irradiation, lead to a conclusion that in radiation therapy of breast cancer one can observe improvement of the blood flow in the pulmonary arterial system whereas in thyroid cancer a similar effect in the irradiated upper pulmonary zone was not marked. Irradiation of the root of the lung produces a more powerful effect on the pulmonary blood flow. Taking into account a sharp suppression of the arterial blood flow in some lung cancer patients (decreasing chemotherapy efficacy) it is suggested that change of the blood content developing after irradiation be used in the planning of combined therapy of lung cancer.

[Hemorrheological changes in microcirculation caused by fats of different dosage, composition and mode administration].

Descriptions of the nature and pathogenesis of disturbances of the microcirculation induced by intravenous infusion of a triglyceride emulsion or by daily oral administration of a cholesterol-in-oil suspension are different and partially contradictory in the medical literature. Hyperlipemic disturbances of the microcirculation are characterized by a general decrease in the rate of flow reflected in all parameters of flow in the terminal blood vessels including a rise of the number of capillaries temporarily functionally isolated from the circulation and by an increase in capillary plasma skimming. The degree is dependent upon the amount of administered fat and on lipid levels. The disturbed capillary perfusion can be attributed to various rheological mechanisms. The hemodynamical relevance of the short-time hyperlipemia produced by intravenous infusion of a fat emulsion and the enduring hyperlipemia produced by the daily oral administration of a cholesterol-in-oil suspension differ. Some of the capillary-rheological bases of these differences are discussed.

Visualization of fluorescent erythrocytes in the microcirculation.

Erythrocytes loaded internally with FITC-BSA can be readily visualized in the microcirculation by using a television camera and a fluorescence microscope. Flow properties of the erythrocytes and their adherence to the vascular endothelium or erythrophagocytic cells can be observed. This procedure should also be useful to delineate the microcirculation under circumstances where infused free FITC-BSA can escape into the interstitial tissue.

[Microcirculation and hyperkeratosis. Pathogenetic hypothesis and treatment].

The orthokeratotic thickening of the epidermis would appear to follow the deceleration or permanent interruptions of the cutaneous microcirculation. This explains the development of calluses, corns and warts in areas where the skin is compressed between bone and a hard external support. Peripheral vasometric anomalies give rise to the same thing. Pentoxifylline, restoring the capillary microcirculation, has a clearly curative effect.

[Microcirculation and permeability of the blood-carrying capillaries in chronic hepatitis patients].

Microcirculation and transcapillary exchange were studied in 40 patients with chronic persistent hepatitis and in 32 patients with chronic active hepatitis (15 patients with a slow progressive course and 17 patients with a rapidly progressive course) using methods of conjunctive biomicroscopy, isotope intracutaneous capillarography and V. P. Kaznacheev's capillaro-venous method with a hydrostatic load. Significant disorders of the microcirculation (extravascular, vascular and intravascular) and pronounced disorders of transcapillary exchange resulting in the development of the syndrome of capillarotrophic insufficiency were revealed in the patients with chronic hepatitis. The degree of microcirculatory and transcapillary exchange changes was in direct relation to the activity of liver pathology.

The influence of carotid body stimulation on oxygen tension and microcirculation of various organs of the cat.

To determine the importance of chemoreflex control for microcirculation, we investigated tissue pO2 and local flow on the surface of muscle, kidney, liver, and heart under systemic hypoxia and under isolated hypoxic perfusion of the carotid body in anaesthetized (pentobarbital) and artificially ventilated cats. Under systemic hypoxia, local blood flow mainly decreased in muscle and kidney, whereas liver and heart showed a flow distribution mainly with a flow decrease. Under systemic hypoxia, tissue pO2 decreased to values in the range of 0 to 10 Torr. A linear relationship was found between the pO2 change under hypoxia and the normoxic pO2 in the investigated organs, indicating local regulatory mechanisms of microcirculation. Isolated hypoxic perfusion of the carotid body caused a microflow increase in kidney in about 63%, in liver in about 55%, and in heart in all cases. Hypoxic perfusion of the carotid body caused pO2 increases in kidney and liver with mean pO2 values of 3 and 4 Torr, respectively, whereas, it produced no significant pO2 change in heart and muscle. The pO2 changes in kidney and liver significantly depended on the normoxic basic tissue pO2 level, indicating, in connection with local flow increases, a redistribution of local flow in these organs.

[Collagen diseases and microcirculation].

The microcirculation (terminal vessels with a calibre of less than 30 mu) was examined by means of capillaroscopy which was correlated with histological studies. According to the classification of vasculitis, microvasculitis is a pathological process involved in a number of collagen diseases. The study of the cutaneous lesions of leukocytoclastic angiitis is one of the best method for approaching the study of these microangiopathies, especially in the context of hypersensitivity vasculitis. The study of the microcirculation may also be valuable in other connective tissue diseases (various forms of necrotic vasculitis, lupus, scleroderma, polydermatomyositis) and in Behçet's disease. However, this study is only in its preliminary stages and the data of capillaroscopy need to be interpreted critically.

[Effect of short-term cerebral ischemia on the microcirculation in the rat mesentery].

The effect of brain ischemia, produced by constriction of the common carotid arteries, on microcirculation in the mesentery was studied in experiments on rats. The spreading and intensity of microcirculatory disturbances were dependent on duration of ischemia and post-ischemia. The state of systemic hemodynamics and microcirculation in the mesentery was compared. Possible mechanisms of the microcirculatory disturbances are discussed.

[Disorders of microcirculation in diffuse toxic goiter and their correction in the early postoperative period].

A study was made of microcirculation in 45 patients suffering from diffuse toxic goiter at the early postoperative period with the aid of microphotography of the bulbar conjunctiva vessels, and investigation of the rate of the isotope elimination from the intradermal depot and the capillary permeability by V. P. Kaznacheev's method. The data obtained permitted to draw a conclusion that on admission to the clinic patients with toxic goiter were suffering from arteriolar spasm, had poor vascular pattern in the bulbar conjunctiva, the sledge phenomenon in small venules, an acceleration of the isotope elimination from the intradermal depot and an increase of the capillary permeability. Preoperative preparation improved the microcirculation indices, without completely normalizing them, however. Microcirculatory disturbances reached their maximum the first two days after the operation and declined by the 3rd-5th day. Recommendations on correction of the microcirculatory disturbances are given.

Functional assessment of the pulmonary microcirculation during postnatal development.

It has been well documented that the lungs are able to remove or metabolize several vasoactive substances passing through the pulmonary circulation. One such function, the conversion of angiotensin I to angiotensin II and hydrolysis of bradykinin to inactive peptide fragments is performed by angiotensin-converting enzyme (ACE). Although a precise role of these functions for systemic vasoregulation has not been defined, measurement of pulmonary metabolic capacity has the potential for providing substantial information about the integrity and physiology of the pulmonary microcirculation. Studies are described using 3H-benzoyl-phenylalanyl-alanyl-proline (a synthetic substrate for pulmonary ACE) in developing conscious lambs and older sheep. Outflow curves from indicator dilution measurements contain information regarding apparent kinetics of ACE activity. Using a nonlinear model for saturable pulmonary metabolic functions, the derived data demonstrate a) a marked increase in Vmax, the apparent maximal velocity of ACE, with age; and b) no significant change with age in apparent Km, the concentration at which the velocity of ACE is one-half Vmax. This approach has the potential for providing a biochemical tool for examining postnatal growth of the pulmonary microcirculation as well as functional integrity of the pulmonary endothelium during a variety of conditions.

[Alpha blockers and microcirculation (author's transl)].

The new compounds such as Prazosin are antihypertensive agents. Their action on blood pressure are a consequence of a direct action on vascular smooth muscle with little or no effect on cardiac output. This action is very important to reduce total peripheral resistance. In our Vascular Laboratory, we study 14 patients treated by Prazosin. They were all studied by capillaroscopy, ultrasonic investigations and plethysmography. Since Prazosin is primarily an alpha receptor blocker, it might be expected to have an action in microcirculation and in vasospastic diseases. We don't find modifications of microcirculation in seven people treated for hypertensive disease. Among seven patients with Raynaud phenomenon, we have only a good result in one case (Buerger disease) and two cases with a little increase of the fingertips blood flow.

[Substance P and microcirculation during stress].

The experiments have been made on rats with the use of animals' immobilization on the back during 1 or 24 h as a stressor. Intravital study of the microcirculation in the rat mesentery has shown that P substance in a concentration of 7 X 10(-8) M aggravated the disturbances caused by the immobilization, while antiserum to P substance with an activity of 150 ng SP/equiv. normalized the microcirculation. The action of P substance and antiserum was similar as regards disturbances of vascular permeability for colloid carbon particles. It is concluded that P substance participates in the mechanisms of microcirculatory and vascular permeability disturbances during stress.

Acute effects of Escherichia coli endotoxin on the pulmonary microcirculation of anesthetized sheep structure:function relationships.

Infusion of Escherichia coli endotoxin into sheep causes a syndrome analogous to the adult respiratory distress syndrome. Physiologic measurements show an initial phase of marked pulmonary hypertension followed by a phase characterized by the production of large quantities of protein-rich lung lymph. The present study relates the structural changes that occur during endotoxemia to concomitant functional changes. In five anesthetized open-chest sheep, we monitored pulmonary and systemic artery pressure for a 1 hour baseline period and for 4 hours after the start of E. coli endotoxin infusion (1.25 microgram/kg, intravenously). We also measured cardiac output, arterial blood gases and pH, and number of circulating leukocytes. In addition, we sequentially biopsied random lobes from the lungs of each sheep at baseline and at 15, 30, 60, 120, 180, and 240 minutes after the start of endotoxin. Five control sheep were treated identically except that they received saline instead of endotoxin. By 15 minutes after the start of endotoxin infusion, light microscopy revealed margination and accumulation of leukocytes in the lungs' microcirculation. Counts of the number of peripheral lung granulocytes in biopsy specimens showed a 3-fold increase above baseline by 15 minutes and a 6-fold increase by 4 hours. By electron microscopy, the leukocytes were identified as both granulocytes and lymphocytes, present in approximately equal numbers. Some granulocytes were fragmented, and specific granules were found free in the vessel lumen. By 30 minutes, some leukocytes were migrating into the interstitium. By 60 minutes, interstitial edema was seen, and there was focal endothelial cell damage. Correlation of the structural with the physiologic changes shows that the initial accumulation of leukocytes in the microcirculation occurs when pulmonary hypertension develops. The migration of leukocytes into the interstitium and endothelial cell damage precedes the physiologic changes that we interpret as increased pulmonary vascular permeability. Since gram negative septicemia is a frequent occurrence in the adult respiratory distress syndrome the changes described here may be similar to the alterations that occur early in the development of the syndrome in man.

Microcirculation of the pancreas: a correlative study of intravital microscopy with scanning electron microscopy of vascular corrosion casts.

The microcirculation of the pancreas in anesthetized rats was examined by intravital microscopy. Scanning electron microscopic observation of vascular corrosion casts of the same parts of the pancreas as viewed light microscopically in vivo, allowed detailed analysis of microcirculation in particular vessels. Blood vessels with flowing blood cells were clearly observed under incident ultraviolet illumination following systemic injection of an intravascular fluorescent tracer. The islets of Langerhans were easily identified by their brighter appearance than the exocrine portions under incident ultraviolet illumination. Blood leaving the islets passed through the insulo-acinar portal vessels into the capillary network around the acini. The periductular plexus received blood from the periacinar capillary network via either venules or capillaries as well as directly from the inter- or intralobular arteries. The periductular plexus also received blood from the islets located close to the ductules. The results obtained show blood in the pancreas flowing in a manner in which it carries insular secretions in high concentrations through short and direct vascular routes from the islets to the pancreatic exocrine tissues, including their duct system.

[Disorders of microcirculation in patients with acute surgical diseases of the biliary tract].

The authors provide the results of studying the microcirculation in 85 patients with acute cholecystitis and 56 patients with mechanical jaundice caused by choledocholithiasis. The data were obtained with the aid of bulbar capillaroscopy during conservative and surgical treatment. Considerable alterations were detected in all the components of the microcirculation (perivascular, vascular, intravascular), correlating with the gravity of destructive lesions in the gallbladder, duration and degree of jaundice. The times and degree of the normalization of microcirculatory disorders were defined.

[Microcirculation and central hemodynamics of hypertension patients during long-term hypotensive therapy].

Microcirculation and central hemodynamics were studied in 62 patients with essential hypertension and in subjects with borderline arterial hypertension during long-term (up to 5 years) hypotensive therapy. Systematic therapy was found to be efficacious in the aspect of the improvement of the resistive component of microcirculation and blood pressure drop. Occasional treatment did not lead to significant beneficial shifts in micro- and macrocirculation.
