Canines erupted through grafted alveolar cleft defects in patients with alveolar clefts: a pulp testing evaluation study.

An electric pulp tester (EPT) was used to evaluate the pulpal response of erupted canines in 75 patients: 16 patients with unilateral cleft palate who received autogenous bone grafts to correct their alveolar cleft defects before canine eruption; 35 patients without cleft palate whose canines erupted spontaneously, but who required orthodontic treatment for alignment; and 24 patients without clefts whose canines required surgical uncovering and orthodontic assistance for eruption. Thirty-one percent of the erupted canines in the patients with grafted unilateral clefts did not respond to EPT, whereas all canines in the contralateral noncleft side responded to EPT. There was no clinical or radiographic evidence of pulpal pathosis of the canines that had erupted through the grafted alveolar clefts. However, these teeth had a high incidence of nonresponsiveness to pulp stimulation, therefore necessitating close follow-up in case they show eventual signs of pulpal degeneration that may require endodontic intervention.

[Clinico-surgical evaluation of dental elements in bone inclusions].

Dental inclusions are assessed on the basis of clinical and operating statistics relating to the years 1981-1985 and comprising 815 patients and 1445 treated teeth. The results confirm the predominance of inclusions among female patients and the prevalence of the mandibular site; the most frequently included teeth are the lower eighths followed by the upper eighths, the upper canines and the second lower premolars. Inclusions are bilateral in most cases. These findings are in substantial agreement with the epidemiological data recorded in the literature. The operating series shows that the type of surgical treatment varies with the localisation of the tooth (anterior or posterior), its relationship with contigous teeth and with the surrounding anatomical structures and the age of the patient. Teeth in the posterior sector are usually extracted. At young age, germectomy is carried out for orthodontic purposes; at mature age, teeth are extracted to prevent the formation of periodontal pockets and the aggravation of caries and their complications. On the other hand, for teeth in the anterior sector, surgery is usually merely the first stage in treatment and is followed by treatment of orthodontic type with traction, in view of the important aesthetic and functional role of the labial teeth.

[Wisdom teeth in the fracture line].

The evaluation of 139 mandibular fractures with wisdom teeth in the fracture line permits to draw the following conclusions: 1. Partially retained teeth in the fracture line frequently lead to infections. For this reason, they should be removed on principle prior to immobilization. 2. Retained teeth or tooth germs caused in no case an infection; consequently, they must not be removed. 3. Fully erupted teeth produced but occasionally an infection of the fracture line. In such cases, the therapy will depend on the functional value of the respective tooth.

[Cleidocranial dysplasia--histological findings in the dental cementum].

Up to now only very few reports on histological studies of teeth in cleidocranial dysplasia have been published. We are presenting a 12-year old boy with this syndrome. Seven of his teeth were studied microscopically. The most relevant results are as follows: acellular cementum in all teeth, almost complete missing of cellular cementum, normal intermediate cementum. The pathogenesis of the retarded eruption of teeth in this disorder is explained as a result of eburnation of the alveolar bone as well as fibrosis of the gingiva. The therapy of choice in order to stimulate eruption is a rigid orthodontic treatment of the impacted teeth using fixed appliances.

Maxillary anterior displacement and impaction in the mixed dentition.

Attention has been given to problems of eruption in the maxillary anterior region. It is a region where a variety of anomalies occur. Since the maxillary anterior region influences appearance so greatly, early detection of difficulties and careful planning and treatment can be extremely beneficial to patients.

[Considerations on composite odontoma].

Odontoma in most cases presents in the mixed dentition period and can remain for long unrecognised. Asymptomatic during its long latency period, odontoma gives signs of life only in an aspecific way: the absence in the arch of one or more teeth should direct the clinician towards investigations which make it possible to identify the cause, as this may be due to odontomas. Radiography may clearly point to the presence of this lesion and may direct the operator towards surgical/orthodontic treatment so as to restore to the dental arch a tooth that would otherwise have remained included. It is, furthermore, important to remove the lesion, even though it is quite benign, because it is easily subject to septic complications. It should be recalled that it is always necessary to back up clinical with histological diagnosis so as to differentiate the odontoma from other lesions with different prognosis and therapy.

[Bony changes in chronic odontogenic maxillary sinusitis].

The authors undertook histological studies on the bone of the facial wall of the maxillary sinus from 64 out of 84 patients who had been operated on for chronic maxillary sinusitis of dental origin in the course of 16 months. Apart from mucosal changes, the specimens from 16 patients showed also osseous changes in the form of periosteal reactions of varying extent. With regard to therapy, it is concluded that patients suffering from odontogenic sinusitis should for preference be treated by surgical intervention, because the conservative treatment will not necessarily result in complete healing.

[Hydantoin hyperplasia--the interesting case].

The author describes a case of excessive gingival hyperplasia in a 9-year-old girl under hydantoin medication. The hyperplasia of the gingivae inhibited the regular uptake of food and the eruption of the permanent teeth.

[Interferences with the development of the jaws and teeth in mixed dentition].

On the basis of results from animal experiments, the authors emphasize a series of factors interfering considerably with tooth eruption. It is dealt with tooth eruption disorders and dental malformations due to premature loss of teeth, fused deciduous teeth, certain dystrophies, submersion of deciduous teeth, dysembryoplasias, and rickets.
