Molecular dynamics at a cytoplasm/membrane interface of a signal sequence from an E. coli maltoporin.

We used a recently developed software that mimics a cytoplasm/membrane environment, with an interface separating two continuous media of different dielectric constants (1). This software has been designed to allow modelling of different kinds of molecules of biological interest such as proteins and drugs, in each of the isolated continuous media, as well as in their interactions with membrane-like structures, making use of the dielectric discontinuity represented by the interface. In the present study we have applied this program ("THOR") to model a polypeptide sequence corresponding to a 50% active mutant of the signal sequence of the lamB gene product of E. coli, known as maltoporin or lambda receptor (2). The peptide was first submitted to optimization of its molecular geometry followed by molecular dynamics in water (epsilon = 80) until thermalization was achieved. The conformation evolved from a rather extended random conformation to increasingly folded structures. The presence of the dielectric discontinuity induced the movement of the molecule's center of mass from water towards the interface. The entry of the peptide into the lower dielectric constant medium (epsilon = 2) through the interface was paralleled by a decrease in the total potential energy, indicating the affinity of the peptide for the lipid-mimetic phase.

[Functional anatomy of the upper airway: cephalometric and reflective acoustic studies].

Cephalometric analysis is an effective tool for the evaluation of craniofacial and pharyngeal changes in obstructive sleep apnoea. Meanwhile even for pharyngeal changes a lot of average values exist, which show significant differences for many cephalometric parameters. Approximately 40% of patients with obstructive sleep apnoea proved to have a retrognathic, dolichofacial appearance, which seems to contribute to obstructive apnoeas by retropositioning of the skeletal fixation for the muscles of the floor of the mouth and the tongue. Acoustic rhinometry for transnasal cross-sectional measurements of the upper airways is still under clinical trial. Whether it is a valuable supplement to cephalometric evaluation in the future will be mainly a question of software development.

Measurement of DNA double-strand breaks in mammalian cells by pulsed-field gel electrophoresis: a new approach using rarely cutting restriction enzymes.

A new method is presented for measuring DNA double-strand breaks (DSBs) which combines the technique of pulsed-field gel electrophoresis (PFGE) with the idea of velocity sedimentation. Purified DNA samples were treated with restriction enzymes, which results in DNA of sizes which can be separated by PFGE. After electrophoresis, unirradiated DNA shows a size distribution (obtained with the help of a specially developed software program) similar to that obtained with the sedimentation technique; with X irradiation, this distribution is shifted to lower molecular weight with increasing dose. The rate of DSB induction was calculated by comparing the curves obtained experimentally with theoretical distributions (based on the assumption that breaks are formed according to Poisson statistics). The method was tested by measuring X-ray-induced DSBs in P3 (derived from human epithelial teratoma) cells. The induction of DSBs was found to be linear with dose and a rate of 5.4 x 10(-3)/Mbp/Gy was obtained.

[Brachytherapy of ocular melanoma: physical and dosimetric aspects of 125I plaques].

Off-axis absorbed dose distribution of ophthalmic plaques is a useful parameter for the explanation of some observed biological effects. From a specific dosimetric software, developed in our brachytherapy department, and from our measurements around 125I eye plaque, we studied the dose distribution effects on the spread of activities inside the plaques. After analysis of experimental data, we observed also that the dose distribution in the penumbra area may be described easily with decrement lines.

Supporting ad-hoc queries in an integrated clinical database.

Whether caring for patients or conducting research, medical decision-makers need access to clinical data. To fulfill that need, commercial software developers have produced a wide range of database query tools that differ greatly in functionality and cost. Generally, tools that have a greater ability to conceal database complexity from the user also require more effort for administrative setup. We describe a cost-effective, commercially-available query tool that requires no special setup to perform most simple queries, yet can be customized to satisfy users' more complex querying requirements.

[Quantitative magnetic resonance tomography and the severity of deficits in dementia of the Alzheimer type].

The aim of the present study was to investigate the severity of dementia of the Alzheimer type (DAT) with respect to morphological changes revealed by quantitative magnetic resonance imaging (MRI). Seventeen patients with DAT (NINCDS-ADRDA guidelines) and 10 healthy elderly controls were included. The severity of dementia was evaluated on the Mini Mental State Examination (MMSE), the Global Deterioration Scale (GDS) and the Brief Cognitive Rating Scale (BCRS). Three-dimensional MRI sequences were acquired using a 1.5-T Siemens Magnetom. Whole-brain volume and the volume of the amygdala-hippocampus complex (AHC) were assessed using the newly developed software NMRWin. This software provides a semi-automated measure of the whole brain volume, while measurement of substructures requires manual guidance. In addition, the ventricle-brain ratio (VBR) was assessed. All morphometric parameters differed significantly between the two groups. AHC volumes discriminated best between them with only a small overlap. These findings were confirmed when only the data of the mildly demented patients were included in the analysis. The degree of AHC atrophy exceeded that of generalized cerebral atrophy. The severity of dementia as indicated by the MMSE, GDS and BCRS scores was correlated significantly with the volumes of the AHC bilaterally, but not with whole-brain volume or with VBR. These results underline the importance of the mesial temporal substructures in the etiology and progression of DAT and indicate that the volume of the AHC can be monitored by MRI and may be used to follow up the disease process.

Eye movement in amyotrophic lateral sclerosis: a longitudinal study.

Eye muscles and the sphincter muscles of the bowel and bladder were formerly thought to be spared in amyotrophic lateral sclerosis (ALS), a neurodegenerative disorder. As frequent subclinical impairment of the anal sphincter muscles in ALS has recently been reported, We suspected an earlier and more frequent, if subclinical, involvement of the oculomotor system than hitherto described. Starting in 1993, we repeatedly studied oculomotor involvement in eight patients with (ALS) using routine orthoptic examination techniques as well as electro-oculographic recordings of ocular movements. Three patients had consistently normal clinical examinations but progressive changes in electro-oculography (EOG). Three patients showed intermittent impairment of eye movements with normal EOG (one patient) or with progressive changes on EOG examination. In two patients, both clinical examination and EOG were progressively pathologic. These findings provide further evidence of early oculomotor involvement, e.g., prior to respiratory failure, in ALS than previously suspected. Since EOG changes seemed to be detectable in all but one patient, EOG changes may allow earlier, subclinical detection of impaired eye movement and thus reveal even an increased frequency of oculomotor impairment in ALS. Due to the heterogeneity of ALS the number of patients examined to settle finally the question of oculomotor involvement in ALS needs to be increased. Newly developed software will allow further interpretation and comparison of more data and, thus, should offer further help in detecting early changes.

Assessment tools: functional health status and patient satisfaction.

This article addresses an aspect of outcomes assessment that includes outcomes from the perspective of the patient; namely, functional health status and patient satisfaction. For rehabilitation care they are the only outcomes that can be measured. Measurement tools exist and are being used to quantify the qualitative perspectives of patient care to produce valuable information about the quality and the efficiency of rehab services. The biggest challenge is lack of standardization to allow comparability of data. Different users bring different perspectives to the issue. The researcher, the software developer, the health care facility, and the payer all have an interest in using functional status and patient satisfaction to evaluate outcomes.

[Optimizing complicated treatments in dental medicine indirectly through the coordination center. Current challenges].

Patient care and processing of the many tasks surrounding a 'multi disciplinary patient' give problems which are difficult to attend, even in well organised practices. Activities, medical and other, are seen as tasks which are standardised as much as possible. The follow up is delegated to a co-ordination centre. Special developed software supports the co-ordinator(s). There are four stages: 1. introduction of the patient and his team, 2. organisation of the preliminary examinations, 3. the treatment planning involving the organisation of a team conference, the work out of the different treatment proposals and support of the patient while choosing the treatment plan of his choice and 4. the implementation of the treatment plan. Throughout the whole procedure care is taken to have a maximal involvement of the general practitioner. The concept allows him to keep his difficult, yet challenging, patients.

A probabilistic classification system for predicting the cellular localization sites of proteins.

We have defined a simple model of classification which combines human provided expert knowledge with probabilistic reasoning. We have developed software to implement this model and have applied it to the problem of classifying proteins into their various cellular localization sites based on their amino acid sequences. Since our system requires no hand tuning to learn training data, we can now evaluate the prediction accuracy of protein localization sites by a more objective cross-validation method than earlier studies using production rule type expert systems. 336 E. coli proteins were classified into 8 classes with an accuracy of 81% while 1484 yeast proteins were classified into 10 classes with an accuracy of 55%. Additionally we report empirical results using three different strategies for handling continuously valued variables in our probabilistic reasoning system.

Smoking attributable mortality and economic costs in Alaska 1992-94.

Tobacco is one of the leading preventable cause of death in the United States and Alaska. Alaska has one of the highest smoking prevalences. The Smoking-Attributable Mortality, Morbidity and Economic Cost software developed by the Centers for Disease Control and Prevention was used to estimate the deaths and economic impact due to smoking in Alaska. In Alaska during the three year period 1992-94, 1416 deaths were estimated to be attributable to smoking, accounting for 19.8% of the 7159 deaths during that time. Direct medical care costs in 1993 due to smoking related illnesses were estimated at $96.5 million. Additional smoking related costs include an indirect mortality cost of $81.61 million and indirect morbidity cost of $15.94 million. The total economic cost attributable to smoking related illness for 1993 is estimated at $194 million [corrected]. In summary, for the time period 1992-94, smoking was estimated to result in 470 deaths per year and in economic costs of almost $300 million per year.

Assessment of a cooperative workstation.

Groupware and new Information Technologies have now made it possible for people in different places to work together in synchronous cooperation. Very often, designers of this new type of software are not provided with a model of the common workspace, which is prejudicial to software development and its acceptance by potential users. The authors take the example of a task of medical co-diagnosis, using a multi-media communication workstation. Synchronous cooperative work is made possible by using local ETHERNET or public ISDN Networks. A detailed ergonomic task analysis studies the cognitive functioning of the physicians involved, compares their behaviour in the normal and the mediatized situations, and leads to an interpretation of the likely causes for success or failure of CSCW tools.

[Pulse oximeter: a component of medical computer-aided monitoring].

A nonautonomous pulse oximeter (a device) that is controlled by a personal computer (PC) is described. The division of functions between the device and PC is shown. Algorithms are presented for signal treatment and device gain control. A list of the developed softwares and an outline of the functions fulfilled are given. The prototypes of the device have been approved and are used at the intensive care units and operating rooms of the First Republican Clinical Hospital of the Udmurt Republic.

Information mining on the Internet.

As with most software, developers tend to create interfaces which seem logical and user-friendly. The only problem is that 'logical' to one person is confusing to another. The best approach is try out several search engines and spend enough time trying sample searches that you get a feel for the degree to which it satisfies your needs. All of these sites have adequate help areas and should be your first stop. Hopefully, these tools will help you to spend less time aimlessly surfing and more time realizing the potential of the Internet.

Cerebellar size and cognition: correlations with IQ, verbal memory and motor dexterity.

The objective of this study was to examine the structure/function relationship between in vivo cerebellar size and higher cognitive function in a sample of healthy young subjects. The design of the study involved correlation of in vivo cerebellar volume measurements with measures of general intelligence (WAIS-R V&P FSIQ, Vocabulary, Block Design, and Digit Span subtests), motor dexterity (Halstead-Reitan Finger Tapping), verbal (WMS Logical Memory), and visual (Rey-Osterrieth Figure) memory covaring for cerebrum size. A similar analysis was performed using left temporal lobe volumes as a control region. The sample consisted of 62 healthy subjects (30 females, 32 males) enrolled as controls at the MHCRC at the University of Iowa Hospitals and Clinics. This independent sample does not overlap with the groups studied in our previous report on the relationship between cerebellar and brain size and IQ. Cerebellar and total brain size were estimated through automatic, atlas-based volume measurements using MR images obtained with a T1-weighted three-dimensional SPGR sequence on a 1.5-T GE Signa scanner and locally developed software. Cerebellar volume significantly correlated with Finger Tapping (left hand: r = 0.218, p < 0.05; right hand: r = 0.211, p < 0.05) and with memory retention of complex narrative material (r = 0.27, p < 0.02). Cerebellar volume correlated with general intelligence in the expected direction (r = 0.19, p < 0.07). This study confirms previous work indicating that the cerebellum may make a contribution to several aspects of cognition. Cerebellar volume significantly correlated with the ability to retain already encoded information in the verbal domain and with fine motor dexterity. Cerebellar volume positively correlated with general but the relationship did not reach statistical significance. The structural/functional relationship between cerebellum and verbal memory abilities is consistent with evolutionary theory for the phylogenetical increase in the size of the cerebellum.

The heart's fibre alignment assessed by comparing two digitizing systems. Methodological investigation into the inclination angle towards wall thickness.

Myocardial contractile pathways which are not aligned strictly parallel to the heart's epicardial surface, give rise to forces which also act in the ventricular dilating direction. We developed a method which allows us to assess any fibre orientation in the three-dimensional myocardial weave. Decollagenized hearts were prepared by peeling-off fibre strands, following their main fibre orientation down to near the endocardium. In the subepicardium the strands followed a course more or less parallel to the epicardium, whereas from the mid-wall on they tended to dive progressively deeper into the wall. The preparation displays more or less rugged surfaces rather than smooth layers. The grooves and crests on the exposed surfaces were sequentially digitized by two methods: (1) Using a magnet tablet (3 Draw Digitizer System, Polhemus, Cochester VTO 5446, USA) on a dilated pig heart we manually followed the crests using a stylus, handling each groove and crest as an individual contractile pathway. (2) A constricted cow heart was digitized using a contact-free optical system (opto TOP, Dr. Breuckmann, Meersburg, Germany), which is based on the principle of imaging triangulation. Using specially developed software the inclination angles of selected crests and grooves with respect to the epicardial surface were calculated. The two digitizing methods yield comparable results. We found a depth- and side-specific weave component inclined to the epi-endocardial direction. This oblique netting component was more pronounced in the inner 1/3 of the wall than in the subepicardium. The inclination angle probably increases with increasing wall thickness during the ejection period. Manual digitizing is an easy and fast method which delivers consistent results comparable with those obtained by the cumbersome high resolution optical method. The rationales for the assessment of transmural fibre inclination are (1) the putative existence of dilating forces inherent in the myocardial weave and (2) the possible overproportional increase in the oblique transmural weave component during myocardial hypertrophy, which would entail a reduction in efficiency of ventricular performance in terms of haemodynamic work.

Computer-assisted psychotherapy.

The rationale for using computers in psychotherapy includes the possibility that therapeutic software could improve the efficiency of treatment and provide access for greater numbers of patients. Computers have not been able to reliably duplicate the type of dialogue typically used in clinician-administered therapy. However, computers have significant strengths that can be used to advantage in designing treatment programs. Software developed for computer-assisted therapy generally has been well accepted by patients. Outcome studies have usually demonstrated treatment effectiveness for this form of therapy. Future development of computer tools may be influenced by changes in health care financing and rapid growth of new technologies. An integrated care delivery model incorporating the unique attributes of both clinicians and computers should be adopted for computer-assisted therapy.

[Development in the National Hospice Care Service in Korea].

The urgent needs to establish hospice care systems in Korea arise from the following reasons: (1) a drastic increase in chronically ill patients with the increase of aged population: (2) rapid changes in living environment from the traditional habitation (e.g., Many Koreans living in apartment complexes, which is the most popular form of modern residence in recent years, prefer to die in the hospital.): the overall increase in patients with advanced cancer: (4) recent trends in early discharge of terminally ill patients from the limited hospital facilities to accommodate other medical insurance beneficiaries: (5) easy acceptance of euthanasia owing to the recent social atmosphere that belittles the dignity of human life: (6) medical and nursing care of AIDS patient in terminal stage: (7) and the problem associated with inhumane medical care system, overtreatment, and groundless fears against narcotics. Terminally ill patients were used to be treated in the hospital in the past. In these days, however, they are forced to have home cares with little assistance from the qualified medical personnel because of insufficient hospital facilities, which are even short for the need of emergency patients and provide priority cares to medical insurance beneficiaries with other acute problems. And yet, neither are there any administrative organizations nor systematic medical studies that deal with the level of terminally ill patient's need, their family's problems and resources of hospice care systems in Korea. Thus, most patients are not able to get appropriate medical care at the terminal stage of their lives. The objective of this study is to make comprehensive database for various hospice care organization currently in operation, link them through medical information system, and develop an easily accessible hospice care model that meets the need of most Korean people. Our survey results may be summarized as follows: Nationally there are 40 organizations that provide partial or full hospice care. However, these organizations are not linked to any formal medical service network. Furthermore, the objective of hospice care, care principles, personnel with appropriate training, educational programs, standard for care, costs, consulting service to patients' family members, the extent of medical care from professional staff members, status of hospice facility, and management of those institutions are neither clearly defined nor organized compared to the international hospice care standards. The surveys on patients of terminal stage, grouped in hospice and non-hospice care patients, reveal what they want visiting nursing care to help their pain control, psychological, social and spiritual demands. While the more than 90% of hospice care patients want to reduce their pains, the non-hospice care patients, in addition to their desire for pain control, demanded more psychological, social and spiritual helps as well. The results of this research could be utilized to (1) define the standard of hospice care, (2) provide the guidance for hospice medical care costs, (3) establish the database of hospice care systems, (4) develop softwares, (5) build communication network through Medinet, and (6) provide an organized visiting home nursing care system. These information should be a valuable resource to many medical staffs who are involved in cancer therapy, nursing care, and social welfare programs.
