Forecasting physician workforce requirements in Minnesota.

The existing oversupply, specialty imbalance, and geographic maldistribution of clinicians are inconsistent with the goals of health system reform in Minnesota. Health professional workforce projections can help the private and public sectors make informed decisions to assure that the future supply of health professionals reasonably approximates requirements. Appropriate methodologies for projecting national health professional requirements, based on existing HMO staffing patterns, should be applied to Minnesota. In the absence of workforce projections, a variety of assumptions about changes in Minnesota's health care system that will affect workforce supply and requirements can, nonetheless, be made. Limitations on the state's ability to influence the health professional workforce within its borders are noted.

FORECAST 2000: a prediction of skills, knowledge, and abilities required by senior medical treatment facility leaders into the 21st century.

This paper reports results from a Delphi study conducted among the Commanders and Deputy Commanders for Administration of 37 Army medical treatment facilities (MTFs), who identified the most important issues challenging their institutions for the remainder of this decade, and the skills, knowledge, and abilities required by MTF leaders to deal successfully with those challenges. A Delphi mail-out was conducted in two iterations. Respondents identified 187 health care issues which were divided by content into nine domains by a panel of health care experts. The domains, ranked by importance, were cost-finance, health care delivery, access to care, quality and risk management, technology, professional staff relations, leadership, marketing, and ethics. In the second Delphi iteration, MTF leaders agreed upon the necessary skills, knowledge, and abilities of future leaders. Results indicated that future leadership will require enhanced financial, quantitative, and technical skills, as well as competence in a broad array of interpersonal and communication skills. Implications for military medical leader development initiatives are discussed.

[Incidence of NIDDM in Daqing and forecasting of NIDDM in China in 21st century].

Based on the survey of the prevalence of NIDDM in 110 660 people aged 25-74 in 1986, the incidence of NIDDM was investigated in 1990 in 36 471 people (M/F 18,801:17,670) who had normal glucose tolerance and/or plasma glucose concentration less than 6.7 mmol/L 2-hour after breakfast (carbohydrate 80 g) four years ago in Daqing, Heilongjiang province, northeast China. 103 male and 88 female diabetics were diagnosed among the 36 471 subjects from June 1986 to June 1990. This result showed that the annual incidence of NIDDM in this area is 131/100,000 (137 in males, 125 in females), and is 130/100,000 standardized to the 1982's chinese population (95% confidence interval, 94-168/100,000). We may estimate that there would be 754 thousand new diabetics per year in 25-74 years old Chinese if the total population were 1.3 billion in China in the 21st century.

Dyslipidemia and atherosclerosis. A forecast of pharmaceutical approaches.

Five classes of lipid-lowering drugs are approved by the Food and Drug Administration (FDA) for use in the United States: nicotinic acid, bile acid sequestrants, fibric acid derivatives, reductase inhibitors, and probucol. None of the agents has an antiatherosclerotic indication. Cholestyramine and gemfibrozil have received indications for preventing complications of atherosclerosis, namely, myocardial infarction and coronary artery disease death. Foreseeable pharmacological strategies to reduce lipid-related cardiovascular risk might be divided into three categories. First, the present approach of lowering lipid and lipoprotein concentrations might be extended through modification of available agents (e.g., a more potent or soluble bile acid resin) or development of agents of novel mechanism (e.g., acyl-CoA:cholesterol acyltransferase [ACAT] inhibition or inhibition of cholesterol biosynthesis at a step other than HMG-CoA reductase). Second, blood lipids could be directly addressed outside of lipid-lowering strategies. Raising high density lipoprotein (HDL) cholesterol levels has not been fully explored, or the target might be modification of the lipoproteins themselves rather than their concentrations. Areas of particular interest in the latter regard are hepatic lipase activity, cholesteryl ester transfer protein activity, and differences between oxidized or otherwise modified low density lipoprotein (LDL) particles and normal LDL. Third, it may be possible to directly lessen the atherosclerotic potential of the vessel wall (e.g., through protecting it from the effects of certain growth factors or altering its state of relaxation.

A forecast model for hazel (Corylus) and chestnut (Castanea) pollen emission.

In this paper a simple conceptual model is introduced in order to predict the atmospheric pollen concentration threshold above which pollinotics show allergic symptoms. In addition, the range of the pollen sampler with respect to the local and remote emission of pollen grains, produced by the vegetation in its flowering phase, is highlighted. Two different pollen types have been considered; their emission in the atmosphere takes place in two periods of the year, which greatly differ both in climatological respect and concerning the atmospheric of mixing particles. The data analysis is carried using a methodological approach, taking into account phenological, aerobiological and meteorological parameters.

Year to end budget forecasting: income and expenditure.

The introduction of devolved management structures within the NHS has exposed many clinical staff to the disciplines of financial management. This short article looks at the principles behind the preparation of budgets, the purpose of the income and expenditure account and the tools used to monitor expenditure at directorate and department level within a hospital.

[A forecasting trial of the dento-skeletal and cutaneous facial profile, as a function of sex, in Class II, division 1 malocclusion, using regression analysis of the coordinates].

For a pattern of 36 leucoderm subjects, aged 12 years, with Class II, division 1, prediction of results are been calculated, in each sex, proceeding bony and cutaneous points coordinates, in axis defined by S-N, with "regression" method. Bony, dental and cutaneous profiles expected 3 years afterwards can be established in a personalized way, as well as correspondent security limits. A logical make easy utilisation of this process.

Executive skills 21: a forecast of leadership skills and associated competencies required by naval hospital administrators into the 21st century.

Those in the Naval Medical Department are experiencing an exciting time of bridled chaos and creative change. Many mid-career officers are uncertain of the leadership behaviors and skills that will be necessary for successful managerial careers. Changes in the method of health care delivery of this nation combined with the reengineering of the armed forces' world-wide mission has driven military medical leaders to expand their professional skills, knowledge, and abilities beyond the clinical sciences. This research identifies the most critical domains in the science of health care administration and differentiates and ranks job skill, knowledge, and ability requirements that will be necessary for successful health care management into the 21st century. Top Naval hospital executives responded to two iterations of a Delphi inquiry. These medical leaders identified 106 unique issues that were content-analyzed into nine domains by a neutral, expert panel. Domains, in order of ranked importance, were leadership, health care delivery systems, cost-finance, technology, accessibility, professional staff relations, marketing, quality-risk management, and ethics. In the second Delphi iteration, hospital executives reviewed domain results and rated identified job requirements on their required job importance. The top-10 rated skills, knowledge, and abilities are reported. Results indicated that although a business orientation is needed for organizational survival, an emphasis on person-oriented skills, knowledge, and abilities is required for future success as a health care administrator in the Naval health care system.
