Comparative study of the effects of two once-a-month injectable steroidal contraceptives (Mesigyna and Cyclofem) on glucose metabolism and liver function. United Nations Development Programme/United Population Fund/World Health Organization/World Bank, Special Programme of Research, Development and Research Training in Human Reproduction, Task Force on Long-acting Systemic Agents for Fertility Regulation.

A 1-year, longitudinal, multicenter study was undertaken to monitor the effects of two once-a-month, injectable contraceptive preparations, Mesigyna (norethisterone enanthate, 50 mg, and estradiol valerate, 5 mg) and Cyclofem (medroxyprogesterone acetate, 25 mg, and estradiol cypionate, 5 mg) on carbohydrate metabolism and liver function. A total of 357 volunteers from four centers (Hangzhou, Havana, Jakarta, and Shanghai) were monitored during one pretreatment cycle, at the end of the ninth injection interval and during one posttreatment cycle, approximately 3 months after the 9-month period of treatment was completed. Both preparations induced minor changes in glucose tolerance, with a slight elevation of glucose response and minimal changes in insulin response to a glucose challenge. These changes were greater with Mesigyna but were of no clinical relevance in either group. With regard to liver function, both preparations induced a rise in bilirubin, a decrease in alkaline phosphatase, and no change in enzyme serum levels. However, these changes were of small magnitude and levels remained well within the normal range in all subjects.

[Trouble of liver function in polymyalgia rheumatica (author's transl)].

The possible existence of liver function disease such as cholestasis during temporal arteritis or polymyalgia rheumatica has been recently discovered. Two cases are reported of cholestasis without jaundice during polymyalgia rheumatica. The altérations most frequently observed are the elevation of the alcalin phosphatases, 5' nucleotidases, gamma G.T. and the retention of BSP. The syndrome disappears by treatment as well as the rheumatologic disease. It seems to be interesting to diagnose polymyalgia rheumatica through this trouble of liver funcion.

Use of rose bengal iodine 131-tagged isotope for testing liver function in 4000 patients.

Within 12 years, 4000 patients with hepatic lesions, were examined by the author. Examinations included: clinical, laboratory, biopsy, together with radioactive Rose-bengal 131I. RBR test prooved to be more sensitive than known laboratory liver function tests. In diagnosis of chronic hepatitis and cirrhosis, histological examinations remain to be decisive, though biopsy is not always recommended from view point of prognosis. The isotope procedure has a great advantage besides to its sensitive registration of liver cells functions, it offers good informations about flowness of bile tract. The bladder diagrams increase by action of milk-chocolate meals (24 hours), after which, urine analysis, estimation of blood activity and resting-liver activity can be of value. Harm due to radiation in RBR test is a minimum, so it can be repeated several times in order to follow progression of liver diseases. RBR examination is recommended for all big hospitals.

Role of Ito cells in the liver function.

Ito cells (perisinusoidal fat-storing cells, stellate cells, lipocytes) of the liver are mesenchymal cells located in the space of Disse. They are the main place of vitamin A storage in characteristic lipid droplets. Moreover, they demonstrate synthetic activity of collagens and other extracellular matrix proteins involved in hepatic fibrosis. Their long, contractile cytoplasmic processes encompass the sinusoids and can affect sinusoidal blood flow. Because of their location in the space of Disse and connection with other sinusoidal and parenchymal cells, they are probably involved in local neurotransmission and paracrine regulation of numerous liver functions. As a source of cytokines, prostaglandins and other bioactive substances, produced in the response to noxious agents, they play a crucial role in the mechanisms of liver injury, regeneration and fibrosis. This article is a review of the current state of knowledge on Ito cells structure and function.

[Hepatic clearance of 99mTc-p-butyl HIDA in liver diseases. Correlations with routine hematochemical parameters of liver function].

The aim of the present investigation was to study the clearance of 99mTc-p-butyl IDA in some acute and chronic liver diseases, it being considered that the typical parameters obtained with this method are as indicative as any of the others put forward for the study of liver function using radioisotopes. 46 subjects were examined: 6 with acute hepatitis, 10 with chronic hepatitis, 18 with liver cirrhosis and 12 with dyspepsia but otherwise normal haematochemical tests. Two basic 99mTc-p-butyl IDA clearance parameters, Tu (semi-take up time) and Te (semi-excretion time), were determined plotting the data obtained using a Gamma-Camera on semi-logarithmic paper. Mean Tu values were as follows: 5'06'' +/- 1'24'' in dyspeptics, 12'30'' +/- 6'31'' in subjects with acute hepatitis, 6'30'' +/- 1'45'' in subjects with chronic hepatitis and 13'30'' +/- 4'30'' in subjects with cirrhosis. The values were: 34'30'' +/- 4'30'' in dyspeptics, 49'54'' +/- 2'36'' in subjects with acute hepatitis, 42'24'' +/- 12'24'' in subjects with chronic hepatitis and 65'30'' +/- 39'36'' in subjects with cirrhosis. The Tu parameter was found to be delayed more significantly in cirrhotic patients and less in subjects with acute or chronic hepatitis, compared to dyspeptics with normal haematochemical parameters. Te was significantly delayed in subjects with cirrhosis and acute hepatitis, while there was no difference for subjects with chronic hepatitis. Of the routine haematochemical tests, the albumin/gamma-globulin ratio and unconjugated bilirubin were found to correlate significantly with the Tu parameter, whereas conjugated bilirubin was found to bear a significant correlation to the Te parameter.
