[Study of the correlation between drug exposure and blood concentration during cyclosporine microemulsion absorption phase in Chinese adult liver transplant recipients].

OBJECTIVE: To analyze the correlation between drug exposure (AUC0-4) and blood concentration of different sample points during Neoral absorption phase in Chinese adult liver transplant recipients, and to evaluate the possibility of using post-dose 2 hour level (C2) as a surrogate marker of AUC0-4 for the therapeutic drug monitoring (TDM) of Neoral. METHODS: Neoral levels at 0, 1, 2, 3, and 4 hours (C1, C2, C3, C4) after morning dose of 22 de novo Chinese adult liver transplant recipients were monitored during different posttransplant periods. Liner regression was used to analyze the correlation between CsA concentration at different time points and the AUC0-4. RESULTS: The best correlation was found at C2, while the correlation of C0 was the lowest. During following-up, the correlation between C2 and AUC0-4 was very stable. CONCLUSION: C2 had the best correlation with drug exposure. This correlation is very stable. C2 may be used as a good surrogate marker of AUC0-4 for the TDM of Neoral.

[Correlation of mercury, selenium and other elements in the tissues of fishes from the regions at different mercury exposure level].

The contents of selenium and other elements in fish liver and muscle tissues collected from mercury polluted area of Wanshan, Guizhou province of China and non-known mercury polluted one of Beijing were determined with instrumental neutron activation analysis, and that of mercury was determined with atomic fluorescence spectrometry. The correlation among the determined elements, especially between mercury and selenium, in the fish tissues were studied. For most of the elements significant difference of elemental content was found between tissues of liver and muscle, and mostly the content in liver was higher than that in muscle. It was interesting to note that the average content of mercury in Guizhou fish liver was 25-fold higher than that in Beijing one, and 5-fold higher in Guizhou fish muscle than that in Beijing one. For most of the elements no significant difference on the average content was found between the same tissues of the two areas. The molar ratio of Hg/Se increased with the increasing of Hg content at the low Hg level, but the alteration trend became gently and reached to a higher constant value (about 0.2) at the higher Hg level. Only the element of bromine and rubidium was found to have strong linear correlation between the two different tissues of liver and muscle. Though certain correlation existed among the different elements of fish tissues from the two areas, the most significant and the closest one existed between the mercury and selenium of muscle tissue of Guizhou fishes. A closer correlation was also found between them of liver tissue of Guizhou fishes. As for Beijing fish tissues, the correlation of mercury and selenium only existed in liver but no correlation of them was found in muscle. Our results confirm the fact that a certain interaction exists between mercury and selenium in organisms including fish, and it is more evident as the mercury-exposed level is higher. Some significant correlation was also observed between other elements, such as potassium and sodium, cesium and rubidium, iron and zinc, in fish tissues and it could be well explained by their similar chemical and physical properties or their different biological functions.

Correlation between preoperative symptom scores, quality-of-life questionnaires, and staging with computed tomography in patients with chronic rhinosinusitis.

BACKGROUND: This study evaluates the correlation between preoperative symptoms, quality-of-life questionnaires (chronic sinusitis survey and 20-Item Sinonasal Outcome Test [SNOT-20]), and staging on computer tomography (CT). It consisted of a prospective cohort study of all consecutive patients undergoing surgery for medically nonresponsive chronic sinusitis, which took place at a tertiary care center. METHODS: Two hundred twenty-one patients completed the Chronic Sinusitis Survey (CSS) questionnaire and the SNOT-20. A visual analog scale (VAS) symptom score was completed also. The average age of the patients was 44.5 years and the male/female ratio was 1.3:1. The symptom scores of all three questionnaires were compared with the Lund-MacKay CT scan score of the sinuses. RESULTS: The median Lund-MacKay CT score was 12 (mean +/- SD, 12.7 +/- 5.7). There was no significant correlation between the SNOT-20 questionnaire and the Lund-MacKay CT score (p = 0.026; p = 0.764) and between the CSS and the Lund-MacKay CT score (p = -0.158; p = 0.058). Furthermore, there was no significant correlation between a single VAS symptom score relating to overall sinonasal symptom severity and the Lund-MacKay CT scan score (p = 0.135; p = 0.121). However, a weak but statistically significant correlation was found between the VAS score based on the sum of five sinonasal symptoms and the Lund-MacKay CT scan score (p = 0.197; p = 0.020). CONCLUSION: The score, based on the sum of five sinonasal VAS symptoms, correlates to the disease severity as measured by the Lund-MacKay CT scan score. The SNOT-20 questionnaire and the CSS symptom score do not correlate to the Lund-MacKay CT scan score, although a correlation was found between the CSS score and the Lund-MacKay CT score in various sub-groups of our patient population.

Correlation and agreement analysis between standard videotape and digital echocardiography: a cross-sectional study.

BACKGROUND: Several studies have analyzed the correlation between digital and standard videotape echocardiographic images. The advantages of digital echocardiography are a faster exam, lower costs and a greater number of exams performed. Our study's aim was to evaluate the correlation and agreement between cardiac dimensions measured by M-mode and two-dimensional echocardiography, using the Echo Off Line free software, and by standard echocardiography. METHODS: This was a cross-sectional, randomized study, in 56 patients, whose images were acquired during echocardiographic examination. Left ventricular systolic and diastolic diameters, septal and posterior wall thicknesses, and left atrial and aortic diameters were measured using the Echo Off Line program. These measures were compared to those acquired using the standard method by a different sonographer, through correlation and agreement analysis as described by Bland and Altman. Values of p<0.05 were considered significant. RESULTS: The correlation index between the methods varied from 0.85 to 0.98 (Pearson's correlation coefficient) for all the variables studied. The analysis showed that there was good agreement between the methods for most of the measures, the mean difference being close to zero. The variability in absolute values, for most measures, did not reach clinical significance. The method's reproducibility (intra-observer) was adequate. This measurement tool demonstrated correlation indices similar to those in the literature consulted, as well as adequate reproducibility indices for intra-observer measures. Its low cost, compared to that of imported workstations on the market, means it can be widely used without significant costs (free software). CONCLUSION: There is a direct correlation and good agreement between M-mode and two-dimensional echocardiographic measures of cardiac dimensions obtained by the Echo Off Line program and by the standard videotape-based method. The Echo Off Line program can be used routinely in echocardiography laboratories.

Coronary atherosclerosis and hair elements contents. A correlation study of 102 autopsy subjects using the energy dispersive X-ray fluorescence analysis and applying an atherometric system.

An atherometric system (AS) was used to study the coronary atherosclerotic lesions of a consecutive series of 102 autopsies divided into two groups according to the principal cause of death, the myocardial infarction and the low atherosclerosis groups, respectively (MIG 67 cases, LAG 35 cases). The hair elements contents (K, Ca, Zn, Br, Sr, Pb, S, Cl, Cu, and their rates Ca/Sr, Zn/Cu and Ca/K) were determined by energy dispersive X-ray fluorescence (EDXRF) analysis. A PEARSON correlation analysis was done between each element and the following descriptive arterial variables of the AS: fatty streaks (X), fibrous plaques (Y), severe (complicated plus calcified) plaques (Z) and total atherosclerosis (sigma) and the three weighted indices of the AS: obstruction (omega), stenosis (P) and benignity (B). These variables were obtained from the three major epicardial branches of the coronary arteries (right RC, left anterior descending LAD and left circumflex LC). From a total of 189 possible correlations between the 9 elements and their 3 combinations and the 7 variables of the AS in the three coronary arteries, 86 (45.5%) were significant. The fatty streak was the best correlated variable. It correlated with all elements with the only exception of chloride. The fibrous plaque was poorly correlated. The severe plaques and total atherosclerosis correlated mainly with Zn and Pb in the MIG and Sr in the LAG. The weighted indices have the same behaviour. Also some correlations were found between K, Ca, Zn, Br, Sr, Pb, Ca/Sr, Zn/Cu and Ca/K ratios with some of the variables of the AS in each coronary artery.

The risk factors of fall and their correlation with balance, depression, cognitive impairment and mobility skills in elderly nursing home residents.

OBJECTIVES: The purpose of this study was to investigate risk factors of fall and to examine their correlations with balance, depression, cognitive impairment and mobility skills in elderly nursing home residents. METHODS: This study included 30 elderly people living in Seyranbaglari Nursing Home in Ankara City Centre, Turkey and was carried out in 2004. Risk factors for fall were obtained through individual interviews and observation. Cognitive function was assessed by Mini Mental State Examination (MMSE). The Geriatric Depression Scale (GDS) was used for determining the depression status of the elderly. Balance of the subjects was evaluated by the Berg Balance Scale (BBS) and the Get Up and Go Test (GUGT). In addition, mobility skills were assessed by Rivermead Mobility Index (RMI). The correlation analysis was used for establishing the correlations of the total number of risk factors with the other parameters. In addition, the correlations among all parameters were evaluated. RESULTS: At the end of the study, it was found that the correlation among all parameters was significant (p<0.01, p<0.05). For example, there was a significant correlation of MMSE score with the total number of risk factor for falls, GDS, BBS, GUGT and RMI. The RMI score was also correlated with GDS, MMSE, BBS and GUGT scores. CONCLUSION: The result of the present study confirm that impairment of cognitive function, balance and mobility skills, and depression increases the risk of falling. In addition, all the parameters affect each other, too. There is a vicious circle among these parameters. If this vicious circle is not broken, an increasing risk of fall will be inevitable. That is why this information will be useful for patients therapy, and for development of fall prevention programs for elderly people.

18F-FDG PET and CT/MRI in oral cavity squamous cell carcinoma: a prospective study of 124 patients with histologic correlation.

UNLABELLED: Accurate evaluation of primary tumors and cervical lymph node status of squamous cell carcinoma (SCC) of the oral cavity is important to treatment planning and prognosis prediction. In this prospective study, we evaluated the use of 18F-FDG PET, CT/MRI, and their visual correlation for the identification of primary tumors and cervical nodal metastases of SCC of the oral cavity with histologic correlation. METHODS: One hundred twenty-four patients with pathologically proven diagnoses of oral cavity SCC underwent 18F-FDG PET and CT/MRI within 2 wk before surgery. We interpreted 18F-FDG PET, CT/MRI, and visually correlated 18F-FDG PET and CT/MRI separately to assess the primary tumors and their regional lymph node status. We recorded lymph node metastases according to the neck level system of imaging-based nodal classification. Histopathologic analysis was used as the gold standard for assessment of the primary tumors and lymph node involvement. We analyzed differences in sensitivity and specificity among the imaging modalities using the McNemar test. The receiver-operating-characteristic (ROC) curve and calculation of the area under the curve were used to evaluate their discriminative power. RESULTS: The accuracy of 18F-FDG PET, CT/MRI, and their visual correlation for the identification of primary tumors was 98.4%, 87.1%, and 99.2%, respectively. The sensitivity of 18F-FDG PET for the identification of nodal metastases on a level-by-level basis was 22.1% higher than that of CT/MRI (74.7% vs. 52.6%, P < 0.001), whereas the specificity of 18F-FDG PET was 1.5% lower than that of CT/MRI (93.0% vs. 94.5%, P = 0.345). The sensitivity and specificity of the visual correlation of 18F-FDG PET and CT/MRI were 3.2% and 1.5% higher than those of 18F-FDG PET alone (77.9% vs. 74.7%, P = 0.25; 94.5% vs. 93.0%, P = 0.18, respectively). The area under the curve obtained from the ROC curve showed that 18F-FDG PET was significantly superior to CT/MRI for total nodal detection (0.896 vs. 0.801, P = 0.002), whereas the visual correlation of 18F-FDG PET and CT/MRI was modestly superior to 18F-FDG PET alone (0.913 vs. 0.896, P = 0.28). CONCLUSION: 18F-FDG PET is superior to CT/MRI in the detection of cervical status of oral cavity SCC. The sensitivity of 18F-FDG PET for the detection of cervical nodal metastasis on a level-by-level basis was significantly higher than that of CT/MRI, whereas their specificities appeared to be similar. Visual correlation of 18F-FDG PET and CT/MRI showed a trend of increased diagnostic accuracy over 18F-FDG PET alone but without a statistically significant difference, and its sensitivity was still not high enough to replace pathologic lymph node staging based on neck dissection.

Correlation of HBV and HCV with CH, LC, HCC in liver biopsied tissue at Rajavithi Hospital.

Retrospective study in Clinicopathology of 66 surgical liver tissue from adult Thai patients admitted at Rajavithi Hospital, in Bangkok, during December 2002-September 2003 (10 month periods). The main purposes are: 1) To find the correlation of HBV, HCV with CH, LC, HCC. 2) To compare the correlation of Hepatocyte, AFP, CEA (IHC) in malignant cells, which one is the best usage to confirm the diagnosis of HCC in both primary and metastasis. 3) To review the clinicopathology of all these 66 liver samples. The results were significant correlation of HBsAg (serology) with HCC (p = 0.010), and also significant correlation of HBsAg (IHC in liver tissue) with CH, LC (p = 0.038, 0.021 respectively). Although no significant correlation (p > 0.05) of HCV (positive anti HCV) with CH, LC, HCC; the causes due to the small sample sizes and short period study are possibly bias factors. The authors concluded that Hepatocyte or Hep-Par I is the best immunocellular marker for malignant liver cells both in primary and metastasis (p < 0.001). The AFP, CEA show no correlation (p = 0.999, 0.670). The authors found other interesting non-viral related liver disease, common, uncommon, tumor and tumor-like (pseudotumor) lesions in the liver from the present study. The results of significant correlation of HBV (HBsAg) with CH, LC, HCC is one good evidence to further support The National HBV Vaccine Program for the uninfected population, which has been sponsored by the Thai Government, The Ministry of Public Health since 1992 and be one of the best and successful Thai Public-Health Policy.

A study on the prognostic value of cyclins D1 and E expression levels in resectable gastric cancer and on some correlations between cyclins expression, histoclinical parameters and selected protein products of cell-cycle regulatory genes.

Cyclins condition the course of a cell cycle through the activation of appropriate serine-threonine kinases. Any variation in the cyclins' expression result in pathologies of the cell division, including neoplastic proliferation. Activity of the complexes of cyclins D1 and E with appropriate cyclin-dependent kinases may be inhibited by protein P21 (WAF1/CIP1) which functions as a cell growth cycle inhibitor. As yet, there have been rather few reports on the prognostic value of this cyclin expression assessment in gastric cancer, the kind of neoplasm still characterized by very poor prognosis. The study aimed at the assessment of expression levels of cyclins D1 and E in surgically removed gastric cancers, including the analysis of this prognostic value parameter, and attempted to determine some correlations between the expression of the examined cyclins and selected histoclinical and molecular parameters such as: patients' age and gender, histological type according to the Lauren classification, cancer stage (TNM), degree of histological malignancy (G) and level of expression of the cell-cycle regulatory genes protein products--P53, P21, P27. Immunohistochemical analysis was performed on specimens obtained from radical stomach resections of 80 patients treated in the period 1992-1997 for gastric cancer stage I-IIIB (TNM-UICC) at the Department of Surgical Oncology, Medical University of Lodz. For immunohistochemical examinations, the LSAB system was used, designed for assessment of antigen expression. In statistical analysis, Fisher's exact test was applied to evaluate correlations between the analyzed variables and Mantel-Haenschel's test to evaluate their collinearity. For the evaluation of the effect of the analyzed variables on postoperation survival and recurrence-free survival the Cox regression model was used. When analyzing the prognostic value and survival period in association to the cyclins D1 and E expression levels, a statistically significant correlation was found only in relation to cyclin E expression: a survival period of minimum 5 year duration was significantly higher in the group displaying a negative, or only faintly positive, reaction to the presence of cyclin E, than in the group with a strongly positive response. Moreover, the analysis showed statistically significant non-linear dependence between the histological type of cancer in the Lauren classification as well as a degree of histological malignancy and the level of cyclin E expression, and a negative correlation between the level of cyclin E expression and the stage of cancer; In addition, a positive correlation between the level of P53 and cyclin E expression as well as statistically significant non-linear correlation between the level of cyclin E expression and the level of protein P21 expression was observed. However, no statistically significant correlations were found between the level of expression of the two cyclins and the level of protein P27 expression or between the levels of cyclin D1 and E expression in gastric cancer. Out of the two types of evaluated cyclins only cyclin E can be considered a significant regulatory factor and a useful prognostic parameter in gastric cancer.

[Expression of AKT2, cyclin D1, and MMP-9 and their correlations to clinicopathologic features of non-small cell lung cancer].

BACKGROUND & OBJECTIVE: AKT2, a serine/threonine kinase, is confirmed to be an oncogene. Abnormal expression and activation of AKT2 is observed in many kinds of tumor tissues. AKT2 is associated with the proliferation and invasion of cancers. This study was designed to investigate the expression of AKT2, Cyclin D1, and matrix metalloproteinase-9 (MMP-9) in human non-small cell lung cancer (NSCLC) tissue and their correlations to clinicopathologic features of NSCLC. METHODS: The expression of AKT2, Cyclin D1, and MMP-9 in 68 specimens of NSCLC, 38 specimens of corresponding adjacent tissues, and 14 specimens of no-cancerous lung tissue were assessed by immunohistochemistry; their correlations to clinicopathologic factors were analyzed using Chi-square test. RESULTS: The positive rates of AKT2, Cyclin D1, and MMP-9 were significantly higher in NSCLC than in adjacent tissues and no-cancerous lung tissues (91.2% vs. 3.8%, 76.5% vs. 0%, 72.1% vs. 13.5%, P<0.05). The expression of AKT2 wasn't correlated to age, sex, histologic subtype and differentiation, and TNM stage of NSCLC patients (P>0.05), but was correlated to lymph node metastasis (P<0.05). The expression of Cyclin D1 and MMP-9 was correlated to lymph node metastasis and differentiation of squamous cell carcinoma (P<0.05); the expression of MMP-9 was correlated to TNM stage. The expression of AKT2 was positively correlated to the expression of Cyclin D1 and MMP-9 (P<0.05). CONCLUSIONS: AKT2, Cyclin D1, and MMP-9 is related to development of lung cancer. The overexpression of Cyclin D1 and MMP-9 may relate to AKT2 regulation.

[Long-term dynamic of fecal corticosterone and its ecological and social correlates in males of great gerbil (Rhombomys opimus Licht.). Non-invasive approach in studies of stress in natural populations].

The relationship between fecal corticosterone concentrations and characteristics of the environment and population demography were studied in adult male gerbils (Rhombomys opimus Licht.) at the southern border of Kyzylkum desert (Reserve "Ecocentre Dzeiran", Bukhara region, Republic Uzbekistan) in spring and fall seasons from 1999 to 2004. We extracted hormones from air-dried fecal samples and analyzed their concentrations by radioimmunoassay (Gerlinskaya et al., 1993). An analysis for year-specific relationships between hormone concentrations and environmental variables of temperature and precipitation using Pearson's r statistic revealed that corticosterone concentrations correlated positively with total precipitation in January and February and negatively with precipitation during March and April. There was also a significant negative relationship between fecal corticosterone and the number of hot days in March (>20 degrees C). Demographic variables that characterized population densities (percent of burrow systems occupied, mean and maximum number of burrow systems/1 ha, number of females in the burrow system) correlated positively with corticosterone concentrations in feces in the beginning of spring, but these relationships were small compared with mean concentrations of corticosterone for the entire spring season that were strongly and positively correlated with number of gerbils, including all pups emerged, in burrow systems owned by one male (within its home range). In contrast, correlation coefficients of corticosterone concentrations with characteristics of feeding resources in the spring were low and negative. In the long-term perspective (interannual comparison), mortality among adult males was highly negatively correlated with mean corticosterone concentrations in the beginning of spring, which is within the period of maximum reproductive effort and potential stress. Body mass was independent of corticosterone concentrations in males in either the beginning of spring, or during the whole spring. In the fall, mean concentrations of fecal corticosterone in males was positively correlated with the number of days from June to October with mean daily temperatures exceeding 30 degrees C, and with percent of burrow systems where at least one adult, > or = 1 year old gerbil had survived. Mortality from fall to spring of the next year and the fall body mass did not correlate with concentrations of corticosterone in feces collected in the fall. When we analyzed corticosterone concentrations in spring seasons of all years combined using a stepwise regression analysis of a sampling of individual males (we analyzed residuals after withdrawal of year effect) on a set of variables representing habitat resources, distances between nearest neighbor males, and variables representing group demography we found low R2 values not exceeding 0.17. Within the six-year period, concentrations of corticosterone in the spring related negatively with abundance of annual herbs and positively with number of females in a male's social group. When only years of high density were analyzed, fecal corticosterone concentrations in males in the spring were again negatively determined by abundance of herbs, as well as by the nearest neighbor distance, and positively determined by the number of females within a male's home range. At the beginning of spring the only determinants were distance to the nearest neighbor male and number of females. In years of low density none of the variables were found to affect corticosterone levels during the whole spring, while in the beginning of spring only partial regression coefficients of abundance of herbs were negative and significant. Stepwise logistic regression analysis revealed positive dependence (P = 0.05) of disappearance of adult males during summer drought on concentrations of fecal corticosterone in the spring, but only when burrow systems with at least one adult (male or female) surviving after the summer were considered. Our results provide evidence ground for the assumption that in a desert rodent with non-regular population fluctuations such as the great gerbil, density may be more suppressed by external factors and not by density dependent mortality mediated by stress. Density dependent increases of stress caused by intense reproductive effort occurred when feeding and climatic conditions were favorable to compensate for negative effects on survival. However, in individual gerbils mortality mediated by stress can take place because we found higher stress in the beginning of spring in males, which did not occur in the population after the summer drought.

Correlation of carotid stenosis diameter and cross-sectional areas with CT angiography.

PURPOSE: Carotid stenosis quantification traditionally uses measurements of narrowest stenosis diameter. The stenotic carotid lumen, however, is often irregularly shaped. New PACS workstation tools allow for more precise calculation of carotid geometry. We compare the narrowest stenosis diameter with 2D area stenosis measurements, with the hypothesis that the narrowest diameter is a good predictor of the more precise area measurement. METHODS: Two neuroradiologists evaluated 178 stenosed carotids in a blinded protocol. Carotid artery bulb stenosis was identified on axial CT angiography and measured in millimeters at its narrowest diameter. An AGFA Impax 4.5 Volume Tool (VT) using Hounsfield units was used to estimate the cross-sectional area of the contrast luminogram. Pearson correlation coefficients were calculated between the millimeter stenosis and the VT area, as well as between the VT area and the calculated area (radius based on narrowest diameter). Regression analysis was performed with the VT area and narrowest diameter datasets. RESULTS: Excellent interobserver correlation (correlation coefficients, 0.71-0.85; 2-tailed significance = .01) permitted averaging of measurement data. There is excellent correlation between the VT area and the narrowest diameter (correlation coefficient, 0.88; n = 176). The VT area was generally greater than the calculated area by an average of 2.77 mm2. There was excellent correlation between the VT area and the calculated area (correlation coefficient, 0.87; n = 176). Regression analysis shows the ability of the diameter measurements to predict corresponding area stenosis. CONCLUSION: Although some carotid stenoses are irregularly shaped and noncircular, measurement of the narrowest stenosis is a reasonably reliable predictor of the cross-sectional area.

[Correlations among GDx-variable corneal compension, optical coherence tomography, and Heidelberg retina tomograph and relationships between these structural parameters and visual field indices].

PURPOSE: In glaucoma patients, to compare the correlations among parameters measured by Heidelberg Retina Tomography(HRT) and retinal nerve fiber layer thickness (RNFLT) measured by GDx-variable corneal compensation (GDx-VCC) and optical coherence tomography (OCT), and to assess the relationship among these structural parameters, and visual field damage. METHODS: A unilateral eye of 110 patients with open-angle glaucoma were included in this study. Each individual was analyzed by HRT (version 3.04), GDx-VCC (version 5.3.2), and OCT-1 (version A6 X1). The correlations among these parameters regarding global area, superior sector, and inferior sector were obtained from the 3 instruments and compared. The correlations among the mean deviation (MD) in the Humphrey field analysis and the parameters in global area from the 3 instruments were calculated. RESULTS: The MD showed quadratic regressions to each global parameter (GDx-VCC; 0.625 (nerve fiber indicator): OCT 0.616 (average RNFLT); HRT, 0.501 (rim area)). Most parameters in OCT and GDx-VCC had highly significant correlations. The rim area showed the highest correlation in each sector to RNFLT with GDx-VCC or OCT among HRT parameters. CONCLUSIONS: These structural parameters derived from GDx-VCC, OCT and HRT analyses had bilinear correlation to the MD. Retinal nerve fiber loss detected on GDx-VCC and OCT correlated linearly with the neuroretinal rim area measured by HRT. These instruments may be useful in monitoring structural changes in glaucoma.

Prevalence and clinico-serological correlations of anti-alpha-enolase, anti-C1q, and anti-dsDNA antibodies in patients with systemic lupus erythematosus.

OBJECTIVE: To evaluate the prevalence and clinico-serological correlations of anti-enolase, anti-C1q, and anti-dsDNA antibodies in patients with systemic lupus erythematosus (SLE). METHODS: Sixty-eight sera randomly obtained from SLE patients were examined. Anti-alpha-enolase antibodies were detected by immunoblot on recombinant protein; anti-C1q and anti-dsDNA antibodies were detected using an ELISA. RESULTS: Anti-alpha-enolase, anti-C1q, and anti-dsDNA antibodies were positive in 21%, 62%, and 63% of patients, respectively. A correlation was found between anti-dsDNA and anti-C1q antibodies, while anti-enolase antibodies did not correlate with the other 2 specificities. Anti-alpha-enolase antibodies were not correlated with any of the clinical and serological variables examined. Anti-C1q antibodies were correlated with ECLAM score, leukopenia, complement levels, and active renal involvement. Anti-dsDNA antibodies correlated with arthritis, leukopenia, complement levels, and the presence of renal involvement, independent of activity. In patients with active renal disease anti-dsDNA antibodies were correlated with a poor renal outcome, occurring after a mean period of 24 months. CONCLUSION: These data suggest the association of anti-C1q antibodies with disease flares and active renal disease in SLE. The observed association of anti-dsDNA antibodies and renal disease was expected. Further analysis is required to fully assess the clinical significance of anti-a-enolase antibodies.

Cognitive and psychosocial outcome in survivors of severe traumatic brain injury: correlations with cerebral perfusion pressure, frontal lobe damage and somatosensory evoked potentials.

OBJECTIVE: To correlate neuropsychological outcome in patients after severe traumatic head injury, with neurophysiological and neuroradiological data collected during the intensive care unit (ICU) period of care. METHODS: Patients admitted to Waikato Hospital ICU with severe traumatic head injury were studied. Respiratory difficulty at the accident site, admission Glasgow Coma Score (GCS), anatomic traumatic brain disruption as quantified by a cerebral computed tomography score, prolongation of the central conduction time (CCT) of somatosensory evoked potentials and the percentage time that the cerebral perfusion pressure was less than 70 mmHg (%CPP < 70) were measured. Neuropsychological outcome was assessed, in terms of cognitive and behavioural function, by the Controlled Oral Word Association (COWA) test (performed by the patient) and Head Injury Behaviour rating scale (HIBS, performed by their caregiver) respectively, one year following injury. RESULTS: Sixty-eight patients with a median post-resuscitation GCS of 6 were able to complete the neuropsychological follow up. Most patients had significantly impaired cognitive and behavioural function (mean COWA = 32 and HIBS = 9.7). Cognitive function did not correlate significantly with behavioural function (COWA vs HIBS, r = -0.14, p = 0.27). There were no significant correlations between either GCS (r = 0.15, p = 0.28) or estimates of respiratory difficulty at the accident scene and neuropsychological outcome. Poor cognitive outcome (COWA) was correlated with %CPP < 70 (r =-0.41, p = 0.005) and prolonged CCT (r = 0.26, p = 0.03). There was an insignificant correlation between the CT score and cognitive outcome (frontal lobe score vs COWA, r = -0.12, p = 0.33). However, the group of patients with the most severe frontal lobe injury tended to have a worse behavioural outcome as assessed by the HIBS. CONCLUSIONS: Behavioural outcome as quantified by the caregiver (HIBS) does not correlate well with the degree of cognitive impairment as measured directly from the patient (COWA). Severely head injured patients with poor neurophysiological indicators (%CPP < 70 or prolonged CCT) have a poor neuropsychological outcome. However, anatomical disruption of the brain as estimated by the frontal lobe CT score correlated poorly with outcome.

Correlation between colposcopically directed biopsy and large loop excision of the transformation zone and influence of age on the outcome.

OBJECTIVES: To assess the accuracy and correlation between Colposcopically Directed Biopsy (CDB) and Large Loop Excision of the Transformation Zone (LLETZ) and the influence of age. MATERIAL AND METHOD: A comparative analysis was conducted from the 352 women referred to Rajavithi Hospital from January 1998 to December 2003, to compare between CDB and LLETZ. Correlation was assessed by the percentage of concordance (accuracy), Spearman's rank correlation coefficient (r) and Kappa statistics (K). A subgroup analysis was performed in women aged > or = 50 years to evaluate the influence of age on the correlation. RESULTS: A 66.2% concordance was found between CDB and LLETZ. The correlation and agreement between the two procedures were low (r = 0.32, p < 0.0001; K = 0.24, p < 0.0001). In women aged > or = 50 years, the accuracy was 60.7% and stronger correlation and agreement were noted (r = 0.50, p < 0.0001; K = 0.31, p = 0.001). CONCLUSION: CDB is an inadequate diagnosis tool for Squamous Intraepithelial Lesion (SIL) in woman below 50 years of age. In women aged 50 years or more with satisfactory colposcopy, the accuracy and correlation between two procedures are not compromised.

Correlation between fasting plasma glucose, post prandial glucose and glycated haemoglobin and fructosamine.

This study was done to determine the correlation between glucose monitoring by fasting blood glucose or 2 hours postprandial blood glucose with HbA1c and fructosamine in type 2 diabetic patients. A total of 82 patients from the Primary Care Clinic were enrolled in the study. Fasting blood was drawn for fasting plasma glucose (FPG), glycated haemoglobin (HbA1c) and fructosamine. Two hours after a standard breakfast, blood was again drawn for prandial plasma glucose (PPG). Both PPG and FPG significantly correlated with both HbA1c and fructosamine but PPG showed better correlation to HbA1c than FPG (r= 0.604 vs.0.575) whereas that of FPG and PPG were equally correlated to fructosamine (r= 0.566 vs. 0.551). In predicting good glycaemic control (HbA1c < 7.0%), the sensitivity, specificity and positive predictive value of PPG were 75.0%, 80.6% and 82.5% whereas FPG were 81.8%, 58.3% and 70.6% respectively. These results show that PPG correlated better than FPG to HbA1c and both equally correlated to fructosamine levels. Thus, PPG predicted overall glycaemic control better than FPG. Compared to HbA1c, fructosamine correlated least well with mean glucose profiles. Hence, using HbAlc in monitoring overall glycaemic control is better than fructosamine.

A multivariate procedure for identifying correlations between diagnoses and over-the-counter products from historical datasets.

A general problem in biosurveillance is finding the optimal aggregates of more basic data to monitor for the detection of disease outbreaks. We developed a multivariate procedure for identifying the set of over-the-counter (OTC) healthcare products that correlates best with a set of diagnoses. To ensure that the procedure produces results that agree with clinical knowledge of diseases and (OTC) products, we applied it to a set of products and set of diagnoses for which the correlation was known to be high. Our hypothesis was that the model could achieve parsimony in the set of diagnoses that correlate with sales of pediatric electrolytes while still producing a high correlation. The procedure narrowed the set of diagnoses that correlate with pediatric electrolytes from 51 diagnoses to eight diagnoses. The correlation of the set of 51 diagnoses with electrolyte sales was 0.95 and the correlation of the set of 8 diagnoses with electrolytes was 0.96. We conclude that the procedure functions as intended and is suitable for further testing with other problems in finding optimal aggregates of OTC products, and more generally of other types of biosurveillance data, to monitor for the detection of various disease outbreaks.
