Detection and size estimation of acute myocardial infarction using 99Tc-glucoheptonate.

Twenty-seven patients with suspected acute myocardial infarction were studied by precordial scanning after intravenous administration of 99mTc-gluchoheptonate 2-48 hr after the onset of chest pain. Fifteen of the patients had clinically documented acute myocardial infarctions. Twelve of these 15 (80%) had areas of distinctly increased tracer uptake in the region of the heart. The three infarctions not identified by scan had peak serum CPK values of less than 300. In seven patients without infarction, no distinct areas of increased tracer uptake were found in the region of the heart. Five patients could not be classified as to whether infarction had or had not occurred. Three had abnormal scans, the significance of which is uncertain. Infarct size was estimated from the area of increased 99mTc-glucoheptonate concentration on scan and compared to peak serum CPK values. A linear correlation with a correlation coefficient of 0.77 was found. Technetium-99-m-glucoheptonate scanning was useful for the identification and size estimation of moderate- to large-size transmural and nontransmural acute myocardial infaractions.

The synthesis of 3-deoxyheptulosonic acid 7-phosphate.

3-Deoxyheptulosonic acid 7-phosphate was obtained by a one-step chemical synthesis through condensation of oxalacetate with erythrose 4-phosphate. This reaction occurs at measurable rates only in the presence of a metal ion; Co2+ and Ni2+ are the most effective catalysts. The Co2+ catalyzed condensation of oxalacetate and erythrose 4-phosphate proceeds at room temperature and neutral pH. Since erythrose 4-phosphate can be replaced by any free aldehyde tested thus far, this type of a homogeneous catalysis opens new synthetic routes to alpha-keto-gamma-hydroxy-fatty acids and their derivatives.

Plasma ionic calcium levels following injection of chloride, gluconate, and gluceptate salts of calcium.

The ionic equivalency of three calcium salts was tested in 15 patients undergoing cardiac surgery with extracorporeal circulation. Of the three salts tested (chloride, gluconate, gluceptate), only calcium chloride showed a reproducible and highly significant relationship between the increase in total calcium and the increase in ionic calcium. It is suggested that extracorporeal circulation may be one clinical situation in which use of a calcium electrode may be of major value. The marked distortion of plasma proteins and pH, the addition of large amounts of citrate, and the differences between various calcium salts indicate that it is probably not possible to predict ionic calcium with assurance and that direct measurement may be necessary for optimal therapy.
